Aspiration of a foreign body is a potentially life-threatening problem that often necessitates an anesthetic for removal of the foreign body. Foreign body aspiration is most common among children aged 1 to 4 years old and has a wide variety of symptoms ranging from a mild, nagging cough to complete airway obstruction. Definitive diagnosis and treatment of foreign body aspiration involve flexible or rigid bronchoscopy. The urgency of the procedure depends on the type of object aspirated and the location of the foreign body in the airway. The appropriate anesthetic for removal of the foreign body is dependent upon the surgeon’s plan and involves several steps in decision-making: intravenous versus inhalational induction, airway maintenance (endotracheal tube vs. supraglottic airway vs. mask), spontaneous versus controlled ventilation, maintenance of anesthesia (total intravenous anesthesia vs. volatile agents). Good communication with the surgeon or proceduralist is key to a safe and effective anesthetic.

Massive and sustained candlelight vigils in 2016–2017, the most significant citizen-led protests in the history of democratic South Korea, led to the impeachment and removal of then President Park Geun-hye. These protests took place in a South Korean media environment characterized by polarization and low public trust, and where conspiracy theories and false claims by those opposing impeachment were frequently amplified by extreme right-wing media outlets. How then was it possible for pro-impeachment protests seeking major social change to succeed? And why did pro-Park protesters and government efforts to defend Park ultimately fail? An agent-affordance framework is introduced to explain how key participants (agents), including journalists, citizens, social media influencers, bots, and civic organizations, together produced a broad citizen consensus that Park should be removed from office. This was accomplished by creatively employing affordances made available by South Korea’s history, legal system, and technologies. New empirical evidence illustrates the ongoing significant roles of both traditional and nontraditional agents as they continue to co-adapt to affordances provided by changing information environments. Interviews with key players yield firsthand descriptions of events. The interviews, original content analyses of media reports, and examination of social media posts combine to provide strong empirical support for the agent-affordance framework. Lessons drawn from citizen-led protests surrounding Park Geun-hye’s removal from office in South Korea are used to offer suggestions for how technology-enabled affordances may support and constrain movements for social change elsewhere in the world.

Drug-induced acute tubulointerstitial nephritis (ATIN) is the most common aetiology of ATIN and a potentially correctable cause of acute kidney injury (AKI). An interval of 7–10 days typically exists between drug exposure and development of AKI, but this interval can be considerably shorter following re-challenge or markedly longer with certain drugs. It occurs in an idiosyncratic and non-dose-dependent manner. Antibiotics, NSAIDs, and proton pump inhibitors are the most frequently involved agents, but the list of drugs that can induce ATIN is continuously increasing. The mechanism of renal injury is postulated to involve cell-mediated immunity, supported by the observation that T cells are the predominant cell type comprising the interstitial infiltrate. A humoral response underlies rare cases of ATIN, in which a portion of a drug molecule (i.e. methicillin) may act as a hapten, bind to the tubular basement membrane (TBM), and elicit anti-TBM antibodies. The classic symptoms of fever, rash, and arthralgia may be absent in up to two-thirds of patients. Diagnostic studies, such as urine eosinophils and renal gallium-67 scanning provide only suggestive evidence. Renal biopsy remains the gold standard for diagnosis, but it may not be required in mild cases or when clinical improvement is rapid after removal of an offending medication. Pathologic findings include interstitial inflammation, oedema, and tubulitis. The time until removal of such agents and the severity of renal biopsy findings provide the best prognostic value for the return to baseline renal function. Poor prognostic indicators are the long duration of AKI (> 3 weeks), a patient’s advanced age, and the high degree of interstitial fibrosis. Early recognition and appropriate therapy are essential to the management of drug-induced ATIN, because patients can ultimately develop chronic kidney disease. The mainstay of therapy is timely discontinuation of the causative agent, whereas controversy persists about the role of steroids.

