Emerging technologies such as electronic health records, dashboards, communication tools, and new-generation monitoring devices offer significant opportunities to advance the treatment and recovery management of substance use disorders. This chapter explains the emerging technologies that are being used in addiction treatment and proposes guidelines for how these systems can be adopted for older adults by drawing on experiences from ElderTree, an interactive health technology designed for older populations.

After its implementation in 1980, shock wave lithotripsy (SWL) became the first-line treatment for more than 80% of patients with urolithiasis. During the last three decades, SWL technology has advanced rapidly in terms of shock wave generation, focusing, patient coupling, and stone localization. Indications for SWL have evolved as well. Although endoscopic treatment techniques continue to improve, SWL continues to be considered first-line therapy for the treatment of many urinary stones. This chapter reviews the fundamental principles of SWL and presents advances in lithotripsy technology such as shock wave generation and focusing, advances in stone localization (imaging), different energy source concepts, and coupling modalities. Our understanding of the pathophysiology and the physics of shock waves can enhance extracorporeal SWL efficacy while limiting complications. Finally, current indications for and contraindications to SWL depending on stone location and in context of the updated AUA/EAU Guidelines are discussed.

The presentation of bipolar disorder in young people can be different from that of adults; therefore, the approach to treatment differs slightly. Treatment is described for early intervention, acute mania, bipolar depression, relapse prevention, and refractory bipolar disorder. A strong therapeutic alliance with the patient and engagement and involvement of the patient’s family is critical to successful intervention. The evidence informing treatment is limited, but there is emerging research focused on the management of acute mania favouring monotherapy with a second-generation antipsychotic (SGA) over a mood stabilizer. Preliminary data favour a combination of an SGA and antidepressant over monotherapy with an SGA for the treatment of bipolar depression. Guidelines endorse electroconvulsive therapy for refractory mania and bipolar depression but there is no clinical trial evidence to support this practice. The development of algorithms to guide the management of all phases of bipolar disorder is a work in progress.

The germ theory of disease, mostly based on single causal pathogenesis, has improved life expectancy and changed human life. Distilling general guidelines on a new generation of public health interventions that acknowledge and use ecology and evolution to reduce the burden of disease remains difficult. Interactions are numerous; their consequences vary, depending on the context, making forecasting difficult. “Last-mile” problems are pervasive, with implementation problems keeping medical knowledge from reaching those most in need. Much of the avoidable burden of infectious disease results from lack of access to modern prevention and treatment methods, from the lack of health structures in poor countries. This chapter concerns major causes of death and disability and suggests that if used strategically, this knowledge could alleviate the burden of infectious diseases. Understanding pathogen-pathogen interactions (e.g., gastrointestinal nematodes and malaria, schistosomiasis and malaria and Wolbachia and filariae) may open opportunities for public health gains.

The past two decades have seen significant advances in the development of evidence-based treatments for pediatric bipolar disorder. Practice guidelines recommend pharmacotherapy with mood stabilizers or second-generation antipsychotics (SGAs) as the first-line treatment. Lithium, risperidone, aripiprazole, quetiapine, and olanzapine are approved by the U.S. Food & Drug Administration for treating bipolar disorder in children and adolescents. The pharmacological literature suggests that SGAs are faster and more effective than mood stabilizers in treating acute manic or mixed episodes, but they have significant side effects and require careful monitoring. While mild to moderate bipolar disorder can be treated with monotherapy, combination pharmacotherapy with an SGA and a mood stabilizer is recommended for youth with severe bipolar disorder. A growing body of literature also suggests the efficacy of psychosocial interventions, with family psychoeducation and skills building as adjunct treatments to pharmacotherapy. More type 1 studies of pharmacotherapy and psychosocial treatments are needed.

Monitoring Framework for Universal Health in the Americas has been prepared with the aim of supporting the analysis of progress and performance of public policies, the generation of evidence and the decision-making to transform or strengthen health systems. Similarly, its purpose is to be integrated into the national processes of planning, monitoring, evaluation, and accountability of progress Member States in the implementation of the measures contained in Resolution CD53.R14. It is expected that the application of this framework will contribute to better policies as well as collaborative work and learning in the Region. This publication offers general guidelines for technical units of national health authorities, which may adapt them according to their own context and needs. The proposed activities could be implemented in an integrated manner with the technical cooperation work of the Pan American Health Organization and related tools developed to strengthen capacities for monitoring, evaluation, and analysis of transformation processes of health systems to advance toward universal health in the Region of the Americas.

Thrombolysis can be used as an alternative reperfusion therapy if the time delay to primary percutaneous coronary intervention is more than 2 hoursPatients with very high-risk non-ST-segment elevation acute coronary syndromes (recurrent or ongoing chest pain, profound or dynamic electrocardiogram changes, major arrhythmias, or haemodynamic instability) should undergo urgent coronary angiography within less than 2 hours after the initial hospital admissionAll moderate- to high-risk (GRACE score >140 or at least one primary high-risk criterion) non-ST-segment elevation acute coronary syndromes patients should undergo coronary angiography before discharge; the ideal timing is within 24 hours after admission for high-risk groups, and within 72 hours for moderate-risk groupsOther patients with recurrent symptoms or at least one high-risk criterion should undergo coronary angiography within 72 hours of first presentationLow-risk non-ST-segment elevation acute coronary syndromes may be treated conservatively, and the indication for an invasive evaluation can be done, based on the evidence of ischaemia during exercise stress testingStents should be used during all percutaneous coronary intervention procedures, whenever technically feasible.
