The aim of the present study was to observe surgical team members’ compliance with basic hand hygiene and intraoperative hygiene guidelines during the anaesthetic and surgical procedure. A secondary aim was to investigate the team members’ knowledge and attitudes toward these guidelines. A structured observation schedule was designed to capture observational data on compliance with hygiene guidelines. A questionnaire was also designed to elicit written responses on attitudes, beliefs and knowledge about hygiene routines. The results showed that the clinical procedures for basic hand hygiene and intraoperative hygiene were flawed in three areas. Hand disinfection before and after direct patient contact was incomplete. Secondly, the team members used gloves in an incorrect way. Thirdly, the scrub nurse did not always change sterile gloves after intraoperative skin disinfection before handling sterile instruments. The quantity of 0.5% chlorhexidine with 70% ethanol used varied and the mechanical performance of skin disinfection varied. Knowledge of hand hygiene routines and intraoperative hygiene routines among surgical team members is incomplete and adherence needs to improve. Regular routine observations and continuous feedback to all staff may be necessary to improve compliance and avoid deterioration of practice.

Handbooks and consultants offer guidelines for customer-complaint reception which seem quite uniform across cultures. But one would expect different behavior patterns in different cultures. This paper describes a pilot investigation of this paradox. Four complaint-handling dialogues exhibiting different levels and types of politeness were written and shown to business students of various European nationalities, predominantly Danish and Spanish. The results showed that the Danes were much less tolerant of polite phrases and promotional language than the Spaniards, but that there was a ’concise, brief, sincere’ style acceptable to all cultural-national groups.

AbstractObjective(i) To examine associations between young adults’ meal routines and practices (e.g. food preparation, meal skipping, eating on the run) and key dietary indicators (fruit/vegetable, fast-food and sugar-sweetened beverage intakes) and (ii) to develop indices of protective and risky meal practices most strongly associated with diet.DesignCross-sectional survey.SettingMinneapolis/St. Paul metropolitan area, Minnesota (USA).SubjectsA diverse sample of community college and public university students (n 1013).ResultsMeal routines and practices most strongly associated with healthy dietary patterns were related to home food preparation (i.e. preparing meals at home, preparing meals with vegetables) and meal regularity (i.e. routine consumption of evening meals and breakfast). In contrast, factors most strongly associated with poor dietary patterns included eating on the run, using media while eating and purchasing foods/beverages on campus. A Protective Factors Index, summing selected protective meal routines and practices, was positively associated with fruit/vegetable consumption and negatively associated with fast-food and sugar-sweetened beverage consumption (P<0·001). A Risky Factors Index yielded significant, positive associations with fast-food and sugar-sweetened beverage consumption (P<0·001). The probability test for the association between the Risky Factors Index and fruit/vegetable intake was P=0·05.ConclusionsMeal routines and practices were significantly associated with young adults’ dietary patterns, suggesting that ways in which individuals structure mealtimes and contextual characteristics of eating likely influence food choice. Thus, in addition to considering specific food choices, it also may be important to consider the context of mealtimes in developing dietary messaging and guidelines.

Purpose – The purpose of this paper is to connect sociology, criminology, and social psychology to identify specific factors that keep protests peaceful, discusses empirical examples of effective peacekeeping, and develops practical peacekeeping guidelines. Design/methodology/approach – The analysis systematically compared 30 peaceful and violent protests in the USA and Germany to identify peaceful interaction routines and how they are disrupted. It employed a triangulation of visual and document data on each demonstration, analyzing over 1,000 documents in total. The paper relies on qualitative analysis based on the principles of process tracing. Findings – Results show that specific interaction sequences and emotional dynamics can break peaceful interaction routines and trigger violence. Single interactions do not break these routines, but certain combinations do. Police forces and protesters need to avoid these interaction dynamics to keep protests peaceful. Communication between both sides and good police management are especially important. Research limitations/implications – The paper highlights the need to examine the role of situational interactions and emotional dynamics for the emergence and avoidance of protest violence more closely. Practical implications – Findings have implications for police practice and training and for officers’ and protesters’ safety. Originality/value – Employing recent data and an interdisciplinary approach, the study systematically analyzes peacekeeping in protests, developing guidelines for protest organizers and police.