This chapter develops the analysis of the ‘punishment beyond the legal offender’. It illustrates how parental imprisonment was experienced differently within and across families, and while not all experiences were negative, there were common experiences of hardship. The chapter considers these personal and social hardships ‘referred pains of imprisonment’. Its analysis shows how these experiences were shaped by the direct contact families had with criminal justice agents, the strength of the relationship with the imprisoned parent, and the anticipated and actual response of others within the local community. The chapter introduces a distinction between ‘acute’ pains that were experienced in the early stages of engagement with the criminal justice process (the arrest, trial, and removal of the father from the family) and ‘chronic’ pains that persisted and burdened family members over the longer term.

This chapter investigates the first stage of recovery in early modern perceptions, the removal of disease. It shows that three agents were responsible for ousting illness: God, Nature, and the physician. While scholars are familiar with the first and last of these forces, the vital agency of ‘Nature’, the divinely endowed ‘intrinsic agent’ of the body, has been largely overlooked. Personified both as a hardworking housewife and a warrior queen, Nature removed disease through processes that resembled cooking/cleaning and fighting, the ‘concoction’ and ‘expulsion’ of the humours. Particular attention is paid to the complex power–gender dynamic between female Nature and the male doctor: in theory, the physician was ‘Nature’s servant’, but in practice, he often became her commander, a situation which illuminates wider cultural attitudes to womankind. To demonstrate the prevalence of belief in Nature’s healing role, Galenic ideas are compared with those of the Flemish physician J. B. van Helmont.

As far as the origins and the development of special collections are concerned, library history often focuses on institutions and the cultural property in writing they preserve. By contrast, the contributions in this volume on the history of the Oriental collections, a special department in the Berlin State Library since 1918, are devoted primarily to the actors involved. It is not so much the Oriental Department and its holdings that are examined, but rather the people in its environment: the collectors, in many cases scholars themselves, who gathered the material in the Orient, the busy antiquarians, mediators and sales agents on the spot, the political promoters in the Berlin ministries, as well as the academic librarians and the compilers of the manuscript catalogs. Academic researchers in their role as users of the Berlin collections are also featured prominently. The periods covered range from the beginnings of the library in the 17th century to its division after 1945 as a result of the removal of the holdings during World War II.

Cyclophosphamide and plasma exchange are the standard of care in rapidly progressive glomerulonephritis or lung haemorrhage caused by antiglomerular basement membrane (anti-GBM) disease, and it is unusual to encounter patients at earlier stages. Steroids are universally used in addition. There is some evidence that plasma exchange may not be a critical part of treatment at an earlier stage. There is no more than anecdotal evidence for other therapies. Slower-onset therapies such as antibodies to B cells are rarely appropriate. If untreated, patients with severe anti-GBM disease will not recover renal function and are at risk of pulmonary haemorrhage. Evidence for the pathogenicity of circulating anti-GBM antibodies provides rationale for removal of circulating antibodies as rapidly as possible, whilst simultaneously inhibiting their synthesis. This was behind the introduction of the combination of plasma exchange with immunosuppressive therapy in mid 1970s, which revolutionized outcomes. Plasmapheresis aims to remove circulating pathogenic antibodies against GBM and possibly other mediators; cyclophosphamide prevents further synthesis of autoantibodies; and steroids act as anti-inflammatory agents to attenuate the glomerular inflammatory response initiated by anti-GBM antibodies. It is clear from experimental models and occasional observations in man that the anti-cell mediated effects of current therapies are important too. Outcomes vary, but in general patient survival is now good, while renal survival remains poor, in many series less than 50% at 1 year. Treatment is toxic and after an early peak in deaths due to pulmonary haemorrhage, secondary infections are the next threat. It may therefore be best not to immunosuppress patients with a very poor renal prognosis who appear to be at low risk of pulmonary haemorrhage. Treatment can usually be curtailed after 3 months without recurrence. ANCA and anti-GBM antibodies occur together in some patients. This is typically an older group which often has features of vasculitis, and the anti-GBM response may often be secondary. Longer treatment as for small vessel vasculitis is usually indicated.
