De nombreux travaux relient aujourd'hui les complications de la maladie hypertensive à un défaut de perfusion des organes cibles. Ces anomalies sont essentiellement représentées par une raréfaction artériolo-capillaire, et une dysfonction endothéliale. La correction efficace du niveau de la pression artérielle reste un objectif essentiel qui pourrait cependant être complété par l'objectif de préserver ou de restaurer les perfusions tissulaires. Dans un premier travail nous avons validé la mesure de la réactivité de la circulation cutanée mesurée par vélocimétrie laser Doppler après administration locale de doses cumulatives d'acétylcholine par iontophorèse. La réponse cutanée à Pacetylcholine a été comparée à la dilatation flux-dépendante de l'artère humérale après hyperémie post-occlusive de la main. Nous avons retrouvé une très bonne corrélation entre réactivité cutanée et réponse humérale (r = 0. 910, PO. 000001). Nous avons ensuite démontré que la densité capillaire cutanée peut être normalisée par un traitement anti-hypertenseur. Il semble que, malgré un contrôle tensionnel équivalent, toutes les classes de médicaments n'ont pas le même effet sur la microcirculation. Nous avons notamment démontré la supériorité d'une association fixe périndopril-indapamide dans ce domaine. La relation de causalité entre l'atteinte microcirculatoire et l'hypertension artérielle reste toujours en suspens. Nous avon démontré que l'augmentation de la pression artérielle observée sous traitement par le bevacizumab, un anti-VEGF utilisé en carcinologie, pourrait être, au moins partiellement, expliquée par la raréfaction capillaire et la dysfonction endothéliale dans la microcirculation<br>Many works link the complications of the hypertensive disease to a defect of perfusion of the target organes. These anomalies are primarily represented by an arteriolo-capillary rarefaction, and an endothelial dysfonction. The effective adjustment of the level of blood pressure remains a crucial objective, although preserving or restoring the tissue perfusions should not be neglected. In the first work, we have validated the measurement of the reactivity of the cutaneous circulation using laser Doppler flowmetry after local deliverance of cumulative amounts of acetylcholine by iontophoresis. The cutaneous response to acetylcholine was compared with the flow mediated vasodilation of the brachial artery. We found a very significant corrélation between cutaneous reactivity and brachial answer (r = 0. 91, P&lt;0. 000001). We then showed that the cutaneous capillary density rarefaction can be reversed by an antihypertensive treatment. Moreover, in spite of a blood pressure control equivalent, all the classes of drugs do not have the same effect on the microcirculation. We in particular showed the superiority of fixed association of périndopril-indapamide in this field. The relationship between the microcirculatory damage and arterial hypertension is not yet clearly established. We showed that the increase of the blood pressure under treatment by the bevacizumab, an anti-VEGF used in oncology, could be, at least partially, explained by capillary rarefaction and endothelial dysfonction in the microcirculation

Le sepsis constitue une des complications les plus graves pouvant survenir dans le domaine de la réanimation ou dans le traitement des malades immunodéprimés. Il se caractérise par l'apparition de nombreuses complications, tel que l'hypotension associé à une diminution des résistances vasculaires périphériques et le syndrome de défaillance respiratoire (SDRA) responsable une hypoxie systémique. Le but de ce travail est d'étudier d'une part les effets des médiateurs du sepsis (" tumor necrosis factor ", interleukines 1 et 6) sur le tonus et la réactivité à la norépinéphrine (NE) du réseau artériolaire et d'autre part les effets de l'hypoxie systémique sur l'adhérence des leucocytes au niveau du réseau veinulaire. Ces travaux sont effectués sur un modèle expérimental original permettant l'étude in vivo de la microcirculation du muscle crémaster chez le rat anesthésié. Dans la première partie de ce travail, nous avions montré que le TNF, l'IL-1 et l'IL-6 ont des effets différents sur le réseau artériolaire du muscle crémaster de rat. L'administration de TNF entraîne une vasodilatation rapide des artérioles. Ces cytokines ont aussi des effets différents sur la réactivité des artérioles à la NE. Une hyporéactivité à la NE apparaît 2heures d'exposition en présence de TNF. Par contre, l'IL-1 induit une hyporéactivité à la NE dans les minutes qui suivent son administration sur la préparation à la NE dans les minutes qui suivent son administration sur la préparation. Aucune modification de réactivé à la NE n'est observée en présence d'IL-6. Dans la deuxième partie de ce travail, nous avons montré que l'hypoxie systémique augmente l'adhérence des leucocytes sur l'endothélium veinulaire du muscle crémaster de rat. Cet effet rapide de l'hypoxie systémique se produit même lorsque la préparation musculaire est suffisamment oxygénée. Cette observation peut expliquer la dissémination de la réponse inflammatoire au niveau des tissus périphériques lors du SDRA.

