Abstract Background: Three-dimensional (3D) technologies have numerous medical applications and have gained a lot of interest in medical world. After the advent of three-dimensional printing technology, and especially in last decade, orthopedic surgeons began to apply this innovative technology in almost all areas of orthopedic traumatic surgery. Objective: The aim of this paper is to give an overview of 3D technologies current usage in orthopedic surgery for patient specific applications. Methods: Two major databases PubMed and Web of Science were explored for content description and applications of 3D technologies in orthopedic surgery. It was considered papers presenting controlled studies and series of cases that include descriptions of 3D technologies compatible with applications to human medical purposes. Results: First it is presented the available three-dimensional technologies that can be used in orthopedic surgery as well as methods of integration in order to achieve the desired medical application for patient specific orthopedics. Technology starts with medical images acquisition, followed by design, numerical simulation, and printing. Then it is described the state of the art clinical applications of 3D technologies in orthopedics, by selecting the latest reported articles in medical literature. It is focused on preoperative visualization and planning, trauma, injuries, elective orthopedic surgery, guides and customized surgical instrumentation, implants, orthopedic fixators, orthoses and prostheses. Conclusion: The new 3D digital technologies are revolutionizing orthopedic clinical practices. The vast potential of 3D technologies is increasingly used in clinical practice. These technologies provide useful tools for clinical environment: accurate preoperative planning for cases of complex trauma and elective cases, personalized surgical instruments and personalized implants. There is a need to further explore the vast potential of 3D technologies in many other areas of orthopedics and to accommodate healthcare professionals with these technologies, as well as to study their effectiveness compared to conventional methods.

CONTEXT AND OBJECTIVE: Although Colles' fracture is a common clinical situation for the orthopedist, we did not find any information in the literature that would allow safe decision-making on the best treatment for each fracture type. The aim of the present study was to investigate Brazilian orthopedists' opinions regarding the main aspects of treatments for Colles' fracture. DESIGN AND SETTING: Cross-sectional study conducted during the 34th Brazilian Congress of Orthopedics and Traumatology. METHODS: Five hundred questionnaires containing 12 items were randomly distributed to orthopedists who were attending the congress; 439 were filled out correctly and were considered in this study. RESULTS: The main factors in making decisions on interventions in fracture cases were whether the fracture was intra-articular, the existence of shortening of the distal radius and the patient's age. The classification method most used was Frykmann. The closed reduction method most used was manual reduction. The principal surgical interventions were percutaneous pinning (39%), external fixation (27%) and volar plate (19%). Most of the interviewees only used bone grafts for osseous gaps in special cases. The most frequent complications were impairment of joint mobility and residual pain. CONCLUSIONS: Brazilian orthopedists have concordant opinions regarding conservative treatment methods and the use of bone grafts. There were conflicting opinions regarding surgical treatment methods, classification types and complications.

Introduction: Postoperative complications (PCs) constitute any deviation from the normal postoperative course. Reporting of PCs remains a challenge, multiple classification systems have been proposed, however these have not been validated across surgical specialties. Clavien and Dindo (DCCS) developed a system for General Surgery and has been adopted in different fields. Nonetheless, this classification has not been adapted to Orthopedics. The objective of this study was to adapt the Clavien-Dindo classification to orthopedic scenarios and to determine the intra and interobserver reliability. Methods: The designer team adapted the Clavien Dindo classification to orthopedic scenarios. Ten orthopedic observers with different degrees of training and experience were selected to evaluate the classification. 48 simulated clinical scenarios of complications and another negative outcomes such as failure to cure and sequelae were sent by electronic format independently. A second round of scoring was performed 30 days later to assess the intraobserver concordance. Results: We found a high interobserver and intraobserver reliability for both the first and second evaluation (Kappa 0.88 and 0.91, respectively). In addition, the intraobserver analysis showed a very good correlation (Kappa 0.93). Discussion: The DCCS classification has been developed for general surgery and has been widely applied in the different surgical subspecialties. In Orthopedics, this classification was validated to hip and pediatric surgery. Therefore, our study involved an adaptation of the classification to general and specific orthopedic scenarios of the different orthopedic subspecialties. This classification may be a useful tool for documenting complications in orthopedic surgery. Keywords: postoperative complications; orthopedics; health systems

