Abstract Prostate cancer is distinguished by unique histological alterations in glandular architecture, observable in Whole Slide Images (WSIs). The disease is further characterized by pronounced genomic instability, exemplified by biomarkers such as DNA ploidy. While the Gleason Score effectively assesses the histological changes, the manual evaluation of DNA ploidy is subjective and time-consuming. Addressing the need for an objective diagnostic tool, our study developed a deep learning model aimed at accurately predicting DNA ploidy using publicly available WSI from the Cancer Genome Atlas (TCGA). Utilizing a ResNet-18 convolutional neural network, we trained the model on WSIs from 200 TCGA patients to extract features and predict total mRNA expression. The model was further refined using data from 19 TCGA patients, focusing on ploidy number prediction. Comparative analysis against a random reference model revealed significant improvements of ploidy prediction in Mean Absolute Error (MAE) by 43.52%, Mean Absolute Percentage Error (MAPE) by 43.59%, and Mean Squared Error (MSE) by 67.86%. These enhancements were statistically validated (p-values: MAE - 7.07E-06, MAPE - 7.69E-06, MSE - 3.14E-05) and substantiated by large Cohen's d values (MAE - 2.88, MAPE - 2.86, MSE - 2.42), confirming the model's advanced predictive accuracy and practical utility. This approach, validated further on a separate TCGA hold-out test set, signifies a major leap in prostate cancer diagnostics. By integrating mRNA prediction checkpoints, our model not only refines ploidy assessment but also sets the stage for correlating these predictions with patient clinical outcomes. These developments promise to enhance existing diagnostic methods, offering a more objective and efficient tool for DNA ploidy assessment in prostate cancer prognosis and treatment planning. Abstract Although the development of single-cell RNAseq (scRNAseq) has improved the knowledge of the tumor microenvironment (TME), there is not a clear understanding of specific cell subtypes, markers that define these cell populations, nor the interaction between them. Previously, we showed that inflammasome activation is correlated with antitumor responses triggered by immune checkpoint blockade (ICB). This highlights a central role for TMEM176B, mostly expressed in myeloid cells, that impairs the recruitment of cytotoxic immune cells in the TME by inhibiting NLRP3 inflammasome activation. Still, the roles of the different subsets of myeloid and T cells in the TME and to what extent they are connected remain controversial. Here, we used scRNAseq data from human tumors to identify and characterize cDC2, Th17 and exhausted CD8+ T cell subpopulations in cancer. Five scRNAseq data sets of melanoma, basal cell carcinoma, breast, lung, colorectal, and ovary cancer patients were integrated, accounting for 382,019 immune cells in total. We applied and compared different bioinformatic tools for scRNASeq data normalization and data sets integration to find conserved immune cell populations across samples, data sets, and cancer types. Then, we evaluated different parameters using well-established methods for clustering and cell type classification to identify subtypes of DCs and T cells in the integrated data. We combined automatic methods with manual analysis of cluster markers to improve robustness and confidence in the cell populations identified. Three clusters of cDC2 were defined and characterized using different scRNAseq analysis tools for pathway enrichment analysis and transcription factors (TF) activation prediction. We found a subset of inflammatory cDC2 with high gene expression of IL1B and NLRP3, in contrast with another cluster characterized by high expression of TMEM176B and CD14. A third cluster showed an intermediate transcriptomic profile. The TMEM176B+ CD14+ cluster was characterized by the activation of JAK-STAT and hypoxia pathways, and the TF FOXL2, ETS2, and ESR2. The NLRP3+ IL1B+ cluster showed more activation of MAPK, Trail, and PI3K pathways, and the TF CEBPE. Moreover, the activation of TNFα and NFkB signaling pathways was found in both clusters. In addition, a conserved population of Th17 cells in the integrated data was observed. Th17 cells were heterogeneous, making the subpopulations and transcriptome signature difficult to define. Further analysis is being performed to identify Th17 subsets and their potential interactions with the cDC2 subclusters. A robust identification and characterization of these immune cell subsets will build towards our understanding of the interactions between these subpopulations triggered by inflammasome activation within the TME and the mechanisms behind ICB response. Abstract Introduction: Multiplexed imaging at single-cell resolution is becoming widely used to decipher the role of the cellular microenvironment in cancer and other complex diseases. To identify spatial patterns of single cells on a tissue, accurate cell-type phenotyping is a crucial step. We present Tribus, an interactive, semi-supervised classifier that avoids thresholds and manual labeling, with user-friendly visualization of the results and automated quality control reports. Materials and Methods: We developed Tribus, an open-source Python 3 software utilizing self-organizing maps for data clustering with an integrated Napari plug-in for fast visualization. It assigns the cell phenotypes based on user-defined label description tables and median marker expressions, following a hierarchical structure to minimize overspill bias. It has a modular architecture and can be run via command line, Jupyter Notebook, or the Napari interface. For exploring the Tribus performance, we used 1) manually expert annotated datasets with different tissue origins 2) dataset with cluster-based annotation 3) previously unpublished dataset. To ensure usability, we utilized datasets generated by using different multiplexed imaging technologies, and of variable size including large tissue microarrays and whole-slide images. Results: Using Tribus we accurately annotated the cell phenotypes in an expert-labeled Co-detection by indexing (CODEX) dataset, consisting of 8 regions of tissue samples from a single donor, including both healthy colon and small bowel sections. Tribus analysis of a high-grade ovarian cancer (HGSC) tumor microarray dataset generated by tCyCIF revealed a subset of lymphoid cells previously mislabelled using a cluster annotation approach. Finally, we utilized Tribus for deep cell phenotyping on a large whole-slide image tCyCIF dataset consisting of over a million cells from five matched HGSC samples collected before and after neoadjuvant chemotherapy (NACT). In the paired samples we successfully identified cell subtypes crucial for immune surveillance. Our analysis accurately differentiated M1 and M2 macrophage subpopulations. In addition, the analyzed post-NACT samples showed a higher proportion of tumor-infiltrating CD8+ T-cells, interestingly displaying occasional spatial clusters by the tumor borders. Therefore, Tribus has shown a great potential to study the spatial patterns and cell-cell interactions of relevant immune cells in response to neoadjuvant chemotherapy treatment. Conclusion: Overall, Tribus is an accurate, fast and efficient way to analyze different sizes of datasets across different technologies, resulting in accelerated analysis of the spatial tumor microenvironment (TME). The accurate cell-type phenotyping with Tribus uncovered high-resolution cellular phenotypes and their spatial patterns in the TME in ovarian cancer after neoadjuvant chemotherapy. Abstract Altitude is an important factor when explaining species distributions. However, many biogeographical studies register altitude directly through topographic maps; a time-consuming manual process prone to mistakes. Currently, GIS and DEMs allow this work to be done more efficiently. Two methods for registering altitude for locations of amphibians and reptiles in the Iberian Peninsula were compared in order to evaluate their efficiency and similarity: manually through topographic maps, and automatically with a GIS, where two DEMs were analysed: the SRTM DEM and a DEM generated through topographic maps. The SRTM altitudes were similar to those recorded by the manual method (only Rana perezi presented differences), whereas the other DEM had differences in almost all amphibians and three reptiles. The differences between both DEMs corresponded mostly to rivers and band effect of radar sensor. The GIS method was faster, efficient and no time was consumed in searching and correcting human errors.

