From an historical point of view, epistemologic thought in psychoanalysis was always dependant on positivist orienta, tions. These are based on the identity principle which enables knowledge's axiomatisation. Unconscious is the other of consciousness, and this onto- logical condition of being-other, requires other logical prin, ciples than those based on identity. These principles are sup, ported by a paradoxical structure where opposites bear a re, lation of complementarity and do not exclude one another. Contemporary french thought in psychoanalysis as well as in philosophy provides a reflection about otherness. A theoretical paradigm is thereby formed which can contribute to constitution of an apistemology adapted to psychoanalysis. Starting from a descriptive angle and a selection of authors and theories, a normative angle rises up in the re, search, concerning a possible psychoanalytic epistemology. Every theorisation in psychoanalysis carries a limited knowledge. This limit is connected with the unknown unconscious and with transformability of its notions. The negativerepre- sents these limits in our research. This integrative approach of otherness by means of a paradoxality logic and the negative, can contribute to the development of an epistemology suitable to psychoanalysis

He intends to show how the function of the impossible articulates in the connection both, and not only from psychopathologic and descriptive point of view, but also cleanly operating one. From their respective report to the real, how these two domains of Knowledge, determining as for the status of the modem subject - the psychoanalysis and the mathematics - can not only shed light mutually on the respective lawsuit of their Knowledge, 'but also isolate a report to the impossible operating in a comparable way in both. The study of this report drove us to put both notions of mathematical discourse and of mathematical subject; Is there a meaning to characterize in psychoanalytical terms, the modes of discursivity of the mathematical historic or individual progress ? For example, can the categories of the psychopathology, as the phobia, the hysteria, the obsessional neurosis or the schizophrenia, specify the invention process in mathematics ? Then, if such seems to be the case, a locatable function of the subject as the Knowledge at work in mathematics can articulate with any Kind of mathematization of the unconscious in psychoanalysis ? Freud has stated that the mourning process was an enigma. How can you mourn someone you don't recall? Is it possible to mourn a shadow? Could traumatic amnesia be considered as an identification to the deceased? Mourning therapy demands an opening up of memory, the images have to be exhibited. The ghost, within mourning therapy, might then be considered as an hallucinatory fiction, as a mean to give its figure back to the disappearance, so that the outline of the deceased can reappear. Mourning therapy could then be seen as an hallucination factory. The story of a survival, the survival of a story. The ghost is, at first, a psychic tool designed to elaborate and give life to a form. The mourning process would then be a time for reviving the ghosts, a time marked by a movement among the fossils that allows a redefining of the burial place's outline. The sepulture work creates a place where movements within the life of the psychism are made possible again. It gives a look and a voice to the tombstone. It generates a psychic space which transforms the living as well as the dead. The mourning process would then consist, for the analyst, in allowing, through transference and within transference, the creation of images that have the power to open up the word and give a voice to the mourning, an erotic one, as well as a violent, or even a comic one. Our research consists in questioning this division. We retraced the history of autism from J. Itard's description of "the savage of the Aveyron". The inquiry continued with S. Freud's influence on the scientific community at the beginning of the XX century. E. Bleuler originated the word "autism", used to describe a symptom of schizophrenia. In 1943 were published two works, taht of H. Asperger on "autistic psychopaths", and taht of L. Kanner on "autistic disturbance of affective contact". From this point on, autism aroused the interest of the scientific community. B. Bettelheim published in 1967 "The Empty Fortress", inspired by analysts such as M. Klein and D. Meltzer. Frances Tustin continues this tradition. E. Schopler, formerly a disciple of Bettelheim, became his detractor. He contests the psychogenic thesis of autism and criticizes his mentor views on the mother's role in the etiology of that entity. He incited the creation of a state program (TEACCH). Schopler is influenced by two trends of thought. On the one side the cognitive with authors such as U. Frith and S. Baron-Cohen, who attempt to establish a link between cognitive impairments and neurophysiology. They define autism as an impairment of the "theory of mind". On the other side is behaviorism : I. Lovaas works on the application of behavioral techniques to autistic disorders, without taking interset in etiology. J. Lacan's teachings introduce language structure as the basis for the subjective position. His students worked on autism in order to derive the reference points necessary for psychoanalytic intervention. His remarks are illustrated by three clinical cases. Psychoanalysis is not opposed to the existence of a somatic origin of autism. However, it considers that the subject, caught in the structure of language, cannot be reduced to his or her organism

