Abstract Regimen-related toxicity, one of the main reasons for discontinuation of chemotherapy in acute lymphoblastic leukemia (ALL), can not only be life-threatening but may affect the risk of relapse. In 246 children with newly-diagnosed ALL who were treated using a single protocol (St. Jude Total XIIIB), we used a candidate-gene approach to determine whether acute toxicities [gastrointestinal (stomatitis or diarrhea), hyperbilirubinemia, infection, or neurotoxicity] were related to 16 common polymorphisms in genes plausibly linked to the pharmacodynamics of the drug therapy. During the high-dose methotrexate consolidation therapy, only genotypes related to methotrexate disposition were associated with grade 3–4 gastrointestinal toxicity, adjusting for demographic factors and ALL risk group. Patients having the RFC AA/AG (p = 0.025) genotypes or those having the MTHFR 677 TT (p = 0.048) genotypes had a higher risk of the toxicity (OR = 10.4 and 3.2 respectively), than those with RFC GG or MTHFR CT/CC. In addition, the risk of hyperbilirubinemia during consolidation was higher among those with the lower activity UGT1A1 7/7 genotype than those with other UGT1A1 genotypes (OR = 12.2, p < 0.0001). Adjusting for time at risk over the entire therapeutic period, RFC AA/AG was again prognostic for gastrointestinal toxicity (p < 0.0001); the GSTP1 AA/AG genotypes (p = 0.021) and TPMT heterozygous genotypes (p = 0.047) were also risk factors. The RFC AA/AG genotype was also a modest risk factor for infection (p = 0.05). There were only weak predictors of neurotoxicity, with the MDR1 exon 26 TT genotype a risk factor (p = 0.025) for presumed methotrexate-related neurotoxicity over the entire therapeutic period. We conclude that germline polymorphisms influence the toxicity of antileukemic therapy and their identification may provide a tool for tailoring therapy in childhood ALL.

446 Background: Renal cell ca (RCC) is one of solid tumor with relatively highest incidence of venous thromboembolism (VTE). But, there have been to no large-scale studies that focused on VTE in RCC. The aim of this study was to investigate the incidence, time course of VTE, risk factors, and prognosis associated with VTE in RCC patients in Korea. Methods: The medical records of RCC patients (n=1248) histologically diagnosed at Severance Hospital, Yonsei University College of Medicine, Seoul, Korea from Jan 2005 to Mar 2011 were retrospectively reviewed. Results: Among 1248 RCC patients, 69.6% were men, median age was 56years (range: 4∼89), and stage distribution was stage I 65.1%, stage II 9.2%, stage III 10.4%, stage IV 12.5%. The 2-year cumulative incidences of tumor induced VTE (tVTE) was detected in 78 patients (6.3%), while 1-month, 6-months and 1-year cumulative incidence of tVTE were 5.3%, 5.7% and 6.0%, retrospectively. Two-year cumulative incidence of tVTE seemed to increase with stage (0.6%, 3.5%, 22.3% and 24.3% in stages I, II, III and IV, retrospectively). Almost tVTE events developed in the first few months after diagnosis. Stage and metastatic disease were independent risk factors for developing tVTE. In multivariate analysis, the development of tVTE was a significant predictor of survival (P=0.004) and stage, age, ECOG PS was also independent predictor of survival. Conclusions: This is the first study that specially focused on tVTE in RCC. The 2-year cumulative incidence of tVTE in Korean patients with RCC was 6.3%, which is similar to other ethnic group. As tVTE related to poorer survival, RCC patients with advanced stage and metastasis with higher risk of tVTE, close follow-up is recommended and proactive prophylaxis of VTE might be needed.

