AbstractIntroduction:The pressures facing emergency medical services (EMS) in Wisconsin and their effects on the delivery of prehospital emergency medical care were not known by the Wisconsin EMS Board. In an effort to assess these pressures and the needs of the emergency medical services in the State as perceived by the services, the Board undertook a survey of the EMS providers in Wisconsin.Methods:A survey instrument was developed and approved by the EMS Board and distributed to all of the licensed emergency medical services in Wisconsin.Results:Of the 453 survey instruments distributed, 323 (71.3%) were completed and returned. Intermediate- and paramedic-level services were more likely to respond than were the basic services, but 235 (72.8%) of the respondents identified their service levels as basic. In addition to providing information about the service characteristics, each responding service also rated the importance of their perceived needs. Lack of medical direction was perceived as the greatest need by all levels of service. However, the second greatest area of need for basic and intermediate services related to difficulty in recruiting new staff. For paramedic services, the second greastest need was associated with dispatching. When comparing services by rural versus urban, difficulty in recruiting new staff and collecting ambulance fees were seen as second and third to lack of a medical director by rural services, whereas urban services noted local training to be in the top three. In the assessment of educational needs, patient-care issues dominated. A review of written comments also demonstrated a difference between rural and urban services, but both noted Medicare and Medicaid reimbursement issues more commonly than any other problem.Conclusion:In the restructuring of health care, it will be important to consider the various needs of prehospital providers and recognize that such needs may be unique to the providers' location and level of service.

Deinstitutionalization in the 1960s shifted the care of the mentally ill from state-funded institutions to community settings. Unfortunately, funding to support the treatment needs of this population has continued to be minimized, and countless individuals have not received much needed care. This has resulted in a large increase in mentally ill patients surfacing in jails, homeless shelters, and emergency departments. Subsequently, the federal government has begun to prioritize funding to address the mental health needs of our communities. Pharmacists are in a unique position to influence treatment outcomes for patients with mental illnesses, and the current state of health reform provides avenues for the pharmacist to become an essential part of the health care delivery team in numerous ambulatory care clinical settings. The challenges with adherence, medical and psychiatric comorbidities, polypharmacy with psychotropics, potential for life-threatening adverse effects, and the ever-present need for patient education are but a few reasons why pharmacists can be utilized to provide Medication Therapy Management services for these patients.

Purpose Services for children with complex communication needs require well-trained practitioners. Yet, research shows that augmentative and alternative communication (AAC) practitioners often receive insufficient preservice training for direct services to these children. Therefore, we explored the preservice and in-service training experiences of AAC practitioners to inform improvements in practitioner training and ultimately improve services for individuals with complex communication needs. Method A focus group study was conducted with AAC practitioners (three focus groups with 13 AAC practitioners in total) in one midwestern U.S. state to explore preservice and in-service training experiences and gain knowledge to inform future training efforts. Constant comparative analysis was used to analyze data gathered from the focus groups. Results This study highlighted important gaps in preservice and in-service training for AAC practitioners, barriers related to AAC training, and suggestions for improving AAC training. Conclusions This study provides important insights into ways AAC practitioner training might improve. We provide clinical implications and future research directions to continue improving AAC practitioner preparation and subsequent services for individuals with complex communication needs.

AbstractObjectives: To determine the extent of mental health services provided in the community in one Irish health board area. To examine the influence of postgraduate mental health training of GPs on provision of mental health services.Method: Questionnaire and focus group methods were employed to determine views on mental health service provision. Data analysis was with parametric and non-parametric tests of association including student's t and chi-squared tests. Thematic analysis of the focus groups was carried out.Results: Twenty-five per cent of patients attending general practice have mental health problems and over 95% of these problems are dealt with in primary care. Only 32% of GPs had received postgraduate training in psychological therapies. GPs with postgraduate training in psychological therapies were more likely to estimate a higher proportion of their patient population with mental health problems and less likely to refer to psychiatric services. A need for support from other health care professionals in primary care was also identified.Conclusion: The majority of patients with mental health problems are treated in primary care. Further training of GPs and increased resources would improve mental health care in primary care and lead to fewer referrals to psychiatric services.