Hypoxia and coldness take hold of the alpinist in high altitude. Psychophysiology is put to test with the potential damage of brain or lung oedema which may cause death. The field which induces accidents is dangerous for subjects trying to move forward and reach a summit over 26000 feet high. As we thought a parallel could be drawn between the phenomenology linked to the infant's distress and the one described by the himalayist climbers, we examined the Freudian psychoanalytical concept "hilflosigkeit". To the "primitive agonies" endured by the nurseling, the adult would respond with real agonies experenced when being in the field of very high altitude. The fantasy of mastering any danger is reinforced with a fantasy of omnipotence and immortality denying the symbolic castrations. The metaphore used to describe The Mountain are very often archaic and sexual. The moutain is a make-believe incarnation of the utmost "Autre" (a Lacanian concept). The subject would attempt an incestuous close-up on the heroic mode. The possibility of an access to "Das Ding" via la "jouissance" would lead straightaway to the extinction of the subject of desire, need and greed. The ordeals and feats would enable the individual to re-originate himself in an inversion of "i did not choose to be born". The illusive ideal of autonomy is backed up by the denials about risk-takings. Grown-up in search of physical as well as psychic ordeal may have been infants craving for a reassuring "holding" and later, they may have become hyperactive children. Our study refers to the clinical and psychoanalytical field. In the first part, we set up the historical context of phantasm’s notion, the emergence and evolution of the conceptions, up to the logical and topological formalization. The developments about the notion of object “a” allowed us to specify the phantasm’s situation related to concepts such as the desire and the jouissance. In a second part of the research, we introduce the question of psychosis, while arguing the radical absence of the phantasm as a fundamental structure in the subjective inscription. In the third part, we consider the possible development of a phantasm in psychosis, as attempted construction supporting the structure far from a clinical and psychopathological decompensation. We can notice this suppleance’s mode often related to the major need to achieve a cutting, splitting the subject from the object of jouissance

This Lacanian outlook undermines the neo-humanistic and scientific-like approach of psychosis promoted by the DSM, in which psychosis is either turned into a banality or chastised, placed as it is on the continuum neurosis-psychosis and disconnected from the talking cures even by many psychoanalysts. This pre-Freudian perspective sustains the scientific psychiatry as well as the neurosciences; in their prevalent discourse, hallucination is still considered as a false perception which can disappear through medication try to show here that a perspective which is both logic and inspired by the fundamentals of modern science can maintain that, for a human being, speech and the organic factor are interconnected following the three Lacanian registers, the real, the symbolic, and the imaginary. The resulting body (corps) is neither the corpse examined in laboratories nor an element of statistics. And if the subject as an effect of language vanishes within the field of the drive and of "jouissance", this does not mean that it is non-existent, since a subject can be supposed as soon as speech encounters the living body. Based on this distinction, I propose a conceptual and clinical investigation of American psychoanalysis, especially of one of its "post modern" branches, intersubjectivity. Two case studies, one by an American psychoanalyst, the other my own, are meant to demonstrate that it is the analyst's position in transference that plays the decisive role with respect to "any possible treatment of psychosis"

Der volle Inhalt der QuelleTo address the matter of the singularity of psychoanalysis compared to other approaches on human psyche, we have chosen to focus on the Freudian concept of overdetermination.Psychoanalysis has a singular place within the intellectual domain and its relationship to the matter of care. As well, psychoanalysis is being held half-way between science and philosophy. But it also seems to lead to a certain form of resistance, sometimes quite persistent and visceral which shows of its specificity. In order to study this matter we will work on a both historical and epistemological level, focusing on the notion of Overdetermination. We will see in-depth the birth of the concept in S.Freud’s work and the influence of overdetermination on Freud’s emancipation from medicine principles. We will then turn to B. Spinoza, L. Althusser and G. Bachelard, in particular to better illustrate the significance of the notion of causality in the scientific process. Lastly the work of Lacan will provide some understanding of the importance overdetermination has had in the epistemology of psychoanalysis as well as its relationship to psychopathology by introducing an irreducible degree of indetermination and a specific approach to the notions of truth and certainty. We will also revisit the relevance of overdetermination within the plethora of theories and psychoanalytical practices, as well as in their relationship to other scientific fields

