Pregnant women are often excluded from routine clinical trials. Consequently, appropriate dosing regimens for majority of drugs are unknown in this population, which may lead to unexpected safety issue or insufficient efficacy in this un-studied population. Establishing evidence through the conduct of clinical studies in pregnancy is still a challenge. In recent decades, physiologically-based pharmacokinetic (PBPK) modeling has proven to be useful to support dose selection under various clinical scenarios, such as renal and/or liver impairment, drug-drug interactions, and extrapolation from adult to children. By integrating gestational-dependent physiological characteristics and drug-specific information, PBPK models can be used to predict PK during pregnancy. Population pharmacokinetic (PopPK) modeling approach also could complement pregnancy clinical studies by its ability to analyze sparse sampling data. In the past five years, PBPK and PopPK approaches for pregnancy have made significant progress. We reviewed recent progress, challenges and potential solutions for the application of PBPK, PopPK, and exposure-response analysis in clinical drug development for pregnancy.

The objective of this work is to investigate the influence of combined application of prostaglandin and oxytocin on the duration of farrowing in sows, number of newborn piglets and interval between two expulsions of piglets. The experiment was carried out on 133 pregnant sows of Danish Landrace-Yorkshire Large white breed, which were divided into three groups depending on the day of the induction of partus (112th, 113th and 114th day of pregnancy). Within each group, the sows were divided into three subgroups, and they were given synthetic prostaglandin PGF2-alfa (dinoprost-DINOLYTIC) analogue in dosage of 10 mg (eq. 2ml of preparation)/animal (I subgroup) single or combined with one (after having fifth piglet) (II subgroup), or two (after having fifth and tenth piglet) (III subgroup) application of oxytocin (OXYTOKEL) in dosage of 20 i.j. + 15 i.j. (eq. 2 ml + 1,5 ml preparation/animal), (two applications). Obtained results show that in the sows treated only with prostaglandin, the longest farrowing (6,41?2,20 h) was in those which obtained the drug on the 112th day of pregnancy, and the shortest (4,56?1,30 h) in the ones treated on the 114th day of pregnancy. In the the sows in which (beside prostaglandin) oxytocin was administered once, the longest farrowing (6,38?1,30) was in those treated on the 113th day of pregnancy, and the shortest (6,38?1,30) in those treated on the 114th day of pregnancy. In the sows treated (beside prostaglandin) with oxytocin twice, the longest farrowing (7,17?2,30) was in the group treated on the 112th day of pregnancy, and the shortest (5,10?1,20) in the ones treated on the 113th day of pregnancy. Generally speaking, the longest duration of farrowing (7,17?2,30) was in the animals which (beside prostaglandin) obtained oxytocin twice, and the shortest duration (4,56?1,30) was in the ones which (beside prostaglandin) were treated only with prostaglandin. The greatest number of newborn piglets (358) was in sows which obtained prostaglandin on the 114th day of pregnancy combined with one application of oxytocin, and the smallest number (250) in sows which obtained perostaglandin on the 112th day of pregnancy combined with two application of oxytocin. The longest interval (0,22 ? 0,06) between two expulsions of piglets was in sows in which partus was induced on the 112th day of pregnancy in combination with two applications of oxytocin, and the shortest interval (0,11?0,03) in which partus was induced on the 114th day of pregnancy, witout oxytocin application.

Bringing a new life to the world is a wonder to every mother. Experience faced by pregnant mothers vary from one pregnancy to another pregnancy. There is a vast amount of information available on the Internet and printed materials. Yet, this knowledge is too complex or lengthy and very few applications provide customized information to pregnant mothers. In a time where smart phones have become a necessity in our life, a mobile app is one of the easiest ways to obtain prenatal information. Hence, we have developed a mobile application to help pregnant mothers. This application includes an artificial intelligence (AI) based chatbot. AI chat bot communicates and guides the mother in a way that creates the illusion as if they are talking to their unborn child. The Spiral Model was used as the development methodology and the application was developed in an environment of continuous integration and deployment with GIT personal repository. This application was implemented using React Native and the Node.js. Chat bot was created with Dialogflow agent and integrated with the firebase through the Google cloud functions. Some existing applications were studied to identify the features and limitations of current pregnancy care mobile applications. This solution is realistic and successful and it has an upgradable model of growth. The rapid development of the Internet and mobile devices in the world has changed people's lifestyles. This mobile app will be helpful for pregnant mothers living in rural as well as in metropolitan areas alike and can enhance education and health.