The article analyzes the experience of leading countries in implementing the service policy of the state (Great Britain, France, Germany, Malaysia, etc.) in order to identify effective practices for reforming state policy in the context of its service orientation. The Charter of the Citizen (UK) was adopted in 1991 and was a 10-year improvement program of the public services quality. The main principles of the Charter (standards; openness; choice; politeness and courtesy; providing services properly; efficiency and low cost) were observed. Moreover, the «Charter on the delivery of public services» (France, 1992) is researched. It presented basic principles of work regulation of French civil service on the basis of equality, impartiality and continuity, as well as new principles of activities, as a reaction to changes in expectations and demands of the population, the emergence of consumers’ new needs and requests, the need for transparency and accountability, simplicity and accessibility.It is proved that the Services Consumer Charter for Public Services in Belgium (1993) is an example of a comprehensive program that seeks to improve the relationship between state bodies and institutions on the one hand and the population - customers and consumers (clients) of services - on the other one.Analysis of mantioned above and other charters helps to detect the main tasks of the public service systems reforms, in particular: increasing the efficiency of public services and reducing the cost for their delivery; deliivery of services to citizens of the proper quality; reduced deadlines for preparing responses to requests of citizens; conducting external audits to provide creation of a system aimed at continuous improvement; emphasis on achieving results, but their planning; better definition of work processes; improving the quality of informing citizens about public services delivery; openness and transparency of authorities activities; liberalization of licensing institutions; implementation of the «e-government».A characteristic feature of reforming the system of public services delivery in the countries taken under consideration is detected. This is a duration of this process. The explanation of the long terms of the reforms in the sphere increasing the efficiency of the public sector lies in the complexity and scope of the set goals and objectives, and also in a large number of parties involved in the reform process. The further research of the problematic can be dealt with analysis of the state, quality, and special features of the domestic reforms dedicated to the state service policy implementation.

There are few studies exploring the need to develop and manage culturally competent health services for refugees and migrants from diverse backgrounds. Using data from 50 interviews with service providers from 26 agencies, and focus group discussion with nine different ethnic groups, this paper examines how the Victorian state government funding and service agreements negatively impact on the quest to achieve cultural competence. The study found that service providers have adopted ?one approach fits all? models of service delivery. The pressure and competition for resources to address culturally and linguistically diverse communities? needs allows little opportunity for partnership and collaboration between providers, leading to insufficient sharing of information and duplication of services, poor referrals, incomplete assessment of needs, poor compliance with medical treatment, underutilisation of available services and poor continuity of care. This paper outlines a model for cultural consultation and developing needs-led rather than serviceled programs.

Despite growing awareness of crime victims’ needs for support and services, few studies have examined the training needs of those who provide such services. This research presents the results of statewide needs assessment of victims services providers (VSPs) conducted in preparation for designing a state victim assistance training academy. Respondents were asked to indicate their needs for training on various topics pertaining to victims services. Results indicate that extent of formal education and years of experience in the field are the primary determinants of reported needs for training. Respondents with less formal education and less experience in the field, regardless of the nature of their organizational position (client services vs. management), report greater needs for training. Implications for designing training programs for VSPs are discussed.

International students at our institutions of higher education have educational and personal needs which extend beyond the classroom. These needs are supposedly provided for by student personnel services, but little data has been collected which examines how international students use student services, and if their use differs significantly from use by American students. If their perceptions of students services and use of these resources does differ, what does that signify? This study measures the use and perceptions of student services by international and American students at Southern Oregon State College. Twenty-one philosophical objectives were examined for levels of importance, effectiveness, and resultant need. A list of twenty resources at the College were compiled and respondents were asked to rate the helpfulness of each resource. Demographic data on students included residence, length of time on campus, involvement in extracurricular activities, and other characteristics and was compared to knowledge of student services resources. International students were asked to identify their English language proficiency and this was compared with their need for student services objectives. All three hypotheses that there would be no differences between American and international students; in their perceptions cf the;:, importance, effectiveness, and helpfulness of student services were rejected at the . 05 level of significance. International students found three objectives to be ineffective, and the focus of these objectives suggested that cultural differences may contribute to student problems. A correlation between self-reported English language proficiency and satisfaction of needs for student services objectives was noted, while no relationship between TOEFL score and satisfaction of needs was found. Southern Oregon State College may want to reconsider its orientation procedures in light of the number of "don't know'' responses from students when asked about a variety of student services. Awareness of student services and the effectiveness of those services for both American and international students are issues which this study aims to help the college address.

