There is a small group of patients undergoing surgery who comprise the majority of perioperative deaths. Morbidity and mortality resulting from tissue hypoxia in the perioperative period can be predicted and prevented by identification of the at-risk group and targeted interventions. Management of these patients requires an understanding of oxygen delivery, the use of cardiac output monitoring to guide fluid and inotrope administration to attain a predefined goal of supranormal oxygen delivery, and the attainment of physiological goals. There are both patient outcome and economic benefits to this management strategy which support the individualized goal-directed therapy approach to managing high-risk patients.

The 2030 Agenda for Sustainable Development (2030 Agenda) established in 2015 sets guiding principles to “achieve a better and more sustainable future for all.” The Sustainable Development Goals (SDGs) included in the 2030 Agenda make explicit what this means by specifying relevant statistical indicators and setting clearly defined targets in them to be achieved by 2030. Given the emphasis on the collection and availability of SDG-related data, it is possible to track universal progress towards the SDG targets. One of the SDGs, SDG 3, includes targets to improve health and well-being. In general, SDG health-related indicators measure health outcomes and coverage at the country level by employing averages. However, given the nature of the data, inequalities in health outcomes and the access to health services tend to be masked. Since it is important to strive for gains in health and well-being to be equitably distributed among individuals regardless of their wealth, educational attainment, and other factors relating to their social background, it is essential to first identify and quantify existing social inequalities in health. To this end, this publication provides an overview of social inequalities in several indicators related to the health of women, children, and adolescents in a region deemed as one with high levels of inequality: the Latin America and the Caribbean (LAC) region. In order for it to serve as a baseline for the 2030 Agenda, emphasis is placed on examining these inequalities around year 2014. The analysis suggests that reducing within-country disparities is a priority, as widespread social inequalities in health are identified among LAC countries.

This chapter covers the multiple measures currently used to assess hope theory. Hope, as theorized by Snyder and colleagues, was originally determined to be a global construct measuring agency and pathways toward goal attainment. Using much of the original theory, hope research has expanded, resulting in multiple measures across different applications and domains. By exploring the context specificity, these scales have been shown to consistently predict outcomes across differing domains, supporting the reliability and validity of new hope measurement. It is anticipated that with more specific hope measurement, the more accurate hope assessment and intervention can become. Treatment of individuals with mental illness who have justice involvement has the same goals as the treatment of those without justice involvement: ameliorating symptoms, minimizing disability, maximizing community functioning, and supporting individualized recovery goals. The attainment of these goals are challenged, if not precluded, when a person is incarcerated, making the overrepresentation of individuals with mental illness all the more tragic, and decreasing criminal justice activity all the more important for both clinical success and public safety. Understanding and addressing the risk of criminal recidivism (i.e., re-arrest and return to jail for offenders with mental illness) is, therefore, a critical skill for the forensic clinician. In this chapter, we review risk factors for recidivism with specific focus on those relevant for offenders with mental illness, and present a holistic approach that incorporates both traditional treatment and recidivism-focused psychosocial interventions.

This chapter examines ethical issues in research involving human subjects. The Nuremberg trials, which exposed to all humankind the outrageous things that could be done in the name of medical science, led to the Nuremberg Code, the first international document from public sources setting out an ethic for research on human subjects. Basic differences are outlined between the goals of therapy for the good of a patient and those of research that focus on attainment of knowledge and the good of society. To protect the interests of human subjects who take part in research, guidelines such as the Declaration of Helsinki have been established for assessing risks and benefits, voluntary consent, protection of privacy and confidentiality, and equity in subject selection. The chapter focuses on the difficulty in calculating harms and benefits to subjects and challenges of obtaining informed consent for participation in research.

This chapter provides information about music therapy practice with children receiving care in a hospital, including information about music therapy service development in new settings. Music therapy offers opportunities for children and their families to be supported while undergoing difficult experiences following an injury or during an illness. Children with life-limiting conditions, and with chronic illness are also treated in hospitals, and can benefit from specialized support and help to cope with their circumstances. Music therapy can be provided to address treatment goals relating to psychosocial care, rehabilitation, pain management, developmental skill attainment, palliative care, and family issues. Music therapists use music improvisation, song singing, instrumental playing, music listening, and music composition, particularly song writing, to support young patients and their families. There is an emerging evidence base to support the role of music therapy as helpful to children and families needing support to cope with hospitalization and treatments.

Enabling achievement of full height potential in a child with chronic kidney disease (CKD) is one of the major and most challenging goals for the paediatric nephrologist. Short stature is associated with psychological maladjustment and with increased morbidity and mortality. The causes of poor growth are multifactorial and include poor nutrition, and metabolic, haematological, and endocrine disturbances. The most vulnerable times are the periods of most rapid growth, that is, infancy and puberty. Growth during infancy is principally dependent on nutrition so many infants need supplemental enteral feeding. Growth delay correlates with severity of CKD, with those on dialysis faring the worst such that by CKD stage 5, approximately 25% of patients are below the normal range for height. Height achieved post transplant is dependent on graft function and is better in younger children and those who have the best height attainment pre transplant. The use of steroid-free immunosuppressive regimens is encouraging. The prognosis for final height is improving.

Chapter 8 considers the implications of this analysis for how we think about public policy and its value for achieving national goals. It also highlights the lessons that the success of landmark higher education policies holds for how lawmakers can promote equal opportunity. To fully understand the progress that American women have made in the last half century, we must take into account the landmark federal higher education programs that have contributed to the dramatic increase in women’s higher educational attainment. In a political context characterized by considerable hostility toward traditional welfare state programs like Temporary Assistance for Needy Families and food stamps, federal higher education programming represents one of the most politically viable methods of redistribution in the United States. Considering the value of higher education for promoting socioeconomic stability and political engagement, American lawmakers interested in promoting equal opportunity would do well to find ways to continue expanding access to college degrees.