Ultrasonography is becoming a useful diagnostic toolin the field of cattle pathophysiology, aiding to meet the demands of the expanding cattle breeding. Transrectal ultrasonography is a rapid, reliable and safe technique for both the animal and the operator. With the use of ultrasonography in research, the hypothesis concerning the wave - like pattern of follicular growth has been confirmed. In addition it has been proved that follicular growth, development and regression continue during early pregnancy. The application of ultrasonographic techniques in concert with modern biotechnology, boosted the production of embryos from genetically superior cows and prepubertal heifers and indirectly solved the problem of infertility in valuable animals. In clinical practice, the use of ultrasonography for early pregnancy diagnosis and for diagnosis of pathologic conditions which lead to prolonged postpartum anoestrous, makes a considerable contribution to the reduction of calving interval. In this review basic physical principles of ultrasonography are described and selected applications in the field of cow reproduction are discussed.

BACKGROUND: Early detection of risk factors for pregnancy is carried out through regular prenatal examinations from an early age to detect abnormalities/disorders/illnesses suffered by pregnant women early. Mobile applications are very helpful in the process of early detection and recording of health workers’ reports, especially during a pandemic. AIM: This study aims to assess the knowledge, attitudes, actions, and skills of pregnant women in the early detection of high-risk pregnancy before and after the intervention and the effectiveness of M.D-Risti modules and applications. METHODS: This study used a quasi-experiment design research design with a pre-post only group design consisting of an intervention group and a control group at 4 health centers in Palu City with a purposive sampling technique of 43 respondents per group with univariate and bivariate analysis. RESULTS: The results showed that there was a significant difference in the level of knowledge (p = 0.00), attitude (p = 0.00), action (p = 0.00), and the skills of pregnant women in early detection of pregnancy (p = 0.00) before and after treatment in the intervention group. There was a significant difference between the two groups with a significant value on knowledge (p = 0.00), attitude (p = 0.00), action (p = 0.00), and skills of pregnant women in doing early detection of pregnancy risk independently (p = 0.00). CONCLUSION: The intervention of M.D-Risti modules and applications had a significant effect on changes in knowledge, attitudes, actions, and skills of pregnant women in early detection of pregnancy risk compared to pregnant women who were not given intervention at all. The M.D-Risti application is one of the solutions during the COVID-19 pandemic and in the long term, all health centers in Palu city can use it to help monitor the risk of pregnancy more quickly

Angiogenesis is one of the main processes that coordinate the biological events leading to a successful pregnancy, and its imbalance characterizes several pregnancy-related diseases, including preeclampsia. Intracellular interactions via extracellular vesicles (EVs) contribute to pregnancy’s physiology and pathophysiology, and to the fetal–maternal interaction. The present review outlines the implications of EV-mediated crosstalk in the angiogenic process in healthy pregnancy and its dysregulation in preeclampsia. In particular, the effect of EVs derived from gestational tissues in pro and anti-angiogenic processes in the physiological and pathological setting is described. Moreover, the application of EVs from placental stem cells in the clinical setting is reported.

Annotation:
The present costs and efficiencies of producing cloned embryos, pregnancies and offspring using the simplified nuclear transfer techniques developed in the authors’ laboratories are compared with those required for the large-scale application of such cloning technologies in cattle. The current costs in the laboratory of producing large numbers of genetically identical cloned embryos for transfer is around $15.00 per blastocyst, which is within the cost estimated to be commercially viable for cloned female dairy embryos for transfer. However, the pregnancy and calving rates from the transfer of such embryos are still well below that required for large-scale commercial application for which ongoing pregnancy rates of at least 50% per recipient will be required. The current pregnancy rate (30–40 days post-transfer) following the transfer of an average of three cloned embryos per recipient is 37%, and the calving rate 17%, representing high losses between pregnancy diagnosis and term. In the beef industry and in some dairy situations the final product (cloned bulls for natural mating) will have a much higher inherent value and different parameters will therefore apply. Recent developments in the technologies that are likely to increase the probabilities of large-scale application are discussed, including recycling nuclear transfer embryos, somatic cell cloning, new cryopreservation techniques and automated oocyte harvesting.