Aim. To justify the use of marketing methods in the activities of dental institutions. Methods. During the period 2008-2011 conducted was a sociological survey of 857 patients with detailed clinical examination; eight predictive models were developed. Used were variational statistics, regression analysis, mathematical modeling, expert method. Results. 171 (27.0%) men and 460 (73.0%) women went to seek care in commercial dental institutions. 267 (33.0%) men and 459 (67.0%) women went to seek care in state dental institutions. The referability of women to dentists was higher in both cases (p 0.05). Women aged 20-29 years preferred to go to commercial dental institutions (32.0±2.3%), than to the state ones (17.0±1.8%, p 0.001), while men, on the contrary, went to the state (37.0±2.4%) rather than commercial dental institutions (20.0±2.0%, p 0.001). At the age of 30-39 years the women went to state dental institutions (33%) more frequently than to commercial dental institutions (29%), while men - mostly to commercial dental institutions (26%) compared with the state (17%). Among the reasons for seeking dental treatment in a commercial organization the first one is the lack of queues at the reception: to surgeons 46.1±1.7%, therapists 58.3±1.7% and orthopedists 60.0±1.85%; the second place is held by a convenient patient schedule: for the profile of surgery 34.2±1.6%, therapy - 26.7±1.5%, orthopedics - 22.0±1.4%, and the contribution of other causes did not exceed 10.0±1.0%. Proposed were eight prognostic models in the form of regression analysis equations for complex evaluation of the influence of factors, which determine the choice of a health care institution, and the referral to the therapist-dentist, surgeon-dentist, orthopedist-dentist, and orthodontist both in public and private health care organizations. Conclusion. Prognostic matrices, calculated using the formulas of mathematical analysis, can be used when planning outpatient dental care, justifying the required number of dentists’ positions in the state and commercial dental organizations.

ABSTRACT Objective: Rheumatoid arthritis is a prevalent disease in the population (range 0.5% to 1%) and involves both orthopedic and rheumatologic treatment. The Time Trade-Off (TTO) technique, which determines the number of years the patient or the professional would be allowed before a successful procedure in terms of life expectancy and value of the procedure, has been gaining ground in clinical protocols. From this standpoint, we sought to compare evaluations provided by the patients, orthopedists, and rheumatologists in determining the TTO and to correlate their responses with the clinical repercussions using previously established scores such as the Brief Michigan Hand Questionnaire and the Disease Activity Score-28 (DAS-28). Methods: A prospective study was conducted that involved 37 patients with rheumatoid arthritis, orthopedists, and rheumatologists. The TTO questionnaire was administered by an independent evaluator for evaluation using the DAS-28 and the Brief Michigan Hand Questionnaire. Results: The descriptive analysis revealed similar medians between the orthopedists, rheumatologists, and patients for single assessments. However, there was a weak correlation between the results from the patient and rheumatologist, the patient and Brief Michigan Questionnaire, and those of the orthopedic surgeon and the DAS-28. Conclusion: Similar median values demonstrated equivalent TTO among the orthopedist, rheumatologist, and patient. However, given the weak correlations between the scores, it was not possible to substitute results using a single evaluation scale. Level of Evidence II, Prognostic Studies.

CONTEXT AND OBJECTIVE Orthopedic surgery implies high costs for both public and private healthcare. The aim of this study was to better understand the differences between the public and private sectors regarding treatment of a damaged anterior cruciate ligament, which is a common knee injury. DESIGN AND SETTING Descriptive cross-sectional study conducted during the Brazilian Orthopedics Congress in Brasília. METHODS We applied questionnaires during the 2010 Brazilian Orthopedics Congress, with participation by 241 knee surgeons from 24 Brazilian states. This was followed by statistical analysis on the data that were obtained. RESULTS The orthopedic surgeons who were evaluated used different approaches and treatment options in different Brazilian states, comparing between the public and private systems. CONCLUSION Both in the public and in the private systems in Brazil, because of non-medical issues surrounding the treatment, the best medical decision is not always made. This may be harmful both to patients and to physicians.

Fractures have brought great pain to patients, and treatment requires a lot of time and yield slow results, which seriously affect the production and life of people. Fractures are mostly treated with traditional conservative treatment methods. For orthopedic trauma, image enhancement technology has gradually played an important role in the clinical treatment of orthopedic trauma and has become a kind of suffering. It has become a new treatment method that attracts people’s attention. In order to study the application of image enhancement technology based on the deep trust network model in the clinical treatment of trauma and orthopedics, this paper conducted a related survey of fracture patients in the city’s first hospital, reviewed relevant literature, and interviewed professionals, and we collected relevant material, constructed case templates, and created clinical research models using comprehensive quantitative and qualitative analytical techniques. Studies have shown that the use of image enhancement techniques in the treatment of fractures has been successful, with healing efficiency approximately 20% faster than conservative treatment. In the clinical treatment of trauma and orthopedics, image enhancement technology can effectively reduce the incidence of complications in the prognosis of patients. Symptom Drop. This shows that the image enhancement technology of the deep trust network model can play an important role in the clinical treatment of trauma and orthopedics.