Past traumas can be repeated in the psyche of subjects of the same family, as an individual or family symptoms, through the movement of an unconscious within the family. Then we are in front of a "symptom-subject" of the family which fonctions as a link in a signifying chain. The absence of paternal fonction keeps pending the symbolic System and the signifier. Hence transgenerational trace develops. Unbounded subjects, free from symbolic law, can frequently persist in acting out and self destruction. We are interested with all the metapsychology : we can see the process of "dis-idealization", the weak super-ego and ideal ego, the dominating death drive, the rift in symbolic System, the real stealing a lead over the symbolic System. We observe the subject repeating the real as a symptom in order to find the symbolic realm. The taboo works as a trauma or a debt with a symbolic transmission, an unknown heritage, according and an unconscious culpability conveyed from one generation to another. The writing of the trauma and the effect produced by the culpability leave a trace. We call it writing of the family history through the use of symptoms, as an attempt to represent traumatic effects. The repulsed psychic event will take a disastrous magnitude in the unconscious, carries on symptoms, which form a discourse, as an appeal to the symbolic System

Der volle Inhalt der QuelleThis research aims to find out what psychopathology is theorized by Sigmund Freud's psychoanalysis, how it is defined and what new developments it inaugurates. We hypothesize that Freudian psychoanalytic psychopathology is not as simple to define as if it were unique or immutable. On the contrary, we believe we will find within psychoanalysis movements and developments which make it possible to highlight different ways of describing psychological suffering. In other words, we assume that, although Freudian psychoanalysis has a unique thread that guides its theory, it does not provide a single psychopathological description, but that different readings of the disease emerged from the clinic and from each theoretical invention. To investigate our hypothesis, we divided the present work into four chapters, the first being historical, the second theoretical, the third nosological and the fourth clinical. Firstly, we will give a brief history of how the “pathos” was portrayed throughout history, and we will illustrate it through a comparison between the periods of Ancient Greece, the Middle Ages and the Modern Age. This chapter will aim to present not only the changes in the ways of portraying the disease in the transition from one era to another, but also to demonstrate the different psychopathological readings within the same historical period. In the second chapter, we will enter the Freudian psychoanalytic field itself. We will take different Freudian concepts as a basis and investigate whether these different conceptual inventions presented throughout the author's work were promoting, as we suppose, reinterpretations in his psychopathological field. In the third chapter, we will find out how Freud describes the clinical condition of melancholia. We chose melancholy because it is an issue that runs through all of Freud's work. And as its theory is marked by movements and evolutions based on the creation of new concepts, we seek to know whether this dynamic nature of the theory produces consequences in the way of conceptualizing this clinical presentation. To conclude, we will investigate seven Freudian clinical cases in an attempt to find out how they are constructed and which theoretical resources the author uses to construct his psychopathological hypotheses. From the clinic, we seek to demonstrate whether our research hypothesis is true or not. This research is based exclusively on the psychoanalytic framework and is primarily a theoretical-clinical research. Therefore, the analysis methodology is subordinated to the assumptions of this theory, which determines, in the development of this research, the exclusive use of methodological resources characteristic of theoretical-bibliographic research, such as readings, information collection, bibliographic survey, text comparison, without any empirical part or even quantification. Because the question asked concerns Freudian concepts, the sources to be used will be primarily primary, justified based on theoretical relevance and consistency and reliability to the concepts involved. Freud marked the direction of a research perspective based on psychoanalytic clinical experience. He left as a legacy a work that points to the articulation of this experience in the form of theory. And it is from this perspective that we allow ourselves to be guided in the proposal and development of this research project, articulating clinical and theory in the investigation of Freudian psychoanalytic psychopathology