Background: The maternal mortality rate is still very high. This also happens in other countries in the world. This situation causes anxiety for each country. One of the causes of death in pregnant women is maternal knowledge about health information during pregnancy. The use of cellular-based applications is currently the leading choice in finding information, and the same thing also happens in pregnant women. Pregnant women currently use many cellular-based applications to access information about pregnancy. Aim: To find out how pregnant women use cellular-based applications as a medium of information in pregnancy. Method: Five review stages are used in this review. The five stages include: identifying research questions, identifying relevant studies, choosing studies, mapping data, compiling, summarizing, and reporting the results. Result: A total of 23 publications were included. Thematic analysis is used to identify key concepts. The International Journal Of Alcohol And Drug Research, 5(3), 131-139. doi:http://dx.doi.org/10.7895/ijadr.v5i3.219Aims: To analyse how social services relate to compulsory care legislation in applications for compulsory care for substance abuse in cases involving pregnant women, given that such commitments cannot be made solely for the sake of the fetus/unborn child.Design: Applications for compulsory care to administrative courts involving pregnant women categorized according to how the pregnancy was presented or emphasized.Setting: Compulsory care for substance abuse in Sweden.Participants: 116 cases involving 107 individuals who were pregnant at the time of application for commitment to compulsory care between 2000 and 2009.Findings: In 43% of the cases the risks for both the woman and the fetus’/child’s health was emphasized. In 28% of the cases the applications were primarily for the sake of the fetus/child. In 17% of the cases the pregnancy was mentioned in a neutral manner, while in 8% of the cases the fact that the woman was abusing substances during pregnancy was presented as an indication of the severity of the problem. Conclusions: References were commonly made to the interests of the woman and the fetus/child as an entity, but social services also openly claimed the need for commitment primarily for the sake of the child. Arguments also mirrored the debate when legislation was first introduced; for example, that substance abuse during pregnancy is a clear indication of how serious the problem is, and how this situation may be hazardous to the woman’s mental health should she become the cause of severe and irrevocable harm to her child.

ABSTRACT Cell-free fetal DNA (cffDNA) is available in the maternal circulation throughout pregnancy and can be used for noninvasive prenatal diagnosis including, determination of fetal sex, identification of specific single gene disorders, typing of fetal blood groups (RhD), paternity determination and potentially routine use for Down's syndrome (DS) testing of all pregnancies. I searched published literature on the PubMed and databases on Scopus interface systematically using keyword's cffDNA, noninvasive diagnosis, fetal DNA in the maternal serum. Reference lists from the papers were also searched. cffDNA representing only 3% of the total cell-free circulating DNA in early and rising to 12% in late pregnancy, clinical investigations has already demonstrated the potential advantage, such as improving safety, earlier diagnosis and comparative ease of testing using cffDNA technology. The discovery of cffDNA circulating in the maternal serum has opened the door to noninvasive prenatal diagnosis testing with novel clinical implications. How to cite this article Ghorbian S. Background. Miscarriage is a very common complication during early pregnancy. So far, clinical therapies have limitation in preventing the early pregnancy loss. Chinese Medicine, regarded as gentle, effective, and safe, has become popular and common as a complementary and alternative treatment for miscarriages. However, the evidence to support its therapeutic efficacy and safety is still very limited. Objectives and Methods. To summarize the clinical application of Chinese Medicine for pregnancy and provide scientific evidence on the efficacy and safety of Chinese medicines for miscarriage, we located all the relevant pieces of literature on the clinical applications of Chinese Medicine for miscarriage and worked out this systematic review.Results. 339,792 pieces of literature were identified, but no placebo was included and only few studies were selected for systematic review and conducted for meta-analysis. A combination of Chinese medicines and Western medicines was more effective than Chinese medicines alone. No specific safety problem was reported, but potential adverse events by certain medicines were identified.Conclusions. Studies vary considerably in design, interventions, and outcome measures; therefore conclusive results remain elusive. Large scales of randomized controlled trials and more scientific evidences are still necessary to confirm the efficacy and safety of Chinese medicines during early pregnancy.