[Truncated abstract] Articular cartilage (AC) covers the surface of synovial joints providing a nearly frictionless bearing surface and distributing mechanical load. Joint trauma can damage the articular surface causing pain, loss of mobility and deformation. Currently there is no uniform treatment protocol for managing focal cartilage defects, with most treatment options targeted towards symptomatic relief but not limiting the progression into osteoarthritis (OA). Autologous chondrocyte implantation (ACI) and more recently matrix-induced autologous chondrocyte implantation (MACI), have emerged as promising methods for producing hyaline or hyaline-like repair tissue, however there remains some controversy regarding the exact histological nature of the tissue formed. Histological characterisation of AC repairs requires destructive tissue biopsy potentially inducing further joint pathology thereby negating the treatment effect. OA is recognised as a major cause of pain, loss of function and disability in Western populations, however the exact aetiology is yet to be elucidated. The assessment of both OA and cartilage repair has been limited to macroscopic observation, radiography, magnetic resonance imaging (MRI) or destructive biopsy. The development of non-destructive AC assessment modalities will facilitate further development of AC repair techniques and enable early monitoring of OA changes in both experimental animal models and clinical subjects. Confocal laser scanning microscopy (CLSM) is a type of fluorescence microscopy that generates high-resolution three-dimensional images from relatively thick sections of tissue. ... Biomechanical analysis suggested that the mechanical properties of MACI tissue remain inferior for at least three months. This study showed the potential of a multi-site sheep model of articular cartilage defect repair and validated its assessment via LSCA. Finally, the LSCA was used to arthroscopically image the cartilage of an intact fresh frozen cadaveric knee from a patient with clinically diagnosed OA. Images were correlated to ICRS (Outerbridge) Grades I-IV and histology. The LSCA gave excellent visualization of cell morphology and cell density to a depth of up to 200'm. Classical OA changes including clustering chondrocytes, surface fibrillation and fissure formation were imaged. Fair to moderate agreement was demonstrated with statistically significant correlations between all modalities. This study confirmed the viability of the LSCA for non-destructive imaging of the microstructure of the OA cartilage. In conclusion, the LSCA identified histological features of orthopaedic tissues, accurately quantified chondrocyte morphology and demonstrated classical OA changes. While the development and investigation of an ovine model of cartilage repair showed the treatment benefit of MACI, some biomechanical issues remain. Ultimately, the LSCA has been demonstrated as a reliable nondestructive imaging modality capable of providing optical histology without the need for mechanical biopsy. The evaluation of students and medical residents through Miniex is already well grounded in literature, with several papers demonstrating their validity and reliability. This paper aims the comparison of two groups of residents of Orthopedics in the Orthopedics Oncology area, utilizing Miniex assessments and a theoretical test examination. A total of 56 Miniex evaluations were carried out divided into two groups of residents being seven of them 2 nd year residents of Orthopedics and another seven attending the 3 rd year medical residency. There was a best performance of 3 rd year residents regarding skill competence of medical interview as well as on theoretical test examination. It has been also identified that skill in medical interview was the best marker for a good performance in the evaluation of Miniex. Analyzing some features of patients, such as gender and complexity of the case shows the possibility of these influencing the score giving to the student. We also observed that there is no association between the score giving to evidence and theoretical Miniex assessments. The findings of this study suggest there are peculiarities in the evaluation process as in the charging of individual knowledge that subjective considerations deserve, in addition to the score offered by traditionally carried out assessments. The use of Miniex is perfectly feasible as a evaluation tool in medical residency; its use can be can be spread to other medical education institutions
A avaliação dos estudantes e residentes de medicina através do Miniex já está bem fundamentada na literatura, com diversos trabalhos demonstrando sua validade e confiabilidade. Este trabalho objetiva a comparação de dois grupos de residentes de ortopedia na subárea de oncologia ortopédica, utilizando as avaliações do Miniex associadas a uma prova teórica. Foram realizadas um total de 56 avaliações do Miniex divididas em dois grupos de residentes, 7 residentes do 2° ano de ortopedia e 7 residentes do 3° ano. Verificou-se um melhor desempenho dos residentes do 3° ano para a competência de habilidade da entrevista médica, bem como nas notas da prova teórica. Identificou-se também ser habilidade na entrevista médica o melhor marcador para um bom desempenho na avaliação do Miniex. Analisando algumas características dos pacientes, tais quais gêneros e complexidade do caso, observouse a possibilidade dessas poderem influenciar na nota recebida pelo estudante. Observou-se também não haver associação entre as notas da prova teórica e as avaliações do Miniex. Os achados deste estudo sugeriram haver peculiaridades tanto no processo avaliativo quanto na carga do conhecimento individual que merecem considerações subjetivas além das notas oferecidas pelas avaliações tradicionalmente realizadas. A aplicação do Miniex como instrumento avaliativo é perfeitamente factível nas residências médicas, podendo seu uso ser ampliado nas instituições de formação médica