The cerebral microcirculation consists of a complex network of small blood vessels that support nerve cells with oxygen and nutrition. The blood flow and oxygen delivery in the microcirculatory blood vessels are regulated through mechanisms which may be influenced or impaired by disease or brain damage resulting from conditions such as brain tumors, traumatic brain injury or subarachnoid hemorrhage (SAH). Monitoring of parameters relating to the microvascular circulation is therefore needed in the clinical setting. Optical techniques such as diffuse reflectance spectroscopy (DRS) and laser Doppler flowmetry (LDF) are capable of estimating the oxygen saturation (SO2) and tracking the microvascular blood flow (perfusion) using a fiber optic probe. This thesis presents the work carried out to adapt DRS and LDF for monitoring cerebral microcirculation in the human brain. A method for real-time estimation of SO2 in brain tissue was developed based on the P3 approximation of diffuse light transport and quadratic polynomial fit to the measured DRS signal. A custom-made fiberoptic probe was constructed for measurements during tumor surgery and in neurointensive care. Software modules with specific user interface for LDF and DRS were programmed to process, record and present parameters such as perfusion, total backscattered light, heart rate, pulsatility index, blood fraction and SO2 from acquired signals. The systems were evaluated on skin, and experimentally by using optical phantoms with properties mimicking brain tissue. The oxygen pressure (pO2) in the phantoms was regulated to track spectroscopic changes coupled with the level of SO2. Clinical evaluation was performed during intraoperative measurements during tumor surgery (n = 10) and stereotactic deep brain stimulation implantations (n = 20). The LDF and DRS systems were also successfully assessed in the neurointensive care unit for a patient treated for SAH. The cerebral autoregulation was studied by relating the parameters from the optical systems to signals from the standard monitoring equipment in neurointensive care. In summary, the presented work takes DRS and LDF one step further toward clinical use for optical monitoring of cerebral microcirculation.

The models of the microcirculation of blood and interstitial fluid in the human liver lobule are developed based on the classical hexagon model of Kiernan. Both blood and interstitial flows in the lobule are treated as flows in porous medium connected via the fenestrated membrane of sinusoids. Several important physiological components are developed and included in the models. The lobule with tissue elasticity shows that the pressure-flux relationship is non-linear and the poroelastic model has more compliance than the solid elastic model. Models of the interstitial flow in both a single lobule and the whole liver are also developed. The results show that our models can predict the amount of interstitial fluid drainage including the ascites. From the parameter studies, we find that the permeabilities of the sinusoids and the interstitial space, and the portal pressure are the most important factors on ascites production. We further investigate the oxygen transportation and uptake by liver cells using the advection–diffusion equations and Michaelis-Menten kinetics. The studies show that the main mechanism of oxygen transportation within the sinusoids is advection; however, the transportations within the interstitial space and across the fenestrated endothelial cells are mainly from diffusion process. The effect of the arrangement of the vessels and the geometry of the lobule on blood perfusion and oxygen distribution is also studied. The results show that the classical hexagonal lobule with the vascular septa provides the optimal perfusion compared to other geometries of the lobule. In summary, this thesis contributes to the development of mathematical models of several important features in the liver microcirculation such as the tissue elasticity, the interstitial flow, the oxygen distribution, and the arrangement of the vessels in the lobule.

L'application d'une pression sur la peau provoque une vasodilatation cutanée, appelée vasodilatation induite par une pression (PIV), et retarde l'apparition de l'ischémie tissulaire liée à cette pression. La pression appliquée sur la peau active les fibres nerveuses capsaïno-sensibles, qui sécrètent en réponse des neurotransmetteurs, qui eux provoquent au niveau de l'endothélium la synthèse et la sécrétion de facteurs endothéliaux qui induisent une relaxation du muscle lisse. Ce travail avait pour objectif d'étudier l'effet du vieillissement sur la PIV.<br />Chez la souris âgée, sans neuropathie périphérique, la PIV était altérée en raison d'une diminution de la vasodilatation endothélium-dépendante ; et la contribution des facteurs endothéliaux de la vasodilatation était modifiée : l'Endothelium-Derived Hyperpolarizing Factor jouait un rôle primordial en raison d'une diminution du monoxyde d'azote et de la prostacycline. Chez les sujets âgés (60-75 ans), la PIV était altérée en comparaison avec les sujets jeunes (20-35 ans) en raison d'une diminution de la vasodilatation endothélium-dépendante mais aussi d'une altération des fibres capsaïno-sensibles et/ou des neurotransmetteurs. En présence d'une neuropathie périphérique, la PIV était abolie. Ces modifications de la microcirculation cutanée au cours du vieillissement expliqueraient la plus grande vulnérabilité de la peau à l'ischémie et l'augmentation du risque d'ulcère de pression liée à l'âge. La compréhension des modifications de la PIV avec l'âge permet d'entrevoir de nouvelles perceptives de prévention et de traitement de l'ulcère de pression chez le sujet âgé.