Studies of the biology and behavior of nonhuman primates have played an integral role in biomedical research, from the development of models of human disease, injury, and brain function, to toxicity testing and the evaluation of novel environments. In most situations primatologists are consulted for their expertise in the anatomy, physiology, genetics, behavior, and ecology of human and nonhuman primates. Opportunities for applied primatologists as consultants, salaried research scientists, data analysts, and administrators exist in a wide range of nonacademic settings including medical centers, nonprofit research organizations, commercial animal supply houses, government agencies, museums, and zoos. Several broad topical areas are usually of interest: 1) behavior, including observation and measurement of psychological and social variables in projects such as pharmacological/toxicological testing, child abuse, and surgery; 2) nutrition, including causes, effects, and prevention of dietary deficiencies and obesity, and their interaction with disease; 3) reproduction, including documentation and measurement of sexual cycles, fertility, pregnancy, perinatal studies, and birth control; 4) human chronic and infectious disease models; and 5) miscellaneous studies on genetics, exercise physiology, aging, artificial limbs, organ transplants, and vehicular safety.

ObjectivesIt is the aim of the current research to identify some common functionalities of postnatal application, and to determine the quality of the information content of postnatal depression application using validated scales that have been applied for applications in other specialties.Settings and participantsTo determine the information quality of the postnatal depression smartphone applications, the two most widely used smartphone application stores, namely Apple iTunes as well as Google Android Play store, were searched between 20May and 31 May. No participants were involved. The inclusion criteria for the application were that it must have been searchable using the keywords ‘postnatal’, ‘pregnancy’, ‘perinatal’, ‘postpartum’ and ‘depression’, and must be in English language.InterventionThe Silberg Scale was used in the assessment of the information quality of the smartphone applications.Primary and secondary outcomes measureThe information quality score was the primary outcome measure.ResultsOur current results highlighted that while there is currently a myriad of applications, only 14 applications are specifically focused on postnatal depression. In addition, the majority of the currently available applications on the store have only disclosed their last date of modification as well as ownership. There remain very limited disclosures about the information of the authors, as well as the references for the information included in the application itself. The average score for the Silberg Scale for the postnatal applications we have analysed is 3.0.ConclusionsThere remains a need for healthcare professionals and developers to jointly conceptualise new applications with better information quality and evidence base.

Abstract Background: Molecular analysis of plasma DNA during human pregnancy has led to the discovery that maternal plasma contains both fetal and maternal DNA. This valuable source of fetal DNA opens up new possibilities for noninvasive prenatal diagnosis. Approach: Published data from the last 3 years demonstrating the feasibility and utility of analyzing fetal DNA in maternal plasma are reviewed. Content: The detection of fetal DNA in maternal plasma is much simpler and more robust than detecting fetal nucleated cells in maternal blood, and does not require prior enrichment. This approach has been shown to have application in the prenatal diagnosis of fetal rhesus D status, sex-linked disorders, and other paternally inherited genetic disorders. Abnormal fetal DNA concentrations in maternal plasma and serum have been found in common pregnancy-associated disorders, including preterm labor and preeclampsia, as well as in pregnancies complicated by fetal trisomy 21. After delivery, fetal DNA is cleared rapidly from maternal plasma, with a half-life in the order of minutes. These clearance kinetics exhibit an important difference from fetal cell clearance, where long-term persistence has been demonstrated. Summary: It has been only 3 years since fetal DNA was first detected in maternal plasma, and much remains to be learned about the biology of this phenomenon. In addition, additional diagnostic applications beyond those discussed here can be expected in the near future.

