Aim: The purpose of this work was to identify and assess safety features on a production line of paper manufacturer called Renova. The assessment includes technical as well as organisational factors. The study was carried out through the evaluation of safety functions (SF), either present or absent in the system analyzed. Methods: The methodology applied was the SFA (Safety Function Analysis), which was developed by Harms-Ringdahl in 2001 and was updated further, in 2011 (draft version). The analytical framework was applied in two processes (raw material loading and transversal cut of log) of a production line (Line H4) of Renova. Results: In the first process analyzed (raw material loading), 47 safety functions (SF) were identified and evaluated, whereas 36 SF were assessed in the second case (transversal cut of log). The evaluation has shown that most of the SF considered are in good condition and being well monitored, therefore they do not need any improvements. In contrast, this work has also identified a number of safety functions that need essential improvements. Conclusions: As a consequence of this SFA analysis, the author proposes a number of specific recommendations to improve safety and the system’s performance in general. Since Renova is a manufacturer of paper products, fire safety is of paramount importance and one of the most relevant recommendations is perhaps the implementation of thermo graphic tests to identify possible hot spots that may originate a fire.

Annotation:
Managing safety in the energy industry requires continuous assessment of the factors with potential to contribute to accidents. Investigations into major incidents across highhazard industries have repeatedly highlighted the critical role of management factors in relation to safety performance. As leadership from site-level managers has been identified as a key non-technical skill with potential to measurably affect safety variables, this thesis investigates how the concept of ‘safety leadership,’ a term commonly used in the energy industry, might be applied at the level of senior management. After a review of the empirical literature revealed minimal consistency across existing work on this topic, four studies were conducted to support the operationalisation of the term ‘safety leadership’ in language relevant to practicing managers. In the first study (Study 1) semistructured interviews with subject matter experts (19 senior managers, 3 health and safety professionals) supported the identification of a set of behavioural ‘elements’ of safety leadership. In Study 2, a second set of interviews with contractors (n=11) and regulators (n=11) facilitated the refinement of the element set, and a preliminary safety leadership framework was proposed wherein behavioural elements were organised into broader categories. In Study 3, a documentary analysis study, data from published incident reports from the energy industry were used to test the preliminary safety leadership framework and assess the framework’s capacity to encompass senior-level behaviours that have been implicated in major investigations. Finally, Study 4 used structured interviews with experienced senior managers (n=15) to assess the face and content validity of the framework. This research informed the development of a behavioural framework, labelled the safety leadership framework (SLF), that includes three categories (Maintaining risk awareness, Leads by example, Setting and managing safety standards) encompassing seven safety leadership elements (Promotes continuous exchange of safety information, Monitors the reality of operations, Incorporates safety into decision-making, Acts as a safety role model, Provides direction, Reinforces with rewards and consequences, Supports and motivates)

This study examines safety perceptions of manufacturing employees involved in a safety behavioral modification process, and the perceptions of employees who are not involved in the same process, based on the idea that such a process helps to decrease injuries in the workplace. The Neal-Griffin Safety Climate/Safety Performance Instrument was used in a specific workplace to determine if race, gender, age, years of service, union membership, and education affect employees' perceptions of safety in an industrial/manufacturing setting. The results of this quantitative study found that, overall, the vast majority of the workers, regardless of demographic and group membership, provided relatively high survey ratings, which indicate that they had positive perceptions regarding their company's safety procedures and miscellaneous safety issues.

Annotation:
Industrial contexts, and in particular the port areas, are very complex systems to be monitored and controlled due to the combined presence of vehicles and people. The port areas are the gateway between navigation and terrestrial transportation and are of great importance in transport logistics. Unfortunately, the management of port areas is quite complex because the safety of the workers must be always assured. Therefore, in such a context, a centralized control system for the monitoring and the prevention of risks is of particular importance. In this thesis, a real-time control system for the monitoring of people and vehicles in industrial areas is proposed. The proposed system is based on the Internet of Things paradigm, i.e. a network of “things” (such as sensors, tag RFID, actuators etc.) which can communicate and interact with each other within a shared IP addressing range, in order to share data and contribute to the management and development of advanced applications. Specifically, the thesis is focused on the design of a wearable sensors network based on RFID technology, and specifically on WISP sensors, for assuring the safety of the workers. In this network, wearable devices that can be inserted directly on the textile have been selected. Differently from conventional sensors, wearable sensors ensure a higher level of comfort, and provide higher electromagnetic performance. Furthermore, textile materials are easily available. Microstrips are good candidates for these applications because they mainly radiate perpendicularly to the planar structure, and their ground plane allows a good shielding on the body tissues. Therefore, I have designed specific antennas for RFID, that unlike the classical microstrip antennas have the radiating surface composed of several "side by side" conductive "threads of textile". Since the microwave model does not allow the design of an antenna with these characteristics with a good approximation, a specific microwave model for coupled lines has been designed. With this model, the specific antenna for RFID has been designed, with Jeans as substrate. The particular antenna’s substrate allows direct integration into garments, but since the wearable antennas are placed very close to the human body, biological issues which may arise on the human body from the use of these sensors have been analysed. The Specific Absorption Rate (SAR) has been considered and simulations have been conducted for evaluating the effects on the human body, and especially on the head, when irradiated with the electromagnetic waves generated by the wearable antenna realized with different materials. Dosimetric effects have been evaluated in function of the distance from the body, in order to define a safe distance for placing the antenna on the human body. The SAR has been evaluated also for full patches with different textile substrates, whose surface is larger than that of the proposed model of coupled lines. Therefore, if the SAR values evaluated for the full patch are satisfying, the SAR values for the model of coupled lines will surely be acceptable.