Summary<p>Brain dysfunction is a frequent complication of sepsis and is usually defined as “sepsis-associated encephalopathy” (SAE). Its pathophysiology is complex and related to a number of processes and pathways, while the exact mechanisms producing neurological impairment in septic patients have not been completely elucidated. Alterations in cerebral blood flow (CBF) have been suggested as a key component for the development of SAE. Reduction of CBF may be caused by cerebral vasoconstriction, induced either by inflammation or hypocapnia. More importantly, the natural mechanisms that protect the brain from reduced/inadequate CBF can be impaired in septic patients, especially in those with shock, and this further contributes to cerebral ischemia if blood pressure drops below a critical threshold. Hypercapnia is associated with a narrower autoregulatory plateau, which may potentially results in large CBF variations when mean arterial pressure (MAP) varies within usual targets. However, few data are available on the role of PaCO2 on cerebral autoregulation (CA). Finally, as SAE occurs also in patients without hemodynamic instability, alterations in brain tissue perfusion could occur independently from hypotension; thus, alterations in cerebral microcirculation, which largely regulates regional flow and blood-cellular nutrients exchanges, could contribute to SAE. In septic animals, these microcirculatory abnormalities could be implicated in the development of electrophysiological abnormalities observed during sepsis and contribute to neurological alterations. However, these findings were limited by several factors, including the technique used to assess the microcirculation, the short time of observation and the limited amount of fluid resuscitation used in those models. <p>In the first part of this work, I evaluated CA and the potential influence of PaCO2 on CA in patients with septic shock. In 21 mechanically ventilated patients, I observed that 14 of them had impaired CA. All the 7 patients with a PaCO2 ≥ 40 mmHg but only 7 of the 14 patients with a PaCO2 <40 mmHg had an impaired CA (p=0.046). Specifically, 4/9 (44%) patients with PaCO2 < 35 mmHg, 7/9 (77%) with PaCO2 between 35 and 42 mmHg, and 3/3 (100%) with PaCO2 > 42 mmHg had impaired CA. The Receiver Operating Characteristic (ROC) analysis showed that a PaCO2 threshold of 38 mmHg had a sensitivity of 50% and a specificity of 100% for the prediction of impaired CA, with an area under the ROC curve of 0.76 (95% confidence interval: 0.52–0.91).<p>In the second part of this work, I hypothesized that altered cerebral microcirculation may occur in the early phase of sepsis and contribute to brain hypoxia. In a clinically relevant model of ovine fecal peritonitis, I showed that there was a progressive deterioration of cerebral microvascular flow in septic animals (n=10) when compared to sham animals (n=5), starting already after 6 hours from sepsis induction and becoming significant at 12 hours thereafter. Moreover, changes in the cerebral microcirculation were not related to changes in MAP, cardiac output or blood lactate levels, suggesting that these alterations in the brain may occur even when global perfusion pressure is maintained, i.e. in non-hypotensive conditions. In a second study, including 10 septic and 5 sham animals, I found that cortical microvascular alterations were associated with decreased cerebral oxygenation. Furthermore, cerebral metabolic disturbances compatible with tissue hypoxia (i.e. increased brain lactate/pyruvate ratio, LPR) occurred mostly during shock, suggesting that hypotension is a critical factor in the development of anaerobic metabolism in the septic brain. Nevertheless, I showed in a third study (n=8) that the reversal of hypotension using vasopressor agents, although increased cerebral oxygenation and slightly reduced LPR, did not significantly influence the alterations of cerebral microcirculation and was associated with an increase in glutamate and glycerol, suggesting ongoing excitotoxicity and cellular damage. These alterations in cerebral microcirculation, oxygenation and metabolism may then contribute to the pathogenesis of SAE.<p><p><p><p><p>Résumé<p>La dysfonction cérébrale est une complication fréquente du sepsis et elle est généralement identifiée comme « encéphalopathie associée au sepsis » (sepsis-associated encephalopathy, SAE). La physiopathologie de la SAE est complexe et liée à des nombreux processus et voies de signalisation, même si les mécanismes qui induisent cette dysfonction cérébrale chez les patients en sepsis n’ont pas été clairement élucidés. Des anomalies du débit sanguin cérébral (cerebral blood flow, CBF) ont été proposées comme une des déterminants pour le développement de l’SAE. La réduction du CBF pourrait être induite par une vasoconstriction cérébrale, élicitée pas l’inflammation ou par l’hypocapnie. De plus, les mécanismes qui naturellement règlent le CBF pour qu’il soit ni diminué ni inadéquat aux besoins cellulaires peuvent être altérés pendant le sepsis, particulièrement en cas de choc septique, et ceci pourrait davantage contribuer au développement de zones d’hypoperfusion cérébrale si la pression artérielle diminue au-dessous d’un seuil critique. Un autre point important est que l’hypercapnie est associée à une diminution du plateau d’autorégulation du CBF, ce qui pourrait potentiellement causer des larges variations du CBF endéans des valeurs de pression artérielle considérés comme normaux en pratique clinique; malheureusement, très peu de données sont disponibles sur le rôle de la PaCO2 sur l’autorégulation cérébrale (cerebral autoregulation, CA). Enfin, vu que l’SAE survient aussi chez des patients qui n’ont pas d’instabilité hémodynamique, des anomalies de la perfusion cérébrale régionale pourraient se produire en absence de toute hypotension artérielle ;en effet, des altérations de la microcirculation cérébrale, qui règle le débit sanguin au niveau des tissues et l’échange d’oxygène et nutriments entre la circulation sanguine et le cellules, peuvent aussi contribuer au développement de la SAE. Dans des modelés expérimentaux de sepsis, les altérations microcirculatoires ont été associées à des troubles électrophysiologies et à la présence d’anomalies « cliniques ». Cependant, ces données ont été biaisées par le type de technique utilisée pour évaluer la microcirculation, le temps d’observation très court et la quantité limitée de fluides administrés au cours de la réanimation liquidienne dans ces modelés. <p>Dans la première partie de ce travail, j’ai décrit les anomalies de la CA et l’impact de la PaCO2 sur la CA chez des patients en choc septique. En étudiant 21 patients en ventilation mécanique, j’ai pu observer que 14 d’entre eux avaient une CA altérée, y compris 7/14 avec une PaCO2 < 40 mmHg et 7/7 avec une PaCO2 ≥ 40 mmHg (p = 0.046). De plus, 4/9 (44%) avec PaCO2 < 35 mmHg, 7/9 (77%) avec PaCO2 between 35 and 42 mmHg, and 3/3 (100%) avec PaCO2 > 42 mmHg avaient une CA altérée. L’analyse selon la « Receiver Operating Characteristic » (ROC) montrait une sensibilité de 50% et une spécificité de 100% pour prédire une CA altérée, avec un seuil de PaCO2 de 38 mmHg (l’aire sous la courbe de l’analyse ROC était à 0.76 [95% ICs: 0.52–0.91]).<p>Dans la deuxième partie de ce travail, j’ai émis l’hypothèse que des anomalies de la microcirculation cérébrale peuvent survenir dans la phase précoce du sepsis et contribuer au développement d’une hypoxie tissulaire. Dans un modelé de péritonite fécale induite chez le mouton, très proche de la situation clinique, j’ai pu montrer que il existe une détérioration progressive de la microcirculation cérébrale chez les animaux septiques (n-=10) quand comparés aux animaux témoins (n=5) qui commence déjà a la sixième heure de l’induction du sepsis and devient significative après 12 heures. De plus, les changement de la microcirculation cérébrale n’étaient pas corrélés à ceux de la pression artérielle, du débit cardiaque ou des taux de lactate, ce qui suggère que ces anomalies peuvent se produire aussi en conditions de stabilité hémodynamique. Dans une deuxième étude comprenant 10 animaux septique et 5 témoins, j’ai observé que les anomalies microcirculatoires étaient associées à une diminution de l’oxygénation cérébrale tissulaire. Toutefois, les anomalies du métabolisme cérébral compatible avec une hypoxie tissulaire (des valeurs élevées du rapport lactate/pyruvate, LPR) se développaient que dans la phase du choc septique, indiquant que l’hypotension artérielle est le facteur déterminant pour ces anomalies métaboliques au cours du sepsis. Cependant, dans une troisième étude sur 8 animaux en sepsis, j’ai démontré que la correction de l’hypotension par administration de vasopresseurs, même si elle était associée à une augmentation de l’oxygénation cérébral et une diminution du LPR, n’améliorait pas de façon significative la microcirculation cérébrale et s’accompagnait par une augmentation des taux de glutamate et glycérol, ce qui plaidait plutôt pour un excitoxicité persistante et une progression des lésions cellulaires. Toutes ces anomalies de microcirculation, oxygénation et métabolisme cérébral pourraient contribuer à la pathogenèse de l’SAE.<p><br>Doctorat en Sciences médicales<br>info:eu-repo/semantics/nonPublished