The clinical outlook has often been viewed as a practical application of knowledge either in medicine or human science, this study will be devoted to the analysis of clinical facts in so far as they encompass characteristics and processes which reach beyond the circumstantial purposes be they conceptual or practical of such and such an intervention. This essay aims at questioning the structural characteristics of the clinical situation itself, and in order to do so, it will use some points that the psychoanalytical field has highlighted, it will postulate that the processes involved are likely to be those which are affected in a number of different pathologies that the theory of mediation links with some malfunctioning in the structure of the person, this essay will not seek to offer a nosographical study, but will rather intend to examine the framework which can probably be considered as a reagent capable of revealing the specific features of these pathologies. The study will endeavour to analyse the relationships and differences in method which can be noticed between, on the on an , the field of scientific approach of the human being and, on the other, that of clinical practice, it will indeed be assumed that although these two approaches can be used together in so far as each proceeds from "cultural faculties", they are to be distinguished one from the other as far as their organisation is concerned. The study will tend to draw a line between the clinical approach and its various applications, be they the therapeutics it allows or the aiming at verifying or testing procedures which places it within the frame of scientific approach. For Lacan the psychosomatic phenomena are related to the creation of a conditioned reaction of the organism through signifiers, which having frozen and therefore lost their signifying function are transformed into signs, which coming from the Other, obtain an imperative quality for the organism, thus disturbing its function. Peirce’s phaneroscopy helps us conceptualise this procedure as a semiotic reduction which proceeds from the thirdness of the signifier to the secondness of the signal (which can bring about conditioned reflexes or reactions ofa different kind) or even go further to thefirstness of complete automation. Yet, beyond the typical psychosomatic phenomena this process of semiotic reduction could also apply to mood disorders (depression, bipolar disorder, delirious mood, athymhormia, panic disorder and so on) and other clinical conditions likewise. According to Post this clinical phenomenon of the tendency towards automation f is a result ofthe kindiing neurophysiological mechanism which could explain passing from mood disorders of reactional type to more automatic mood states. Its aim is to point out essential evolution of the clinical outlook beginning with its origins, in the medicine of Antiquity and the Classical Age, up to the contemporary transformations following the “technicist” postmodern conceptions regarding suffering and psychic health. The basic contribution brought by the psychoanalytic conception at the beginning of the XXth century consists in the reintroduction of the Subject which suffers within a problematic largely dominated by an organicist determinism issued from XIXth century psychiatry. Due to Freud and to its definition of the symptom as realisation of an unconscious wish, and then as a defence as a response to anxiety, a particular conception of the subject of the unconscious is substituted for a conception purely pathological. It was then with Lacan that the path opened by Freud, concerning the real as well as the symbolic dimension of the symptom, will be actualized from a conceptual point of view, which also allowed for a treatment and clarifying of the clinic phenomena that characterise our epoch. This contribution constitutes a commentary on the set of these phenomena as well as on the logic that guides them, and it considers in detail two particular issues: depression and the Alzheimer type dementia. Following the conceptual argumentation, its main interest consists in bringing to light the clinical and ethical scope which concerns the fact of maintaining psychoanalysis as a effective offer in order to meet and treat the discontent of the subject of our times

