Treatment of diseases of the musculoskeletal system to date remains not only a serious medical, but also socio-economic problem. Chronic diseases of the joints and spine are one of the most common causes of disability in patients. The main clinical manifestations of most diseases of the musculoskeletal system are pain and inflammation, which requires adequate antiinflammatory therapy. The data on various mechanisms of action of non-steroidal anti-inflammatory drug (NSAID) diclofenac (Voltaren), as well as the results of clinical studies and meta-analyses confirming its effectiveness and good tolerability are presented. The data of experimental and clinical studies of the efficacy and safety of local forms of diclofenac-Voltaren Emulgel (diclofenac-diethylamine) –are presented. Provides information about the form of the Voltaren Emulgel 2% for 12 hours. It is shown that a wide range of analgesic and anti-inflammatory effects of diclofenac (Voltaren), and good tolerance can be used as symptomatic therapy in the treatment of diseases of the musculoskeletal system.

The article describes a method of manual manipulation where the necessary movement of the element of the musculoskeletal system, which was selected for exposure, is performed in accordance with the principles of achieving vibrational and translational mechanical resonance. This method of mechanical and resonance manipulation proved to be more effective and less traumatic in clinical practice when compared with the traditional manipulation method, and the intended effect, which consisted in increasing mobility and more physiologically correct arrangement of the exposed elements of the musculoskeletal system, was more pronounced, namely, greater mobility was achieved with a more uniform increase in the mobility of all the exposed elements of the musculoskeletal system. At the same time, fewer manual therapy sessions by 30% were required for achieving the planned therapeutic effect, and, due to less trauma to biological tissues during mechanical and resonance manipulation, the sessions were less painful.

Musculoskeletal ultrasound (MSKUS) has been recognized as an important method for the evaluation of diseases of the musculoskeletal system, and contrast-enhanced ultrasound (CEUS) technology is becoming an important branch of it. The development of novel materials and tiny nano-formulations has further expanded ultrasound contrast agents (UCAs) into the field of nanotechnology. Over the years, nanoscale contrast agents have been found to play an unexpected role in the integration of precise imaging for diagnosis and treatment of numerous diseases. It has been demonstrated that nanoscale UCAs (nUCAs) have advantages in imaging over conventional contrast agents, including superior biocompatibility, serum stability, and longer lifetime. The potential value of nUCAs in the musculoskeletal system is that they provide more reliable and clinically valuable guidance for the diagnosis, treatment, and follow-up of related diseases. The frontier of advances in nUCAs, their applications, and insights in MSKUS are reviewed in this paper.

Successful implementation of modern treatment modalities for children with oncological and hematological diseases resulted in significant increase in overall and relapse-free survival. These patients are at risk of developing musculoskeletal late effects. The assessment of musculoskeletal late effects and mineral metabolism and the development of solutions for comprehensive rehabilitation and prevention of late effects are challenging issues for pediatrics. A literature review summarizes information related to musculoskeletal late effects associated with special treatment and demonstrates a need for the optimization of diagnostic approaches and corrective therapy of mineral metabolism. Up-to-date multidisciplinary rehabilitation treatment for children in remission can correct the consequences of their main disease and treatment-related side effects.

This review article elucidates the application and viability of electromagnetic fields as therapy and rehabilitation method for the treatment of musculoskeletal pathologies. All studies were found in different databases such as Science Direct, Scielo, IEEE, ProQuest, Pubmed, among others, where the search for information contemplated a period of 5 years (2016-2020). Results evidenced that electromagnetic fields used to treat musculoskeletal conditions showed positive results and improvements in reducing chronic and acute pain in different musculoskeletal pathologies such as osteoarthritis, osteoporosis, nonunions, fractures, fibromyalgia, muscular system injuries, among others. It was possible to evidence that one of the most used magnetic treatments were pulsed electromagnetic fields, which have been implemented to treat joint, muscle and nervous diseases. Additionally, several devices to generate the magnetic stimulation have been designed with a therapeutic and rehabilitation approach, which elucidate the progress in the implementation and acceptance of this type of therapeutic alternatives to treat musculoskeletal conditions. Finally, magnetic stimulation has been also implemented to enhance biomaterials function, evidencing that scaffolds stimulated with electromagnetic fields improve musculoskeletal tissue regeneration.