The development of this PIV depends on the activation by pressure of sensory C-fibres, leading to the release of neurotransmitters that acts at the endothelium level to stimulate the synthesis and release of endothelial factors inducing smooth muscle relaxation. This work had for objective to study the modifications of the PIV with the ageing. In old mice, without peripheral neuropathy, the PIV was reduced because of a decrease of the endothelium-dependent vasodilatation and the endothelium factors of the vasodilatation were redistributed with a contribution of Endothelium-Derived Hyperpolarizing Factor because of a decrease of the nitric oxide and the prostacyclin. In old subjects (60-75 years), the PIV was altered in comparison with young subjects (20-35 years) because of a decrease of the endothelium-dependent vasodilatation and a sensory fibres and/or neuromediators alteration. In the presence of a peripheral neuropathy, the PIV was abolished. These modifications of the cutaneous microcirculation with the ageing would lead to an early occurrence of ischemia related to an increased risk for pressure ulcers and the understanding of PIV modifications allow to glimpse of news perceptives of prevention and treatment of pressure ulcers in elderly

Notch3 est une protéine membranaire impliquée dans la différenciation des cellules musculaires lisses et la régulation du réflexe myogénique cérébral. L’objectif de ce travail est de préciser le rôle de Notch3 dans la microcirculation rénale. Chez des souris Notch3 KO, nous avons analysé la réactivité vasculaire en réponse à des agents vasoactifs et la réponse à un modèle d’hypertension artérielle (HTA) chronique. L’étude invasive de l’hémodynamique rénale nous a permis de mettre en évidence une altération de la vasoréactivité des souris Notch3 KO en réponse aux vasoconstricteurs (Norépinéphrine/Angiotensine II ; AngII) et aux vasodilatateurs (Bradykinine/PGI2). Ex vivo, la réponse à l’AngII des artérioles afférentes des souris Notch3 KO n’est pas affectée par le retrait du calcium du milieu extracellulaire. Ces résultats démontrent que les souris Notch3 KO présentent une altération majeure de la vasoréactivité rénale et suggèrent que cette anomalie est liée à une défaillance dans le processus d’internalisation du calcium extracellulaire. En réponse à un modèle d’HTA induite par l’AngII, les souris Notch3 KO présentent une insuffisance cardiaque (diminution de la fraction de raccourcissement) responsable d’une importante mortalité de ce groupe (56%, contre 14% dans le groupe contrôle). L’exploration fonctionnelle des souris Notch3 KO a mis en évidence une polyurie osmotique en rapport avec une natriurèse anormale. Cette observation inattendue suggère des implications extravasculaires de la voie de signalisation Notch3 dans le rein. Ce travail démontre que Notch3 joue un rôle clé dans plusieurs mécanismes de la physiologie rénale et de l’adaptation cardiovasculaire à l’HTA