An illness or injury sustained by a family member affects all family members. It is consequently important that a child’s need to be involved in a family member’s care is clearly recognized by healthcare professionals. The aim of this study was to describe healthcare professionals’ approaches to children as relatives of a parent being cared for in a clinical setting. A web-based study-specific questionnaire was sent and responded to by 1052 healthcare professionals in Sweden. Data were analysed using descriptive statistics and qualitative analysis. The results show that guidelines and routines are often lacking regarding involving children in the care of a parent. Compared to other areas, psychiatric units seem to have enacted routines and guidelines to a greater extent than other units. The results indicate that structured approaches based on an awareness of the children’s needs as well as a child-friendly environment are vital in family-focused care. These aspects need to be prioritized by managers in order to support children’s needs and promote health and wellbeing for the whole family.

The aim of the present cross-sectional study was to analyze the differential impact of the first COVID-19 lockdown (3 April 2020) on stress, health practices, and self-care activities across different Hispanic countries, age range, and gender groups. One thousand and eighty-two participants from Spain, Chile, Colombia, and Ecuador took part in this study. Irrespective of the country, and controlling for income level, young people, especially females, suffered a greater level of stress, perceived the situation as more severe, showed less adherence to health guidelines, and reported lower levels of health consciousness, in comparison to their male peers and older groups. However, in the case of self-care, it seems that older and female groups are generally more involved in self-care activities and adopt more healthy daily routines. These results are mostly similar between Colombia, Ecuador, and Spain. However, Chile showed some different tendencies, as males reported higher levels of healthy daily routines and better adherence to health guidelines compared to females and people over the age of 60. Differences between countries, genders, and age ranges should be considered in order to improve health recommendations and adherence to guidelines. Moreover, developing community action and intersectoral strategies with a gender-based approach could help to reduce health inequalities and increase the success of people’s adherence to health guidelines and self-care-promoting interventions. Future studies should be addressed to explore the possible causations of such differences in more cultural-distant samples and at later stages of the current outbreak.

Background Good healthcare hygiene can reduce the number of healthcare associated infections by one-third and with current knowledge we can with simple means observe the precautions necessary to ensure good healthcare hygiene. Yet these hygiene rules are often not followed. Aim To elucidate the factors that facilitate for nursing staff in hospital wards in to follow basic hygiene procedures. Methods The study design is a descriptive literature study. This method was chosen to get an idea of the current state of knowledge in the field. The method describes the state of knowledge of a field by reviewing, analyzing and describing selected scientific studies. Results Analysis of data from the articles gave seven themes: sense of responsibility, workload, attitude and behaviour, knowledge and education, responsibilities with regard to hygiene, feedback and availability. The most important factors are “attitudes and behaviour” and “management’s responsibility considering the hygiene”. Conclusion A clear leadership with motivated managers and an encouraging environment gives the best opportunity to change the staffs' behaviour and attitudes. However, to improve compliance with hygiene routines all seven factors need to be considered. Thus patients suffering can be avoided through a reduction in health care associated infections and days of care.

Pressure ulcer is a care related injury that has a broad prevalence in the hospitalsof Sweden. This dermatological injury appears when a constant pressure createsdamage to the underlying tissue. Guidelines have been formed towards the workof pressure ulcer prevention, which aims to minimize the prevalence of pressureulcers. The aim of this study was to examine nurses’ view on pressure ulcerprevention in relation to patients admitted to a somatic ward. The methods ofexamination used had both a qualitative and a quantitative approach. The materialwas collected through questionnaires and the final number of informantsamounted to 29. The methods of analysis used were manifest content analysis aswell as descriptive statistics. The result of the study demonstrated that 72 % of theinformants considered that they had a need to increase their knowledge aboutpressure ulcers. Among the informants that participated in the study, 86 %acknowledged knowledge about the wards’ routines and guidelines for pressureulcer prevention. Only half of the informants that were deputies or had a monthlyemployment knew about these routines and guidelines. All of the informants thathad any form of further education thought of pressure ulcer prevention as aprioritized act of nursing, a result that differs from the informants with no form offurther education amongst whom only 75 % thought of pressure ulcer preventionas prioritized.