The design of domestic environments is fraught with decision-making, a process often dictated by fashion. The resulting inhabitation of domestic spaces blends the routine and the banal, with occasional forays into the extraordinary. The spaces of the domesticity range from single rooms to elaborate palaces. These can be functionally prescribed or open-ended, they support furniture, décor, behaviours, and narratives. The writer Georges Perec (1936–82) provides a way of looking at the domestic realm and ordinary life through his many inter-related writings on the subject. In his quest for an ‘anthropology of everyday life’, he explored notions of the ‘ordinary’ and ‘infraordinary’. In this text two important works by Perec are examined to explore how he framed and questioned notions of domesticity; can this reading be construed as a theory of domesticity?Perec’s text Species of Spaces describes a spatial continuity between city and dwelling that is characterised by spatial types, thresholds/boundaries, objects, and everyday practices often of an autobiographical nature. He begins with the page, ascends through the apartment building and the city, and ends with the world in a sequence of embedded spatial conditions. A close read of Species of Spaces uncovers a kind of sociological work, a critique or manifesto, and an evolution from Perec’s previous writings. In the text he asks the most fundamental questions, such as ‘What does it mean, to live in a room?’In his monumental text Life A User’s Manual, Perec examines the lives of residents in a typical Parisian apartment building, it remains one of the most significant imaginings of how a building, or work of architecture, can be occupied. Through the vast scope of Perec’s project the book captures the intertwining lives of the occupants of the building at 11 Rue Simon-Crubellier, in Paris’s 17th arrondissement, at precisely 8:00 pm on 23 June 1975. To accomplish this Perec devised forty-two ‘preprogrammed’ factors to structure each of the ninety-nine chapters to ensure that he covered plot, actors, and setting in a systematic way. The apartment building ultimately provides an armature for the study of the very small to the very large.Life A User’s Manual describes a domestic world, how we organise our residences into compartments of space, how we furnish the rooms, how our stories create our realities, and how the lives of people in an ordinary apartment building intertwine in so many ways. Although frozen in a moment, the novel captures the vagaries and complexities of the everyday. It describes the routines of living, unexpected happenings, the connections between people living together at close quarters, the role of interiors in defining a particular period, the histories that can support and damage a life, the common aspirations and tragedies of urban dwellers, and so on. Perec's work does not constitute an actual theory of domesticity, although it precisely describes a domestic order that provides a sense of place by attending to both the minor and major aspects of an environment.

Permasalahan mitra dalam bidang produksi, mitra menggunakan alat penggiling yang masih manual dengan pemutar tuas untuk menghancurkan singkong menjadi pasta singkong. Mitra sering mengeluhkan sakit pada bahu karena memutar alatnya secara manual. Selain itu, waktu penggilingan juga lebih lama dan kurang efisien. Selain itu, dari segi kualitas, kurangnya pengetahuan mitra mengenai batas maksimum penggunaan bahan pewarna makanan sintetis. Hasil dari pengabdian ini adalah pengadaan alat penggiling/pelumat singkong dengan penambahan mesin dinamo, pendampingan penggunaan alat penggiling dengan dinamo, serta pelaksanaan penyuluhan kepada mitra mengenai penggunaan bahan pewarna makanan sintetis yang aman. Alat penggiling singkong dengan penambahan mesin dinamo ini dapat meningkatkan produksi sebesar 60% dibanding alat penggiling secara manual, yaitu yang awalnya proses penggilingan 10 kg / jam, bisa meningkat menjadi 30 kg / jam. Kemudian dalam meningkatkan kualitas dan keamanan pangan gethuk lindri, batas maksimum penggunaan pewarna makanan sintetis seperti tartrazine adalah 70 mg / kg makanan atau 1 sendok teh peres untuk 43 kg adonan atau berdasarkan ADI 0 – 7,5 mg/kg berat badan.

