Der volle Inhalt der QuelleThis thesis is about external communication of university hospital in France, more precisely on communication through social networks. Understanding and analysing the most shared and interactive information make our aim easier to design a model for hospital communication. Firstly, we need to better understand the French hospital system and its history, as well as the change of meaning of the word “health”. To demonstrate this, we will address issues such as E.health as being the meeting point of media, communication, and healthcare technologies, as well as patient management. We notice that the patient’s behaviour towards the doctor has changed since the arrival of new media technologies allowing people to get some resources at any time and freely. These changes have a direct impact on hospitals communication which have to become more opened, clear, and also modern. Then we choose to analyse the most used networks by the university hospitals, Youtube, Facebook and twitter. We also mention two other communication aids, the patient booklet which has to be delivered to every patient, and internet websites. The present research proposes a comparison of the hospitals on the content of each of their aids and the convenience to have access to them. It opens a new perspective in the field of hospital external communication and allows a more precise response to patient’s enquiries. Thanks to surveys among professionals, we have also found that there was a need for more fluid communication between the different departments within the hospital. Our research makes visible the developments and the limits of the current external communication in hospitals but above all opens the way to new communication means in these establishments
Electronic Thesis or Diss., Université de Lorraine, 2024. http://www.theses.fr/2024LORR0170.
Der volle Inhalt der QuelleFour years after the start of the COVID-19 pandemic, it is estimated that almost 100 million people worldwide are suffering from long-term consequences, defined under the umbrella term of Long Covid. The development of remote monitoring solutions has been recommended by the National Institute for Health Excellence (NICE), and the use of voice and vocal biomarkers is particularly relevant in this context. This thesis project consists of two axes. The first one aims to improve knowledge on Long COVID based on data from the Predi-COVID cohort in Luxembourg, and on the other, to develop an innovative voice-based digital health solution to monitor the health of people affected by Long Covid. We have shown that Long Covid is not a single entity, but appears to be made up of sub-phenotypes of varying severity, and that symptoms tend to occur in clusters. Trajectories of symptom evolution from initial infection to 2 years later were modeled, showing that 30% of Predi-COVID participants experienced high levels of persisting symptoms, with a strong impact on their quality of life at 2 years, and a predominance of fatigue and anxiety. These data underline the need for innovative tools to enable personalized management. The UpcomingVoice study described the needs and expectations of people affected by Long Covid in terms of a digital, voice-based symptom-tracking solution, defined the expected specifications and allowed the development of a first version of the Long Covid Companion application. This application, created for and with people affected by Long COVID, provides a concrete response to their needs, enabling them to monitor their health and symptoms on a daily basis, providing a graphic visualization that can be used as a basis for discussion with healthcare professionals, and offering validated resources on Long COVID. Long COVID Companion is the first application of its kind accessible to all, free of charge and in 3 languages. This thesis project thus has a direct impact on people with Long COVID, providing them with a tool to support them in their daily lives. The Long COVID Companion application also represents a virtual cohort of people with Long Covid, making it an additional unique resource for research on Long COVID and vocal biomarkers. Collaborations have been set up with the #Apresj20 Covid long France association, as well as with specialized COVID Long care networks in Luxembourg and France, and will contribute to improving knowledge on Long COVID. Finally, further studies to assess the potential clinical benefit of Long COVID Companion and improvements to obtain medical device certification are envisaged to ensure that this tool is fully integrated into the care of people living with Long COVID