Introduction: Ochratoxin-A (OTA) is one of the most abundant food-contaminating mycotoxins, known for its nephrotoxicity, neurotoxicity, gonadotoxicity, teratogenicity, immunosuppression and carcinogenesis. OTA has been linked to several genitourinary pathologies, including Balkan nephropathy and genitourinary malignancies. We examine OTA levels in serum samples and tumour specimens collected from patients with renal and testicular tumours.Methods: Frozen samples were obtained from the Ontario Tumour Bank. Serum specimens, along with renal and testicular tumour biopsies, were included in this study. Normal tissue from the negative surgical margins of each tumour served as a control. OTA levels in serum was measured using the enzyme-linked immunosorbent assay (ELISA), while OTA detection in tissue specimens was determined using immunohistochemistry (IHC).Results: We included specimens collected from 56 patients (36 men and 20 women). Histopathology of the 52 renal tumours included 31 (60%) conventional type renal cell carcinomas (RCC),5 (10%) chromophobe RCC, 5 (10%) papillary RCC, 1 (2%) oncocytoma and 10 (19%) upper tract urothelial carcinoma (UC). The 4 testicular tumours included 1 seminomatous (25%) germ cell tumour and 3 (75%) non-seminomatous germ cell tumours. OTA was detected in the serum of renal tumour patients, with a range from 0.004 to 0.25 ng/mL (mean: 0.07 and median 0.06 ng/mL).There was no OTA signal detected by IHC staining in all testedrenal and testicular tumours.Conclusions: The OTA levels detected in the serum of patients were highly variable and relatively low. No OTA was detected in the tissue samples.

The effect of substituting fishmeal protein with soyabean meal protein on serum metabolism and some haematological indices, have been assessed, using Hyperco broiler chickens. Six diets (isonitrogenous and isocaloric) were fed, in which, 0, 20, 40, 60, 80 and 100% of dietary fishmeal (FM) protein was substituted with soyabean meal (SMB) protein. Results showed significant (P < 0.05) differences in clotting time, packed cell volume (PCV), red blood count (RBC) and mean corpuscular volume due to dietary treatments. Clotting time was longest for birds on diet 6 in which FM was completely with SMB and shortest for those on diet 3. The variations in the PCV, RBC and MCV followed no particular trend with dietary treatments. The hemoglobin concentration (Hb), mean corpuscular hemoglobin (MCH) and mean corpuscular hemoglobin concentration (MCHC) did not differ significantly (P > 0.05). Total serum protein (TSP), albumin, globulin and albumin/globulin ratio, were not significantly (P > 0.05) influenced by the dietary treatments. However, significant (P < 0.05) differences were obtained in the serum urea levels as the values seemed to increase with increasing Revel of FM substitution with SBM. It was inferred, that an all-SBM feeding may have serious implications for the routine debeaking processes, especially in layers, given the much higher clotting time for birds on diet 6 m which there was 100% substitution of FM with SBM protein

Activation of Rac1 GTPase signaling is stimulated by phosphorylation and release of RhoGDI by the effector p21-activated kinase 1 (PAK1), but it is unclear what initiates this potential feed-forward mechanism for regulation of Rac activity. Phosphatidic acid (PA), which is produced from the lipid second messenger diacylglycerol (DAG) by the action of DAG kinases (DGKs), is known to activate PAK1. Here, we investigated whether PA produced by DGKζ initiates RhoGDI release and Rac1 activation. In DGKζ-deficient fibroblasts PAK1 phosphorylation and Rac1–RhoGDI dissociation were attenuated, leading to reduced Rac1 activation after platelet-derived growth factor stimulation. The cells were defective in Rac1-regulated behaviors, including lamellipodia formation, membrane ruffling, migration, and spreading. Wild-type DGKζ, but not a kinase-dead mutant, or addition of exogenous PA rescued Rac activation. DGKζ stably associated with PAK1 and RhoGDI, suggesting these proteins form a complex that functions as a Rac1-selective RhoGDI dissociation factor. These results define a pathway that links diacylglycerol, DGKζ, and PA to the activation of Rac1: the PA generated by DGKζ activates PAK1, which dissociates RhoGDI from Rac1 leading to changes in actin dynamics that facilitate the changes necessary for cell motility.

SummaryTo evaluate and compare the fiveyear outcome after treatment of a varicose GSV by endovenous thermal laser ablation (EVLA) and radiofrequency ablation (RFA).In this non-randomized, prospective study, patients treated in 2007 and 2008 for complete varicosis of the GSV (CEAP: C2–C6) were divided according to the treatment technique used into three groups: RFA (VNUS ClosureFast™), EVLA 980 nm (ELVeS 980™) and EVLA 1470 nm (ELVeS 1470™). Ultrasound-guided follow-up consultations were conducted 3 days, 3 months, 1 year and 5 years after treatment.589 patients with 643 GSVs (223 RFA, 185 EVLA 980, 235 EVLA 1470) were treated under tumescent local anaesthesia. No major complications occurred and minor complications were rare (4.7–12.2 %, p=0.135).66 % (RFA), 15 % (EVLA 980) and 32 % (EVLA 1470) of the patients were pain-free without post-operative analgesics. After 5 years, 86 % (RFA), 90 % (EVLA 980) and 93 % (EVLA 1470) of the veins were occluded (p=0.096). Ultrasound imaging revealed reflux in treated GSVs in 5–8 % of cases (p=0.73). A second ablation of the previously treated GSV was required in 5 % (RFA), 2 % (EVLA 980) and 2 % (EVLA 1470) of the patients (p=0.28).Endoluminal thermal ablation is an easy, safe and well tolerated method for the treatment of varicosis. Radiofrequency ablation is less painful than EVLA. RFA and EVLA 1470 are equally effective for vein closure.