Most pediatric orthopaedic interventions are intended to improve or preserve physical function, yet their outcomes have been assessed using primarily surrogate measures (e.g., radiographic indices) that may not accurately represent patients'function. Physical function may be more appropriately measured with activity-based scales, but these have been infrequently applied in surgical studies. The purpose of this study was to identify existing activity-based physical-function scales appropriate for pediatric orthopaedics, to present criteria useful for scale selection, and to discuss the special problems of measuring physical function in children. Twenty-one scales relevant to pediatric orthopaedics are described according to their target population, purpose, method of administration, content, and quality of standardization. These scales have been further classified according to a new taxonomy. The unique aspects of measuring physical function in children are discussed and include the effect of age and development, method of reporting, and question formats. Standardized measures of physical function based on physical-activity ability exist and should be used more frequently to assess pediatric orthopaedic interventions

Annotation:
Movement simulation and musculoskeletal modeling can predict muscle forces, but current methods are hindered by simplified representations of joint structures. Simulations that incorporate muscle forces, an anatomical representation of the natural knee, and contact mechanics would be a powerful tool in orthopedics. This study combined a validated anatomical model of a knee joint with menisci and a musculoskeletal model of the human lower extremity. A forward-dynamics muscle driven simulation of the stance phase of a walk cycle was simulated in LifeMOD (Lifemodeler, Inc) and muscle forces and ground reaction forces were estimated. The predicted forces were evaluated using test data provided by Vaughan CL. et al. (1999).

In this study the tribological analysis of Al2O3 nanocomposites/Al2O3 pair; proposed as a candidate material to fabricate hip prostheses was carried out. Nanopowders of Al2O3 (AKP 50, 300 nm), TiO2 (PS-25, 50 nm) and Co metallic powder (Nilaco, 28 nm) were mixed and hot pressed. Wear test was carried out in a pin-on-plate tribometer, with a frequency of 1 Hz, a load of 49 N, for 4h; the counterface used was Al2O3. Mechanical properties as Vickers hardness, fracture toughness and Young’s modulus were estimated using the indentation method. Distilled water and fetal bovine serum solution (FBSS) were used as environment. It was found that the specific wear rate of Al2O3 nanocomposites was about 10−8mm3/N*m and the coefficients of friction were around 0.3–0.5 for FBSS. Worn surfaces were observed using SEM.

A computer-aided simulation model is developed to aid in the design and optimization of orthopaedic trauma implants. The developed model uses digital imaging, computer-aided solid modeling, and finite element methods in order to study the effects of various geometric parameters of fixation devices in orthopedic surgery practice. The results of the present simulation model would lead to the determination of the optimum implant design that provides the best match with the geometry of the human femur — reducing the risk of over-stressing bone tissue during implant insertion. The effectiveness of the presented simulation model is demonstrated through the design of intramedullary (IM) nails used in treating femoral shaft fractures. CT scans were taken of forty intact human femora. A technique was developed in order to digitally reconstruct the scans into 3D solid models using image segmentation, surface simplification, and smoothing methods while maintaining accurate representation of the original scans. Each resulting surface model is characterized by a network of nearly equilateral triangles of approximately the same size allowing for quality finite element meshing. Femoral lengths, curvature, shaft diameters, and location of maximum curvature were then quantified. An average geometric model was then generated for the investigated sample by averaging corresponding nodal coordinates in each femur model. Using the average model, a length-standardized function representing the curvature of the medullary canal was derived to create a geometrically optimized IM nail for the entire sample. “Virtual surgery” simulating the insertion process was then performed using finite element methods in order to validate the proposed optimal IM nail design. The results of both the optimum nail and a current nail were compared using the femur having the highest curvature in the sample. The present study shows that the developed simulation model leads to a nail design that reduces the insertion-induced stress within the femur to an acceptable level compared to current nails.

Nowadays, Virtual Prototyping (VP) methods are widely used for product design and development purposes. In particular, VP methods are now used also for the orthopedic products development process, to better understand the functional performance of prostheses or implants within the musculoskeletal system. In fact, developing validated virtual models of joints or of other anatomical structures may reduce design and prototyping costs and compress development cycles. The purpose of this paper is to point out first the state of art of both the VP technologies and the kinds of virtual models used in the medical field, with particular attention to those used for the design and development of orthopedic products. Then, it focuses on a qualitative analysis of some biomechanical simulation software packages (LifeMOD, AnyBody, and OpenSim) as tools for the improvement of the product design and development processes.