The purpose of this study is to investigate the guidelines, methods and strategies of social workers supporting children and young people who live with honour-related problems – in order to deepen the knowledge of tensions between the guidelines of social work and its practices. The result of the study has emerged from qualitative interviews with six social workers working with children and young people and three local authority documents containing guidelines and routines. The social workers were asked questions about their experiences of guidelines and routines and their current work situation. To analyse and understand the results a thematic method was used along with theories on discretion. The result shows that social workers use different assessment manuals in order to assess that a child/young person is exposed to honour-related problems, but their experience is that the manuals are not complete and adapted to children and young people who live with honour-related problems. Therefore, many social workers have a discretion, where they can take available methods and adapt them to the honour-related cases. The social workers have several different voluntary and family-oriented interventions to offer, but in honour-related cases the interventions are not optimal. The social workers experience difficulties with working with the families of children and young people with honour-related problems because they are not receptive to change, which was the first tension that could be identified. The overall experience of the interviewed social workers regarding honour-related cases shows that their work is complex in relation to laws, guidelines and methods. Thereby another tension was discovered in the result and analysis between forced interventions relating to the laws and guidelines and principles of the child's best interests. The social worker experiences are that it is difficult to motivate forced interventions as it does not always improve the situation of the child.

Annotation:
Background: Increasing attention has been paid to preventative health over the past few decades. However because of constraints on consultation time and medical funds general practitioners (GPs) are often unsure which measures are appropriate and when to carry them out. They need to be well informed about the cost-effectiveness and evidence regarding each preventative measure to help their patients make informed choices about what needs to be done. Due to the large number of recommended screening measures general practitioners are often unsure which to prioritise and also forget to carry out all recommended measures. Recommendations for screening in South Africa and research into preventive strategies used by general practitioners are lacking. This research attempts to find out whether the prevention strategies used by general practitioners in private practice in Grahamstown follow recommended guidelines. Methods: To obtain a broad understanding of prevention strategies used by general practitioners in Grahamstown, the following tracer conditions were selected for the study: screening for smoking, breast cancer, cervical cancer, colorectal cancer, hyperlipidaemia, prostate cancer and human immunodeficiency virus (HIV) infection. Research on routine annual health checks was included as these are used by many GPs to screen for tracer conditions. The research was done in 2 parts: 1. Review of the literature to obtain evidence on the recommended prevention strategy for each of the selected tracer conditions and 2. Interviews with GPs to evaluate the prevention strategy they used for each tracer condition. The literature was reviewed for evidence on the following parameters for each tracer condition: burden of the disease prevented; cost-effectiveness of the screening measures; sensitivity and specificity of screening tests; whether the screening measure for and treatment of the tracer condition is acceptable to patients; appropriate duration between repeated screening tests and whether there is effective treatment for the tracer condition. Eleven general practitioners were interviewed on the prevention strategies they use for each of the selected tracer conditions. Transcriptions of the interviews were analysed qualitatively and qualitatively. The prevention strategies used by the general practitioners was then compared to recommended guidelines. Results: Evidence from the literature regarding the burden of and optimal prevention strategy for each tracer condition is reported. Using this evidence an appropriate prevention strategy for each tracer condition is outlined. The prevention strategies used by the GPs for each tracer condition and the routine annual health check is reported from the analysis of the interviews. The results show a wide range of differing strategies used by the GPs, often not following recommendations from research. Discussion: The prevention strategies used by general practitioners for each tracer condition is compared with the recommendations from the literature. Important differences between what are recommended and what general practitioners are doing is discussed. Some general practitioners are practicing largely curative medicine and are not adequately screening their patients. Others are over screening with too many unnecessary tests being done annually as a routine. The interviews reveal that generally GPs do not discuss the potential harms and limitations of screening tests with their patients; do not keep check lists for each patient and do not use registers or recall systems to ensure all screening is done. Conclusion: General practitioners need to ensure their prevention strategies follow recommended guidelines. To do so they can use the routine annual health check or opportunistic case finding and prevention. They need to ensure that routine health checks are targeted to the individual patients’ health risks and avoid doing unnecessary tests. Check lists can help to ensure all screening is done on every patient. While registers and recall systems improve screening rates they are not always possible in busy general practices. Recommended prevention strategies for each of the tracer conditions are made.