The educational needs of Asian Pacific American students in higher education frequently are not addressed because of being viewed as a "model minority." Thus, student services and academic support programs are oftentimes not appropriately provided for Hmong students. This study was conducted at California State University, Sacramento to determine the student service and academic support needs of currently enrolled Hmong students, whether or not the services were available, and the factors which contributed to their academic success or failure. The qualitative case study method was utilized so that an in-depth understanding of the situation could be determined. A purposive or criterion-based sampling method was used to identify twenty-two currently enrolled Hmong students. Personal interviews were conducted using an interview guide (Appendix B). The data obtained through the taped interviews were transcribed and the Microsoft Excel computerized statistical software was used. The information was retrieved for an in-depth analysis for text interpretation and theory building. The University provides a comprehensive range of student services and academic support programs, however, only approximately one-half of the services were known to the students. The programs or services which focused on Hmong students, and provided financial, advising and tutoring services were viewed as most beneficial. A majority (50 percent) of the students felt uncomfortable, unaccepted, detached, and not part of the campus community. A higher percentage of males (60 percent) expressed these feelings, and were more critical and less positive than the females. A significant number of students stated that their English as a second language and math needs were being met. Overall, it appears that the students have performed academically at an acceptable level. The Hmong University Student Association provided a culturally and academically supportive environment. An additional key service the University could provide is a more effective method of disseminating information on availability of services. Recommendations for future research include: determine whether or not peer support and study groups enable ethnic students, particularly Hmong students, to persist and do better academically; and reasons for Hmong students leaving the University.

The researcher used observations and interviews in the research design. The information was divided into common themes. The themes were demographics, reasons for returning to higher education, concerns, classroom procedures, advising procedures, and campus resources. The conclusion includes a discussion on adult learner needs on-campus and how to address them. There is also a discussion on the responsibilities of the institution.. Included in the thesis are recommendations for future research.

