Authors tried to pay special attention to disclosure of specifics of psychophysiology of the person, to difference of physiological mechanisms of regulation of mental functions of the person in comparison with model researches on animals. Prolonged difficulty identifying and regulating emotions is another essential symptom of PTSD, and has been associated with hormonal dysregulation, social and academic difficulties, and structural and functional brain deficits in youth and adults. Individual subject variance in personality, disposition, sex, and genotype has been shown to uniquely modulate the prefrontal and limbic brain regions associated with emotion processing. The current chapter examines how the component processes of emotion regulation, such as fear conditioning, can be dysregulated by the experience of traumatic stress, by which the brain centers that manage reactions to emotionally charged stimuli are over- or underactivated. The preclinical literature that serves as the basis for our understanding of these systems is reviewed, as well as studies of adults and children who have experienced trauma. Future directions, such as clinical care based on neuroendocrine research, are also discussed.

Selective attention is essential for all aspects of cognition. Using the paradigmatic case of visual spatial attention, we present a theoretical account proposing the flexible control of attention through coordinated activity across a large-scale network of brain areas. It reviews evidence supporting top-down control of visual spatial attention by a distributed network, and describes principles emerging from a network approach. Stepping beyond the paradigm of visual spatial attention, we consider attentional control mechanisms more broadly. The chapter suggests that top-down biasing mechanisms originate from multiple sources and can be of several types, carrying information about receptive-field properties such as spatial locations or features of items; but also carrying information about properties that are not easily mapped onto receptive fields, such as the meanings or timings of items. The chapter considers how selective biases can operate on multiple slates of information processing, not restricted to the immediate sensory-motor stream, but also operating within internalized, short-term and long-term memory representations. Selective attention appears to be a general property of information processing systems rather than an independent domain within our cognitive make-up.

Developmental age and symptom variations influence treatment needs for trauma- and stressor-related disorders (TSRD). TSRD include disorders found in children age 6 and under (reactive attachment disorder, disinhibited social engagement disorder, post-traumatic stress disorder [PTSD] < 6) and those described for individuals who are older than age 6 (PTSD, PTSD with dissociative symptoms, acute stress disorder, adjustment reactions, and other specific TSRD, e.g., complicated grief). Treatments for children under age 6 primarily focus on caregiver–child dyads. Post-trauma symptoms such as those described for PTSD with dissociative symptoms, complicated grief, and complicated trauma require alterations in proven trauma-focused methods. In addition to appropriately timed processing of the trauma, treatments for youths are best when they are multifaceted (also include, for example, focus on support systems and relationships; self-skills, e.g., regulation, coping; and other age, symptom, and trait-related factors). For children, treatment methods often include creative methods as well (e.g., drawings, storytelling).

Complementary therapies are modalities that are not traditionally part of Western medical care. Some of these therapies have demonstrated a favourable benefit:risk ratio in recent research and many can be incorporated into a multimodality pain management plan. In general, complementary therapies reduce pain by interfering with the processing of pain signals or lessen the impact of pain on the patient’s emotional state. Mind-body therapies, such as hypnosis, meditation, yoga/qigong, and music therapy, can reduce anxiety, depression, and stress-all common in patients experiencing pain. Acupuncture appears to have direct analgesic effects and reduce nausea and vomiting, which are potential side effects from opioid therapy. Massage therapy may reduce anxiety, and to a lesser degree, depression and pain. Complementary therapies are generally safe when provided by trained practitioners, although certain safety precautions still need to be exercised. The origin of pain, the factors complicating it, burdens and risks to patient, and each patient’s belief system and cultural background should all be considered when selecting from among the complementary modalities for pain.

Furthermore, brutality and atrocity are recurrent characteristics of any culture, often aggravated in situations of historical stress independent of religious systems.

Social structures can be fruitfully studied as networks of social relationships. These should not be conceptualized, and examined, as stable, acultural patterns of ties. Building on relational sociology around Harrison White, the book examines the interplay of social networks and meaning. Social relationships consist of dynamic bundles of expectations about the behavior between particular actors. These expectations come out of the process of communication, and they make for the regularity and predictability of communication, reducing its inherent uncertainty. Like all social structures, relationships and networks are made of expectations that guide social processes, but that continuously change as the result of these processes. Building on Niklas Luhmann, the events in networks can fruitfully be conceptualized as communication, the processing of meaning between actors (rather than emanating from them). Communication draws on a variety of cultural forms to define and negotiate the relationships between actors: relationship frames like “love” and “friendship” prescribe the kinds of interaction appropriate for types of tie; social categories like ethnicity and gender guide the interaction within and between categories of actors; and collective and corporate actors form on the basis of cultural models like “company,” “bureaucracy,” “street gang,” or “social movement.” Such cultural models are diffused in systems of education and in the mass media, but they also institutionalize in communication, with existing patterns of interaction and relationships serving as models for others. Social groups are semi-institutionalized social patterns, with a strong social boundary separating their members from the social environment.