AbstractObjectiveTo characterise plasma and red-blood-cell (RBC) folate status among pregnant women in an area with an extremely high prevalence of neural tube defects, and to compare them with those of women from a low prevalence area.DesignA cross-sectional survey conducted in 2003.SettingOne county and one city from each of the high prevalence area and the low prevalence area in China.SubjectsFive hundred and sixty-two women in their first trimester of pregnancy in the high prevalence area and 695 pregnant women in the low prevalence area.ResultsWomen in the high prevalence area had less than half the plasma and RBC folate concentrations (12.2 and 440.0 nmol l− 1, respectively) of women in the low prevalence area (33.5 and 910.4 nmol l− 1, respectively). In the high prevalence area, 40% of rural women were deficient in RBC folate and 50% were deficient in plasma folate; 20% of urban women were deficient in RBC folate and 30% deficient in plasma folate. In contrast, only 4% (RBC folate) and 6% (plasma folate) of rural women, and 2% (RBC folate) and 1% (plasma folate) of urban women, were folate-deficient in the low prevalence area. Less than 10% of rural and about 26% of urban women in the high prevalence area took folic acid periconceptionally, compared with 70% and 60% of women in the low prevalence area.ConclusionsBlood folate deficiency is highly prevalent among pregnant women in an area of China with a very high prevalence of neural tube defects.

We report a case of an unanticipated intra-operative transesophageal echocardiography (TEE) finding of pulmonary artery thromboembolism in a 72yr woman being prepared for radical nephrectomy and caval thrombectomy. Intra-operative TEE performed to evaluate the extent of caval thrombus found the presence of pulmonary artery tumor thromboembolism in an otherwise asymptomatic patient after induction and prior to commencing surgery. A chest Computed Tomography (CT) scan confirmed a large saddle tumor thromboembolus. A multidisciplinary approach was utilized to facilitate radical nephrectomy with caval thrombectomy and pulmonary artery thromboembolectomy. This case shows the importance of adequate perioperative imaging and utilization of intra-operative TEE to evaluate the extent of disease. To our knowledge, we are the first to present a case of RCC with cava tumour thrombus in which the pulmonary artery tumour thromboembolism was detected incidentally on intraoperative TEE.

Abstract The enantioselective resolution of racemic styrene oxide (rac-SO) to (R)-SO by whole cells of a recombinant Escherichia coli expressing epoxide hydrolase (EH) activity in aqueous and biphasic system were studied. Some parameters that may alter this bio-resolution, such as the concentration of recombinant cell, substrate and product were evaluated. The effect of the addition of different additives on the course of rac-SO biotransformation was also investigated. The results showed that the yield and the enantiomeric excess (ee) of (R)-SO were dependent on these variables. When the kinetic resolution was conducted with 350 mM of rac-SO, enantiopure (R)-SO with high (≥99%) ee was obtained with a yield of 38.2% yield at 12.2 h in the presence of 10% (v/v) Tween 80. An isooctane/aqueous system was developed to overcome the adverse factors in the aqueous phase, resulting in an improvement of yield from 38.2% to 42.9%. The results will provide a useful guidance for further application of this enzyme in the biocatalytic production of chiral synthons.