ENGLISH ABSTRACT: Continuing Professional Education (CPE) is a method by means of which radiographers and other health care professionals can systematically maintain, improve and broaden their knowledge and skills. Chances are that mandatory CPE will be introduced for South African radiographers by the year 200t. For this reason it was decided to undertake a needs assessment survey of Free State Province diagnostic radiographers. The aim of this study wasto assess the perceived needs of Free State Province radiographers. In this way adults, as consumers of CPE, were involved in the planning process. Results obtained from the needs assessment provide valuable information to providers of CPE activities. With the aid of the results, providers can plan activities responsibly, which will satisfy their target population. The needs assessment was done by means of a maH-administered questionnaire-, and a response rate of 58% was obtained. Data analyses showed that most of the radiographers were employed in small x-ray departments and, due to this fact, experienced constraints regarding CPE participation. Constraints such as staff shortages and a lack of backup staff were identified. New developments-in radiography, management skills, computer skills and UUrasound were tbe topic areas indicated by the highest percentage of respondents as high level of need areas. Flexible learning strategies- with adequate support mechanisms must be developed. In order to provide high-quality CPE activities, all stakeholders, such as the Technikon Free State, the Society of Radiographers, the employers- and the radiographers, must share the- respon-sibHity of CPE. The study showed that close co-operation between stakeholders is essential to the success of CPE in the Free- State Province,
AFRIKAANSE OPSOMMING: Voortgesette Professionele Onderwys (VPO) is 'n strategie wat radiograwe en ánder protessionele gesondheidsorgwerkers in staat stelom hulle kennis en vaardighede sistematies in stand te hou, te verbeter en uit te brei. Die-moonUikheid Destaan dat Suid-Afrikaanse radiograwe teen die jaar 2001 verplig sal word om aan VPO deel te neem. Om hierdie rede is daar besluit om 'n behoeftebepalingsopname van die- diagnastiese radiograwe in die Vrystaat Provinsie te doen. Die doel van hierdie studie was om die behoeftes van die Vrystaat Provinsie se radiograwe-, soos deur hulle self geïdentifiseer, te bepaal. Op hierdie manier kon radiograwe, as verbruikers van VPO, by die beplanningsproses betrek word. Die- resultate- van die behoeftebepalingsopname wat bekom is, bevat waardevolle inligting vir verskaffers van VPO-aktiwiteite. Met behulp van die resultate sal verskaffers van VPO-aktfwiteite wat hulle-teikengroep se- behoeftes sal bevredig, met verantwoordelikheid kan beplan. Die behoeftebepaling is met behulp van pos- vraelyste gedoen. 'n Respons van 58% is verkry. Data-ontleding het getoon dat die meeste van die radiograwe werksaam was in klein x-straaldepartemente wat gevolglik bygedra het dat hulle beperkinge met betrekking tot VPObetrokkenheid ervaar het. Ander struikelblokke soos personeeltekorte en 'n gebrek aan aflospersoneel is geïdentifiseer. Nuwe ontwikkelinge- in radiografie, bestuursvaardighede, rekenaarvaardighede en Ultraklank is deur die grootste persentasie respondente as die areas waarin die grootste behoefte bestaan, geïdentifiseer. Buigsame leerstrategieë met toereikende ondersteuningsmeganismes moet ontwikkel word. Om hoë gehalte VPO-aktiwiteite te verskaf moet alle- belangegroepe, onder andere- die Technikon Vrystaat, die Vereniging van Radiograwe, die werkgewers en die radiograwe self gesamentlik die verantwoordelikheid vir VPO deel. Die- studie- toon dat noue samewerking tussen belangegroepe noodsaaklik is vir die sukses van VPO in die Vrystaat Provinsie.

Abstract Portugal is a traditional country of emigration with a multigenerational diaspora spread across a large number of countries. In the last 50 years, especially after the 1974 revolution, it developed a welfare state that responds to the needs of residents (including immigrants). Traditionally, this welfare state has been described as fragile in comparison with other welfare regimes in Europe. Nevertheless, it was built as a universal welfare system based on jus solis and deterritorialized jus sanguinis regime. The study of the extension of social protection to Portuguese citizens living abroad had not yet received sufficient attention, albeit recurrent news on measures and strategies that the state put in practice to assist Portuguese emigrants in need. This chapter will survey some of the recent policies to reach citizens abroad by showing the limits of the Portuguese welfare state in action. Through these policies and actions, Portugal projects an image of an always-present state that extents its national powers beyond the limits of its territorially confined borders.

Abstract The policies of the Greek state vis-à-vis Greek citizens residing abroad are better developed in some areas (pension, cultural/education policy), but very embryonic in others (social protection, family-related benefits). The institutions representing and aggregating the interests of the Greek diaspora, such as the General Secretariat for Greeks Abroad and the World Council of Hellenes abroad of the Ministry of Foreign Affairs, reflect earlier periods of Greek migration during the post-war period, but meet less adequately the needs of recent migrants, especially following the post-2010 Greek economic crisis. At the same time, political parties continue to play an active role in the relationship between diaspora and the homeland. The policies of the Greek state, especially when exercised informally or with regard to cultural and educational programs, are also characterized by an emphasis on blood, language and religious ties, and are offshoots of a long-standing history of migration to Western Europe, North America and Australia. Possible developments, such as the long-overdue implementation of the right to vote from abroad, an official registrar for Greek citizens residing abroad, new programs of social protection in Greece and new economic incentives for return might change the diaspora policies of the Greek state in the next decades.