Background:Dual Energy CT Scan (DECT) can detect monosodium urate crystal deposits in multiple tissues. EULAR gout guidelines (Richette, 2020) recognized the value of DECT in making a clinical diagnosis when joint aspiration is difficult. DECT shows crystal deposits in almost 50% of gout patients without tophi (Dalbeth, 2017). Tophi are known to predict all-cause and cardiovascular mortality (Vincent 2017, Perez-Ruiz 2013) and it is plausible that DECT could as well. A prognostic measure should be reliable and valid. DECT validity would be evident for death, disability and distress.Objectives:This study used a best evidence synthesis approach to synthesize the evidence for DECT as a prognostic measure in gout.Methods:PUBMED and EMBASE were searched from initiation to December 2019; keywords (Dual Energy Computed Tomography OR DECT, gout, tophaceous gout, chronic gout, monosodium urate crystals OR monosodium urate burden OR tophi OR monosodium urate volume OR flares OR pain OR distress OR death OR disability OR function). Human studies in English were considered. Titles, abstracts and full articles were reviewed. A manual search of secondary sources was conducted. Key gaps identified were considered throughout 2020 when reviewing emerging articles and presentations. Data extraction was conducted by both authors; data presented represents consensus.Results:Of 344 articles, 81 titles/abstracts met screening inclusion criteria (24%) in the 2019 search; review of the full manuscript led to 41 articles selected (51%). Additionally, 3 key papers and 2 ACR 2020 presentations were identified through 2020. DECT is highly reliable with inter-class correlation coefficients >0.9. DECT has content validity. Dalbeth (2015) showed DECT and X-Rays findings correlated in tophaceous patients, r=0.70, p<0.001. Hand function correlates with DECT burden, with r2=0.59, p=0.024 (Dalbeth 2007). Dalbeth (2017) showed DECT associated with greater flares at 3 and 12 months (p<0.01) in 152 patients. Pascart (2018) confirmed that subjects with flares had nearly doubled DECT feet volumes (0.9 vs 2.1 cm3, p=0.05) versus those not flaring. Dalbeth (2017) showed abnormal DECT scans occurred in 47% of patients with normal uric acid (<6.0 mg/dL) without palpable tophi and in 90% with elevated uric acid and palpable tophi. DECT is very sensitive to change (Araujo 2015) with 95% volume reduction in 152 patients on pegloticase treated up to 12 months. Three studies show DECT is correlated to cardiovascular risk factor prevalence (Pascart 2020, Gamala 2018, Lee 2017). Marty-Ané reported that DECT volume predicts mortality (Marty-Ané ACR 2020). Limited evidence from 3 studies suggests that the minimum important volume for DECT is 1.0 cm3 at feet and ankles, including Pascart 2020.Conclusion:DECT imaging is highly reliable, has evidence for content validity and is highly sensitive to change. DECT appears to predict future gout flares, cardiovascular risk factor prevalence and mortality. Minimum important DECT volume approximates 1.0 cm3. Este ensayo se inspira en la crítica como sabotaje de Manuel Asensi para analizarlas colecciones de poemas La muerte en Beverly Hills y De “Extraña Fruta” y otros poemas de PereGimferrer. Su poesía juvenil anticipa una plétora de obras literarias y fílmicas españolas querecurren a un arte técnicamente reproducido (en términos de Walter Benjamin) –específicamentelos discos del jazz y el cine negro clásico– para modelizar sujetos y acciones antitéticos alas tendencias fascistas de la historia. La modelización de nuevos sujetos se logra gracias a unaidentificación afectiva (lo que Asensi llama afepto) facilitada por una experiencia no cronológicadel tiempo asociada con el cine. Esta poesía transmite un modelo del mundo preocupado conel subalterno histórico luchando por encontrar su voz.

The purpose of this sketch to present Georges Perec’s work in the context of drama. As Perec wrote just one play for the theater, it’s hard to call him a dramatist. However, it is easy to see that all the literary tricks he uses in his works are a part of a well-thought-out game. The author of Life a User’s Manual employs and interweaves various literary conventions, prompts dialogue between his own works and draws the reader into the very center of his tricks, nuances and simulations. Perec’s writing could be deemed a pure act of dramatization, where the author himself, to some extent, takes on the role of director, making his audience not so much the recipients of his texts but the actors performing them.

He suggests that filmmakers such as Jonathan Perel, whose Corporate Accountability (2021) he discusses at length, extend this notion of accountability not only to their subjects but to their audience through untraditional approaches that force viewers to sit with their discomfort and actively engage with the images before them.

This article tries to describe the achievements of general linguistics within Catalan culture from the mid-19th to the middle of the 20th century. The main linguistic directions that manifested themselves in the Catalan countries during this period and their main authors are the following: comparative linguistics with Lluís Nicolau d'Olwer, idealism with Manuel de Montoliu and structuralism with Ricard Aragó. The German influence is particularly present in the first two directions mentioned. During this time, the work of Pere Barnils on experimental phonetics as well as that of Antoni M. Alcover on dialectology and Alexandre Galí on didactics of syntax emerged.

Jean Monnet, one of the founding fathers of the European Union, once said that “Nothing is possible without men, but nothing lasts without institutions”. This is also true for academic initiatives. Subsiguientemente se propone el análisis de varios textos literarios vinculados a ese republicanismo radical, y se aportan indicios sobre el modo en que su interpretación y su uso eran presumiblemente orientados por el contexto periodístico en el que esos textos vieron originalmente la luz.

Demostraremos que lamayoría de ellos no representan verdaderos problemas, sino que son elresultado de la negligencia y la pereza por parte de los ajustadores, queintentan alcanzar un perfecto sincronismo labial sin tener en cuenta lasnecesidades de una comunicación real.

Through a careful analysis of official government documents such as national statutes, national agency memorandum orders, policy directives reports, and national and local plans, among others, pertinent to the Philippine Government's response to climate change mitigation and adaptation, the paper elucidated the policy directions of the country concerning climate change response. Aimed at proposing recommendations to enhance the current legal, organizational, and fiscal frameworks of the Philippine Government in its efforts to mainstream its climate change strategy, the paper identified some of the gaps and possible areas of improvement in the standing legal and structural constellations of policies employed in the country. It has been found by the research that though there have been actions related to the international mandate of promoting sustainable development, there are still areas that can be reformed and revisited if the government programs and institutions are to remain in fidelity to the commitments of the Philippines to combat the impacts and mitigate the effects of climate change. Euthanasia or assisted suicide are now legal in several European countries. Spain has approved related legislation in 2022, and in Portugal the subject is currently under debate after preliminary ratification. This remains a controversial topic, raising passionate discussions that sometimes hamper the need to consider, in depth, the ethical, clinical, and operational difficulties of processes of this type. Older age people do not necessarily lose their autonomy with age, nor should they lose their right to decide on euthanasia or assisted suicide whenever the jurisdiction allows it. However, there is a growing concern of older people being coerced into decisions about ending their lives, not least due to their own fears of becoming a burden both for their families and the society. Manuel Martín-Carrasco will analyze how ageism can influence or model a wish to die in elderly people, especially those living with greater physical and/or mental disability. The so-called rational suicide focuses on the possibility that a healthy subject eventually decides to end his own life as a reflex of a free decision, and not within the context of a mental illness. Manuel Sánchez-Perez will discuss how difficulties in assessing the rationality of suicidal ideation include its dimensional character, together with the evidence that rationality is not always absent in depression and that the absence of depression does not imply rationality. Next, Lia Fernandes will provide an overview of years of public and parliamentary debates on euthanasia and physician-assisted suicide in Portugal. She will focus on issues regarding the exact role of psychiatrists (old age psychiatrists in particular) in the decision process, as proposed by the recently approved legislation. Finally, Javier Olivera will provide a brief overview of the recent implementation of legal access to euthanasia in Spain. A discussion of several controversial aspects of the operationalization of such a complex process will follow, including those related with conscientious objection.