The researchers predict a future, where people’s interaction with things is decreased and things become more autonomous, requiring less control from people, aiming to make life easier. However, the question is if we really are ready to let go of the control over such systems in order to have easier life. This is what this research through design project is aiming to find out through a research question How the different user interaction modalities with IoT devices at home influence the sense of control for users? and sub-question How the sense of control over IoT devices would influence how people trust them? With the help of prototype, questionnaire and online sessions, two lists of guidelines were created, consisting of points to consider while designing for such systems from control and trust perspective. The angle was narrowed down to a morning routine and interaction with the alarm clock connected to a coffeemaker.

Rawdon, Paul David. „Experiences of families of people with Autism Spectrum Disorder in the Canterbury/West Coast area“. Thesis, University of Canterbury. Social and Political Sciences, 2012. http://hdl.handle.net/10092/8209.

Der volle Inhalt der Quelle
Annotation:
Many people would argue that New Zealand has had a poor reputation for providing support services for the families of people with an Autistic Spectrum Disorder (ASD). The limited support services available for families in this country were highlighted during the trial and the manslaughter conviction of Janine Albury-Thomson. Albury-Thomson was found guilty of the manslaughter of her autistic daughter, Casey (MOH, 1999). The sentencing judge called for an independent inquiry to be held to examine the extent of support services that were available for families affected by the disorder with a view to identifying any gaps that may exist and ascertaining whether or not government policy could be blamed (Norris, 1998). More than twelve years have now elapsed since the Albury-Thomson trial and research was needed to ascertain the experiences of families raising a child with ASD. This research examines the experiences of families who have a child who has an ASD diagnosis. Research participants were families of people with Autism Spectrum Disorder who reside in the Canterbury/West Coast area. The research explored the support needs of families and the informal and formal supports that are available to them in this region. This research was conducted in three parts, with each part having a distinct method of data collection allowing triangulation of research findings. Part one was a survey questionnaire distributed via a range of mediums using a snowball sampling method. The second data set was made up of two case studies that were undertaken with a semi-structured interview approach. The third set of data was elicited via a semi-structured interview process from five local service providers that provide services to those with ASD and their families. A qualitative methodology was adopted to analyse research findings. This methodological approach was selected because it gave participants the opportunity to tell their stories about the diagnostic pathway they travelled and their experiences using available services. This research has social policy relevance as it was designed to examine the participant’s experiences while using services and resources available for the families of people with ASD in the Canterbury/West Coast area. Participants told of various social, emotional, and practical impacts and of the experiences they encountered while using the various services available. While the level of awareness of ASD has increased, the diagnosis process proved to be problematic for some when using the public health system. Consistent with a 2001 study commissioned on behalf of the Autistic Association of New Zealand, participants in this study who sought a diagnosis through the public health system found it to be a protracted process. Participants attributed delays to a lack of resources or staff that had a limited knowledge of ASD and saw these factors as barriers to obtaining a diagnosis within an acceptable time frame. A more timely diagnosis was reportedly received when the private health system was used. With regards to support offered by the agencies involved with ‘day to day’ contact with families it was found that overall, competent, experienced people worked for the existing agencies and they were capable of offering support where needed. However specific issues were identified by participants including a perception that case managers failed to understand their situation because they were unfamiliar with the implications of having a family member with ASD. Significantly, family members and service providers both identified a concern about a lack of suitable accommodation that can be utilised by a person with ASD. Because there is limited research into the support needs of families of a child with ASD this research provides an important insight into the needs of a fairly marginalised, and sometimes socially isolated, group. Implications for ongoing service provision and support of families are discussed in the thesis.

This chapter provides a framework and guides for teaching the online music appreciation class in ways that suit the online modality and a variety of learners. Because it is much more challenging to build community and connect with others in an online classroom when compared to face-to-face learning, and community is essential to learning, a framework for teaching online that includes aspects most likely to promote community among learners and between students and their instructor is needed. Readers will find helpful guidance to develop a robust community of inquiry within the online music appreciation classroom, along with communication strategies and routines typical of online teaching. The chapter ends with a brief summary of important points and an infographic designed to visually highlight essential elements for teaching online music appreciation, including teaching, social, and cognitive presence; guidelines for online communication while teaching a class; time management tips; and weekly routine suggestions.

