Background: Hernia is one of the most common ailments encountered in general surgery practice. The inguinal hernia constitutes majority of cases. The available literature describes umbilical, femoral, lumbar, perineal and many others rarer varieties. Our preliminary observations indicated a changing scenario. With a view to ascertain it, investigations were made to find out the incidence of hernia with frequency data of different types and demographic details of patients during last decade at tertiary care teaching hospital in Pune and then comparing it with the existing literature.Methods: Ten years data from October 2008 to October 2018 of all the adult population suffering from hernia reporting to this center was collected from the archives of hospital records maintained in OPD, wards and the department.Results: Inguinal hernia was the most common constituting 76.9% of the total cases. The right side was more affected than the left side with 51.0% cases being on right side. This was followed by umbilical hernia, 12.26% of the total cases. Males were more affected than the females except in incisional hernia. Only two cases of spigelian hernia had undergone repair over last one decade. There were no cases of femoral hernia, lumbar hernia and perineal hernia encountered during the last ten years in this center.Conclusions: The relative frequency of the incidence of different types of hernia was inguinal, umbilical, epigastric; incisional which have been in descending order. In the last decade at this centre which is in contrast with hernia incidence figures quoted in common reference books and the available literature.

Cette thèse compare six affection afin de donner un cadre unifié à la catégorie de maladie rare. Elle reconstitue les différentes modalités dïntégration des maladies rares dans le monde médical depuis les années 1950. Le problème apparaît aux États-Unis dans les années 1960. Une ambition de restaurer une division du travail plus adaptée se fait jour au début des années 1980 et fixe la catégorie dans des dispositifs réglementaires. L'étude traite ensuite de l'expérience de ces maladies et de l'action collective en situation de rareté. Elle explore ce que représente le fait d'être rare et distingue deux formes de rareté, objective et subjective, cette dernière étant un enjeu de l'action collective. Une enquête dégage un modèle associatif propre aux associations de maladies rares. L'étude de l'action collective inter-associative montre en revanche les difficultés de ces petits groupes à se fédérer dans la durée
This thesis is a comparative study of six ‘rare disorders’ aiming at providing a coherent frame of analysis for this category. The first part presents the history of connections rare disorders had with medical organisation since 1950. At first well integrated, they became a problem in the United-States during the 1960s. The attempts to reorganise the division of medical labour then framed the regulatory dimension. The second part addresses the experience of these disorders and collective action in a context of rarity. I explore what “being rare” means, which leads me to distinguish two types of rarity, an “objective” and a “subjective” one, the latest being a stake in collective action. The inquiry describes a model of association characteristic for rare disorders. Collective action at the inter-associative level shows how long-term coalitions challenge these small groups

