In the 1980s, sudden infant death syndrome (also known as SIDS or cot death) was one of the most common post-neonatal causes of death in the UK. The dramatic 80% fall in these deaths over the last three decades is a testament to evidence-based research using the observational case–control study and even more impressive given the difficulties in funding studies without a randomized controlled trial design. This chapter will describe the initial breakthrough in the early 1990s, the characteristic profile of SIDS, the associated factors identified, the potential causal mechanisms, and the current risk reduction messages. Most of the risk reduction messages are non-controversial and have been readily accepted by health professionals and parents alike. The divergence of opinion regarding how we give preventative advice surrounding infant bed sharing and the strategies employed is addressed in more detail.

Patient safety suddenly burst into public consciousness in the late 1990s and became a “celebrated” cause in the 2000s. It has since gradually faltered, and little improvement has been noted over almost 20 years. Both the rise and fall of patient safety demand explanation. Medical harm had been known long before the 1990s, so why did it suddenly become popular? And why were safety efforts ineffective? The authors propose that this rise was due to a discursive shift that reframed “medical harm” into “medical error” in the setting of anxiety about industrialization and great change in healthcare. The “error” framing, with its inherent notion of agency, was useful in advancing the agenda of a technocratic, managerial group of health professionals and diminishing the authority of the old guard based on clinical expertise. The fall was due to this “medicalization” of safety. Health professionals and managers with little knowledge of safety science came to dominate the patient safety field, crowding out expertise from the safety sciences (e.g., psychology, engineering) and thus keeping reform under the control of the healthcare establishment. Operating with a sort of delusional clarity, this scientific-bureaucratic cabal generated a great deal of activity but made little progress because they failed to engage with expertise in the safety sciences. Twenty years after sudden popularity, there is general agreement that little of value has been achieved. The future of patient safety is in doubt, and radical reform in approaches to safety will be required for progress to be made.

Attention can be defined as a multifaceted gateway to consciousness. In this chapter we summarize the current knowledge on coronary vascular development and identify the essential mechanistic cellular and molecular components of coronary morphogenesis. The second part of the chapter describes how Shakespeare uses and adapts recognizably ‘medieval’ details in his tragedies. Cardiovascular disease is the leading cause of death in the United States and accounts for more deaths than all cancers combined. Many apparently healthy adults first present to the health system with a myocardial infarction or sudden cardiac death. It is therefore imperative to assess the risk of cardiovascular events among currently asymptomatic individuals to try and intervene early in the disease process to prevent future events. Risk factors for the development of coronary artery disease in particular, and cardiovascular disease in general, have been identified from epidemiological studies and used to develop global risk scores to risk stratify populations. Although these scores work well at the level of the population, unfortunately they are imperfect at estimating risk at the level of the individual. In this Chapter we will discuss the use of exercise stress testing and other imaging modalities as screening methods for the detection of coronary disease and predicting risk in asymptomatic adults. The hope is to develop a screening program to identify cardiovascular disease early in the disease process to introduce interventions that will favorably impact outcomes in a manner similar to the current screening programs available for several cancers.

RPA commonly presents with sore throat, fever, torticollis, dysphagia, neck pain, muffled “hot potato” voice, cervical lymphadenopathy, and respiratory distress. Epiglottitis symptoms classically include the triad of drooling, dysphagia, and distress but may include inspiratory stridor. Parotitis is recognized by a sudden firm, erythematous swelling of the preauricular and postauricular areas. Immediate airway management and otolaryngology consultation are required. Most patients will require transoral or transcervical incision and drainage in addition to IV fluid resuscitation and close observation.