Der volle Inhalt der QuelleEnsuring that patients can access medical care promptly and improving the standard of that care are crucial aims for healthcare facilities. Healthcare providers however face the challenge of a decreasing number of physicians across all specialties, complicating the effort to keep up with the rising demand for care. Departments specializing in rheumatology, which manage chronic conditions requiring ongoing care, are especially impacted. Emergency services, too, are frequently overwhelmed due to the suboptimal allocation of their ever-shrinking resources. The ongoing health crisis has intensified existing challenges, leading to a widespread implementation of digital tools to aid both patients and healthcare professionals. Nonetheless, the practicality, effectiveness, and clinical value of many digital solutions remain unverified through scientific research. Against this backdrop, the principal aim of this doctoral research was to investigate the integration and application of three cutting-edge digital health technologies (DHT). The purpose was to empower patients, enhance patient autonomy in daily disease management and to improve professional healthcare monitoring. The technologies examined include: (1) symptom checkers (SC), dedicated software for symptom assessment and care seeking guidance; (2) self-sampling devices (SSD), tools enabling patients to collect biological samples independently at home; and (3) electronic patient-reported outcomes (ePRO), systems for capturing patients' self-reported health status.Three objectives were therefore successively pursued. These were: (1) to evaluate the accuracy, acceptance and usability of SCs in patients seeking rheumatology and emergency care; (2) to evaluate the accuracy, acceptance and usability of SSDs in patients with rheumatic diseases; and (3) to evaluate patient-reported electronic outcomes in patients with rheumatoid arthritis and axial spondyloarthritis. To achieve these objectives, we conducted 12 studies (including six randomized controlled trials) involving a total of 3971 participants (3356 patients presenting with rheumatological complaints, 450 patients admitted to the emergency department and 165 healthcare professionals). These studies have resulted in 20 scientific articles, 18 of which have been published in international peer-reviewed journals and 2 are currently submitted for publication. Our results indicate a high level of patient acceptance of SCs. However, their diagnostic accuracy is still highly inadequate, and their use tends to produce overdiagnosis and omission of certain diagnoses. More worryingly, SCs have failed in a substantial number of cases to diagnose life-threatening emergencies. In addition, our results underline a patient preference for the use of SSDs applied to the upper arm, judged to be less painful than traditional fingerpricking. Ultimately, granting rheumatologists access to patient-generated data from home, merging insights from symptom checkers and self-sampling devices, has markedly enhanced diagnostic accuracy. Employing machine learning algorithms to analyse patient symptom reports, along with the integration of imaging data, has further increased diagnostic accuracy. Moreover, the incorporation of electronic patient-reported outcomes (ePRO) alongside data on inflammatory markers obtained through SSDs has substantially decreased in-person consultations and released new resources. The findings of this doctoral study underscore the viability and promise of a digitally supported patient pathway. This pathway is particularly encouraging for managing individuals with chronic diseases such as rheumatic conditions however also for patients requiring emergency care

Information science plays a fundamental role in the information society, to devise ways for social inclusion through digital inclusion. Accordingly, we developed this research for the Masters Program in Information Science at the Federal University of Paraíba, to intervene in the process of informational exclusion experienced by the Community of Santa Clara (CSC) in João Pessoa, Paraíba. The intervention happened through field research for registration, organization and dissemination of "information sources" (study subjects) comprised of people of the Community. An action of information has been implemented into the CSC to create the virtual interface "Blog da Comunidade Santa Clara" in the Wordpress platform, to disseminate the wealth of knowledge of those depositors of social memory and knowledge of Santa Clara, which will be available for future generations. The appropriation of the research results (the Blog) by the community generated the extension project "Blog Management Course", within the PPGCI/UFPB, whose purpose was to develop information skills for residents of the Community volunteers to continue the blog. Thus, the result of the research at Santa Clara was increased, in reciprocal action of the Community and as assumption of the action-researchmethodology, the determination to continue the publication of the blog (a virtual interface for communication of information). In addition to the extension project for training of volunteers of CSC, the treasure of knowledge disseminators of the Community, we developed a learning activity through a tutorial for creating blogs, in partnership with the "Laboratory of intellectual Technologies" (Laboratório de Tecnologias Intelectuais - LTi) Department of Information Science, UFPB. The inclusion of CSC's treasure of knowledge in cyberspace, as well as empowerment of the community in intellectual competence to use digital technology of communication of information, can promote the recovery of cultural identity of residents of the CSC and citizenship.

Der volle Inhalt der QuelleAim of this research is to identify the dynamics of “health data” in the field of digital sovereignty: who can use it to describe and explain situations, predict trends, and induce individual and/or population, or even States, behaviours ? What is – and should be legally protected, and how ? We here report on and analyze the overflowing of historical approaches to regulation, due to the diversification of players, techniques and uses ; the multiplication of data sources and their dissemination, the shaking of legal categories despite their recent establishment ; the porosity of national and joint systems, due to conventional or agressive interactions. As a result, we analyze the accelerated advent of new rules at European level in traditionally regalian fields of cyber infrastructure, qualifications (data, technologies, uses), and mutual guarantees against interferences. Other challenges call for in-depth insight (such as reidentification & synthetic data), in an era where for long technological domination is no more a prerogative of States, and where geopolitics has been extended by new tools and practices