Les éléments de terres rares (ÉTR) sont des métaux de grand intérêt économique et des traceurs géologiques de premier ordre mais leur cycle métallogénique reste encore méconnu. La Province de Grenville expose sa racine orogénique et présente une grande concentration d’occurrences d’ÉTR, notamment associées à des granites pegmatitiques (« PGD ») dans le Grenville central. Deux mécanismes peuvent être à l’origine de ces PGD, (i) la fusion partielle d’une croûte continentale archéenne et/ou paléoprotérozoïque de la racine orogénique, ou (ii) l’extrême différenciation de magmas mantelliques produits en contexte d’extension post-orogénique. Une approche multi-méthodes (terrain, pétrogéochimie, géochronologie et isotopie) permet de caractériser ces PGD à ÉTR et de discuter leur pétrogénèse dans le cadre de l’évolution géodynamique de la province. Ils ont intrudé à ~1005-1000 Ma (U-Pb monazite et zircon) des métaplutons (minéralisation en allanite) ou des séries métasédimentaires (minéralisation en monazite) du segment de moyenne pression de la ceinture Allochtone. La datation U-Pb sur monazite et apatite d’un leucosome de ces paragneiss migmatitiques met en évidence un pic de métamorphisme au faciès granulite à 1080-1050 Ma suivi d’un refroidissement lent à un taux de 2 à 6°C/Ma, d’abord dans les conditions suprasolidus jusqu’à l’intrusion des PGD dans des conditions proches du solidus hydraté, puis dans des conditions subsolidus jusqu’à l’isotherme 450-500°C enregistré à 970-950 Ma. L’intrusion des PGD est synchrone de l’initiation dès ~1005 Ma de la fusion partielle prolongée (> 20 Ma) des métasédiments parautochtones sous-jacents. Le caractère peralumineux des PGD, l’étude de zircons néoformés des PGD (isotopes U-Pb-Hf-O et éléments traces), et l’étude pétrogéochimique des PGD et des métapélites paléoprotérozoïques à archéennes de la ceinture Parautochtone, issues de l’érosion de la marge de Laurentia, traduisent la formation des PGD par leur fusion partielle. Au contraire, un PGD issu de la remobilisation de plutons allochtones est peu minéralisé en ÉTR, confirmant que l’extraction des ÉTR de ces occurrences du Grenville central est associée à des roches d’affinité crustale du Craton du Supérieur
The Rare Earth Elements (REE) are of great economic interest and first order geological tracers. However, their metallogenic cycle remains poorly defined. The Grenville Province exposes its orogenic root and shows numerous REE occurrences that are especially associated with pegmatitic granite dykes (“PGD”) in the central Grenville. Two hypotheses can account for the genesis of these PGD, whether (i) the partial melting of the orogenic root composed of reworked Archean and/or Proterozoic pre-existing continental crust, or (ii) the extreme differentiation of mantle melts produced during post-orogenic extension. A multi-method approach (field geology, petrogeochemistry, geochronology and isotopy) allow to characterize the PGD and discuss their petrogenesis during the geodynamic evolution of the province. They intruded at ca. 1005-1000 Ma (U-Pb on monazite and zircon) metaplutons (mineralization hosted in allanite) or metasedimentary sequences (mineralization hosted in monazite) from the mid-pressure allochthonous crustal segment. U-Pb dating on monazite and apatite from a single leucosome of these migmatitic paragneisses evidence for a peak of metamorphism at ca. 1080-1050 Ma that was followed by a slow cooling at a rate of 2 to 6°C/Ma, first under suprasolidus conditions until the close to wet-solidus intrusion of PGD, and then under subsolidus conditions down to the 450-500°C isotherm dated at ca. 970-950 Ma. The intrusion of the PGD is coeval with the initiation as early as ca. 1005 Ma of the protracted (> 20 Ma) partial melting of underlying parautochthonous paragneisses. The peraluminous character of the PGD, the investigation of pristine magmatic zircon of the PGD (U-Pb-Hf-O isotopes and trace elements), and the petrogeochemical investigations of parautochthonous Paleoproterozoic-Archean metapelites that derive from the Laurentian margin’s erosion, points to the derivation of the PGD from their partial melting. To the contrary, one PGD derived from the reworking of allochthonous plutonic units is poorly mineralized in REE, and confirms that REE from the central Grenville occurrences are issued from crustal rocks of the Superior Craton

Les interactions fluide-roche intervenant dans les systèmes hydrothermaux océaniques ont été étudiées expérimentalement en surveillant les isotopes du Mg et Sr. Les résultats suggèrent que les réactions rapides à températures moyennes pourraient expliquer en partie les divergences identifiées précédemment dans le bilan marin du Sr. De plus, la ré-dissolution de l'anhydrite dans les stades avancés peut expliquer les faibles quantités d'anhydrite trouvée dans les échantillons de basalte océanique altérés. L'évolution du δ26/24Mg dans les fluides indique que le Mg isotopiquement lourd a été incorporé préférentiellement dans les minéraux des argiles.Le fractionnement entre les terres rares et le Ca pendant la précipitation de calcite a été étudié expérimentalement. Ces données suggèrent que l'incorporation du REE3+ dans la structure de la calcite est probablement équilibrée par l'incorporation de Na+ et que les coefficients de partage déduits pour les conditions marines peuvent varier considérablement selon la composition du fluide. Enfin, l'étude de la solubilité de l'hydroxylbastnasite (REE(CO3)(OH)), conduite au cours de ce travail, indique que cette phase est moins soluble que les autres carbonates des terres rares connus
The fluid-rock interactions occurring in hydrothermal systems at or near mid-oceanic ridges (MOR) were studied experimentally while monitoring the Mg and Sr isotopic evolution. The results suggest that fast intermediate-temperature reactions could partly explain previously identified discrepancies in the marine 87Sr/86Sr budget. Furthermore, late-stage anhydrite re-dissolution caused by fluid reduction and temperature decreases in our experiments represent a potential explanation for the low amounts of anhydrite found in altered oceanic basalt samples. The fluid δ26/24Mg indicate that isotopically heavy Mg was preferentially incorporated into the clays. Towards the interpretation of rare earth element (REE) patterns in marine calcite, the fractionation between the REEs and Ca during the precipitation of calcite was studied experimentally. These data suggest that the incorporation of REE3+ ions into the calcite structure was likely charge balanced by incorporation of Na+, and that inferred partition coefficients for marine conditions can vary substantially depending on the fluid composition. Furthermore, the solubility of hydroxylbastnasite (REE(CO3)(OH)) was determined, indicating that this phase is less soluble than other known REE carbonates

