Maternal age on both ends of the reproductive spectrum (teenage and 35+) is associated with increased risk of adverse pregnancy outcomes, as compared with the age range from 20–34 years old. Some of the increase in pregnancy complications in older mothers is caused by underlying age-related health issues such as hypertension and diabetes, the prevalence of which increases linearly with age. The risks associated with young maternal age are more related to nutritional deficits and the fact that pregnant adolescents may still be growing themselves. Poor fetal growth often seen in adolescent pregnancies possibly results from competition for nutrients. Maternal bone loss is also a concern, as adolescent diets are commonly low in calcium and vitamin D. Pregnant adolescents may benefit from calcium supplementation to compensate for the increased need for their own bone growth and should at minimum receive vitamin D supplements, as recommended for all pregnant women.

Traumatic brain injury (TBI) is a leading cause of death and disability throughout the world, particularly among young adults, affecting untold numbers of women of childbearing age. TBIs can disrupt almost any aspect of physical, cognitive, and/or emotional functioning, potentially complicating a woman’s ability to conceive, carry, and deliver a healthy child. For women who are already pregnant and sustain a TBI, medical stabilization of the mother and management of risk of further injury to the fetus are priorities. For women with a previous history of TBI, comprehensive assessment and optimal management of common sequelae of TBI (eg, seizures, endocrine dysfunction, physical and cognitive impairments, and neuropsychiatric symptoms) are essential to maximizing outcomes for both mother and child. Consultation with physiatry and neuropsychology, utilization of rehabilitation therapies to maximize the mother’s functional recovery, and consistent communication among all medical team members throughout pregnancy are essential.

Five programmes which offer additional services to enhance the child health programmes for families at risk of poorer maternal and child health and development outcomes are being evaluated. Flying Start is a community-based programme targeted on families with children under 4 years of age living in deprived neighbourhoods in Wales. A Better Start is a place-based programme implemented by local partnerships in English wards with high levels of economic deprivation, providing services and support for all families living in the target wards from pregnancy until a child’s fourth birthday. The Family Nurse Partnership is a structured programme of home visits for first-time young mothers. The Maternal Early Childhood Sustained Home-visiting programme is a structured programme of sustained home visiting delivered by health visitors in pregnancy and the first 2 years. The Trial of Healthy Relationship Initiatives for the Very Early-years is comparing two parenting programmes for women with additional mental health or social support needs in pregnancy with outcomes up to 18 months. Early published results are reviewed and links given to the websites to signpost the reader to up-to-date information on the effectiveness of each programme.