In this experimental study, we tested the hypothesis that exogenous progesterone and GnRH applications would prevent the detrimental effect of prostaglandin F2α (PGF2α; a) on early pregnancy in ewes by sustaining high plasma progesterone level. For this purpose, 9 pregnant ewes (mating day = 0) were divided into 2 groups on Day 18 as follows: (1) PGF2α group (125 μg of d-cloprostenol injection on Day 18, n = 5); (2) PGF2α + progesterone sponge group (125 μg of d-cloprostenol injection on Day 18 + 20 mg flugestone acetate for 7 days, n = 4). Moreover, gonadotropin-releasing hormone (GnRH; 10 μg of buserelin acetate, n = 4) was injected intramuscularly to PGF2α + progesterone sponge group ewes on Day 22 after mating to induce luteinization in dominant follicles. The progesterone sponge was withdrawn on Day 25. Plasma progesterone (P4) concentration was measured on Days 18 and 19 by ELISA. Pregnancies were examined by using transrectal ultrasonography on Days 18, 22, 25, and 35 after mating. Statistical difference between groups was analysed by Chi-squared test. P4 concentration declined to below 1 ng mL–1 on Day 19 in both groups. While all pregnancies were terminated (5/5) in the PGF2α group by Day 25, progesterone sponge application prevented pregnancy loss (4/4) in the PGF2α + progesterone sponge group (P < 0.02). When the progesterone sponge was withdrawn on Day 25, half of the pregnancies (2/4) continued after GnRH application in the PGF2α + progesterone sponge group until birth. In conclusion, results suggest that exogenous progesterone application is sufficient for maintaining early pregnancy in ewes, even if the corpus luteum is regressed. Furthermore, GnRH application on Day 22 after mating might luteinize dominant follicles and could be sufficient to maintain pregnancy after exogenous progesterone is removed.

Pregnancy care is a topic of interest for both academia and practitioners. Novel pervasive technologies and applications, such as mobile technologies, wearables and IoT, open a wide corpus of possibilities for fostering pregnancy care management, and reducing risks and problems, improving communication among stakeholders and society development. This article introduces a model of a pregnancy care IT ecosystem based on the integration of various services in a semantically enriched e-health ecosystem. As proof of concept, both the web and mobile applications that aim to help pregnant women and gynaecologists were designed and employed in a real environment. An evaluation of the developed ecosystem was performed on a sample of 500 pregnant women and 100 doctors. After pilot usage, a survey was used to collect the data from participants, and assess the acceptance of the developed system. Results show that quality, usability and usefulness are on a high level, and that both pregnant women and doctors are ready for more extensive use of the system. In addition, research findings imply that employing pervasive technologies could bring significant benefits to all the parties in pregnancy care systems.

Gestational diabetes mellitus (GDM) is frequently described as glucose intolerance for pregnancy women. GDM patients currently practice the traditional method (record book) for recording blood glucose readings and keeping track of meal intake. This practice is not efficient and impractical for monitoring glucose level for GDM patients when we compared with mobile health monitoring technologies available today. Although, many applications have been developed for diabetes patients, but we do not found any application appropriate for GDM monitoring. In this study, we describe the design and development of mobile application for GDM monitoring using genetic algorithm that aims to predict recommended meal intake. We developed the mobile application for the GDM patients to maintain their blood glucose level through their meals. We tested the components of the mobile application and found that the prediction algorithm has successfully predicted the next meal intake according to the patient blood glucose levels. We hope this study will encourage research on development of selfmonitoring applications to improve blood glucose control for GDM.

Excessive gestational weight gain is a prevalent problem and an independent predictor of future obesity in both mothers and offspring. Intervention research to prevent excessive gestational weight gain is still in its infancy but results to date have been quite modest. Research in weight control outside of pregnancy over the past 30 years has been more robust and identified several key components of effective programs, including use of caloric restriction, daily diet self-monitoring, self-weighing, behavior therapy and ongoing patient–provider contact. The aim of this article is to summarize intervention components shown to be effective in promoting successful weight control outside of pregnancy and explore potential applications in pregnancy. Available evidence suggests that the strategies shown to be effective for weight control outside of pregnancy may also promote better weight control in pregnancy, but several lines for future investigation remain.