Les granites à métaux rares ont une faible valeur économique mais sont souvent exploités pour leurs minéraux industriels. L’objectif de ce travail est d’évaluer la potentialité de certains résidus issus de la production de kaolin de St Austell (Angleterre) pour la récupération de métaux critiques (CRMs) comme les Terre Rares légères (LREE), le niobium (Nb), le tungstène (W) et l’étain (Sn) via le développement d’un procédé de récupération des CRMs. L’étude des résidus miniers montre que les CRM sont concentrés dans le résidu micacé avec des teneurs en LREE allant jusqu’à 170 ppm avec 40 à 60% des CRMs distribués dans ce résidu. Un procédé de traitement de ce résidu a été développé incluant des étapes de concentration par gravité et flottation. Les teneurs en LREE permettent la récupération d’environ 60% des LREE avec des teneurs de 1,6 % et 0,5% LREE respectivement dans le concentré de gravité et flottation
Rare metals granites have always been considered of poor economic value due to their low metal grades but, when altered, they are often exploited for their industrial minerals. This work address the recovery of critical metals (CRMs) including Light Rare Earth Elements (LREE) and rare metals (Sn, Nb, W) from St Austell (UK) kaolin residues though the development of a beneficiation process. Results shows that CRMs are pre-concentrated in the micaceous residue stream with LREE grades of 170 ppm, which account from 40% to 60% of the mass of CRM entering the plant. Analysis of this residue streams shows a high variability of CRM grades and suggest a cut-off grades for some of them. A combined gravity/flotation process have been developed for this residue which allow to recover up to 60% LREE with LREE grade of 1.6 to 0.5% for the gravity and flotation concentrate respectively

L' intérêt des phases oxydes à base de terres rares est certes multiple et leurs propriétésont été explorées depuis longtemps: mais nous nous intéressons ici aux comportements de cesphases en tant que phases thermiques, catalytiques ou conductrices ioniques à hautetempérature. Il s'agit en particulier de développer des systèmes innovants de matériauxpouvant intervenir dans la conception de dispositifs pour la microélectronique, pour capteursde gaz ou membranes sélectives ou pour systèmes dépolluants.Les phases de structure pyrochlore ou fluorine de type TR2Ce207 où TR désigne unélément de Terre Rare présentent divers potentiels d' applications: elles ont été considéréescomme des phases pouvant résister à de hautes températures. Les oxydes de structurespérovskites, de formule générale ABO), présentent de multiples applications potentielles,notamment en tant que phases diélectriques pour condensateurs, ou phases conductricesioniques (en ions oxygène ou en protons) pour électrolytes solides, du fait même de leur hautestabilité chimique à haute température.Ce travail a été divisé en deux parties. La première a consisté à élaborer la phase" thermique)} de type pyrochlore TR2Ce20 7 (TR = La, Ce, ... ) en utilisant un minéralcomplexe à base d'allanite-monazite et de silico-aluminates issus de déchets industriels, doncà bas coût.La deuxième partie a consisté à élaborer BaCe03 et à étudier ses propriétéscatalytiques et conductimétriques en fonction de la température. Une nouvelle méthode desynthèse reposant sur l' utilisation du mélange EDTA-citrate a été utilisée afin d'élaborer unprécurseur, qui, traité thermiquement à 950°C, a permis d'élaborer des poudressubmicroniques de la phase BaCe03. L'activité catalytique du composé BaCe03 démarre à450°C pour atteindre la conversion totale à 675°C : dans cette gamme de température,l'efficacité catalytique de la phase BaCe03 est maximale. L'évolution de la conductivité enfonction de la température de pastilles compactées de BaCe03 a révélé l'existence d'une sériede modifications électriques fortement corrélées aux transitions structurales connues pourBaCe03 dans la littérature. À basse température (300 à 450°C), la faible conductivité de laphase orthorhombique, associée à la faible énergie d'activation, peut être liée à la migrationdes défauts extrinsèques (gaz adsorbés). Cependant, au-dessus de 500°C, la conductivité de laseconde phase orthorhombique augmente: ceci pourrait être attribué à une mobilité croissantedes atomes d'oxygène.