AbstractWith 10% of Tunisians living abroad, the Tunisian diaspora is a subject of national concern. How can the Tunisian state ensure their access to social protection abroad? This chapter will present the general institutional framework by which Tunisian authorities interact with their nationals abroad, as well as the main policies for engaging with this population. The chapter will provide an overview of the Tunisian diaspora’s main characteristics, its relations with the homeland and the policies, programs and services offered by Tunisian authorities to respond to the transnational social protection needs of Tunisian nationals abroad. By highlighting its diaspora infrastructure, key policy issues and, in particular, its social protection policies for citizens abroad, the chapter seeks to analyse the realities and deficiencies of Tunisia’s diaspora policies.

Abstract Romania is the EU Member State with the highest numbers of emigrants, according to Eurostat. The annual growth of the Romanian diaspora is one of the fastest in the world, and quite recent (over the past 20 years). In light of these developments, the institutional network for engagement with Romanians abroad, first established in the mid-1990s, has recently increased in an attempt to respond to the fast-growing Romanian diaspora. A Ministry entirely dedicated to maintaining the relations with the Romanian diaspora was formally institutionalized 10 years after Romania officially joined the EU in 2017, replacing scattered departments and institutional bodies. However, the role of the recently set up diaspora institutions still needs to be clarified and firmly determined. Policies were developed with the goal of ensuring the integration of Romanian citizens in their countries of residence, but also to encourage return to Romania. In particular, as few other European diaspora populations, the Romanian diaspora is represented in the Romanian Parliament by two Senators and four Deputies who represent the interests of three to five million Romanians abroad.

Concerns about government snooping in the wake of revelations by whistle blower Edward Snowden have deterred enterprises and IT professionals from keeping sensitive data in the clouds. Moving towards cloud-based computing has emerged and has gained acceptance as a solution to the tasks related to the processing of information. However, cloud computing carries serious risks to business information. The questions around risk and compliance are still largely unknown and need to be ironed out. Cloud computing opens numerous legal, privacy, and security implications, such as copyright, data loss, destruction of data, identity theft, third-party contractual limitations, e-discovery, risk/insurance allocation, and jurisdictional issues. This chapter discusses the associated legal risks inherent in cloud computing, in particular the international data transfer between EU and non-EU states.

Millions of migrants, refugees, and asylum seekers live somewhere other than in their regions of origin. More than 244 million people are estimated to be immigrants globally. Worldwide, healthcare systems have to be prepared for this very heterogeneous population, with their different biographies, stress, and resilience factors. Both concepts of health and disease and expectations about treatment depend on cultural background, traditional values, personal experiences, and social worlds, all of which are in a constant state of flux. Barriers to access mental health care must be overcome. To be able to cope with mental health needs among diverse multicultural populations health professionals and service providers need sufficient knowledge and competencies. In this chapter, barriers to access mental health services and the needs of health professionals and mental health services will be identified and discussed.

Content in digital services is often filtered for users based on individual preferences with the possible consequence of creating a state referred to as a “filter bubble”. The objective of this paper is to examine which of a user’s inherent needs that are important to satisfy when a user is consuming personalized content in a digital service. The paper uses a survey to measure the need for autonomy, competence and relatedness of the SelfDetermination Theory when users are consuming filtered content in digital services. The results show that the investigated services fail to satisfy all needs. A satisfactory user experience should include the opportunity for the user to satisfy the need for autonomy, competence and relatedness. For autonomy, transparency of filtering and choice about filtering should be offered. For competence, it is essential to offer content that the user can learn from, and also provide the right amount of choice throughout the service. The danger of filter bubbles is not personalization, but to remove choice about personalization.

The article covers the controversial issue on embracing the client-focused approach with regards to implementing optimisation procedures. The stages highlighted by business consultants are demonstrated and attention is drawn to the lack of a stage for identifying client attitudes towards optimisation measures. The opinion of business consultants is presented: upon the fulfillment of non-standard orders, the profitability of the business decreases. There is a contradiction between the need to optimise business operations to increase profits and the mission of the service enterprise; to satisfy the needs of the target customer group. In order to resolve the contradiction, it is proposed to introduce a customer needs identification phase into the optimisation process to inform the choice of optimisation techniques. The example of a catering company in Ekaterinburg shows the feasibility of selecting optimisation techniques based on an analysis of the reasons why customers refuse services. The following optimisation techniques were chosen: service format modification (the study was run prior to the pandemic outbreak), the optimisation of service assortment and personnel activities. As a result, new varieties of the catering service for the company were selected; menu options for different price ranges and target purposes were proposed; the introduction of additional services was justified. In optimising staff activities, it was decided to abandon hourly pay by linking it to the number of customers served, the complexity of the menu items, master classes, work at an offsite/stationary event, and the number of staff per shift. A new position has been added to the staffing table, allowing the company’s core staff to focus on the quality of their duties. The example illustrates the resolution of the contradiction between the optimisation goal of increasing profits and the principle of customer focus.

