ABSTRACT Introduction: Studies of sedentary behavior in the workplace, where conditions represent a health risk, mainly involving levels of physical activity and health of police officers, have increased in several countries around the world. Objective The objective of this study was to compare the physical activity levels, sedentary behavior and health risks of military police officers involved in overt and specialized patrolling. Methods: A total of 146 military police officers involved in overt and specialized patrolling were assessed, and anthropometric data were collected for body mass - kg; height - m; BMI - kg/ m²; waist-hip ratio (WHR) and fat percentage. Physical activity and inactivity levels were determined using the International Physical Activity Questionnaire (IPAQ), short version 8, with statistical analysis (t-Test, Mann-Whitney U test and Chi-squared test, p≤0.05). Results: The mean time of physical activity – overt patrolling (108.33 ± 92.60 min/week) and specialized patrolling (137.11 ± 90.30 min/week) totaled an overall mean of 122.72 ± 91.94 min/week, p≤0.05; and time seated – overt patrolling (391.27 ± 192.90 min/week) had 30.1% of insufficiently active participants, while specialized patrolling was 319.41 ± 123.10 min/week with 17.1% of insufficiently active participants, with a total mean of 312.00 ± 112.30 min/week and 47.3% of insufficiently active participants. Active police officers are one-third less likely to develop health risk than inactive police officers (OR = 0.3, CI = 0.18-0.67). Overt patrol police officers are 3.6 more likely to develop health risk than specialized patrol police officers due to sedentary behavior (OR = 3.6, CI = 1.48-8.75). Conclusion: Both groups of military police officers have lower than recommended physical activity levels and spend most of their time seated. It was also noted that the variables of overt patrol police officers have indicators that are more detrimental to health than specialized patrol officers, yet both occupational groups should be instructed on how to adopt active and healthy lifestyles. Level of evidence II; Prognostic studies-Investigation of the effect of patient characteristics on disease outcome.

Aim. To evaluate the diagnostic approaches to viral hepatitis B and immune response on hepatitis B vaccination among clinical dispensary of dermatology and sexually transmitted diseases medical staff. Methods. The study included 92 serum samples of healthcare facility staff who underwent hepatitis B vaccination more than 7 years before. To assess the post-vaccination immunity, levels of antibodies to surface antigen of hepatitis B (anti-HBS) were determined by ELISA. Published data on the duration of post-vaccination immunity and annual reporting forms of clinical dispensary of dermatology and sexually transmitted diseases of Kazan in 2013 were analyzed. Results. Patients with human immunodeficiency virus, viral hepatitis B and C are often seeking for medical aid, retaining the risk for medical staff and other patients to be infected. Diagnosis of hepatitis B virus in medical settings is based on the detection of hepatitis B surface antigen (HBSAg), while revealing hepatitis B core antigen (HBCAg), anti-HBC (summary levels), extra-Dane particle antigen (HBEAg) and other markers of viral hepatitis B would allow to diagnose a latent form of hepatitis B. In 56.5% (n=92) of cases, anti-HBs levels were below the protective titer, suggesting higher risk for infection and requiring an additional revaccination. In the remaining 43.5% of cases, protective antibodies titers were revealed, confirming the effectiveness of specific prevention measures. Conclusion. Despite the mass vaccination for hepatitis B, owing to post-vaccination immunity weakening over time, revaccination of medical staff remains important.

In Britain, antenatal care is performed by specialist obstetricians, general practitioners (GPs), and midwives, often in combination. Because most women are delivered in specialist hospitals, obstetricians are involved in booking for place of delivery, and usually in delivery of specialized investigations such as ultrasonic scanning. General practitioners may independently book women for confinement under their care in Cottage Hospitals or (rarely) at home, and may provide all the medical care for those women, in addition to sharing in the antenatal care of women to be delivered at specialist units, and in fact doing most of the care for these women, unless they are very high risk or complicated.

Access to specialized care is essential for people with Parkinson´s disease (PD). Given the growing number of people with PD and the lack of general practitioners and neurologists, particularly in rural areas in Germany, specialized PD staff (PDS), such as PD nurse specialists and Parkinson Assistants (PASS), will play an increasingly important role in the care of people with PD over the coming years. PDS have several tasks, such as having a role as an educator or adviser for other health professionals or an advocate for people with PD to represent and justify their needs. PD nurse specialists have been established for a long time in the Netherlands, England, the USA, and Scandinavia. In contrast, in Germany, distinct PDS models and projects have been established. However, these projects and models show substantial heterogeneity in terms of access requirements, education, theoretical and practical skills, principal workplace (inpatient vs. outpatient), and reimbursement. This review provides an overview of the existing forms and regional models for PDS in Germany. PDS reimbursement concepts must be established that will foster an implementation throughout Germany. Additionally, development of professional roles in nursing and more specialized care in Germany is needed.