Ce travail est consacré au développement de luminophores de type orthophosphates GdPO4 et ultraphosphates YP5O14 activés par les ions Yb3+, Er3+ et Tm3+, synthétisés sous forme de poudres par voie solvothermale (orthophosphates) et par méthode de flux (ultraphosphates), aptes à émettre une lumière blanche par up-conversion IR-visible. Des orthophosphates GdPO4 : Eu3+ ont également été élaborés par chimie douce afin d’établir le lien entre propriétés optiques et propriétés morphologiques. Les conditions opératoires optimales ont été déterminées. La caractérisation des matériaux obtenus a été menée d’un point de vue structural, par DRX et IR, et morphologique, par MEB. Les orthophosphates GdPO4 ont été obtenu sous forme de nanobâtonnets en condition acide et nanodisques en milieu basique. L’étude des propriétés de luminescence des orthophosphates GdPO4 : Eu3+ a montré que les nanodisques présentent de meilleures performances que les nanofils. L’utilisation de la théorie de Judd et Ofelt a permis de relier les propriétés optiques, structurales et morphologiques de ces orthophosphates. Sous excitation IR, un processus de up-conversion est mis en évidence dans les phosphates YP5O14 : Yb3+, Er3+, Tm3+ et GdPO4 : Yb3+, Er3+, Tm3+. Les processus de transfert d’énergie Yb3+ → Er3+, Tm3+, présidant à l’émission d’une lumière blanche, ont été étudiés. La morphologie des matrices phosphatées a une influence notable sur les propriétés de up-conversion. Ainsi, l’orthophosphate GdPO4 : Yb3+, Er3+, Tm3+ obtenu sous forme de nanobâtonnets en milieu acide est un candidat prometteur pour la lutte anti-contrefaçon
This work deals with the development of polycrystalline Yb3+/Er3+/Tm3+ activated phosphate phosphors of formula GdPO4 (orthophosphates) and YP5O14 (ultraphosphates) elaborated by a solvothermal method (GdPO4) and by a flux method (YP5O14), able to emit a white light by a IR-visible up-conversion process. Eu3+ doped GdPO 4 samples were also elaborated by soft chemistry in order to establish the relation between optical and morphological properties. The elaborating conditions were determined. The characterization of these materials has been carried out using X-ray diffraction, IR spectroscopy and SEM analysis. GdPO4 orthophosphates were obtained as nanorods and nanoparticles under acidic and basic media respectively. GdPO4 : Eu3+ orthophosphate nanoparticles show better optical performances than nanorods. Moreover, the optical, structural and morphological properties of orthophosphate materials were corroborated by the Judd and Ofelt theory. Under an IR excitation, an up-conversion mechanism is evidenced in YP5O14 : Yb3+, Er3+, Tm3+ and GdPO4 : Yb3+, Er3+, Tm3+ activated phosphates. Yb3+ → Er3+, Tm3+ energy transfer processes leading to a white light emission are investigated. The up-conversion properties are considerably influenced by the morphology of the phosphates. The GdPO4 : Yb3+, Er3+, Tm3+ orthophosphate obtained as nanorods in an acidic medium is a promising candidate for anticounterfeit labels