Over 900 colleges and universities across the U.S. have adopted the Quality Matters Rubric for the design of their online courses with the intention of providing guidance to both instructors and peer reviewers. Given the challenge of how design components align with Web-based instruction delivery in terms of interactivity and formative assessment, there is a need to develop guidelines to establish a strong connection between design and delivery. Such information could support a dynamic, balanced, and student-centered approach to instructional development in virtual learning environments. This chapter proposes a matrix built on the linkage among well-established design practices, delivery methods or strategies, and assessment routines.

Apex courts face a fundamental problem: they cannot succeed in building systematic effectiveness in the face of regular opposition from the other branches of government, but they must sometimes invalidate the acts of those institutions to make rights effective. Chapter 5 considers how a court can build effectiveness while inducing inter-branch cooperation. A key is proportionality, which provides an infrastructure for the construction of dialogic jurisprudence. Constitutional courts delineate “zones of proportionality” within which policymakers enjoy meaningful policy discretion, albeit within guidelines set by the court. The chapter considers how rights-based, constitutional dialogues play out in three contexts, with respect: to legislating, making and enforcement of administrative law, and in the adjudication of private law disputes. These dialogues not only serve to secure other institutions’ acquiescence to the court’s jurisprudence, but drive proportionality reasoning into their decision-making routines, and hence into the legal domains over which they preside.

Chapter three describes the heart of the MillionTreesNYC campaign: planting one million trees. It focuses on the organizational, governance, and material arrangements involved in transforming the city’s urban forest. A formal public-private partnership was created to run the campaign, an example of truly hybrid governance at work. The prominence and scale of the initiative led to major organizational changes in the two core partners. They both garnered and expended massive amounts of financial and human resources in the planting of a million trees. Counting the number of trees planted was central to the public identity and internal functioning of the campaign. The conduct of urban forestry in New York City was radically altered by the MillionTreesNYC campaign. New guidelines, routines, and procedures were developed to alter the practice of urban forestry on streets, in parks, in “natural areas,” on public housing grounds, and on private land.

Hyperparathyroidism (HPT) is abnormal excessive production of parathyroid hormone (PTH) from the parathyroid gland(s), often with resultant hypercalcaemia. HPT affects 1% of adult population. Incidence increases after the age of 55. Over 80% of primary hyperparathyroidism (PHP) cases in Western countries are found incidentally by routine biochemical tests or on investigation of aetiology of resultant end-organ damage (osteoporosis and renal stones). Diagnosis can be established by demonstrating persistent hypercalcaemia (or serum calcium at the high normal levels) in the presence of elevated (or inappropriately normal) PTH concentrations and elevated urinary calcium excretion. Surgical resection is the treatment of choice for all symptomatic and asymptomatic patients with overactive parathyroid gland who are fit for surgery.

Unselected coagulation testing is less evidenced based in assessing bleeding risk, prior to surgery. This not only delays surgery inappropriately but also causes unnecessary concern in patients who are found to have ‘abnormal’ tests. In addition, it is associated with a significant cost. Patients with a negative bleeding history do not require routine coagulation screening prior to surgery. Patients undergoing surgery should have a screening bleeding history taken. This should include structured detailed history, personal and family history, previous surgery and trauma, and detail of anti-thrombotic medication. Based on a detailed history, a few selected tests should be undertaken. In addition, liaise with the haematologist in the presence of positive bleeding history, significant comorbidities, and when high risk surgery is planned.