State Machine Replication (SMR) is a widely used approach for fault tolerance of important services. Support for SMR implementations on shared infrastructures has emerged, allowing wider adoption. However, there are still non-trivial aspects that developers have to handle to build and deploy their dependable services. In this paper, we tackle the need for recovery to keep fault-tolerance levels, and propose an approach to: (i) simplify the development of logging; (ii) improve resource sharing in shared infrastructures; (iii) alleviate costs with replication in pay-per-use infrastructures. The central idea is to decouple service execution from logging and offer logging functionality as a service attachable to SMR deployments. Beyond the added simplicity to deploy an SMR, we show that this approach does not penalize performance of replicated services, and that a logging service can scale to look to several applications.

The Passenger Rail Investment and Improvement Act of 2008 (PRIIA) was created to reauthorize the National Railroad Passenger Corporation, better known as Amtrak, and strengthen the US passenger rail network by tasking Amtrak, the U.S. Department of Transportation (US DOT), Federal Railroad Administration (FRA), States, and other stakeholders in improving service, operations, and facilities. PRIIA also tasks States with establishing or designating a State rail transportation authority that will develop Statewide rail plans to set policy involving freight and passenger rail transportation within their boundaries, establish priorities and implementation strategies to enhance rail service in the public interest, and serve as the basis for Federal and State rail investments within the State. In order to comply with PRIIA, State rail plans are required to address a broad spectrum of issues, including an inventory of the existing rail transportation system, rail services and facilities within the State. They must also include an explanation of the State’s passenger rail service objectives, an analysis of rail’s transportation, economic, and environmental impacts in the State, and a long-range investment program for current and future freight and passenger infrastructure in the State. The plans are to be coordinated with other State transportation planning programs and clarify long-term service and investment needs and requirements. This paper and presentation will illustrate the steps required in preparing a State rail plan and the benefits of having a properly developed plan.

The government procurement market is a macroeconomic system that includes interacting structural elements within a single system that are an integral part of a country's economic system. The state, as the owner of significant financial resources, acts as the largest subject of market relations in placing budget orders. State procurement (goods, construction works, services) is carried out to meet the most important state needs. Modern market relations have confirmed the need to regulate the process of procurement of goods, services and construction works in country by the state, at the level of an independent institution of executive power/government. In the modern economy, public institutions face the need to regulate the flow of financial resources in order to make procurements for state and social needs, which spending entities must implement efficiently and effectively. In this regard, certain areas of market research are discussed and some conclusions are presented.

Ahmedabad Municipal Corporation (AMC) in the state of Gujarat, India looks after the basic needs of its four million citizens. AMC offers primary, secondary, and tertiary healthcare services at subsidized rates. The load on AMC tertiary healthcare hospitals has been increasing rapidly, stretching the hospital resources beyond limits. Insufficient resources at these hospitals call for increased allocation of AMC funds to provide a satisfactory level of service. However, AMC is unable to allocate a larger share of its finances to the health sector owing to similar demand from other sectors. AMC authorities are therefore concerned about their ability to offer quality healthcare services at subsidized rates. In this paper, we describe a few e-Gov. applications to address and resolve the issues related to costs, revenues, subsidy, and the quality of services, so that AMC can meet its social obligations in the health sector satisfactorily. Some of our recommendations have been already implemented, while others are under consideration.