Synchronous primary tumors of female genital tract are rare with a rate of about 0.7-1.8% of all gynaecological tumours. Most common primary tumours presenting as synchronous lesions are ovary and endometrium. However, sex cord stromal tumors are rare variety of primary ovarian tumor and synchronous with endometrium is even much rarer. These tumors are detected usually in younger, overweight, nulliparous and perimenopausal female. Synchronous primary tumors of endometrium and ovary have a better prognosis than the either of above alone because these are usually low grade and diagnosed at early stage. We present a report of four cases of synchronous endometrial and sex cord stromal tumors of ovary.

When most people think of the genre epic poem, they probably think of the great epics of antiquity and the Middle Ages. The further one follows literary history into the present, the rarer one encounters this art form. Two contemporary German-language authors, however, have deliberately chosen this form to tell heroine stories, in keeping with the tradition of the genre: Ann Cotten’s verse epic ‘Verbannt!’ (2016) and Anne Weber’s ‘Annette, ein Heldinnenepos’ (2020). Although both works are dedicated to the same form and function, they differ strikingly in both aspects.

Introduction: Multiple myeloma (MM) commonly appears after the age of 50, corresponding to 1% of all cancers. Symptoms such as renal failure and hypercalcemia are usually the initial manifestations of the disease. Involvement of the central nervous system (CNS) it´s rare and a marker of poor prognosis. We report a young patient opening the MM with an initial manifestation that is rare in the CNS literature. Case report: Male, 38 years old, previously healthy, admitted for holocranial and progressive headache for 1 month and visual blurring and mental confusion started two days before admission. On examination we evidenced papilledema and negative myoclonus in the 4 limbs and neck. Brain computed tomography, arterial and venous angioMRI were normal. Cerebrospinal fluid puncture show increased opening pressure (32 cm H2 O) and biochemical analysis was normal. Laboratory tests showed 24-hour proteinuria in the nephrotic range (3 g), microcytic hypochromic anemia and reduced creatine clearance (48,8 mL/min — Chronic Kidney Disease Epidemiology Collaboration). Contrast brain and orbits magnetic resonance imaging (MRI) presented thickening of the meninges and bone infiltration, suggestive of lymphoproliferative or hematologic disease. MRI of the spine (cervical and thoracic), thorax and abdomen showed signs of diffuse bone infiltration. HMM was confirmed by electrophoresis of proteins with expressive peak of gamma globulin (43%, VR 11,1–18%). Dexamethasone (4 mg 6/6 hour for three days) was administered, with complete resolution of mental confusion and headache. The patient was followed up with hematology to start chemotherapy. Conclusion: We report a rare case of MM that started with predominantly neurological manifestations which responded very well to corticosteroid therapy in low doses after few days. This case illustrates how intracranial hypertension in a young patient requires extensive investigation for possible rarer differential diagnoses when initial tests are normal.

Introduction: Cryptococcoma is a rare CNS infection caused by Cryptococcus neoformans, with a granulomatous lesion. It’s even rarer in immunocompetent patients. Diagnosis requires histopathological analysis and treatment with amphotericin B, often with surgical resection. The aim is to report four cases of cerebral cryptococcoma In Immunocompetent Patients. This is a case report study. The information of this work was obtained through review of the medical record. Case report: We report on four cases of cerebral cryptococcoma in immunocompetent patients. All cases presented with images of expansive lesions and compatible histopathological analysis. The first case was a 62-year-old man with dysphasia and decreased strength on the right side, and underwent total surgical resection, followed by 23 days of amphotericin B treatment. The second case was a 62-year-old woman who presented with orofacial dyskinesia and decreased strength on the right side, and underwent stereotactic surgery, followed by antifungal treatment. After 10 days of hospitalization, the patient developed respiratory insufficiency, progressed to cardiorespiratory arrest and brain death. The third case was a 54-year-old woman who presented with a skin infection caused by Cryptococcus. During treatment with fluconazole, the patient experienced altered sensory perception and left hemiparesis. Surgical resection of the lesion was performed, followed by 30 days of antifungal treatment. The fourth case was a 54-year-old man who presented with pulsatile headache and right hemiparesthesia, and underwent stereotactic neurosurgery, followed by 12 days of amphotericin B treatment. Conclusion: Cryptococcus neoformans infection in the CNS, manifested in the form of cryptococcoma, although rare, should be considered as a differential diagnosis for brain expansive lesions in immunocompetent individuals.