Infectious diseases complications remain one of the main problems of surgery. Infections are one of the causes of canser patients morbidity and mortality, along with tumor diseases. The frequency of postoperative wound complications varies between 3-34% depending on the type of surgery [5]. Cancer patients are more susceptible to the systematic immunosuppresive state, caused by malignant neoplasms and directly by antitumor therapy. It is obvious that the anticancer problem of treatment and prevention of secondary infections in these conditions becomes even more urgent than before. Objective: To analise infections complications (IC) caused by microorganisms depending on antitumor treatment in cancer patients. Materials and methods: The study included 41 (100%) cancer patients with infectious complications during antitumor treatment in the Azerbaijan Medical University (AMU) Cancer clinic for skin and soft tissue neoplasms. Most often, the pathological process was lokalized in the lower extremities in 19 (46%) patients; in the upper extremities in 14 (34,1%) patients: in the trunk in 8 (19,5%) patients. Of these 12 patients (29,2%) patients received treatment for posteopertive complicattions, 10 (24%) patients received treatment for complications – related to chemotherapy and 19 (46%) patients received treatmentrelated to radiation therapy. There were 28 (68%) men and 13 woman (36,5%) patients. The age of the patients ranged from 34 to 82 years among the studied patients with soft tissue tumors 24 (58%), skin tumors 17 (41,4%) patients .

Introduction. Traction in dynamic mode (kinesitraction) is a new direction protected by patents of Ukraine, which combines the procedure of traction and motor activity. Traction treatment of the spine in a dynamic mode plays an important role in the treatment of the musculoskeletal system, so it is important to understand the structure, mechanisms and features of the structures involved in these processes, including the bath developed by us. The aim of the study. To acquaint with a design of a bath for underwater horizontal polysegmental kinesitraction treatment of the spine and lower extremities lesions, methods of its use. Materials and methods. Content analysis, method of system and comparative analysis, bibliosemantic method of studying the actual scientific researches on baths’ designs for underwater horizontal polysegmental kinesitraction treatment of the spine and lower extremities lesions, methods of its use are used. Sources are searched in scientometric databases: PubMed, Medline, Springer, Google Scholar, Research Gate by tags: kinesitraction, musculoskeletal system, underwater horizontal treatment of the spine, traction, traction system. 22 English and Ukrainian literary sources which describe this problem were selected and analyzed. The description of the bath for underwater horizontal segmental traction treatment of the spine and lower extremities lesions in the dynamic mode was used (Certificate of copyright registration for a scientific work N 99985, 25.10.2020). Results. The designed by us bath for underwater polysegmental kinesitraction treatment of lesions of the spine and lower extremities (Certificate of registration of copyright to a scientific work N 99985, 2020) is intended for the treatment of diseases of the musculoskeletal system, internal organs, disorders of vital systems in the aquatic environment by exposure to dosed motor and traction loads. The design includes a water bath, armrests, handles, footrest, racks, cervical block system, chest block system, lumbar block system, head restraint belt, chest strap, pelvic girdle, shin strap, cable, load. Using a bath it is possible to carry out tractions of the cervical, thoracic, lumbar spine, their combinations, lower extremities, or total provide the traction. Conclusions. Traction treatment of the spine in a dynamic mode plays an important role in the treatment of the musculoskeletal system, being more effective than static. The understanding of the structure, methods and features of the structures involved in these processes, including our developed baths for underwater horizontal polysegmental hydrokinesitraction treatment of lesions of the spine and lower extremities, based on the correction of disorders of the nervous, musculoskeletal systems caused by morphological, biochemical, physiological mechanisms of stimulation of the functions of vital systems, regeneration processes, and the proposed methods of their use is an important component of effective treatment of diseases of the musculoskeletal system. Musculoskeletal disorders, including fractures, scoliosis, heterotopic ossification, osteoporosis, osteoarthritis, disc degeneration, and muscular injury, etc., can occur at any stage of human life. Understanding the occurrence and development mechanism of musculoskeletal disorders, as well as the changes in tissues and cells during therapy, might help us find targeted treatment methods. Single-cell techniques provide excellent tools for studying alterations at the cellular level of disorders. However, the application of these techniques in research on musculoskeletal disorders is still limited. This review summarizes the current single-cell and spatial omics used in musculoskeletal disorders. Cell isolation, experimental methods, and feasible experimental designs for single-cell studies of musculoskeletal system diseases have been reviewed based on tissue characteristics. Then, the paper summarizes the latest findings of single-cell studies in musculoskeletal disorders from three aspects: bone and ossification, joint, and muscle and tendon disorders. Recent discoveries about the cell populations involved in these diseases are highlighted. Furthermore, the therapeutic responses of musculoskeletal disorders, especially single-cell changes after the treatments of implants, stem cell therapies, and drugs are described. Finally, the application potential and future development directions of single-cell and spatial omics in research on musculoskeletal diseases are discussed.

Introduction: There is the increasing number of elderly patients with motion disabilities who require help and infatuations from nursing stuff. According to the Central Statistic Department (GUS), 25,4% people in Poland is over 60. In 2050 this number will increase to 40%(39,9%). This is very important topic because when the number of old people will increase the geriatric care will be more important. Especially because this is partially covered by nursing team. They should be satisfied and content of the job they do, and they should fulfill the aim they had when they started this profession. The aim: Analysis of nurse’s work with patients, nursing procedures evaluation, how this procedure is scored by the nurse, what is the nurse’s role in the rehabilitation process. Material and methods: There were nursing process with geriatric patients analyzed in this article. There were nursing procedures investigated with the opinion of the person who performed this procedure. Role of the nurse in rehabilitation process was and the satisfaction from typical work load was analyzed. Results: Analyzed results showed that there is a deficit in the nurse’s knowledge and the lack of the complex care in elderly patient with disabilities. There is lack of the preparation and education program to perform holistic patient care. This includes monitoring, examination and elderly needs. Conclusions: The nurse is the first link that helps the patient to understand his disease. The nurse develops therapeutic contact that helps the patient to feel safe and positively react for the introduced treatment.