Here is what a psychopathology of recovery mea ns as its theoretical developments are grounded on clinical data. If indeed it seems obvious, in the field of child cancerology, that physical recovery, once acknowledged and foretold by the doctor, refers only to the child that has bee n running a lethal danger, the status of what is normally called psychological recovery is far from being the same. The latter is not just at stake for the child when he or she has become an adult it also implies (if not most of the time ev en more) his parents. And our aim here is to show how, within its psychological dimension, recovery does exist only when each one recovers feeling of his identity or, to mention things with a more commonplace wording, when one is resuming the possibility of saying in everyday language : "i. . . Do. . . ". Yet physical recovery and psychological re covery are far from being easily distinguished even if they do not take place jointly. Such a time lag can be accounted for through the new surge within the parents' thoughts and fantasies of some representations thitherto repressed or conc ealed because of concerns with the child's sickness. These representations wich normally arise through the questioning o f life hopes restored to the child, stress the function of unconscious fantasies around the theme of an unescapable pros pect of death to come. Our hypothesis here held is as follows : if we may believe it pertinent to qualify such represent ations as being infanticide, we cannot accept to admit that they betray death wishes solely aimed at the real child. It intends also to show that variability of the mode of abdictive operating depends on the variability of modes of functionning and of the personality organization in which reveals (registers) the psychopathology of ties. The sample is constituted by two independent groups : a group of 23 subjects registered in a social dynamic rehabilitation situation and a group of 35 subjects in an emergency social situation. The results demonstrate the common process to abdiction (the tie's psychopathology), that depend on the personality's organization and functioning mode. The psychopathology of ties is present in 96 % of cases. The mode of functioning and organisation of the borderline personality superior level covering borderline, narcissistic and dependent personality disorders, is linked to the partial abdiction in 19 % of cases. The functioning and organization mode of borderline inferior level, that is the narcissistic, dependent, evitent and antisocial personality disorders, is linked to the sub-total abdiction in 53 % of cases. The mode of functioning of the psychotic personality as a defensive organization installed by subjects is used to fight against the intolerance of anguish and the pathologically fragmented states, is linked to total abdiction in 27 % of cases

Thus, to read clothing in the field of the symbolic shows the supposed frivolity pinned to the interest of dress. To these aggressions was added a precocious rupture of the link between mother and child posing thas a problem of a deficiency of mother love. We upheld, on the basis following the theoretical psycho-analytic model, that the newborn infant in this premature state could come out of its said state using a psychotherapy of a "materbal" type which allows the installation of an object relation. We were brougth to conclude that the psycho-sensory-moteur developpment installs it self as soon as there is a presence of another mother being which places the newborn in a subject situation with a future and this regardless of foetal age within clearly the limits of possibility

Temporal grammatical inversions pointed out in their discourse about their suicidal reaction question their subjective temporal registration. The tools of research employed to explore this hypothesis are organized in two parts : a clinical interview and some projective tests (T. A. T. , "Phrases à compléter de Stein"). This material is designed for a population made up of two adolescents who never thought of suicide and five suicidal adolescents (girls only). Pathological mourning seems to be a paradigm of the transgenerational transmission, as some suicidal behaviors through the punctuations of its processes. Others suicidal behaviors are found in the destinies of death of the sexual instinct. If the death instinct seems to be a destiny of the sexual instinct, it also appears as its negative double. The Freudian Eros is found in the instinctual one and the thought as its positive double. Thanatos is found in its negative double, covering the traumatic one, but this time as a transformable destiny of the traumatism. From the angle of the work of the transmission and the symbol's transmission, the suicidal behaviors show the effects of the impediment of that work. Three models are proposed : "metaphorical", "métonymical" and a model underlain by the myth of eternal return. Tosquelles, a concern has risen about the place and function of the question : "what does the civil service want?" in the way of "was will das weib" of S. Freud, as elaborated by J. Lacan. This concern falls upon the problem of "dejuridicisation" as a support, in the present social context, of the structure of "forclusion" of the "signifiant du nom du pere", as constitutive of the failure of the paternal metaphor, described by J. Lacan to explain that man may become mad, even as a result of "social psychosis" (J. Lacan). So, the monographic description of the positions taken by the civil service regarding a psychiatric treatement center and its collective actors, which open to psychotic patients the real possibility of being cured ( Freud gave the following definition: "to love and to work") result in an analysis of the logic of contemporary thought abolishing law and genealogy by the failure of representations and their basis: the institution and the myth supporting it. In addition, the field of psychiatric treatement of this center, considered as an attempt to restore the condition of reference to the "paternal function" and to transcendance ("madness is the failure of transcendance". Der volle Inhalt der QuelleThe impact of digital devices in psychopathology is a current issue that is fuelling many debates in the scientific literature and clinical practice. New symptoms are attributed to these technological objects questioning their effects on both social bond and subjectivities. After setting the different ways the literature review presents these issues, we show that the analytical paradigm has regularly drawn inspiration from mechanical and mechanistic theoretical models. The Lacanian subject then appears as a frontier concept, just as the Freudian unconscious, which allows us to extract ourselves from the strict human/machine binarism. We show that clinical study of psychoses' structure has, since the beginnings of psychoanalysis, taught practitioners about the (auto-)therapeutic constructions that subjects could carry out by drawing on the machine. We then question the way in which the analytical paradigm can recognize these devices as possible "digital crutches" for the clinical study of psychotic supplementary devices. On the strength of the therapeutic results obtained during digital workshops, we show that it is first and formost the position occupied by the clinician in the transference which can allows us to find therapeutic levers in these objects. This is less a digital clinical practice than a clinical use of digital media. Finally, we question the extent of to which these devices can facilitate or hinder the implementation of these "transferential partnerships". Therefore we propose some clinical and therapeutic orientations in the use of digital objects with psychotic subjects