FRA derailment statistics between 1996 and 2009 show an average of 13 derailments each year industry-wide are attributed to extreme winds, roughly 7 of which result in vehicles blowing over and derailing. BNSF Railway has developed a hybrid process to identify critical crosswind speeds for various types of railcars to mitigate future blow-over derailment risk. The method uses computational fluid dynamics to compute the wind effects based on the car envelope. These wind effects are simplified in an empirical spreadsheet tool for various wind and train speeds, and wind heading angles. CFD analysis and reduction into the empirical spreadsheet tool have been completed for alimited set of car types. With the resulting external wind loads, vehicle dynamic simulations are used to evaluate the propensity for wheel unloading. Both VAMPIRE® and NUCARS® dynamics routines were used for various parts of this study. In all cases, the wind onset has been assumed to ramp from a zero to full wind condition which remains steady from there forward. The main paper will cover several steps within this project: aerodynamic investigations and generation of appropriate load sets, preliminary and secondary vehicle dynamic simulations, and simplified train operating guidelines proposed thus far. The paper is summarized with a discussion of how these results may be applied to further mitigate wind blow-over derailments in terms of operating rules and the operating environment. An appendix is provided which outlines the relationships between the CFD wind loads and assumed train simulation speeds.

Annotation:
The use of doubler plates or ‘doublers’ has become routine for temporary ship repairs. It is the preferred method for ships’ structural repairs for plate corrosion due to its relative ease and low cost of installation over the more costly permanent welded plate insert repair. A lack of performance data and engineering design guidance are the reasons that repairs with doublers are currently considered only temporary. This objective of this study was to develop a set of guidelines for designing and applying doubler plate repairs to ship structures. The guidelines were established using the following criteria: various stress analyses, buckling strength, corrosion types and rates, weld types, and doubler plate fatigue and fracture assessment. Studying and understanding doubler plate repair performance by comparison to that of the primary hull performance allows critical operational decisions to be made with greater ease and confidence. However, the ultimate goal of this study was to establish the design and limitations on the applications of doubler plate repairs for surface ships.

Even with the many advantages of adopting a structured reporting system, there is little convention on how to disseminate this routine into the report environment. This work proposes a systematic approach to migrate a system routine from free-text reports to structured reports, focusing on the DI- COM Structured Reporting guidelines. We evaluated this proposal by creating a reporting module in the context of a telemedicine system, and performing case study covering ultrasonography reports. Using the AdEQUATE model, the eval- uation showed a high user perception from the system, directly reflecting the quality of our proposal. The results are a set of defined premises and steps that turns a telemedicine system into a complete structured reporting environment.

Vibrating wire (VW) strain gauges have been used by the pipeline industry for over 50 years as part of landslide hazard management programs. This paper provides technical and operational guidelines for the use of these strain gauges based on 20 years of experience managing active but slow moving landslides. Guidelines are provided for the use of strain gauges during 1) routine monitoring 2) cut outs and 3) strain relief. Examples of expected strain gauge responses are provided along with technical considerations for interpreting data. Given the relatively small size of the gauges in relation to the length of pipeline within most landslides, techniques are provided to best locate the gauges including the use of 1) visual/on-site geotechnical assessments, 2) geotechnical monitoring technologies and 3) smart pigging technologies (caliper, IMU and axial strain technologies). Limitations, reliability, and alternatives to VW gauges are also discussed.

This report presents the results of RTI International Education’s study on teachers' guides across 13 countries and 19 projects. Using quantitative and qualitative methods, we examine how teachers’ guides across the projects differ and find substantial variation in the design and structure of the documents. We develop a scripting index so that the scripting levels of the guides can be compared across projects. The impact results of the programs that use teachers’ guides show significant impacts on learning outcomes, associated with approximately an additional half year of learning, showing that structured teachers’ guides contribute to improved learning outcomes. During observations, we find that teachers make a variety of changes in their classroom instruction from how the guides are written, showing that the utilization of structured teachers’ guides do not create robotic teachers unable to use their own professional skills to teach children. Unfortunately, many changes that teachers make reduce the amount of group work and interactivity that was described in the guides, suggesting that programs should encourage teachers to more heavily utilize the instructional routines designed in the guide. The report includes a set of research-based guidelines that material developers can use to develop teachers’ guides that will support effective instructional practices and help improve learning outcomes. The key takeaway from the report is that structured teachers' guides improve learning outcomes, but that overly scripted teachers' guides are somewhat less effective than simplified teachers' guides that give specific guidance to the teacher but are not written word for word for each lesson in the guide.