Ese es el problema de los libros que han viajado mucho y pasan por demasiadas manos, temporal o definitivamente. Aunque el hecho de que yo esté aquí, en esta hoja de papel que no hace parte de mi cuerpo y que, con toda seguridad, terminará en la basura, implica que no hay algo definitivo para un libro, en el sentido estricto del término. Por fortuna en esta librería siempre hay un mataburros a mano: “¡Oye, Diccionarioooo!, dime rápido el significado de la palabra definitivo”.

Exclusive breastfeeding is the best food given to babies during the age of 0-6 months without being interspersed with other additional food. However, in reality, there are still many breastfeeding mothers who do not provide exclusive breastfeeding to their babies due to various factors. In Indonesia, mothers who exclusively breastfeed their babies during the current month are still low, which is 2% of the total number of mothers who give birth. The behavior of breastfeeding mothers is based on Lawrence Green's (1980) behavioral theory, namely predisposing factors (knowledge, attitudes, education level, and work), enabling factors (family economic level), and reinforcing factors (family role). This study was conducted to see the various factors associated with low exclusive breastfeeding in infants in Gandus District. This study used descriptive analytic method with cross sectional approach, where the sample was 100 breastfeeding mothers. Sampling using Random Sampling method, namely Proportioned Stratified Random Sampling. The instrument in this study used a questionnaire. Data processing using manual system was done by univariate and bivariate analysis with Chi-Square test with level of sgnificant (alpha) 0.05. From the results of the study it is known that the value of x2 count is greater than x2 table for work (4.06> 2.71), so there is a significant relationship between work and low exclusive breastfeeding. While the x2 value is smaller than the x2 table (knowledge (1.43 < 2.71), attitude (0.34 < 2.71), education level (0.10 < 4.61), family economic level (0.44 < 2.71), and family peren (0.58 < 2.71), so there is no significant relationship with the low exclusive breastfeeding in infants. The suggestion from this study is that all parties involved in exclusive breastfeeding in infants can make various efforts to increase knowledge and awareness that are adjusted to all the resources available in the area.

Abstract Background: Adherence to BCCPG improves clinical outcomes for patients with breast cancer. However, implementation of international BCCPG may not be feasible in low- and middle-income countries (LMICs) due to limitations in resources and personnel. A potential solution for this issue is developing national/regional guidelines considering local healthcare contexts. In Mexico, the National Consensus on Diagnosis and Treatment of Breast Cancer (known as the Colima Consensus, http://consensocancermamario.com) is considered the national BCCPG, and it is updated bi-yearly since 1994. The aim of this study was to evaluate physicians’ uptake of the Colima Consensus recommendations, and to identify barriers and attitudes that may impact adherence. Methods: A cross-sectional, 30-item survey exploring attitudes towards the Colima Consensus, as well as barriers limiting adherence to its recommendations, was e-mailed to Consensus attendees, members of the Mexican Society of Oncology, and of the Mexican Mastology Association, with answers collected over 3 weeks. Descriptive statistics, including means, medians, and standard deviations were used to analyze the participants’ sociodemographic characteristics, attitudes, and perceived barriers. X2 tests were used to evaluate associations between physicians’ characteristics/attitudes and adherence. Results: Among 1553 physicians invited to participate, 320 (21%) answered the survey, of which 25 (8%) were unaware of the Colima Consensus. Two hundred ninety-five participants completed the entire survey and were included in this analysis. Fifty-six percent were male, 65% were age 30-49, 44% practiced in Mexico City, and 8% were foreign. Regarding specialty, 26% were surgical oncologists, 14% medical oncologists, and 10% radiation oncologists. Over half of respondents (57%) worked in both public and private practice, while 19 and 23% worked only in public or private practice, respectively. Sixty-two percent had access to a multidisciplinary breast cancer team. Eighty-five percent reported using the Colima Consensus to guide clinical decision making and 77% adhered to its recommendations in most/all cases. Forty-two percent used the Colima Consensus as their only guideline reference. Surgical oncologists were more likely than medical oncologists to use the Colima Consensus as their only guideline reference (29 vs. 12%, p&lt;0.01). The most commonly reported barriers to adherence were lack of resources (51%) and logistical issues (31%). Physicians working without a dedicated multidisciplinary breast cancer team were significantly more likely to report a lack of resources as a barrier than those with a multidisciplinary team (63 vs. 43%, p&lt;0.01). No differences were found in other barriers according to participants’ characteristics. Regarding attitudes towards the Colima Consensus, 88% agreed/strongly agreed that it was a valuable teaching tool, 88% considered it a trustworthy source of information, and 89% believed it led to improved quality of care. Seventy-six percent of the participants agreed/strongly agreed that the Consensus’ recommendations were free of bias and 58% considered that its use led to reduced costs of care. Conclusions: Initial results of our study show high levels of adherence and positive attitudes towards the Colima Consensus, with a significant proportion of physicians using it as their main guideline