This thesis forms part of an ongoing long-term project to investigate the suitability of Doppler optical coherence tomography (OCT) as a measurement tool to investigate skin thickness and blood flow in patients with systemic sclerosis. There is a discussion of the characterisation of an electro-optic phase modulator for use in a Doppler OCT imaging system which is being built for the purpose of clinical studies. In addition to this the development of software for the same system is described. The work includes a comparison of two methods of obtaining Doppler information that were tested with the system; a phase resolved method and a correlation mapping method. Initial structural and Doppler images obtained using the system are presented. In addition to this the development of semi-automated software to measure skin thickness from both OCT and high frequency ultrasound images is discussed. The results of a study, for which this software was developed, into skin thickness measurements using both techniques in both patients with systemic sclerosis and healthy controls are presented. Both OCT and high frequency ultrasound were able to measure a statistically significant difference in epidermal thickness at multiple locations on the body. Finally, the modification of a freely available Monte Carlo simulation for light propagation in multi-layered tissue (MCML) to enable the simulation of structural and Doppler OCT images is covered. The simulation was able to extract the magnitude of the simulated flow accurately to within an order of magnitude, and after a simple filter was applied to eliminate fluctuations in the data the structure of the Doppler image closely matched what was modelled.

La microcirculation cutanée a été proposée comme modèle d'étude de la dysfonction microvasculaire globale dans les maladies cardiovasculaires. Par ailleurs, elle est spécifiquement atteinte dans le syndrome de Raynaud, qui est une ischémie paroxystique des extrémités déclenchée notamment par le froid. L'exploration de la fonction microvasculaire cutanée suscite donc un réel intérêt, mais les méthodes d'étude souffrent d'une hétérogénéité importante, et leur variabilité intra-individuelle est mal connue. La première partie de ce travail fait la synthèse des différentes méthodes d'étude la fonction microvasculaire cutanée, et rapporte les résultats de deux études consacrées à leur reproductibilité. Nous avons dans une seconde partie étudié grâce à ces tests la réactivité microvasculaire cutanée dans le syndrome de Raynaud, et mis en évidence des anomalies chez ces patients, notamment du contrôle neuro-vasculaire. La dernière partie de cette thèse est consacrée à l'étude d'approches pharmacologiques ciblées sur les anomalies de la microcirculation cutanée identifiées chez les patients. Nous avons évalué l'effet du sildenafil, un inhibiteur de la phosphodiesterase-5, sur le flux sanguin digital et montré son effet vasodilatateur lors d'un refroidissement local dans le syndrome de Raynaud. Enfin, nous avons étudiés chez l'animal et chez l'homme l'iontophorèse de vasodilatateurs, une approche innovante d'administration locale de médicaments pour augmenter le flux sanguin cutané.

Inflammatory bowel diseases, (IBD), are a group of chronic disorders of the gastro-intestinal tract, and include Crohn’s disease (CD) and Ulcerative Colitis (UC). The pathogenesis is not known, but involves at least in part a loss of tolerance towards the commensal colonic microbiota. In this thesis, we show in animal models of CD and UC that the colonic mucosal blood flow increased compared to healthy animals. This blood flow increase is due to an up regulation of endothelial nitric oxide synthase (NOS). Further, we show in the UC model that the thickness of the firmly adherent colonic mucus layer increased compared to healthy animals. This increase is due to an up regulation of inducible NOS in the epithelium. Both the blood flow and mucus thickness increase appear to be protective mechanisms.  We demonstrate that the firmly adherent colonic mucus layer acts as a partial barrier towards luminal bacteria. In the UC model, this barrier is destroyed, causing increased bacterial translocation. The adhesion molecule P-selectin was up regulated in the UC model, leading to increased interactions between leukocytes and the endothelium, but also increased interactions between platelets and the endothelium. This indicates that not only leukocytes, but also platelets are involved in colonic inflammation. The addition of the probiotic bacterial strain Lactobacillus reuteri prevented disease by normalizing P-selectin levels and endothelial interactions with leukocytes and platelets. Lactobacillus reuteri also decreased bacterial translocation over the epithelium. In summary, this thesis highlights the importance of colonic barrier functions, and investigates the role of the microbiota in experimental IBD.

Low back pain is a costly financial and loss of productivity societal issue. Although its aetiology is unclear, it has been associated with the intervertebral disc and its degeneration which has been thought to be caused in part by poor nutrition. In this thesis the caudal disc of skeletally mature equines is utilised as an animal model. Techniques such as x-ray, light imaging, histology and magnetic resonance imaging were employed to investigate the vascular and structural anatomy of the disc and its surroundings and the uptake of tracers within the disc tissue. A detailed study of the anatomy revealed similarities with the human disc. The equine disc consists of two distinct structural areas; a nucleus and annulus. The surrounding vascularisation is similar; a main anteriorly positioned artery, the median caudal artery splits and encircles the centre of the vertebral body providing nutrition to the vertebral body. Smaller vessels anastomose over the surface of the vertebral body. Within the vertebral body the vessels end in capillary terminations at the edge of the vertebral cartilage endplate. As in humans these terminations were seen to vary along the endplate with shape and density; the capillaries are densest and larger in the area next to the nucleus. The cartilage endplate itself was found to have a variable width; of between 0.16 mm and 0.33 mm being widest at the nucleus. The annulus was seen to consist of lamellar rings which had high collagen content. A marked difference between equines and humans found was the number and width of lamellae present; equines were found to have on average 5 lamellae with a width range of 140 10 1110 microns. The shape of the discs was also found to be different with equines having almost circular coccygeal discs which have a concave superior and inferior surface. The nucleus of the disc, unlike current literature was found to have local order. An important contribution to knowledge which this thesis has made is data collected on diffusion time and partition coefficient on many regions within the disc. It was found that to reach equilibrium it took up to 22 hours in the outer anterior annulus but only 5 ½ hours in the central nucleus. Diffusion was found to be fastest with the neutral ring-shaped molecule Gadovist and slowest in the positive ion, manganese. Partition coefficients between the tracers were also found to vary. The highest partition coefficient was 6 in the central nucleus with manganese and the lowest was 0.5 with Magnevist at 0.5 in the nucleus area. This information will be useful in aiding drug delivery clinically and performing contrast enhanced imaging for pathology detection.