Projected life expectancy continues to grow worldwide owing to the advancement of new treatments and technologies leading to rapid growth of geriatric population. Thus, age-associated diseases especially in the musculoskeletal system are becoming more common. Loss of bone (osteoporosis) and muscle (sarcopenia) mass are conditions whose prevalence is increasing because of the change in population distribution in the world towards an older mean age. The deterioration in the bone and muscle functions can cause severe disability and seriously affects the patients’ quality of life. Currently, there is no treatment to prevent and reverse age-related musculoskeletal frailty. Existing interventions are mainly to slow down and control the signs and symptoms. Mesenchymal stem cell (MSC) transplantation is a promising approach to attenuate age-related musculoskeletal frailty. This review compiles the present knowledge of the causes and changes of the musculoskeletal frailty and the potential of MSC transplantation as a regenerative therapy for age-related musculoskeletal frailty.

The training manual is based on many years of experience in the practical application of reflexotherapy in outpatient settings. The article presents a structural analysis of diseases of children and adolescents receiving treatment in the reflexology room of a childrens polyclinic, which revealed a high level of pathology of the musculoskeletal system and connective tissue and a steady growth trend of this group of diseases. Pathogenetic justifications, indications and contraindications for the use of reflexotherapy in Pediatrics and in diseases of the musculoskeletal system in particular are given. Principal therapeutic approaches and treatment regimens for patients with obstetric hand paralysis, spastic torticollis, migraine, Perthes disease and traumatic limb injuries are considered. Recommendations on the organization of outpatient use of reflexotherapy in Pediatrics, the methods used and their instrumental support are formulated. The training manual is intended for doctors who specialize in reflexology, already practicing reflexologists, orthopedic traumatologists and physiotherapists.

Because of the successful implementation of modern treatment technologies at any stage of therapy of children with oncohematological diseases, their overall and disease-free survival has increased significantly. According to recent observations, 80 % of children after the completion of a special antitumor treatment continue to have changes of different organs. Almost all patients are at risk for late complications from the musculoskeletal system but little attention is paid to this issue. Evaluation of long-term complications from the musculoskeletal system and mineral metabolism in patients, development of their complex rehabilitation and prevention is an actual problem of pediatric oncology and hematology. This review summarizes data on long-term complications from the musculoskeletal system after special therapy. The necessity of diagnostics approaches harmonization and correction of not only the most common complications, but also little studied conditions, such as reduction of bone mineralization, is shown. It is noted that multidisciplinary rehabilitation treatment of children in remission can not only correct the consequences of the underlying disease, concomitant pathology and the consequences of special therapy but also reduce the disability of children.

The main pathogenetic mechanisms of the development of a traumatic disease of the musculoskeletal system are peripheral circulation disorder, hypoxia, and inflammation, which is accompanied by the development of pain. Pain management is a primary component in helping patients with various injuries. The activation of sanogenetic reactions is also crucial in accelerating the processes of damaged tissues regeneration. A scientific search on the impact of acupuncture on typical pathophysiological processes of traumatic disease was conducted in the Medline and Google Scholar databases. The analysis of scientific journal articles showed that despite the ambiguous attitude to acupuncture, the effectiveness of the method is quite high. Acupuncture is a branch of complementary medicine that has been used for thousands of years. Scientific research in recent decades has established a positive therapeutic effect on various links of pathogenesis through the implementation of the main mechanisms of acupuncture, at the segmental, suprasegmental and cortical levels, manifested in the form of neurohumoral and neuroimmune reactions. The presented literature review directs researchers to further study the possibilities of acupuncture in the treatment of traumatic diseases of the musculoskeletal system from the standpoint of evidence-based medicine.

Limitations of the motor sphere as the most severe consequences of diseases of the musculoskeletal system drastically reduce the quality of life. The WHO statistics shows that 80 % of the population, the majority of which are the people of working age, suffer from various diseases of the musculoskeletal system. The proportion of patients of working age who have limitations of the musculoskeletal system increased after the adoption of the law on raising the retirement age, which makes the problem of their treatment and rehabilitation even more important. Great economic damage requires the search for new approaches to repairing a defect of the locomotor system. Total joint replacement is acknowledged effective method of treating patients. However, the results of arthroplasty do not always satisfy the patient and the surgeon. The reason for this is the lack of rehabilitation programs, which have a negative impact on the treatment process. The purpose of this study is to analyze self-control as a personal resource for restoring the social functioning of patients after endoprosthetics of major joints.