reference. Lack of resources and logistical issues were the main barriers hampering implementation of recommendations, particularly for physicians working without a multidisciplinary breast cancer team. Our results highlight the relevance of local BCCPG and of multidisciplinary collaborations, and suggest a need for the creation of stratified recommendations for various healthcare settings and resources. Background:Interferon signaling pathway plays a relevant role in autoimmunity. Genetic variants in theinterferon regulatory factor (IRF) 5gene, that encodes the major regulator of the type I interferon induction [1], have been related to the development of several inflammatory diseases [2].Objectives:To determine the influence ofIRF5on Immunoglobulin-A vasculitis (IgAV), an inflammatory vascular disease.Methods:ThreeIRF5polymorphisms (rs2004640, rs2070197 and rs10954213) representative of 3 different haplotype blocks were genotyped in 372 Caucasian patients with IgAV and 876 sex and ethnically matched healthy controls.Results:No statistically significant differences between patients with IgAV and controls were observed when eachIRF5polymorphism was analyzed independently. Similarly, no statistically significant differences between patients with IgAV and controls were found whenIRF5polymorphisms were evaluated combined conforming haplotypes. Abstract Immunotherapy based on Immune Checkpoint blockade (ICB) has become a significant therapeutic option for advanced Non Small Cell Lung Cancer (NSCLC) patients. However, there is an urgent need to find novel biomarkers that to reliably stratify good responders to immunotherapy. Currently, the available biomarkers are not specific enough. Exosomes are small membrane vesicles with sizes of 30-100 nm secreted by most cell types including cancer cells. Exosomes operate as an intercellular communication system by sending proteins, mRNA and miRNAs among other relevant RNA molecules. Exosomes enriched with miRNAS are involved in proliferation, differentiation, maduration and immune cell activation. Moreover, in cancer cells miRNA and other small RNA molecule expressions are dysregulated. Exosomes produced from cancer patient's plasma have been shown to be accurate diagnostic tools for the disease. In this study we profiled miRNAs and other small RNA cargo by exosome of plasma samples from 77 Non Small Cell Lung Cancer (NSCLC) metastatic patients before and after the first cycle of immunotherapy to evaluate the potentiality of predicting response to immunotherapy. We perform exosomes together with RNA isolation and small RNAseq sequencing from plasma samples before and after the first ICB cycle. Two independent softwares were used to identify small RNAs (RNAtoolbox and mirMaster). Prior ICB treatment we did not find differentially expressed miRNAs or other small RNA between good and bad responders. Interestingly we identified 12 exosomal miRNA differentially expressed between good and bad responders after the first cycle of ICB. Intriguingly, levels of miR-134-5p, miR-142-3p, miR-143-3p among others previously associated with NSCLC were found to be considerably higher in the good responder group than the bad responder group. Regarding other smallRNA molecules we observed a great variety and variability of piRNA, rRNA, scaRNA, lncRNA, snoRNA, snRNA, miscRNA and circRNA. To address the function of miRNA and other differentially expressed RNA molecules, we consulted KEGG, GO and Reactome for gene regulatory networks. Interestingly, KEGG results show pathways in cancer as top hit and Reactome highlight Immune System and cancer hits. El presente monográfico contiene una serie de investigaciones especializadas que, sin duda, contribuirán a ampliar sustancialmente el conocimiento científico que tenemos sobre determinadas realidades habitacionales y materiales del área septentrional de la Península Ibérica. Además, los sitios arqueológicos recogidos en este volumen responden a contextos tanto de carácter rural como urbano y con un amplio abanico cronológico que abarca realidades que van desde la Protohistoria hasta la Edad Media. Así pues, el primero de los trabajos de este número se inicia con el estudio realizado por Hugo Lozano Hermida, Eduardo Ramil Rego, José Manuel Caamaño Gesto y Sara Barbazán Domínguez titulado “Castreños y romanos en el occidente cantábrico: estado de la cuestión y algunas reflexiones”, centrado en el estudio de una serie de zonas arqueológicas emplazados en el área geográfica sita entre Punta de Estaca de Bares y la Ría de Ribadeo que datan de entre la Edad de Hierro y el periodo romano. Seguidamente, el equipo dirigido por Alfonso Fanjul Pereza nos ofrece un análisis sobre la realidad socio-económica del asentamiento asturianos de Las Vallinas, de fines de la Prehistoria e inicios de la dominación romana, comparándolo además con otros sitios conocidas en la costa del Golfo de Vizcaya. Por otro lado, Mario César Vila y Manuela Pérez Mato en su artículo “El yacimiento de San Esteban (Cacabelos, León) y su relación con Bergidum Flavium: síntesis y resultado de la intervención arqueológica realizada en 2014”, nos muestras el resultado de los trabajos de campo desarrollos con motivo de las obras de modernización de regadíos del Canal Alto del Bierzo. También se incluye en este número el análisis que Carlos Fernández Rodríguez, Eduardo González Gómez de Agüero, Natividad Fuertes Prieto y Víctor Bejega García han realizado sobre los productos del mar localizados en una serie de asentamiento del interior del noroeste de la Península Ibérica durante el periodo de dominación romana. En esta misma línea, el artículo que encabeza Esteban Álvarez Fernández está dedicado al análisis de los restos faunísticos exhumados en el poblado zamorano de El Castillón que data de entre los siglos V y VI.