Son efficacite est illustree par la determination simultanee de la structure et des conditions operatoires (consommation d'energie) optimales pour un procede d'esterification continue ainsi que pour un reseau d'echangeurs de chaleur

La néoangiogénèse est impliquée dans le développement des tumeurs. Certains facteurs stimulant l’angiogénèse ont été isolés et peuvent être inhibés. La surveillance de l’activité de ces thérapies inhibitrices, souvent cytostatiques, ne peut se baser sur une observation morphologique. L’étude fonctionnelle de la microcirculation est possible en IRM grâce à l’analyse de la pharmacocinétique de traceurs dans la tumeur. L’activité d’une thérapie antiangiogénique a été surveillée en IRM grâce au Spin Labeling (ASL) couplé à une suppression du bruit de fond. Cette technique permet de quantifier la perfusion tissulaire, grâce à un marquage électromagnétique des protons du sang. L’ASL a permis de distinguer dès le 1er mois les patients répondeurs des patients en progression, ce que les mesures morphologiques ne révélaient qu’à 3 mois. L’analyse de la perméabilité capillaire par l’analyse dynamique des rehaussements (ADR) tissulaires et artériels après injection de produit de contraste a été optimisée grâce à une séquence double écho dont la pondération était adaptée aux concentrations mesurées : T2* pour les concentrations élevées et T1 pour les basses. Cette technique a permis de réduire de 58% l’erreur d’estimation de la perméabilité capillaire. Le développement d’une technique de mesure des temps de relaxation en une apnée a permis d’améliorer les estimations de perfusion (ASL) et de perméabilité (ADR) en évitant d’utiliser des valeurs de T1 tumorales approximatives lors de la quantification. En mesurant la perfusion et la perméabilité tumorale, cibles des traitements antiangiogéniques, l’ASL et l’ADR se présentent comme des méthodes attractives pour le suivi de ces thérapies<br>Angiogenesis plays a key role in tumor growth. New approaches to treat cancer by interfering with angiogenesis stimulating factors are now available. However, clinicians need new surrogates of antiangiogenic activity. Tumor size changes occure 2 or 3 month after the beginning of the treatment, which is to late. Functional analysis of tracer kinetic in tissue in MRI may be an attractive alternative. Antiangiogenic activity of a VEGF receptor inhibitor, was evaluated by tumor blood flow, assessed by arterial spin labeling (ASL) with background suppression. Preliminary results reveal a significant correlation (Spearman r = 0. 90, p =. 0002) between the change in blood flow at 1 month and change in tumor size measured at 4 months or the time of disease progression. A capillary permeability assessment in dynamic contrast enhancement (DCE) was optimized using a dual gradient echo sequence. This dual sequence was sensitive to high concentration thanks to T2* weighted images and to low concentrations with T1 weighted slices. The use of this sequence leads to reduce by 58% the permeability measurements. The development of a new technique to assess to relaxation time in the body within a single breath hold may improve perfusion and permeability measurements. Real relaxation time can be used for quantification instead of theoretical values, reducing systematic error. ASL and DCE have promise as early predictors of clinical response to antiangiogenic therapies and may help to identify non-responding patients

Microcirculation has become a key factor for the study and assessment of tissue perfusion and oxygenation. Detection and assessment of the microvasculature using videomicroscopy from the oral mucosa provides a metric on the density of blood vessels in each single frame. Information pertaining to the density of these microvessels within a field of view can be used to quantitatively monitor and assess the changes occurring in tissue oxygenation and perfusion over time. Automated analysis of this information can be used for real-time diagnostic and therapeutic planning of a number of clinical applications including resuscitation. The objective of this study is to design an automated image processing system to segment microvessels, estimate the density of blood vessels in video recordings, and identify the distribution of blood flow. The proposed algorithm consists of two main stages: video processing and image segmentation. The first step of video processing is stabilization. In the video stabilization step, block matching is applied to the video frames. Similarity is measured by cross-correlation coefficients. The main technique used in the segmentation step is multi-thresholding and pixel verification based on calculated geometric and contrast parameters. Segmentation results and differences of video frames are then used to identify the capillaries with blood flow. After categorizing blood vessels as active or passive, according to the amount of blood flow, quantitative measures identifying microcirculation are calculated. The algorithm is applied to the videos obtained using Microscan Side-stream Dark Field (SDF) imaging technique captured from healthy and critically ill humans/animals. Segmentation results were compared and validated using a blind detailed inspection by experts who used a commercial semi-automated image analysis software program, AVA (Automated Vascular Analysis). The algorithm was found to extract approximately 97% of functionally active capillaries and blood vessels in every frame. The aim of this study is to eliminate the human interaction, increase accuracy and reduce the computation time. The proposed method is an entirely automated process that can perform stabilization, pre-processing, segmentation, and microvessel identification without human intervention. The method may allow for assessment of microcirculatory abnormalities occurring in critically ill and injured patients including close to real-time determination of the adequacy of resuscitation.