The key idea of the well-being concept strives to answer the question about how well the needs of people in a society are met in different spheres of social life - the physical, economic, social, educational, environmental, emotional, and spiritual – as well as individuals’ evaluations of their own lives and the way that their society operates (Gilbert, Colley, Roberts, 2016). One of the possible suggestions for answering the question: “How well are the needs of people in a society met?” could be the monitoring of citizen’s satisfaction with public services while applying a standardized questionnaire for population covering 193 primary indicators (health, social security, culture, public transport, utilities, environment, recreation and sport, public communication, education, etc). Even 23 indicators are about education that makes educational services a considerable part of all social service system. As the researchers aimed to analyze satisfaction of rural population with public services stressing the education issue, indicators about education dominated in the survey. The data were collected in 2016 - 2017 in 2 regional municipalities: municipalities: Jonava and Radviliskis (N=2368). The results of the analysis demonstrate that rural residents' satisfaction with formal general education services is relatively high. The only negative exception is the "the placement of a child in a pre-school institution based on the place of residence". Furthermore, rural residents poorly evaluated educational services that are related to non-formal education, adult education, the education of children with disabilities, child safety, meaningful xtracurricular activities of children and young people during all day, preventive programs. These major conclusions let the researchers state that local self-governmental institutions are not capable to cope with the quality challenges of some educational services without special intervention policy of the central government and the EU responsible structural units. A negative impact is also reinforced by a rapidly deteriorating demographic situation in Lithuanian rural areas.

The cancer cervix is the second most common cancer among women worldwide. About 86% of the cases occur in developing countries and this is responsible for 88% of total deaths. In India 132,000 new cases are diagnosed each year with this disease and 74,000 deaths are recorded annually which accounts for the 1/3rd of the global deaths from cervical cancer. This hospital based study is designed to look at the distribution of the disease in patients coming to the Mahavir Cancer Sansthan with the aim to achieve an early diagnosis and treatment and recognition of disease in its preinvasive state for better outcome and quality of life. The patient registry data in Mahavir Cancer Sansthan showed that the total number of patients from all cancer were 20,746 in year 2013-2014. The cervical cancer constituted 14% of the patients. 50% of the patients belong to the six district of Bihar which falls in Gangetic plain. 700 case notes have been reviewed for clinical staging at the time of the diagnosis. 71% of the patients were in stage 2b at the time of first clinical presentation, 24% in stage 3% and 4% were in stage 4. Only 1% patients were found in stage1. The colposcopy clinic data suggest only 0.04% patients have approached to us at preinvasive stage. We conclude from this study that although this hospital is mainly a referral cancer hospital 99% patient have reported to the hospital at stage 2b and beyond. Given the natural History of cervical cancer this is only the tip of iceberg. A robust system for colposcopy services needed to diagnose this disease at its preinvasive and micro invasive stage to reduce the morbidity and mortality and improvement in the quality of the life of the patients.

This rapid literature review explores how to maintain essential state functions and basic service delivery during escalating conflict situations. It draws on literature and ideas from various overlapping agendas including development and humanitarian nexus; development, humanitarian and peacebuilding nexus (the “triple nexus”); fragile states; state-building; conflict sensitivity; resilience; and conflict prevention and early warning. There has been an extensive exploration of these ideas over the past decades: as the international development agenda has increasingly focussed on the needs of fragile and conflict-affected contexts (FCAS); as violent conflicts have become more complex and protracted; as the global share of poverty has become increasingly concentrated in FCAS highlighting the need to combine humanitarian crisis strategies with longer-term development strategies; as threats emanating from FCAS increasingly affect countries beyond those states and regions e.g. through serious and organised crime (SOC) networks, migration, terrorism, etc; and as global trends like climate change and demographic shifts create new stresses, opportunities, and risks.