The epitope of a mouse monoclonal AB (9.9) which detects a Factor IX (F.IX) polymorphism in the plasma of normal persons (PNAS 82:3839, 1985) has been related to not more than 6 AA residues of F.IX by recombinant DNA technology. The same 6 residues define Smith’s polymorphic epitope (Am. J. Human Genet. 37:688, 1985 and in press). This region of F.IX contains the alanine:threonine dimorphism at residue 148 first suggested by McGraw et al. (PNAS 82: 2847, 1985) and established by Winship and Brownlee with synthetic DNA oligomers (Lancet in press). Using synthetic DNA probes, we have found that the DNA difference between positive and negative reactors to 9.9 is whether base pair 20422, the first pair in the codon for residue 148, is A:T or G:C. We can conclude that 9.9 reacts with F.IX containing threonine but not alanine at position 148.The F.IX immunologic polymorphism-whose epitope we are referring to as “Malmo”-is, not surprisingly, in strong linkage disequilibrium with two F.IX DNA polymorphisms, TaqI and Xmnl. The highest frequency of the rarer Malmo allele in 6 disparate ethnic groups was in Swedes (32%); a lower frequency (14%) was seen in White Americans whose ancestors came overwhelmingly from the Celtic regions of the British Isles; it was at very low frequency or absent in Black Americans, East Indians, Chinese and Malays. A maximum frequency in Swedes and absence in Africans and Orientals suggest that the transition from A:T to G:C occurred in Scandinavia and spread from there. The history of Europe and America plus the geographical distribution of the rare allele lead us to suggest that this locus might be designated: “the Viking gene”.

Amphibians are a Vital Sign indicator for monitoring long-term ecosystem health in seven national park units that comprise the Great Lakes Network. We present here the results for 2019 amphibian monitoring at Mississippi National River and Recreation Area (MISS). Appendices contain tabular summaries for six years of cumulative results. The National Park Service Great Lakes Inventory and Monitoring Network established 10 permanent acoustic amphibian monitoring sites at MISS in 2015. Acoustic samples are collected by placing automated recorders with omnidirectional stereo microphones at each of the 10 sampling sites. Temperature loggers co-located with the recorders also collect air temperature during the sampling period. Eight of the nine species of frog and toad known to occur at MISS were found in 2019. The most well distributed species were Eastern American Toad, Gray Treefrog, Green Frog, and Northern Leopard Frog. Rarer are Blanchard’s Cricket Frog, Cope’s Gray Treefrog, Wood Frog, and Boreal Chorus Frog. American Bullfrog has not yet been detected on GLKN monitored sites but has been recently confirmed nearby (Pigs Eye Lake). Two of the ten sites—MISS02, MISS04—were not sampled in 2019 due to flooding, and occupancy of early calling species at MISS06 was determined inconclusive due to a late sampling start. MISS07 was also deployed late and results may contain some false absences due to late sampling. We expanded analyses and reporting in 2018 to address calling phenology and to provide a second metric for tracking changes in abundance (as opposed to occupancy) across years. Occupancy analyses track whether or not a site was occupied by a species. Abundance is tracked by assessing how the maximum call intensity changes on sites across years, and by how many automated detections are reported from sites across years. Using two independent survey methods, manual and automated, with large sample sizes continues to return reliable results, providing a confident record of site occupancy for most species. There were some data collection issues in 2019, with two ARS units not deployed and two others with late start dates. This did reduce our ability to assess some sites and species. Summaries of 2019 data are shown in Appendices A, B and C, and cumulative data collection result summaries are provided in Appendix E. Since temperature logs show that the threshold of ≥40°F was already exceeded by 1 April in 2019, we recommend a 15 March start date for future data collection.