A mathematical model has been developed which describes the fluid exchange from a capillary network of realistic topology, and calculates the spatial distribution of extravascular pressure. In this model, the capillaries are represented by a superposition of sources and sinks, resulting from a D'Arcy's Law description of flow in tissue of uniform fluid conductivity. The combination of this representation and Starling's Hypothesis, which relates the forces influencing transmural fluid exchange, yields an integral equation of the second kind which is solved numerically for the source strength distribution. Two important features of this approach are that: (i) it allows for interaction between the local tissue pressure field and fluid exchange (the model is called, therefore, the tissue pressure interaction model); and (ii) complex network morphologies are easily modeled. In single capillaries, this interaction, which decreases the predicted fluid exchange, increases with the magnitude of the ratio of capillary wall to extravascular fluid conductivities. For multiple capillaries, in addition to the "self" interaction of a capillary with the local extravascular pressure field, there is the possibility of interaction between capillaries ("capillary-capillary" interaction). The ratio of conductivities, and the additional factors of intercapillary distance and the number of capillaries, also affect interaction in capillary networks. Although interaction is only a weak function of intercapillary distance, it depends strongly on the number of capillaries. The major result from this work is that for the entire physiological range of conductivity ratios, interaction cannot be neglected in predicting fluid exchange. Although tissue pressure interaction affects the magnitude of fluid exchange, it does not greatly alter the pattern of extravascular flow. Therefore, previous models which neglected interaction are not invalidated by the present findings. The effect of interaction on planar capillary networks within a semi-infinite tissue space was also investigated. Flow boundary conditions were imposed at opposed planar boundaries, parallel with the capillary network. Interaction was found to decrease with decreasing distance between the boundary and plane of the capillaries. It still exerted a large effect, however, for distances greater than one-fourth the reference capillary length.

Résumé. Le sepsis, reconnu d’intérêt publique, est défini comme une dérégulation de la réponse de l’organisme face à une infection, pouvant entrainer une défaillance d’organes engageant le pronostic vital. Il se traduit par l’altération des fonctions micro et macrocirculatoires, en lien avec la réponse inflammatoire et métabolique. Malgré l’évolution des thérapies, le pronostic reste sévère, conduisant à l’émergence de stratégies thérapeutiques innovantes en lien avec l’amélioration de nos connaissances physiopathologiques. Ainsi, le lactate de sodium molaire (LSM) a montré des effets bénéfiques dans l’arrêt cardiaque et le traumatisme cérébral. Notre équipe a antérieurement démontré dans un modèle murin de sepsis qu’un pré-traitement avec du LSM permet l’amélioration de la fonction myocardique, de la microcirculation et module le processus inflammatoire. Néanmoins, il s’agissait d’un modèle non réanimé avec une thérapie préventive. L'objectif du travail de thèse était d'évaluer les effets du LSM dans un modèle murin de sepsis, dans des conditions reproduisant au plus près les approches cliniques. Pour cela, nous avons induit le sepsis chez le rat mâle Wistar par ligature et ponction caecale (CLP), dont nous avons suivi l’évolution des paramètres macrocirculatoires par télémétrie. Ainsi, nous avons débuté la réanimation des animaux 10 h après la CLP correspondant à la baisse de la pression artérielle systolique et diastolique. Nous avons comparé les effets du LSM et du NaCl 0,9% (soluté de référence) à une dose équivalente de 2,5 mL/Kg/h durant 18 h, en association à une antibiothérapie sur les paramètres macrocirculatoires, microcirculatoires, et sur la réponse inflammatoire et le métabolisme. Résultats. Nous avons montré un effet favorable du LSM sur la microcirculation mésentérique évaluée par laser speckle comparé au NaCl 0,9%, sans effet significatif sur la macrocirculation (pression artérielle, fréquence cardiaque, fonction myocardique). Nous avons montré que le lactate de sodium permet d’améliorer de façon dose dépendante la réactivité vasculaire ex vivo en condition inflammatoire induite par le LPS. Nous avons observé une modulation du processus inflammatoire dans notre modèle animal induite par le LSM avec une baisse significative du taux circulant de TNF-α. Ceci pourrait être associé à la surexpression de son récepteur GPR81. Sur le plan métabolique, nous avons montré l’utilisation métabolique du lactate sans modification des taux de corps cétoniques. En conclusion, nous avons montré dans un modèle de sepsis expérimental par CLP réanimé que le LSM permet d’améliorer la microcirculation mésentérique et l’inflammation sans effet sur la macrocirculation. Les mécanismes responsables de ces effets favorables restent à définir. Ces résultats obtenus sur ce modèle expérimental soutiennent l’intérêt du LSM comme piste thérapeutique métabolique dans la prise en charge du sepsis<br>Abstract. Sepsis, recognized as a public health concern, is defined as a dysregulation of host response to infection, potentially leading to life-threatening organ failure. It results in impaired micro- and macrocirculatory functions, associated with inflammatory and metabolic responses. Despite advances in therapies, the prognosis remains severe, prompting the development of innovative therapeutic strategies linked to improvements in our pathophysiological knowledge. Molar sodium lactate (MSL) has shown beneficial effects in cardiac arrest and traumatic brain injury. Our team previously demonstrated in a murine model of sepsis that pre-treatment with MSL improved myocardial function, microcirculation, and modulated the inflammatory process. However, this was a non-resuscitated model with preventive therapy. The objective of this thesis was to evaluate the effects of MSL in a murine model of sepsis under conditions closely resembling to clinical approaches. To this end, we induced sepsis in male Wistar rats through cecal ligation and puncture (CLP) and monitored the evolution of macrocirculatory parameters via telemetry. We initiated animal resuscitation 10 hours after CLP, corresponding to the decrease of systolic and diastolic blood pressure. We compared the effects of MSL and 0.9% NaCl (reference solution) at an equivalent dose of 2.5 mL/kg/h for 18 hours, combined with antibiotic therapy, on macrocirculatory, microcirculatory parameters, and on inflammatory response and metabolism. Results. We demonstrated a favorable effect of MSL on mesenteric microcirculation as assessed by laser speckle, compared to 0.9% NaCl, with no significant effect on macrocirculation (blood pressure, heart rate, myocardial function). We showed that sodium lactate improves vascular reactivity in a dose-dependent manner ex vivo under inflammatory conditions induced by LPS. We observed a modulation of the inflammatory process induced by MSL in our animal model, with a significant decrease in circulating TNF-α levels, which could be associated with the overexpression of its receptor, GPR81. On a metabolic level, we demonstrated the metabolic utilization of lactate without changes in ketone body levels. In conclusion, we showed in a resuscitated experimental CLP-induced sepsis model that MSL improves mesenteric microcirculation and inflammation without affecting macrocirculation. The mechanisms responsible for these favorable effects remain to be defined. These results obtained in this experimental model support the potential of MSL as a metabolic therapeutic option in sepsis management