AbstractWith its outstanding soft tissue contrast, spatial resolution, and multiplanar capacities, magnetic resonance imaging (MRI) has become a widely used technique. Whole-body MRI (WB-MRI) has been introduced among diagnostic methods for the staging and follow-up assessment in oncologic patients, and international guidelines recommend its use. In nononcologic applications, WB-MRI is as a promising imaging tool in inflammatory diseases, such as seronegative arthritis and inflammatory myopathies. Technological advances have facilitated the introduction of three-dimensional (3D) almost isotropic sequences in MRI examinations covering the whole body. The possibility to reformat 3D images in any plane with equal or almost equal resolution offers comprehensive understanding of the anatomy, easier disease detection and characterization, and finally contributes to correct treatment planning. This article illustrates the basic principles, advantages, and limitations of the 3D approach in WB-MRI examinations and provides a short review of the literature.

The advent of genomic technologies is considerably leading to developments in the field of molecular medicine. The accumulation of detailed information about gene expression, epigenetic variability, protein transcription and functional modulation is contributing to a new era in medicine. Rapid and early diagnostic procedures, molecular characterisation of degenerative and proliferative diseases and personalized therapies are predicted to lead to advancements in health prevention and treatment of disease. Diagnostic tools and therapies based on local and /or general modulation of cellular processes for traumatic or degenerative musculoskeletal conditions are becoming available. A logical consequence of the information derived from extensive data gathering, systems biology and systemic medicine has lead to significant improvements in understanding biological structure and function in a simultaneous bottom top and integrative, holistic manner. The description of disease mechanism at an intimate, subcellular level has a dual benefit. A thorough understanding of the crosstalk involved in molecular pathways both in the normal and the diseased state are expanding scientific knowledge and simultaneously are enabling design cell-targeted and individualized therapies. This paper presents a brief overview of current molecular based treatments available to the orthopedic surgeon and introduces the concept of systemic medicine from the perspective of musculoskeletal pathology.

Non-steroidal anti-inflammatory drugs (NSAIDs) are one of the most commonly used type of drugs in the world and are prescribed for a wide range of indications, including osteoarthritis, rheumatoid arthritis, other diseases of the musculoskeletal system and soft tissues, as well as for the relief of postoperative pain. It is well known that the use of NSAIDs is associated with the risk of gastrointestinal (GIT), cardiovascular (CVS) and kidneys complications. Lornoxicam belongs to the NSAIDs of the oxicam class and is a balanced inhibitor of both cyclooxygenase (COX) isoforms – COX1 and COX2 – and differs from other oxicams in its rapid onset of action, high elimination rate. The drug is characterized by a pronounced analgesic effect and a favorable safety profile both in relation to the gastrointestinal tract and cardiovascular system, does not require dose adjustment in patients over 65 years of age.

Demographic transformation, characterized by the aging of the population, is causing an increasing problem in developed countries. This change involves a significant increase in the number of chronic diseases, the health damage generated by which causes loss of life years due to deteriorating health and impairs quality of life. Among chronic diseases, the increasing frequency of musculoskeletal disorders has become characteristic of an aging society, which causes the greatest loss of life years in Hungary due to limitations. These problems mean increasing social, economic, and administrative pressure on the population and pose solution challenges for the spa town leaders and health decision-makers. There are several therapies available in the medical and health sciences to prevent and treat musculoskeletal disorders, with increasing emphasis on conservative therapies as the role of health increases. In Hungary, among these procedures, medicinal water treatment services based on natural healing factors available in spa towns play a key role, which is also the basis of medical tourism and part of the health care system. To solve the problems caused by musculoskeletal disorders, it is essential to know the occurrence of the disease and the treatment-use attitude of the patients, mainly due to the COVID-19 pandemic. Therefore, the main goal of our research is to assist spa towns leaders and health decision-makers in the implementation of medical tourism developments and more optimal patient care. One of the part-aims of our research is to reveal the regional differences of the most common musculoskeletal diseases in Hungary based on secondary data. Our other research-part objective is to determine the impact of socio-demographic characteristics, health status, type of musculoskeletal disease, pain, and commitment to bath medicine care system on the future use of medicinal water treatment in patients with musculoskeletal disorders. Based on our results, we declare that the health status of the Hungarian population in terms of the most frequently occurring locomotor diseases is worst in Central Hungary, the greater part of the Southern Great Plain, and the northeastern part of the country. In terms of territory, we concluded that the incidence of musculoskeletal disorders is relatively low, and moderate inequality in Hungary. It also follows from our results that the indicators measuring regional differences selected can be successfully applied to examine the territorial inequalities of musculoskeletal diseases concerning medical tourism. We also found that the respondents' level of family income, place of residence by region, state of health, the degree of commitment to medicinal water treatment/service was found, furthermore the cost of treatment, and the cost of accommodation /travel, significantly affect the planned use of the medicinal water treatment in the future. Our results promote the implementation of more targeted medical tourism and health industry developments in spa towns.