Abstract Background: We previouslyreported that invasive disease-free survival (IDFS), the primary outcome, and distantdisease-free survival (DDFS), a secondary outcome, differed by menopausalstatus in a pre-specified analysis. Here,we report updates on IDFS and DDFS with additional follow-up, as well as distantrecurrence-free interval (DRFI) and post hoc analyses in premenopausal women.Methods: Eligibilitycriteria included women &gt; 18 years of age with HR+, HER2-BC and 1-3 +LN and no contraindications to taxane and/or anthracycline basedCT. DRFI was defined as time to distant recurrence or death from breast cancer.Analyses were intent-to-treat among eligible pts. We performed a post hocanalysis evaluating IDFS between treatment arms in premenopausal pts with pN1mi.In addition, we conducted a two-year landmarked IDFS analysis by ovarianfunction suppression (OFS) or not in the ET arm, as well as by regularmenstrual periods or not in both treatment arms.Results: Among the 4,984eligible pts, there were 553 IDFS events and median follow-up of 6.1 years.Postmenopausal women do not have any IDFS or DDFS benefit with CT; however, 5-yearabsolute benefits for IDFS and DDFS with CT for premenopausal pts were 5.9% and3.3%, respectively. In pre-menopausal pts, CT was associated with improved DRFIfor all RS values &lt; 25, with an absolute improvement of 2.3% for RS0-13 and of 2.8% for RS 14-25. Among premenopausal pts, 12.4% (n=206) had pNmi)disease. In a post hoc analysis, there was a trend for CT benefit for thosewith pNmi [hazard ratio (HR)=0.44, confidence interval (CI)=0.18-1.08]. Therewere only 22 IDFS events. Only 17.2% of premenopausal pts assigned to ETunderwent OFS in the first 24 months and in two-year landmarked analysis, therewas no IDFS difference in those who underwent OFS or not (HR=0.88,CI=0.47-1.63). In premenopausal women assigned to ET, 58.9% stopped havingperiods within the first 24 months, and have anumerically improved IDFS compared to those who continued regular periods (HR=1.48,CI: 0.92-2.40). Among premenopausal assigned to CT followed by ET, 80.8%stopped having periods within the first 24 months and have a numericallyimproved IDFS compared to those who continue to have regular periods (HR=1.56, CI:0.85-2.86).Discussion: In accordancewith the differential IDFS and DDFS benefit based upon menopausal status inS1007, premenopausal pts with 1-3+LN and RS &lt; 25 had a statisticallysignificant improvement in DRFI with the addition of CT. A small proportion of S1007premenopausal participants underwent OFS. The role of OFS as it relates to CTbenefit cannot be determined from this study. Background:Rheumatoid arthritis (RA) is a chronic autoimmune disease with no cure, characterized by episodes of exacerbation and remission, which requires permanent use of medications. Clinics of excellence are multidisciplinary and centralized programs that improve adherence to treatments. Information on the benefits of these models of care has been published but is not definitive. In Colombia, the clinical registry of patients with RA is kept in the Cuenta de Alto Costo (CAC).Objectives:To demonstrate the difference in the percentages of sustained remission at 2 years, between an institution with non-centralized management or standard of care (Hospital Militar Central-HMC) compared to another institution with centralized management or clinic of excellence (BIOMAB-IPS) and determine if the results are determined by any of the intervention variables or by the program.Methods:The 2-year clinical records for the CAC were compared between an institution with non-centralized management (HMC) in comparison with another institution with centralized management (BIOMAB-IPS), performing a sociodemographic description, measuring control of the disease DAS28 clinimetry, Fisher’s test non-parametric bivariate analysis, multiple regression model, and population matching with Propensity score Matching (PSM).Results:Complete information was obtained from 2 years of follow-up, in centralized management 3457 patients and for the non-centralized unit 114 patients. Most of them corresponded to 2962 women (82%), with time of illness of 9.5 years and 10.2 years, respectively, without statistically significant differences. A difference was observed in the 2 programs to maintain remission at 2 years, in favor of the centralized program 54.7% vs 28.6.2% (p <0.00). With the binomial generalized linear regression model, it was confirmed that this difference was not explained by variables such as the use of biological therapy (RR = 0.77; 95% CI 0.69-0.86), use of DMARDs (RR = 0.71; 95% CI 0.62-0.82) and number of rheumatology consultations (RR = 0.97; 95% CI 0.92-1.02) in comparison with the centralized care model (RR = 2.32; 95% CI 1.58-3.35). Due to the biases between the groups due to the non-probability sampling, a PSM was performed, with a 1: 1 match, caliper of 0.065, obtaining a pseudo population with well-balanced covariates (see table 1). In the common support area, statistically significant differences were documented in sustained remission over 2 years, in favor of the centralized care group 45 vs 17.9% (p = 0.001).Conclusion:With the information from the clinical records, statistical strategies can be used to evaluate important differences in centralized care programs, observing favorable results of these types of care that are not related to isolated elements of the program, but to the overall effect of the program.References:[1]Austin PC. Background:With the objective to improve quantification of sacroiliitis maintaining a practical perspective, our group developed SCAISS, a semi-automated method to measure bone marrow edema (BME) in MR images from sacroiliac (SI) joints, combining semi-axial and semi-coronal slices [1]. The 2009 ASAS definition of active sacroiliitis was based on standard semi-coronal slices only, perpendicular semi-axial slices being considered but optional [2]. We hypothesized a simplified SCAISS (s- SCAISS) method using only semi-coronal slices.Objectives:To analyze the validity and reliability and feasibility of a simplified Spanish tool for semi-automatic quantification of sacroiliac inflammation by magnetic resonance in spondyloarthritis (s- SCAISS) using a semi-coronal scan instead of combining semi-axial and semi-coronal slices.Methods:The s- SCAISS was designed as an image-processing software. We performed the following analysis: (1) three readers evaluated SI images of 23 patients with axial SpA and various levels of BME severity with the s-SCAISS and SCAISS, and two non-automated methods, SPARCC and Berlin; (2) 20 readers evaluated 12 patients images, also with the three methods. Convergent validity, reliability and feasibility were estimated.Results:Theinterobserver reliability(ICC and 95% CI) in the three observers’ study