PURPOSE: To determine the relative risk (RR) of developing a metachronous contralateral renal tumor after an initial diagnosis of renal cell carcinoma (RCC), with stratification by renal tumor characteristics, demographic factors, and follow-up duration, in order to develop an improved risk-based surveillance strategy. PATIENTS AND METHODS: The 1973 to 1997 Surveillance, Epidemiology, and End Results database was used to determine the observed and expected number of metachronous contralateral renal tumors developing after an initial diagnosis of RCC. RESULTS: A total of 43,483 patients had a first diagnosis of RCC. Contralateral RCC developed subsequently in 155 (0.4%) of 40,049 patients with follow-up who had no synchronous diagnosis of RCC, with 10.81 expected cases (RR, 14.3; 95% CI, 12.2 to 16.8). The respective RRs (and 95% CIs) for contralateral RCC for white men and women were 16.0 (11.1 to 22.3) and 13.7 (7.7 to 22.6) at less than 2 years, 8.8 (5.0 to 14.3) and 10.5 (5.0 to 19.3) at 2 to 5 years, 13.5 (8.1 to 21.0) and 5.1 (1.4 to 13.2) at 5 to 10 years, and 13.0 (6.2 to 23.9) and 13.7 (5.0 to 29.9) at ≥ 10 years, respectively. The RRs were significantly higher in black compared with white men for the first 5 years, with the RRs (and 95% CIs) in the former group of 95.3 (58.2 to 146.7) at less than 2 years and 41.9 (16.8 to 86.3) at 2 to 5 years. CONCLUSION: The incidence of metachronous contralateral RCC is stable on long-term follow-up, suggesting that surveillance of the contralateral kidney should remain rigorous on extended follow-up. Black men are at a significantly higher risk of developing contralateral RCC in the first 5 years of follow-up.

Introduction: Treatment decisions in localized prostate cancer are complicated by the available choices. A rapid-access cancer clinic (RAC) has been unique to Calgary, AB, since 2007. This RAC offers multidisciplinary prostate cancer education by a urologist, medical oncologist, and radiation oncologist. It is hypothesized that treatment utilization data from decisions taken at RAC may serve to benchmark the appropriateness of treatment decisions on a population level.Methods: Records of patients with clinically localized prostate cancer in Alberta between October 1, 2007 and September 30, 2009 were reviewed with ethics approval. Records were linked to the Alberta Cancer Registry database. Clinical, treatment, and health services characteristics pertaining to patients attending RAC were compared to the general population. The primary endpoint was utilization rates of each initial treatment.Results: During this two-year period, 2838 patients were diagnosed with localized prostate cancer; 375 attended RAC. The utilization rates among RAC patients vs. the whole Alberta population were: prostatectomy 60.3% (95% confidence interval [CI] 55.3–65.2) vs. 48.0% (95% CI 47.1‒50.7; χ2 p<0.001); active surveillance 16.0% (95% CI 12.3‒19.7%) vs. 13.5% (95% CI 12.2‒15.8; χ2 p=0.214); radiotherapy 11.7% (95% CI 8.5‒15.0) vs. 18.0% (95% CI 16.9‒20.5; χ2 p=0.002); and hormone therapy 8.0% (95% CI 5.2‒10.8) vs. 17.4% (95% CI 16.1‒18.9; χ2 p<0.001).Conclusions: A specialized clinic for localized prostate cancer may be associated with a higher likelihood of receiving surgery or active surveillance as initial treatment compared to the prostate cancer population in Alberta.

In healthy young men (age, 20–22 years), we tested the role of prostanoids produced by the COX (cyclo-oxygenase) pathway in cutaneous vasodilatation evoked in the finger by ACh (acetylcholine). To this end, changes in cutaneous RCF (red cell flux), recorded by the laser Doppler technique, evoked by a series of iontophoretic pulses of ACh were tested before and after oral aspirin (600 mg). Increases in RCF produced by successive pulses of ACh up to a mean change of 125.5±11.8 PU (perfusion units) were potentiated 30 min after aspirin (160.0±12.4 PU; P<0.05). By contrast, aspirin had no effect on increases in RCF evoked by iontophoretic application of the NO (nitric oxide) donor and endothelium-independent dilator sodium nitroprusside (mean increases in RCF were 73.8±9.8 PU before and 79.1±12.2 PU after aspirin). The ACh-evoked increases in RCF were also potentiated 3 h after oral administration of the antioxidant vitamin C (1000 mg; 139.1±15.4 PU before and 170.5±13.5 PU after vitamin C; P<0.05). We propose that, in healthy young men, cutaneous vasodilatation evoked in the finger by the endothelium-dependent dilator ACh is limited by constrictor products of the COX pathway, including PGH2 (prostaglandin H2), TXA2 (thromboxane A2) and/or superoxide anions. This effect of the COX products may be an early marker of the increased risk of cardiovascular disease in men compared with women.