The chronic condition of some pathology may lead the patient to a critical condition or even to an end-stage, putting the multidisciplinary group in an ethical conflict; as well as the family. Considering that the objective of healthcare staff has always been the well-being through prevention and correction of the clinical condition, avoiding, at all times, to relief pain and suffer, now having to address the therapeutic help to a “good death” of the patient. The cultural conditions have changed and death is not perceived the same way it used to be, nor by the society nor the group providing attention and care to the critical or end-stage patient, generating expectations for each case, separating the patient from reason and reality (1). Today, science has had a technological advance having a direct impact on the vital function of the patient, having a direct influence on the time, but mainly on the way of death, focusing the attention on the possible decisions of the seriously sick patient, starting the era of an “assisted death”, as opposed to a natural death (1,2).

At many universities, students who get enrolled at faculties of engineering are required to study a preparatory year before they begin their specialized academic programs. This year includes several general engineering and basic science modules. It is claimed that this year gives students the required time and knowledge to decide on which engineering department they would select. This study aims to investigate this issue from students’ and academics’ points of view. Islamic University of Gaza (IUG) has been taken as a case study. The study carried out a survey that includes a questionnaire directed to the students and a structured interview directed to the academic staff. Results showed that the majority of students do not think they need an entire academic year to get a clear perception for the different engineering specialities. They think it would be better if they get specialized directly upon their entry to the university. This is also the recommendation of the academic staff. Despite the benefits claimed for the preparatory year, greater ones could be achieved if students are given the chance to get specialized earlier. This includes giving engineering departments more flexibility in the selection of the first year modules

A randomized controlled trial evaluated the impact of different methods of water agitation on clinical and microbiological outcomes in 31 nursing home residents. Four conditions were tested: a) whirlpool tub, jets on, using standard soap products; b) ultrasound tub, ultrasound on, using the standard soap products; c) ultrasound tub, ultrasound on, using specialized soap and skin conditioner; and d) either tub (randomized), water circulation off, using standard soap products (the control condition). Outcomes of interest included skin microbial flora, water microbial flora, skin condition, time spent bathing, and staff satisfaction. Resident skin condition and skin microbial flora did not differ between the four treatments. The tubs also did not differ in terms of bacterial colonization; however, there was a non-statistically significant trend for the highest counts to occur in whirlpool tubs after being idle overnight. The ultrasound and whirlpool tubs were preferred by staff over the control treatment (still water) in terms of sound and overall suitability. In addition, staff reported that the ultrasound tub using enhanced skin cleansers made bathing residents easier and faster than the same tub using standard cleansers.

ABSTRACTBackground: Attitudes of residential care staff toward residents with dementia affect the quality of care. We examined the attitude of frontline residential care staff toward residents with dementia, and how the presence of specialized care units or programs may affect staff attitude. Methods: Staff working in nursing homes participated in a survey which covered demographic data, current state of dementia care in workplace, opinion regarding dementia care, and perceived importance of dementia behaviors. Results: 1,047 nurses and personal care workers participated. 78.8% respondents reported difficulties in managing dementia residents. Those who ranked positive symptoms as more important were 4.5 times more likely to report difficulties, independent of experience. Independent factors associated with positive attitudes toward further training were working in a non-profit home (OR 2.4, 95% CI 1.1, 5.0; p = 0.024) and having a dementia program or unit in the current workplace (OR 1.8, 95% CI 0.985, 3.302; p = 0.056). Only having a dementia program or unit in the current workplace was associated with a positive attitude toward commitment to stay in dementia care (OR 1.9, 95% CI 1.1, 3.2; p = 0.021), adjusted for gender, type of home, post, dementia prevalence in workplace, and work experience. Conclusion: The majority of long-term care staff felt dementia care difficult yet hold positive attitude toward further training and were committed to stay in dementia care. Having a specialized dementia care unit or program in the current workplace was associated with commitment to stay in dementia care and was marginally associated with positive attitude toward further training.

This study aims to explore the status quo of information security from the perspective of information technology (IT) staff in Jordanian university libraries and to discuss the most prominent difficulties they face. Moreover, it aims to identify the effect of the variables (years of experience, type of university, job level, and specialization) on the staff estimation of the status quo of information security and the difficulties they face. The study population consisted of all (96) staff members of the IT departments in the libraries of public and private Jordanian universities for the academic year 2015-2016, of whom (84) responded. To answer the study questions, a questionnaire that examines the status quo of information security was adopted, it consisted of five themes. The questionnaire also included questions to study the difficulties facing IT staff. The results indicated that the respondents’ estimation of the status quo of information security in Jordanian universities libraries was at medium level, as the item “procedures for the protection of computer systems and networks” and the item “access control to information systems” were ranked first in the study fields with a high rating. Results also indicated that the overall level of difficulties attained a medium level, and that the most prominent difficulties facing IT staff in universities libraries are the shortage of personnel specialized in information security and the lack of budget allocated to information security in university libraries. The study also showed a statistically significant difference at the level of significance (α≤0.05) for the variables of “job level” and “specialization” in the staff estimation of the status quo of information security. Also, a statistically significant difference was found in the staff estimation of the difficulties they face related to the specialization variable. The study recommends paying more attention to the various aspects of information security in libraries, such as the infrastructure in regard to technology and the existence of a precise policy to ensure information security and to increase the number of human resources specialized in the field of information security.

