Crohn’s disease and ulcerative colitis are both chronic inflammatory bowel diseases (IBD). This article summarizes current best practice in treating pregnant patients with IBD, ranging from conservative therapy to endoscopy and imaging methods, including a description of surgical therapy indications. Female patients with IBD should ideally plan their pregnancies for when their disease is in remission. Patients in remission may also have complications during pregnancy, however the risk of complications is lower than in patients with active disease. Any chronic medications they were on before becoming pregnant (with the exception of teratogenic methotrexate) should remain unchanged. According to the current literature, pregnancy does not itself complicate the course of IBD. In cases of a severe relapse or an occurrence of complications in these patients, careful multidisciplinary cooperation is required, especially between the gastroenterologist, surgeon, radiologist, and gynaecologist. Surgical treatment is required only in cases of acute complications of IBD (such as acute severe colitis resistant to medical therapy, perianal abscess, and complications of IBD in the sense of such acute abdomen events as perforations, ileus due to a stenosis, or massive haemorrhage).

In 2018 year, the patent of original adalimumab expired and a new biosimilar version of adalimumab have been introduced on the Czech market. FKB-327 is one of the new biosimilar adalimumab versions and was approved for all indication of the original drug. This is the first experience with biosimilar adalimumab FKB-327 in IBD patients. Patients cohort comprised from 51 patients included 40 (82%) ones with Crohn´s disease and 9 (18%) ones with ulcerative colitis. Most of the patients (78%) have been naive for biologic therapy. A positive therapeutic effect during the median follow up time (37 weeks) was described in 47 (92%) patients. This drug was tolerated very well and none of the treated patients had to stop prematurely the drug administration due to significant side effects.

Introduction: Endoscopic transpapillary drainage is a common method of solving malignant biliary obstruction. An alternative to the transpapillary approach is endosonographically guided biliary drainage using a lumen-apposition stent, which can be used not only in case of ERCP failure. Case report: We describe our experience with endosonographically guided choledocho-duodenal drainage using a lumen-apposition stent, which was complicated by delayed hemobilia. Persistent and severe bleeding was successfully resolved by endovascular embolization using a spiral coiling. Conclusion: Endosonographically guided drainage using a lumen-apposition stent may, despite all advantages, be accompanied by serious complications, including significant bleeding. Therefore, the availability of an interventional radiology unit is appropriate.

The diploma thesis concerns my life project ZaBřehem and my activity in this project, which has the character of cooperation, organization, DIY, home design, web design, painting, crafts and over time also cultural organization and curation. The work goes through several components of the whole project and aims to defend the role of the artist in creating a new place that has the ambition to change the approach to people diagnosed with mental illness. At the same time, it focuses on the space specifically of one of the rooms, the layout of which the author considers her own work, as well as the method of transferring this space to the school premises for defense, which will create another site-specific installation.

The aim of this thesis is to provide a close reading and a philosophical and anthropological interpretation of Burton's understanding of the concept of melancholy (as well as the concept of the human being and the world) as it is presented in his work The Anatomy of Melancholy. The primary objective will be gradually to respond to the following questions: How did Burton perceive the concept of melancholy? How did he make use of it within his notion of the melancholic world? Based on this, his vision of a remedy to the melancholic disease afflicting the whole world, together with the form of this treatment presented in The Anatomy of Melancholy will be thereafter shown and explained. This task will be preceded by an analysis situated on the edge between the history of ideas and intellectual history, cultural history, and philosophical anthropology with a small overlap with the history of mentalities. This analysis will firstly reveal the diversity and rich history of the concept of melancholy and, subsequently, open up the intellectual milieu and ideas which form the basis of Burton's notion of the problem of the melancholic world and its treatment.