was: s-SCAISS = 0.69 (0.490–0.845); SCAISS= 0.770 (0.580–0.889); Berlin = 0.725 (0.537–0.860); and SPARCC = 0.824 (0.671–0.916). In the 20 observers’ study, ICC was: s- SCAISS = 0.66 (0.478–0.863); SCAISS = 0.801 (0.653–0.927); Berlin = 0.702 (0.518–0.882); and SPARCC = 0.790 (0.623–0.923).Spearman correlation coefficientbetween s- SCAISS _BERLIN was r= 0.712 and s- SCAISS_ SPARCC was r= 0.779 and s- SCAISS _SCAISS was r= 0.90. Abstract The irruption of the immunotherapy for the treatment of non-small cell lung cancer (NSCLC) based on Immune Checkpoint Inhibitors (ICI) PD-1 and PD-L1 inhibitors is considered a treatment revolution. However, only a small percentage of patients benefit with ICI treatment over the long term, and precise biomarkers that can recognize these individuals before or early during treatment have so far eluded. PD-L1 expression and tumor mutational burden (TMB) are the most well studied biomarkers for predicting response to PD- (L)1 blockade-based ICI prior to treatment. TMB is still being clinically assessed whereas PD-L1 has several drawbacks for prediction of persistent benefit. Our aim is to evaluate the basal state and dynamic changes of cell-free DNA (cfDNA) concentration to predict and monitor response in NSCLC patients starting ICI. A total of 248 cfDNA concentration measurements were performed from 87 NSCLC patients. The quantification was done before the start of the treatment, at the second ICI cycle, after 6 and 12 months in treatment, and at progression if it was within the first 12 months. Quality and quantity of the cfDNA was assessed using Qubit High Sensitivity and Bioanalyzer 2100. We first explored the association with response of the basal cell-free DNA concentration using Mann-Whitney-Wilcoxon test. The longitudinal analysis between different time points was tested with Wilcoxon signed-rank tests. Response was ascertained using RECIST parameters at 3, 6, and 12 months. We evaluated the utility of the cfDNA concentration as a prognostic factor using Mantel-Cox test. The response association results indicate that early and long-term response is associated with lower levels of basal cfDNA (p&lt;0.002, 3 months; p&lt;0.001, 6 months; p&lt;0.05, 12 months). The longitudinal analysis could also detect a decrease in cfDNA from the second cycle to the month 6 of treatment in responders (p&lt;0.05), and an increase in non-responders (p&lt;0.05). To identify a prognostic concentration threshold, statistical stratification of cfDNA levels based on the selected maximum rank method of cfDNA levels was used to group them into low and high concentration. Patients with high basal cfDNA concentrations showed both worse FPS (log-rank 3.4x10-03, Medians: Low 12.57 [8.36 - 18.46] High 5.50 [3.03 - 10.06]) and OS (log-rank 3.2x10-05, Medians: Low 21.26 [15.33 - 33.96] High 5.45 [3.10 - 15.66]). With these results, we provide proof-of-principle that specifically detecting tumor cfDNA by PCR or NGS methods would not be necessary to predict the long-term clinical benefit to ICI in NSCLC patients. Moreover, concentration cut-offs can be defined to categorize the patients in potential responders and non-responders before the start of the treatment. This would constitute a cost-effective and easily implementable prognostic tool that could be particularly useful for avoiding loss of treatment windows of opportunities. Abstract Introduction: HER2DX is a prognostic (risk-score) and predictive (pathological complete response-score) genomic assay validated in baseline (BL) tumor samples from HER2+ EBC pts. RD after neoadjuvant anti-HER2-based therapy is associated with worse outcomes. Whether HER2DX risk-score analyzed in RD provides additional prognostic information is unknown. Methods: This retrospective analysis included 14 pts from group B of the PHERGain trial (NCT03161353): 6 who developed distant recurrences and 8 matched controls with HER2DX high-risk score at BL who did not. Group B pts had centrally confirmed, stage I-IIIA, HER2+ EBC and were treated with HP (± endocrine therapy), with chemotherapy added for pts without PET response after 2 treatment cycles or pathological complete response (Perez-Garcia et al, Lancet Oncol 2021).HER2DX was evaluated on tumor samples collected at BL and surgery. The primary objective was to determine the association of HER2DX risk-score in RD with the development of distant recurrences. Secondary objectives were to assess gene expression and the HER2DX signatures in BL vs RD and their association with distant recurrences. Cox regression models evaluated the association of variables with distant recurrence. Paired SAM with a false discovery rate &lt;5% identified differences in gene expression between BL and RD samples. Results: Median follow-up was 4.04 years. Five (83.3%) of 6 pts with distant recurrences had HER2DX high-risk scores at BL. The 8 control pts had HER2DX high-risk scores at BL but no relapse. In RD, all 6 pts (100%) who relapsed had a HER2DX high-risk score, while 4 (50%) of 8 pts without distant recurrence presented a HER2DX downstaging. Pts with HER2DX low-risk in RD had better 3-year distant disease-free survival than high-risk pts (100% vs 50%; hazard ratio = 4.34 [1.03-18.23], p=0.045). In RD, high expression of the proliferation signature and 15 genes, including proliferation genes, and low expression of 38 genes, including immune and luminal-related genes, were associated with poor prognosis. In paired analyses of BL and RD samples, 118 genes (63.8%) were differentially expressed in pts that did not relapse, while only 3 genes (1.6%) were differentially expressed in pts that relapsed. Conclusions: These data suggest that HER2DX risk-score in RD provides additional information on the likelihood of developing distant recurrences. In RD, high expression of immune and luminal genes was associated with a reduced risk, while high expression of proliferation genes was associated with a higher risk. Molecular changes after anti-HER2 therapy were mainly observed in RD of pts that did not relapse. Prognostic impact of HER2DX downstaging merits further evaluation. O objetivo deste estudo é verificar o impacto dos custos de diferentes métodos de colheita no preço final de venda, bem como verificar os ganhos monetários obtidos na produção de cafés especiais no município de Carmo de Minas (MG). Os dados deste estudo buscam subsidiar a tomada de decisão quanto ao tipo de colheita e aos retornos esperados em termos de preços de venda do produto. A metodologia adotada foi a de conversão das unidades de campo (balaios) para a unidade de comercialização do café (sacas de 60 kg). Os dados levantados permitem ao produtor desenvolver estratégias de manejo da lavoura de café para que atendam aos padrões do mercado, maximizando o retorno sobre os custos de colheita adotada. Dentre os