Objective: A group of 47 young adults suffering from schizophrenia was interviewed to garner their views on their needs for care. Method: Three members of a specialized multidisciplinary rehabilitation team, who had been caring for these patients, on average, for the past 4 years, completed a questionnaire to assess the needs for care of these individuals. Patient and staff assessments were then compared. Results: Patients and staff do not share similar views on the presence of clinical and social problems. Further analyses of the perceived importance of living-skills deficits, the perceived difficulties in dealing with these, and the recent developments in rehabilitation practices challenge whether patient-staff consensus is indeed essential for rehabilitation. Conclusion: We propose that staff should listen to patients' points of view more carefully, especially in the areas of work, studies, and independent living.

Globalization has influenced the identity of universities worldwide to such an extent that they have become servants for economic growth by producing scientific knowledge and highly educated graduates. In a South African (SA) dispensation, the transformation of higher education (HE) focused on restructuring the system to meet the needs of a technology-orientated economy. The latter resulted in more specialized and complex university work that was reliant on technological devices for its completion. It is on this premise that the author investigated the extent to which technological devices as a ‘job resource’ kept SA University staff constantly connected to their work life. This study was subject to a mixed methods research design whereby three data collection instruments were used to obtain both quantitative and qualitative data, which were combined via triangulation of factor analysis and theme identification. Purposive sampling included both academic and non-academic staff from a selected SA university. Stemming from the results, it became apparent that problematic merger issues led to inadequate workspaces, job resources and IT network. Consequently, staff were experiencing occupational stressors related to work overload and inadequate job resources, which perpetuated staffs’ work-life transcending into their personal life, as they were constantly plugged into their technological devices to complete job tasks.

Purpose Policy makers are applying market-inspired competition and financial incentives to drive efficiency in healthcare. However, a lack of knowledge exists about the process whereby incentives are filtered through organizations to influence staff motivation, and the key role of managers is often overlooked. The purpose of this paper is to explore the strategies managers use as intermediaries between financial incentives and the individual motivation of staff. The authors use empirical data from a local case in Swedish specialized care. Design/methodology/approach The authors conducted an exploratory qualitative case study of a patient-choice reform, including financial incentives, in specialized orthopedics in Sweden. In total, 17 interviews were conducted with professionals in managerial positions, representing six healthcare providers. A hypo-deductive, thematic approach was used to analyze the data. Findings The results show that managers applied alignment strategies to make the incentive model motivating for staff. The managers’ strategies are characterized by attempts to align external rewards with professional values based on their contextual and practical knowledge. Managers occasionally overruled the financial logic of the model to safeguard patient needs and expressed an interest in having a closer dialogue with policy makers about improvements. Originality/value Externally imposed incentives do not automatically motivate healthcare staff. Managers in healthcare play key roles as intermediaries by aligning external rewards with professional values. Managers’ multiple perspectives on healthcare practices and professional culture can also be utilized to improve policy and as a source of knowledge in partnership with policy makers.

ABSTRACTBackground: Persons with dementia hospitalized for an acute illness have a high risk of poor outcomes and add to the burden on acute care systems. We developed a segregated Special Care Unit (SCU) in a somatic hospital for patients with challenging behavior resulting from dementia and/or delirium. This pilot study evaluates the feasibility and patient outcomes. Methods: The SCU was established with environmental features that allow for safe and unrestricted ambulation within the unit and create a home-like atmosphere. Daytime activities structure the day and assure additional professional presence. The staff received intensive specialized training. Feasibility criteria were: acceptance by the staff, avoidance of transfers to geriatric psychiatry, lack of serious falls and mortality. Patient outcome criteria were ADL (Barthel index), mobility scores and behavior scores (Wilcoxon's, McNemar tests, pre-post design). Results: 332 consecutively admitted patients were enrolled. The SCU has been well received by the staff. Length of hospital stay did not differ from other hospital patients (15.3 ± 8.3 vs. 15.0 ± 10.3 days, p = 0.54). Six patients were transferred to geriatric psychiatry. Two patients suffered a fall-related hip fracture. The median Barthel Index improved significantly (admission 30, discharge 45, p < 0.001), with only 8.5% of patients suffering functional loss. Wandering, aggression and agitation were significantly reduced (p < 0.001).Conclusions: The SCU has improved the care of patients with challenging behavior. Decline in ADL function and institutionalization occurred to a lesser degree than would be expected in this group of patients. Despite the selection of patients with behavioral problems, transfer to psychiatry was rare.