The vast differentiation potential and the relative ease for culture expansion have established mesenchymal stem cells as the building blocks in cell therapy and tissue engineering applications for a variety of musculoskeletal diseases, including repair of fractures and bone defects, cartilage regeneration, treatment of osteonecrosis of the femoral head, and correction of genetic diseases such as osteogenesis imperfect. However, research in the past decade has revealed differentiation potentials of mesenchymal stem cells beyond lineages of the mesoderm, suggesting broader applications than originally perceived. In this article, we review the recent developments in mesenchymal stem cell research with respect to their emerging properties and applications in nonmusculoskeletal diseases.

Увеличение продолжительности жизни человека, наблюдаемое во всем мире, требует особого внимания к возрастным изменениям, которые влияют на качество жизни. Остеоартроз и остеопороз входят в список основных заболеваний, которые приводят к ухудшению здоровья, изменению образа жизни и в конечном итоге, к инвалидности. Несмотря на то, что современные методы, применяемые для лечения как остеоартроза, так и остеопороза достаточно широки, разработка новых препаратов для лечения этих заболеваний является весьма актуальной. Среди таких препаратов особое внимание привлекают пептидные биорегуляторы, которые обладают геропротекторным эффектом, а также оказывают профилактическое действие при различных патологических состояниях организма. Одним из таких средств является пептидный препарат Хондролюкс®, который может быть рекомендован для профилактики заболеваний, связанных с изменениями хрящевой и костной тканей. Musculoskeletal diseases are most common in the elderly. The associated physical pain and limited ability to move impair quality of life and lead to disability in older patients. Among musculoskeletal diseases, osteoarthritis and osteoporosis are the most common. Osteoarthritis is a disease associated with age-related changes in the cartilage tissue. Osteoporosis, primarily postmenopausal, is characterized by decreased bone density. The similarity of these diseases is due to the fact that they both develop over a long time and are characteristic of older people. With age, both bone and cartilage tissues tend to lose their structure due to deterioration of metabolic processes. Therefore, it is important to maintain the musculoskeletal system as long as possible and focus on prevention of osteoarthritis and osteoporosis. Treatment of such diseases is associated with substantial economic costs; therefore, development of new, efficient and safe methods, which would qualitatively change the structural and functional state of bone and cartilage tissues, is highly relevant. Experimental studies showed that a new peptide drug, Chondrolux®, can be recommended for prevention and treatment of diseases associated with degenerative and dystrophic changes in the cartilage and bone tissues.

AbstractThe effects of radiation and chemotherapy on the musculoskeletal (MSK) system are diverse, and interpretation may be challenging. The different lines of treatment have effects on diseased and normal marrow, and they may lead to complications that must be differentiated from recurrence or progression. This review analyzes the changes induced by radiotherapy and chemotherapy in the MSK system in the adult and pediatric population, and the expected associated imaging findings. Treatments are often combined, so the effects may blend. Awareness of the spectrum of changes, complications, and their imaging appearances is paramount for the correct diagnosis. The assessment of body composition during and after treatment allows potential interventions to implement long-term outcomes and personalize treatments. Imaging techniques such as computed tomography or magnetic resonance imaging provide information on body composition that can be incorporated into clinical pathways. We also address future perspectives in posttreatment assessment.

WB-MRI provides information about diffuse multifocal pathologies with excellent anatomical definition through high soft tissue contrast and spatial resolution as well as valuable functional information from diffusion-weighted images. In addition to its roles in establishing the diagnosis and assessing the extent and severity of disease, WB-MRI is also useful for monitoring the response to treatment for malignant and benign systemic diseases affecting the musculoskeletal system. This article reviews and updates the applications of WB-MRI in current practice, discussing the role of this helpful tool in various conditions involving the musculoskeletal system including bone metastases, hematologic cancers, inflammatory processes, infections, and multisystemic-multifocal bone, nerve, vascular, and muscle/soft tissue disorders, as well as other idiopathic conditions.

BACKGROUND: There are many risk factors that result in musculoskeletal disorders because of work. This also occurs in the instant food industry, where apart from manual load handling and repetitive work, the production process can also results in risks. AIM: The aims of this study are to predict the musculoskeletal disorders in the next 50 years and the effectiveness of scenarios for controlling musculoskeletal disorders. METHODS: This study employed Research and Development method through a dynamics system approach. This research was conducted in one of the industries that produce instant food in South Sulawesi, Indonesia. The data obtained was based on interviews, which were further analyzed using Interpretative Structural Modeling. RESULTS: Based on the simulation results for 30 years, it was found that there was an increase in the average musculoskeletal disorder incidence by 20.63% per year. At the end of the simulation in 2050, the number of musculoskeletal disorder incidents became 48481.69. In this case, the simulation for 30 years (2020–2050) was conducted on a model of controlling occupational diseases at an instant noodle company in Makassar by providing treatment in the form of reducing risk factors that cause musculoskeletal disorders. Based on the simulation results for 30 years (2020–2050), musculoskeletal disorders have the most significant contribution to the increase of occupational diseases incidents as a whole. The increase in musculoskeletal disorders is an accumulation of several risk factors that exist in the instant noodle production process. CONCLUSIONS: Prediction of the musculoskeletal disorder incidence using a dynamic system approach for 30 years (2020–2050) has increased by an average of 20.63% per year. The behavior of the model after receiving treatment on the occurrence of musculoskeletal disorders has an average decrease in the average incidence of 51.11% per year. In this case, to control the musculoskeletal disorder incidence, the elements or variables controlled simultaneously are work posture, lifting load, and length of work.