Background: Sepsis may be modelled in vivo using lipopolysaccharide (LPS) to cause microvascular disturbances including macromolecular leak and leucocyte adhesion. Such changes may be due to an altered balance of eNOS and iNOS. Statins are widely used to lower cholesterol. Recently evidence indicates statins may exert pleiotropic effects, possibly restoring physiological levels of eNOS and iNOS.

Le sujet de cette thèse est l'optimisation du transport de l'oxygène dans la microcirculation artérielle à travers la recherche d'une valeur optimale de la concentration en globules rouges. Les vaisseaux constituant cette portion du réseau vasculaire présentent une différence d'échelle de plusieurs ordres de grandeurs. Les phénomènes physiques décrivant la circulation sanguine y sont donc de natures très différentes. Par conséquent nous avons divisé notre étude en deux parties. Dans la première partie nous avons modélisé le réseau des petites artères dont le diamètre varie entre 1,5 mm et 500 μ m. Le modèle utilisé tient compte du comportement non-newtonien du sang. Après avoir montré l'existence de valeurs optimales de la concentration en globules rouges, nous avons étudié par des approches analytiques et numériques leur comportement en fonction des variations des paramètres géométriques du réseau ; puis nous avons discuté des interprétations physiologiques de ces valeurs. Dans la seconde partie nous nous sommes intéressés à la capture de l'oxygène dans des capillaires pulmonaires de 8 μ m de diamètre. Nous avons proposé un modèle numérique par éléments finis de capillaire 2D axisymétrique contenant une succession de globules régulièrement espacés. Pour traiter l'interaction fluide-structure entre les domaines fluides et la membrane déformable des globules nous avons utilisé la méthode « caméra » qui est une adaptation de la méthode ALE (Arbitrary Lagrangian Eulerian) standard permettant de suivre le globule dans son déplacement moyen sans avoir à modéllser l'intégralité du capillaire. La modélisation du transport de l'oxygène prend en compte la convection, la diffusion mais également les réactions chimiques entre l'hémoglobine contenue dans les globules rouges et l'oxygène. Nous avons mis en évidence un phénomène « d'écrantage » qui limite la diffusion de l'oxygène et nous avons appliqué ce modèle au cas d'un œdème pulmonaire<br>This PhD thesis deals with the optimization of oxygen transport in arterial microcirculation by the research of an optimal value of red blood cells concentration. In this part of the vascular network, the vessels diameters range over several orders of magnitude, therefore the physical phenomena describing blood circulation are of different nature. So, we divided our study into two parts. In the first part we model a network of small arteries whose diameters range from 1,5 mm to 500 um. The model takes into account the non-Newtonian behavior of the blood viscosity. First, we show the existence of optimal values for red blood cells concentration. Then we study by analytical and numerical means the behavior of those optimal values according to variations of geometrical parameters of the network. Last, we discuss physiological interpretations of these values. In the second part we study the capture of oxygen in idealized pulmonary capillaries of 8 um diameters containing a succession of regularly spaced cells. To treat the fluid-structure interaction problem between the fluid and the deformable cells membrane, we develop a 2D axisymmetrical numerical method based on finite elements method called the "camera" method, which is an adaptation of the standard ALE method (Arbitrary Lagrangian Eulerian). The camera method enables to follow the red cell without the need to model the whole capillary. The modeling of oxygen transport not only takes into account the convection and diffusion of hemoglobin and oxygen, but also the chemical reactions between both species inside the red blood cells. Thanks to our model, we identified a "screening" phenomenon that limits the capture of oxygen by red blood cells, and we were able to study the effect of pulmonary edema