métodos de colheita avaliados, a colheita manual selecionada tem maior potencial de retornos vantajosos para um sistema de produção de cafés especiais em Carmo de Minas (MG). SPECIALTY COFFEE HARVEST: COST AND BENEFIT TO FARMERS IN THE MANTIQUEIRA DE MINAS AREA – MG ABSTRACT: Coffee market has become an overly complex market in terms of crop, cost, and sales management. The aim of this study was to verify the impact of different harvesting methods costs on sales price as well as to verify economic profits obtained by specialty coffees in Carmo de Minas (MG). The data addresses the decision-making regarding the picking system and the expected returns in terms of sales prices. The methodology foresees the conversion of field units (balaio) to coffee sales units (bags of 60 kg). The data collected allows the producer to develop coffee crop management strategies to come into the market standards, maximizing returns among the harvest costs. Amid the picking methods evaluated, the hand-picked coffee has the greatest potential for beneficial returns for a special coffee production system in Carmo de Minas (MG). Abstract Brain metastasis is an unmet clinical need, affecting between 10-30% of cancer patients with 200000 to 400000 newly diagnosis per annum in the US. Recently, several clinical trials have reported benefits using immunotherapy to treat brain metastasis. However, variability of the responses is broad and high benefit is found mainly in asymptomatic brain metastasis, while the benefit is dramatically reduced in the clinically relevant stage. Thus, it is currently unknown how to effectively target symptomatic brain metastases with immunotherapy. We previously reported a clinically relevant protumoral program driven by STAT3 activation in a subpopulation of reactive astrocytes in these advanced stages of the disease. Our current study further exploited the heterogeneity within the metastasis-associated microenvironment as a resource to identify novel therapeutic vulnerabilities to improve the benefits of immunotherapies based on immune checkpoint blocking antibodies (ICB) in symptomatic brain metastasis. Our results demonstrate that reactive astrocytes are strong immunomodulatory cells in brain tumors. We have identified the molecular profile of disease-associated glial cells and defined its connection to modulatory activities on specific lymphocyte populations in experimental brain metastasis as well as human-derived samples. scRNASeq and high content multiplex immunofluorescence allowed us to report a novel local immunomodulatory axis dependent on TIMP1 (astrocytes)/CD63 (CD8 T cells), which is present in brain metastasis patients with high immunoscore and would imply an additional immunosuppressive signal for potential ICB responders in brain metastasis. Genetic and pharmacologic approaches targeting this STAT3-dependent local immunomodulatory axis have allowed us to define the rationale to combine immune checkpoint blockade with a STAT3 inhibitor, which we previously used in patients. We proved that such combined immunotherapy boost the systemic activation of T cells while also preventing the local blockade. Additionally, our comprehensive strategy includes the possibility to stratify patients that are best qualified to benefit from this therapy by measuring TIMP1 in liquid biopsies from CSF. Even more, our data using Patient Derived Organotypic Cultures (PDOC) from fresh brain metastasis neurosurgeries confirms that our therapeutic strategy might benefit brain metastases generated from any primary source. In conclusion, we describe an immunosuppressive mechanism in the brain microenvironment that could explain the lack of response to ICB in patients with advanced brain metastasis. Our finding provides the rationale to implement complementary approaches targeting local immunosuppression to increase the benefit of immunotherapy in symptomatic brain metastasis. Abstract Pediatric germ cell tumors (GCT) are rare and heterogeneous, presenting various histologies. There are no specific markers for diagnosing GCT, and 30% of patients are resistant to standard cisplatin-based chemotherapy, which can be related to epithelial-mesenchymal transition (EMT). Therefore, this study aims to elucidate markers for classifying GCTs, evaluate the molecular mechanisms related to drug resistance, and search for new therapeutic approaches. The expression of miRNAs was assessed for 42 GCT-pediatric patients through miRNA Panel - Nanostring technology. Whole Exome Sequencing (WES) was performed for 16 GCT-pediatric patients to assess somatic mutations. In silico analysis of epithelial-mesenchymal transition (EMT) markers was performed using the cBioPortal database. The expression of EMT markers was evaluated in vitro in parental and cisplatin-resistant models (NTERA-2R). Xenograft model-derived NTERA-2R was established and EMT markers were also assessed. We identified specific miRNA expression signatures for each histology, including 34 miRNAs for dysgerminomas, 13 for embryonal carcinomas, 25 for yolk sac tumors, and one for immature teratoma. We identified 26 miRNAs that were commonly expressed in malignant tumors, with six miRNAs (miR-302a-3p, miR-302b-3p, miR-371a-5p, miR-372-3p, miR-373-3p, miR-367-3p) showing significant overexpression. MiR-302b-3p had a significant association with all the evaluated clinical features. WES analysis showed that Single Base Substitution signature 39 was observed in 68.75% of samples and SBS22 in 12.5%. Copy number alterations were identified in 4, 7, 8, 10, 12, 21, and 22 chromosomes, with amplification of CDKN1B, KRAS, CCND2, ETV6, and KDM5A genes and deletions of KIT and PTEN genes. Somatic mutations in NYAP2, RHBDF2, MTOR, KIT, ATM, KRAS, and PIK3CA were frequent among the samples. KIT (Asp816Val, Ala829Pro) and KRAS (Gln61Leu) mutations were classified with potential clinical significance and were validated. In silico analysis showed that different histologies had distinct expression profiles for EMT markers. Patients with SNAILlowSLUGlow expression had higher PFS than those with SNAILhighSLUGhigh (p=0.006). The median PFS of patients with SLUGlow was 46.4 months, while SLUGhigh was 28.0 months (p=0.022). In vitro analysis using NTERA-2R showed overexpression of EMT markers. The same result was obtained in the xenograft model derived from NTERA-2R. Our study offers vital insights into pediatric GCTs' molecular landscape, using in silico, in vitro, in vivo analyses, and genetic assessments, highlighting three points: 1) The miRNA signatures for each histology may have robust diagnostic and prognostic potential, besides clinical implications as biomarkers. 2) KIT and KRAS genes are possible therapeutic targets of pediatric GCTs. 3) EMT is related to cisplatin resistance and SLUG is a potential molecular target.