Diseases of the musculoskeletal system are the most common pathologies of the modern society, yielding frequency only to diseases of respiratory and cardiovascular systems. Given the fact that they are occurred in a young, working-age population, as well as people in older age groups, the treatment of these patients consumed large financial resources, which results in a high socio-economic importance of rheumatic diseases in general. The article focuses on the prevalence of these forms of pathology in Russia and several foreign countries, material costs of the medical care of such patients (including direct, indirect and additional costs), the change of these ratios in connection with the increased longevity of the population, and the emergence of new high-tech methods of treatment, which led in some cases to a significant rise in the cost of medical services. There are discussed measures to optimize the treatment of these pathologies based on pharmaco-economic aspects.

Locomotive syndrome is a geriatric syndrome that corresponds to the model of autonomy loss and the emergence of addictions due to pathology of the musculoskeletal system with a heavy medical and social burden. Combination of musculoskeletal system diseases, including osteoarthritis, osteoporosis, dorsopathy, sarcopenia, neuropathy, together with impaired physical functioning are the clinical characteristics of locomotive syndrome. Only comprehensive programs, including pain treatment, the risk of osteoporetic fractures, prevention of falls, correction of sarcopenia, physical training, psychotherapeutic methods, etc., can increase the mobility of patients with locomotive syndrome and help reduce addictions in older age.

The article presents the information about the effectiveness and safety of the use of аceclofenac in the treatment of patients with diseases of the musculoskeletal system. The results of numerous clinical studies indicate comparable or superior efficacy of аceclofenac in the treatment of ankylosing spondylitis, rheumatoid arthritis, lower back pain, osteoarthritis. Aceclofenac is characterized by good tolerability, rarely causes adverse events and has a high adherence of patients to treatment.

Diseases of the musculoskeletal system and injuries are identified as one of the main causes of disability of the country's population, which leads to a permanent loss of working capacity of adults. This leads to an increase in production losses, an increase in social benefits, an increase in the share of costs for restorative treatment and ensuring an adequate quality of life for the particular part of the population. Strengthening the rehabilitation direction in the health care system may be a possible solution to the problem. A systematic analysis of the levels of primary disability of the population as a result of injuries and diseases of the musculoskeletal system can become a reliable basis for determining the need for rehabilitation services in the region and planning the appropriate amount of resources for their provision. The aim: to study the features of changes in the levels of disability of the population of the Transcarpathian region during 2011-2020 and to provide an assessment of the needs for rehabilitation services for people with injuries and diseases of the musculoskeletal system in the region. Materials and methods. The data of the Centre for Medical and Social Expertise of the Transcarpathian region for the years from 2011 to 2020 served as research materials. The medical-statistical method and the method of structural-logical analysis were used during the study. The results. It was established that in the region the third and fourth places in the structure of causes of disability for ten years were stably taken by diseases of the musculoskeletal system (13.0-16.9%) and injuries (9.4-10.8%). In total, 3,807 people were recognized as disabled for the first time as a result of these pathologies, and their average annual number was 983±49 people. The positive increase in the absolute number of cases of disability due to diseases of the musculoskeletal system and injuries during 2011-2019 outweighed the negative by 37.8%. With a fairly insignificant increase in the number of cases of newly established disability due to diseases of the musculoskeletal system, their share in the structure of causes of general disability increased from 13.0% to 16.9%, and the decrease of 23.6% in cases of annual disability due to injuries was also accompanied by a decrease in their share in the structure of causes of disability from 10.2% to 9.7%, which is connected with the trends of changes in the total number of cases of disability. The results of the analysis showed the preservation of the relevance of diseases of the musculoskeletal system and injuries as causes of permanent disability, which requires the response of the health care system of the Transcarpathian region by creating effective methods to improve the situation, one of which can be the provision of timely, full-fledged and high-quality rehabilitation assistance to such patients. The used methodological approach to the analysis and interpretation of the dynamics of the population's disability levels can serve as a management tool for regular calculations of the region's population's need for rehabilitation services. Conclusions. Disability of the population of the Transcarpathian region due to injuries and diseases of the musculoskeletal system remains an important issue for the regional health care system, which causes a high level of rehabilitation needs. Administrative analysis of the average annual number of people with a newly established disability for the specified reasons and a detailed interpretation of the dynamics of disability levels will allow to plan the rehabilitation services and their resource provision in accordance with the needs of the population.