In 2017, the State of California adopted landmark legislation to increase the funds available for transportation in the state: Senate Bill 1 (SB1), the Road Repair and Accountability Act of 2017. Through a combination of higher gas and diesel motor fuel taxes, SB1 raises revenue for four critical transportation needs in the state: road maintenance and rehabilitation, relief from congestion, improvements to trade corridors, and improving transit and rail services. To help state leaders identify the most important projects and programs to fund within those four topical areas, we conducted an online survey that asked a sample of 3,574 adult Californians their thoughts on how the state can achieve the SB1 objectives. The survey was administered from April to August 2019 with a survey platform and panel of respondents managed by Qualtrics. Quota sampling ensured that the final sample closely reflects California adults in terms of key socio-demographic characteristics and geographic distribution. Key findings included very strong support for improving all transportation modes, reducing air pollution and greenhouse gas emissions from transportation, and more convenient options to travel without driving. Respondents placed particular value on better maintenance for both local streets and roads, as well as highways. Finally, the majority of respondents assessed all types of transportation infrastructure in their communities as somewhat or very good.

As a public agency, interacting with and understanding the public’s perspective regarding agency activities is an important endeavor for the Indiana Department of Transportation (INDOT). Although INDOT conducts a biennial customer satisfaction survey, it is occasionally necessary to capture public perception regarding more specific aspects of INDOT’s activities. In particular, INDOT needs an effective way to measure and track public opinions and awareness or understanding of a select set of its traffic engineering practices. To evaluate public acceptance of specific INDOT traffic engineering activities, a survey consisting of 1.000 adults residing within the State of Indiana was conducted. The survey population was representative in terms of age and gender of the state as of the 2010 U.S. Census. The survey was administered during the months of July and August 2020. Public awareness regarding emerging treatments not currently implemented in Indiana is low and opposition to the same new technologies is prominent. Older or female drivers are less likely to be aware of emerging treatments, and older drivers are more likely to oppose potential implementation of these treatments. Although roundabouts are commonplace in Indiana, multi-lane roundabouts remain controversial among the public. Regarding maintenance and protection of traffic during work zones and considering full or partial roadway closure, public preference is for partial closure; this preference is stronger in rural areas. The public equally agrees and disagrees that INDOT minimizes construction related traffic delays. Approximately 76% of Indiana drivers believe themselves to above average drivers, while an additional 23% believe themselves to be average. Driver perceptions of average highway speeds speed are not aligned with posted speed limit as the perceived average speed on Indiana’s urban freeways and rural and urban state highways is considerably higher than the actual speed limit.

Data and experiences in Sweden show that it is possible to combine high productivity in animal production with the restricted use of antibiotics. The major key factors that explain Sweden’s success in preventing AMR are: Swedish veterinary practitioners were aware of the risk of AMR as early as the 1950s, and the need for prudent use of antibiotics was already being discussed in the 1960s. Early establishment of health services and health controls to prevent, control and, when possible, eradicate endemic diseases reduced the need for antibiotics. Access to data on antibiotic sales and AMR made it possible to focus on areas of concern. State veterinary leadership provided legal structures and strategies for cooperation between stakeholders and facilitated the establishment of coordinated animal health services that are industry-led, but supported by the State.

Health, United States, 2019 is the 43rd report on the health status of the nation and is submitted by the Secretary of the Department of Health and Human Services to the President and the Congress of the United States in compliance with Section 308 of the Public Health Service Act. This report was compiled by the National Center for Health Statistics (NCHS) of the Centers for Disease Control and Prevention (CDC). The Health, United States series presents an annual overview of national trends in key health indicators. The 2019 report presents trends and current information on selected measures of morbidity, mortality, health care utilization and access, health risk factors, prevention, health insurance, and personal health care expenditures in a 20-figure chartbook. The Health, United States, 2019 Chartbook is supplemented by several other products including Trend Tables, an At-a-Glance table, and Appendixes available for download on the Health, United States website at: https://www.cdc.gov/nchs/hus/ index.htm. The Health, United States, 2019 Chartbook contains 20 figures and 20 tables on health and health care in the United States. Examining trends in health informs the development, implementation, and evaluation of health policies and programs. The first section (Figures 1–13) focuses on health status and determinants: life expectancy, infant mortality, selected causes of death, overdose deaths, suicide, maternal mortality, teen births, preterm births, use of tobacco products, asthma, hypertension, heart disease and cancer, and functional limitations. The second section (Figures 14–15) presents trends in health care utilization: use of mammography and colorectal tests and unmet medical needs. The third section (Figures 16–17) focuses on health care resources: availability of physicians and dentists.