At the crossing point of social and psychopathological logics, it mainly affects teenagers and involves a visual mark that defies classical psychiatric concepts. The circumscribed time of the emergence of this symptom opens the opportunity to study the interaction between contemporary representations of individuality and unconscious dynamics. A convergence point can be found for one part at the symbolical level, with the signification that this praxis can carry out, and for another part at the level of the body, the area of choice for the expression of the individuality and its desires. The adolescence, being precisely the moment where psychical reorganization accompanies the advent of an accomplished status of this individuality, can shed light on determinations that take part in this phenomenon. The redefinition of the individual with regard to a body that is now reduced to a mere image can be considered as one of the major consequences of social transformations. But the adolescence has its own specificities and stands at the tension point between different forces that rule our neoliberal societies. The conflicts that agitate puberty, particularly as they tend to be followed by acts, also tend to express themselves within the frame of these tensions, up to the point of reaching body mutilation. When considered in the perspective of a genealogy that links it up to self-flogging practices, this phenomenon can be considered like a form of subversion of the normalizing forces. This is why it falls under the field of "power clinic"

The second section will follow the life of Sylvie through the form of a monograph to examine what psychosis reveals about the experiences of the patient and / or their psychic structure. Issues concerning the psychotic patient can be understood through organic signs, feelings, and transference. Monitoring the effects of this work over the following period of 11 years allows an account of the benefits and limitations of the methods chosen, leading to an analysis of the progress made between disciplines and of their aporias. I will illustrate the result obtained with Sylvie in particular, while considering the limitations of this type of work. Finally I will show how this research can pave the way forward in the therapy of psychosis from a monistic perspective, taking into account the reciprocal nature and interpenetration of body and spirit

Der volle Inhalt der QuellePart one of this thesis presents various conceptualization and interpretations of the experience as well as behavior of bereavement and grief using 20th century models of psychopathology. Recent descriptive naturalistic, longitudinal and epidemiological studies of the general population helped define the clinical aspects of "normative grief", and in particular the model of "non complicated bereavement (DSM-III, DSM-IV, CIM 10). Grief normally evolves according to four stages (shock, separation anxiety and research behaviour, working through one's grief and spontaneously resolving depression, and finally a stage of recuperation), with each stage having its own characteristics. Conversely, abnormal, psychiatric, or pathological grief has been defined, and in particular the model of "complicated grief" which is characterized by chronicity as well as by psychosocial and somatic complications. The ICG (Inventory of complicated grief, Prigerson et al. , 1995-1996) is a quantitative instrument specific to grief which allows for distinguishing separation distress and traumatic distress on the one hand, and the grief reactions of depression and anxiety on the other. Using a dimensional approach, the ICG helps identify bereaved persons at high risk for psychological and somatic disorders (cancer, heart disease, etc. . . ), and its therefore of major interest in a health psychology. As twenty percent of all bereaved subjects will present with complicated grief, this represents an important public health concern. Part 2 of this thesis reviews the controversys relative to the different qualitative and/or quantitative approaches to bereavement and grief, and in particular the quantitative grief scales such as the TRIG, CBI and ICG. The 3rd part presents an individual empirical study based on a sample of 108 psychiatric inpatients compared to 100 subjects from the general population. The results of the investigation confirme the excellent construct validity, criterion validity and convergent validity of the ICG, compared to the TRIG