Bacteriophage therapy has recently attracted increased interest, particularly in difficult-to-treat infections. Although it is not a novel concept, standardized treatment guidelines are currently lacking. We present the first steps towards the establishment of a “multidisciplinary phage task force” (MPTF) and a standardized treatment pathway, based on our experience of four patients with severe musculoskeletal infections. After review of their medical history and current clinical status, a multidisciplinary team found four patients with musculoskeletal infections eligible for bacteriophage therapy within the scope of Article 37 of the Declaration of Helsinki. Treatment protocols were set up in collaboration with phage scientists and specialists. Based on the isolated pathogens, phage cocktails were selected and applied intraoperatively. A draining system allowed postoperative administration for a maximum of 10 days, 3 times per day. All patients received concomitant antibiotics and their clinical status was followed daily during phage therapy. No severe side-effects related to the phage application protocol were noted. After a single course of phage therapy with concomitant antibiotics, no recurrence of infection with the causative strains occurred, with follow-up periods ranging from 8 to 16 months. This study presents the successful outcome of bacteriophage therapy using a standardized treatment pathway for patients with severe musculoskeletal infection. A multidisciplinary team approach in the form of an MPTF is paramount in this process.

The purpose of this study is to identify key factors that determine a person’s decision to seek treatment from traditional Korean medicine (TKM) instead of conventional medicine through analysis of nationally representative data from Korea, where a dual healthcare system exists. The analysis is based on episodic data from the 2008 and 2009 Korea Health Panel. The main dependent variable is the selection between TKM and conventional medicine. We used a multiple logistic regression model to identify the determinants of TKM while controlling for clustered error. Approximately 5% of all doctor’s visits were characterized as TKM services. Urban residents were 1.441 times more likely to use TKM than rural residentsP=0.001. The probability of choosing TKM over conventional medicine for a range of conditions compared to the reference condition (gastrointestinal disease) was as follows: circulatory system diseases (OR 5.267,P<0.001), nervous system diseases (OR 12.054,P<0.001), musculoskeletal system diseases (OR 20.579,P<0.001), and neoplasms (OR 0.209,P=0.004). Certain diseases are significantly more likely to be treated by TKM than by conventional medicine. This suggests that many people view TKM as being additionally effective for specific diseases, particularly musculoskeletal disorders.

Diseases of the musculoskeletal system are accompanied by nociceptive, neuropathic pain which is, in addition to kinesitherapy, physical therapy and acupuncture, widely treated using the local injection therapy. This treatment method is commonly used by physicians of different specialties for about 100 years and includes intra-articular, periarticular, perineural, intramuscular administration of different medicines, homeopathic remedies or medical devices. Hyaluronic acid salts, nonsteroid anti-inflammatory drugs, glucocorticosteroids, local anesthetics, vitamin B12, botulinum toxin A medications are often used for the treatment of degenerative conditions, posttraumatic joint disease, delayed complications of aseptic, bacterial and viral inflammatory, rheumatic processes within and out of the joints, such as: pathology of periarticular soft tissues as a consequence of local physical overload after acute injury, chronic microtrauma, sport trauma, affected facet joints and periarticular space at patients with dorsopathy, local muscle hypertonus (myofascial pain syndrome), neuropathy. The unique medication Hyalrepair-02/10 chondroreparant® based on the solid-state modificated hyaluronic acid salts and biogenic metabolites (non-drug bioorganic depot complex consisting of hyaluronic acid, amino acids and ascorbyl phosphates) is widely used at spine diseases and diseases of the musculoskeletal system, and it has showed good safety at local injection therapy. Heterogeneity of target tissues at patients with the musculoskeletal disorders, pharmacological substances as a part of injection drugs, and dosages of medicines and medical devices as well as individual patient reactions to the local therapy depending on severity of the disease, compensatory abilities and other reasons determines the necessity of carrying out a clinical comparison between different types of local injections with various groups of preparations.

The analysis of the main etiological factors of the musculoskeletal disorders in athletes is presented. The study of the anatomical, histological and functional features of the bone-cartilaginous structures in athletes made it possible to establish the priority macro- and micronutrients, which should be the basis in modern specialized and functional food products. Brief composition characteristics of the new canned food “Enmit-beef” and dry protein concentrate “Ostov” (for the drink preparation) are presented. The preliminary results of the use of these functional meat-based products indicate their effectiveness in rehabilitation and treatment of athletes with musculoskeletal disorders.