The purpose of this research was to develop a methodology that would evaluate employees’ personality traits, demographic characteristics, and workplace parameters to predict safety compliance along with the moderating effect of risk perception. One hundred and twenty five employees of a manufacturing facility were given questionnaires to gather their demographic and perception information. Surveys were also used to measure their personality characteristics, and periodic observations were recorded to document employee’s safety compliance. A significant correlation was found between compliance and the worker's perception of management's commitment to safety (r = 0.27, p < 0.01), as well as with gender (r = -0.19, p < 0.05). Females showed a significantly higher average compliance (78%), than males (69%). These findings demonstrated the value of developing a model to predict safety behavior that would assist companies In maintaining a safe work environment, preventing accidents, ensuring compliance, and reducing associated costs.

Annotation:
Safety culture and safety climate are terms that are used often in the context of safety management but are not very well defined or differentiated. This research concentrates on safety climate, a summary concept of employee perceptions of safety management practices within their organisation. There is a common assumption that a positive safety climate results in better safety performance outcomes, yet there is little research evidence to support this notion.
Despite being defined as a summary concept, much of the research on safety climate has been empirically driven and has concentrated on identifying the number and content of the dimensions/factors that contribute to the safety climate. Due to a lack of theoretical input in the field, the use of proprietary instruments which are unavailable in the public domain and varied developmental histories of these tools, the findings of past research has been mixed.
This research addresses several of the limitations in the literature on safety climate by using the National Safety Council of Australia’s Safety Climate Survey. The research centres around two main issues; the dimensionality and factor structure of safety climate; and investigation of the applicability of a framework linking safety climate to safety performance.
The project was divided into two studies. Study I is based on archival data collected by the National Safety Council of Australia (NSCA) (N=215, N=127, N=90). Study II utilises data collected specifically for the purposes pf this research (N=226). The survey instrument was further developed for Study II to include scales that allow for a more complete investigation of a framework linking safety climate to safety performance.
A comprehensive validation of the NSCA safety climate survey was conducted. Structural Equation Modelling was utilised to examine various models of the factor structure of safety climate. It was found that safety climate is best understood as a second order concept comprised of several specific first order factors. The factor structure remained consistent across several samples however the strength of the individual factors varied in each sample.
Structural Equation Modelling was also used to investigate the framework of the relationship between safety climate and safety performance. Evidence in support of his model was obtained however some variations to the model were necessary to achieve this support. Reinforcement for the influence of general organisational climate in providing a context for safety climate was revealed. Given that it was not possible to include all aspects of the framework within the statistical model these findings provide a good impetus for further research in this field.

The main aim of using radars in industrial safety system is to detect the presence of target accurately. The conventional methods of radar target detection algorithm such as the Cell averaging constant false alarm rate method (CA-CFAR), Greatest of constant false alarm method (GO-CFAR) and the Smallest of constant false alarm rate method (SO-CFAR) has their own disadvantage when it comes to precise target detection which is a key factor for a safety system. This thesis investigates the above mentioned conventional CFAR algorithms for its pros and cons in target detection and proposes a new and improved method called Cell Evaluation target detection method. The proposed method is shown to mitigate the limitations present and the assumptions made in the conventional target detection method. Further more angular estimation is performed to determine the precise location of the target and the artifacts due to the angular estimation is eliminated by aggregating the detected points from multiple radar modules by linear translation. This gives a better visualization of the target.

As more systems are becoming embedded hardware-based, challenges regarding software safety and considerable consequences of their failure arise. Various safety standards assure certain safety aspects of systems, addressing areas including testing. The safety standards chosen for this thesis are ISO/IEC/IEEE 29119-2 & 3, IEC 61508-1 & 3, ISO 13849-1 & 2, and ISO/IEC/IEEE 12207:2017. This thesis tackles the problem of compliance with safety standards by utilising a lightweight assessment method, leading to recommendations for improving the test process of an industrial control system. A case study is performed on an automation company to achieve the objectives of this thesis. The method used for the qualitative data analysis results in recommendations regarding the compliance of the company's test process with selected safety standards. As the final step, the execution of a focus group research leads to the industrial evaluation of the recommendations and assessment results. The company's development process fully complies with 22% and fails to comply with 58% of the extracted requirements from the selected safety standards. Furthermore, the thesis results in recommendations for improving the test process of an industrial control system. As a result of performing the case study, a method for a lightweight assessment of the development process of industrial control systems is achieved. The generic method follows five steps, firstly tabulating the data to attain assessment criteria and items, used by the assessment step to get a compliance degree per requirement. The analysis step comes next to shed light on areas of strength and weakness, leading to recommendations. The final step evaluates and refines the recommendations according to the results of a focus group. Further development of the method used in this thesis can lead to a generic method for assessing development processes, concerning safety standards, using limited resources.  The results of this generic method can lead to recommendations for test process improvements of control systems via safety standards.