Objective: To evaluate the effectiveness of contrast-enhanced ultrasound (CEUS) in detecting incomplete ablation and local recurrence of renal tumors after percutaneous radiofrequency ablation (RFA). Methods: 31 patients were included for RFA treatment and underwent CEUS examination after RFA, ablation zone and contrast distribution in the ablation area were observed, CEUS images were compared with enhanced CT/MRI images to determine the residual tumors and local recurrence of renal tumors. Results: The average maximum diameters of the tumor and the ablation zone after the first RFA were 32.3 ± 14.7 mm and 35.9 ± 12.2 mm, respectively. A higher rate of complete tumor ablation was achieved if the ablation zone was larger than the primary tumor (p = 0.026). Within 1 month after RFA, contrast-enhanced CT/MRI examinations demonstrated incomplete ablation in 9 of 31 patients (29.0%), while CEUS revealed incomplete ablation in 8 of 31 patients (25.8%). The sensitivity, specificity, positive predictive value and negative predictive value of CEUS in evaluating complete ablation of renal tumors were 88.9%, 100%, 100%, 95.7%, respectively. During the follow-up period, local recurrence was reported in 2 (7.4%) of the 27 patients with complete tumor ablation. Tumor recurrence signs in the two patients were identified by both CEUS and contrast-enhanced CT/MRI. Therefore, both the sensitivity and specificity of CEUS for the evaluation of tumor recurrence were 100%. Conclusion: After percutaneous RFA of renal tumors, the effectiveness of CEUS in the follow-up assessment of residual and recurrent tumors is basically the same as that of contrast-enhanced CT/MRI. Advances in knowledge: In this study, we evaluated the effectiveness of CEUS in the follow-up assessment of residual and recurrent tumors after RFA is basically the same as that of contrast-enhanced CT/MRI. Combining multiple follow-up methods may improve the detection rate of residual or recurrent tumors.

Realizou-se um estudo prospectivo cego sobre a sensibilidade e especificidade do sinal do quinto dedo, do fenômeno dos interosseos de Souques e o teste do desvio pronador como sinais motores sutis em pacientes com tumor cerebral monoemisférico. De 71 pacientes, 57 possuíam lesão cerebral comprovada radiologicamente e 14 indivíduos apresentavam imagem cerebral normal. Tanto o sinal do quinto dedo como o fenômeno dos interósseos de Souques evidenciaram a mesma sensibilidade (31,5%). O teste do desvio pronador apresentou baixa sensibilidade (12,2%). A combinação dos três testes foi positiva em 49,1% dos pacientes.

Objective Endovenous methods are increasingly used to treat varicose veins. We evaluated the outcome of patients treated with the new radiofrequency ablation (RFA)-ClosureFast catheter in an outpatient setting. Method Retrospective analysis of postinterventional duplex ultrasound (DUS), complication rate and quality of life of patients treated for incompetent saphenous veins. Results Between 2007 and 2009, 155 patients had been treated with ClosureFast. DUS was available from 73 (47%) patients (102 great [GSV] and 16 small [SSV] saphenous veins). After a mean follow-up of 12.2 months (range 1–29 months), DUS showed six (5.9%) open GSV and an occlusion of all treated SSV. One pulmonary embolism had occurred. Mean patient's satisfaction was 8.7 (10 = very satisfied), pain after one week 2.0 (no pain = 0, maximal = 10) and absence of work was 0.9 day (range 0–14 days). Conclusion RFA for incompetent saphenous veins can safely be performed in an outpatient setting with a low complication rate, minimal pain and fast recovery.

AbstractThe relic “the sack of Saint Francesco” has for the first time been investigated by scientific means. The sack is kept at the Franciscan Friary of Folloni near Montella in southern Italy. According to legend, the sack appeared on the doorstep of the Friary in the winter of 1224 containing bread sent from St Francesco (St Francis of Assisi), who at that time was in France. The bread was allegedly brought to the friary by an angel. We analyzed samples of the sack to obtain a radiocarbon (14C) date and to search for any remaining traces of bread. The 14C date yielded a calibrated age range of AD 1220–1295 (2σ), which places the textile in the right timeframe according to the legend. Chemical analysis by gas-chromatography with mass spectrometric detection (GC-MS) revealed the presence of ergosterol (5, 7, 22-ergostatrien-3b-ol), a known biomarker of brewing, baking, or agriculture. In this paper we have further substantiated the validity of ergosterol as a biomarker for the past presence of bread. It appears that there is a fine correspondence between the Franciscan legend and the two most decisive scientific methods relevant for analyzing the sack. Although it is not proof, our analysis shows that the sack indeed could be authentic.