Hematogenous osteomyelitis is still a common disease, accounting for 6-10% of all pyoinflammatory diseases [3], while the incidence varies from 0.3 to 0.75 per 1000 child population. The proportion of complications of acute hematogenous osteomyelitis (CSO) in the structure of purulent-septic diseases ranges from 8.6 to 12%, in the structure of orthopedic pathology - from 3 to 6% [4]. Orthopedic complications (dislocations, deformities, shortening, contractures and ankylosis of large joints) are observed in 3171% of cases [5]. In children, the severity of the disease in the long term is aggravated by the defeat of the metaepiphyseal zones, which leads to a violation of the further formation of the musculoskeletal system [1, 2, 23].

Thyroid disease can be accompanied by a variety of rheumatic manifestations, ranging from early growth defects during infancy to adult manifestations such as arthralgias, myalgias, myopathy, acropachy, arthritis, osteoporosis. Objective — to provide a summarizing current literature on the analysis of a variety of musculoskeletal disorders in hyperthyroidism and hypothyroidism and possible mechanisms that explain this connection. Materials and methods. The authors conducted a systematic literature search for relevant Englishlanguage publications published between June 2011 and October 2021 in MedLine, PubMed, and Google Scholar. A variety of rheumatic manifestations in hyperthyroidism and hypothyroidism include: arthropathy, myopathy, adhesive shoulder capsulitis, thyroid acropachy, tunnel (carpal) syndrome, Raynaud’s phenomenon, Hoffman’s syndrome, osteoporosis. Thyroid diseases commonly cause musculoskeletal complaints and may even present with rheumatic syndromes before the nature of the underlying endocrinopathy is apparent. On occasion, thyroid disorders can mimic some rheumatic diseases and leading to diagnostic errors. On the other hand, thyroid disorders can coexist with rheumatic diseases as well as rheumatic symptoms and findings. Musculoskeletal complaints can be related to unidentified and untreated thyroid diseases. However, it is important to keep in mind that the therapy for these diseases might lead to myopathies. As a result, internists, endocrinologists, and rheumatologists should be well-versed in recognizing how thyroid illnesses influence the musculoskeletal system and must be aware of these correlations to ensure that the associated condition is not missed, or the diagnosis is not delayed. This will aid in avoiding diagnostic mistakes and provide early suspicion of thyroid pathology and correct treatment. The evidence-based medicine showed and substantiated in vitro effect of the drug on the pathogenetic processes in the cartilage tissue, its promotion of the inflammation regression in the musculoskeletal system. The authors present a series of clinical studies of top osteoarthritis doctors, which showed that Alflutop had an anti-inflammatory, chondroprotective, analgesic effect, and that the administration of the drug allowed patients to reduce the doses of NSAIDs, and it could be used in patients with comorbid diseases.

Due to demographic changes, the number of older drivers is steadily increasing. Mobility is highly relevant for leading an independent life in the elderly. It largely depends on car driving, which is a complex task requiring a multitude of cognitive and motor skills vulnerable to age- related functional deterioration. The almost inevitable effects of senescence may be potentiated by age-related diseases, such as stroke or diabetes mellitus. Respective pharmacological treatment may cause side effects, additionally affecting driving safety. The present article reviews the impact of age-related diseases and drug treatment of these conditions on driving fitness in elderly drivers. In essence, we focus on diseases of the visual and auditory systems, diseases of the central nervous system (i.e., stroke, depression, dementia and mild cognitive disorder, and Parkinson’s disease), sleep disorders, as well as cardiovascular diseases, diabetes mellitus, musculoskeletal disorders, and frailty. We will outline the role of functional tests and the assessment of driving behavior (by a driving simulator or in real traffic), as well as the clinical interview including questions about frequency of (near) accidents, etc. in the evaluation of driving fitness of the elderly. We also address the impact of polypharmacy on driving fitness and end up with recommendations for physicians caring for older patients.

In this paper, the authors investigated methods for the prevention and treatment of flat feet. It was found that the problem under consideration was and remains relevant, despite the scientific progress and great success in the medical industry over the past decades. The growth of diseases is facilitated by the frivolous attitude of people to this pathology, because few people think about the scale of the problem. It has been confirmed that flat feet negatively affects not only the musculoskeletal system, but also other organ systems and the state of the body as a whole. It has also been found that the presence of flat feet can indicate a person's overall health. Today, the problem of providing assistance to adults and children with impaired foot function remains relevant and significant. Three main factors take part in the development of this disease: changes in the general physical condition, a violation in the distribution of physical activity on the musculoskeletal system, and an innate tendency. An analysis of scientific literature has shown that today the rehabilitation system for children with orthopedic pathology provides for the use of various means and methods of kinesiotherapy, where a special place is given to gymnastic and sports-applied exercises aimed at strengthening the muscles that form the arch of the foot. It was revealed that the basis of most of the diagnosed cases of flat feet is a weak tone of the muscular skeleton of the lower leg and foot, as well as excessive load on the legs caused by a long stay in an upright position. The author provided an optimal definition of the concept of flat feet, described its types and forms, named the main causes and symptoms, considered methods of prevention, treatment and correction.