When severe, epilepsy will often destroy health and shorten life. Sudden unexpected death in epilepsy is a dark reality. Modern drugs and devices for uncontrolled seizures often offer a poor value proposition. Interest in retributive theory, and emphasis on proportionality between crime and punishment as a requirement of justice, revived in English-speaking countries in the 1970s. After less than a half century, however, retributivism’s influence is waning. It is beset by challenges. Some, such as difficulties in scaling crime seriousness and punishment severity, and linking them, are primarily analytical and of interest mostly to theorists. Others, such as trade-offs between proportionality and crime prevention, relate to real-world applications. Both sets of challenges can be explored in their own terms, and solutions can be sought. The bigger question, though, is whether the challenges are epiphenomenal and portend displacement of retribution as the most intellectually influential normative frame of reference for thinking about punishment. Only time will tell whether retributivism is in terminal decline. Most likely, the difficulties contemporary philosophers face are as much a reflection of a change in the zeitgeist, in prevailing sensibilities, in mentalités as of sudden realization that retributive ideas offer less guidance for thinking about punishment than was widely understood.

For me every sound has its own shape or form. This sense of shape is like objects in my periphery. They move around me and change in size and structure depending on how the sounds change. The experience is more intense with more complex sounds and when the source of the sound is not visible. I wonder if it is my brain creating the visual for the sound. The shapes seem to reflect the sound: liquid sounds often create fluid bubbly shapes; sharp clanging sounds have more angular shapes like growing crystals; bass sounds are large and expanding. When there is a loud, seemingly singular sound, this can create a sense of space around me as if I were inside the shape itself. When many sounds occur at once, the shapes often combine, creating a complex structure or a texture. These shapes sometimes blend together, rather as sounds do in the environment, creating a moving landscape. These experiences are part of how I sense the world and rarely stand out as distractions. Sometimes, however, a sudden, unexpected sound will evoke a synaesthetic experience that is distracting, drawing my attention away from whatever I might be doing....

Benign paroxysmal positional vertigo (BPPV) is by far the most common cause of vertigo, and approximately one in five people will have it at some time during their lifetime. Sudden violent spells of vertigo are triggered by a change in position, such as turning over in bed, getting in and out of bed, bending down and straightening up, and extending the head back to look up. During an attack, both eyes oscillate in a characteristic pattern, called nystagmus. Indeed, understanding the mechanism for nystagmus production was key to developing a cure for BPPV. It is difficult to say who exactly deserves credit for discovery of the cure for BPPV, but as with most discoveries in medicine, there is a long list of contributors, each providing a small piece to the puzzle. To tell the story, this book focuses on five remarkable physician scientists who made key discoveries leading up to the cure for BPPV: Prosper Ménière (1799–1862), Josef Breuer (1842–1925), Robert Bárány (1876–1936), Charles Hallpike (1900–1979), and Harold Schuknecht (1917–1996). The final section brings the reader up to the present time, describing the contributions of recent investigators and our current approach to treating BPPV.

The ophthalmologist has a large part to play in the management of many rheumatological diseases. These diseases can cause a number of symptoms from mild ocular discomfort to sudden blindness. In addition, many rheumatological diseases have helpful ophthalmic signs, which can aid diagnosis. This chapter has been written to help rheumatologists identify these signs and symptoms. We have started by summarizing the common pathology found in patients with rheumatological diseases (dry eye syndromes, conjunctivitis, episcleritis, scleritis, uveitis, and optic neuropathy). This has been arranged working backwards from the front of the eye towards the retina and optic nerve. The rheumatological conditions that give rise to ophthalmic signs (giant cell arteritis, systemic lupus erythematosus, polyarteritis nodosa, Wegner's granulomatosis, systemic sclerosis, rheumatoid arthritis, seronegative arthropathies, sarcoidosis, and Behçet’s disease) have then been summarized, including a section of paediatric conditions (juvenile idiopathic arthritis, spondyloarthropathies, and multisystemic illness). Finally, treatment regimes and recent guidelines have been covered for the screening of uveitis in juvenile idiopathic arthritis and the management of patients taking hydroxychloroquine. We hope that both rheumatologists in training and consultants find this chapter a useful clinical aid, and that it encourages them to look closely for subtle signs that will help improve the management of their patients.
