For three decades beginning in 1868, Frederick Law Olmsted and his partners and successors created for Buffalo, the second largest city in New York State, a series of parks and parkways that attracted national and international attention. Olmsted's work for Buffalo occupied a prominent place in his influential career as park planner and urban reformer. In Buffalo, Olmsted and his partner Calvert Vaux implemented a comprehensive series of parks and parkways that pioneered the concept of the metropolitan recreational system. Initially conceived between 1868 and 1870 and substantially constructed by 1876, Olmsted and Vaux's Buffalo park system carefully modified the city's original plan, framed in 1804 by Joseph Ellicott, and introduced progressive design features inspired by the example of the Second Empire in Paris.

Background: Risky sexual behaviors such as sexual concurrency, sexual networks (fish for sex exchange) and unprotected sex, have been reported as the main cause of STIs/HIV infections among the fisher folks. Behavioural change mechanisms such as condom use, and abstinence are some of the interventions used in the prevention HIV/AIDS spread in Kenya.Methods: This was a follow up study on a three tie quasi-experimental study involving 246 randomly selected fisher folks in Mbita and Usenge along Lake Victoria. The study had three phases. baseline, intervention and endline. The survey used questionnaires to collect data among respondents. Using a sample frame in the beach management unit offices, fisherfolks were identified and invited to participate. Consent was obtained from participantsResults: Various variables were influenced by behavior change communication strategies used. These includes use of condom every time of sexual encounter p>0.000, risk associated with non-condom use p>0.004, stopping using condom and fear of getting HIV/AIDS p>0.009, Sexual intercourse without condom use is dangerous p>0.000, whether remembering to use condom every time of sex is difficult P.0.000, whether they are keeping many sexual partners p=0.004, receiving fish/money in exchange for sex in last six months p=0.006.Conclusions: Multiple sexual partners, non-condom use, fish for sex, alcohol consumption and circumcision were significantly associated with risky sexual behavior among the Fisherfolks, therefore other existing interventions need to be intensified to supplement behavior change communication to curb sexually transmitted diseases and further spread of HIV and AIDS.

Background Women in the fisher communities in coastal regions of Bangladesh are engaged in a wide range of fishery activities. However, there is limited evidence available on the occupational hazards and risks experienced by them. Method The study was conducted among fishing colonies in Cox’s Bazar and Chattogram districts in Bangladesh. This was a cross-sectional study blending qualitative and quantitative approaches. The qualitative component comprised five focus group discussions to understand the occupational context, hazards, and risks faced by the fisherwomen, informing the questionnaire design for the subsequent survey. The quantitative survey involved a sample of 207 women from fisher communities, gathering socio-demographic information, occupational hazards, risks, and health and safety practices. Findings The study found a high occurrence of occupational hazards, health risks and limited availability of preventive measures among the women in fisher communities. Occupational hazards include physical safety hazards such as slippery surfaces and fish cutting instruments, and physical hazards like prolonged sun exposure and noise. Chemical hazards like pesticides and saltwater, ergonomic hazards such as prolonged uncomfortable posture and heavy lifting, and biological hazards including inadequate sanitation facilities were prevalent. The study also identified the potential occupational risks, and the outcomes resulting from the hazards, including injuries (87.44%), musculoskeletal conditions (69.08%), skin diseases/conditions (56.52%), eye complaints (33.82%), severe respiratory distress (24.15%) and high incidence of self-reported communicable diseases. Most women (78.26%) did not use personal protective equipment, and the majority (93.72%) lacked a workplace first aid kit. Conclusion The study revealed a high prevalence of occupational hazards and health risks, including injuries and diseases, among Bangladeshi women in fisher communities with insufficient safety measures. Collaboration among government, NGOs, development partners, fisheries stakeholders, and the community is imperative for targeted training, innovative procedures, ergonomic solutions, provision of protective equipment, and advocacy to enhance the well-being of these women.

This paper aims to describe the conceptual model of informal business development applied to “Tibo-Tibo” fisherwomen in North Minahasa Regency. Data and information were obtained by observation and structured interviews to “Tibo-Tibo” fisherwomen, business partners and village governments. The data was analyzed qualitatively by using Miles Huberman perspective, through stages: data reduction, data presentation, conclusion drawing and verification. After going through the validity of experts, this paper produced a conceptual model of informal business development based on intellectual capital namely human capital, social capital and organizational capital. This paper recommends that through the intellectual capital approach "Tibo-Tibo" fisher women's business can be improved through empowerment. Empowerment in the form of human capital will increase the added value products with the availability of a variety of products for frozen food businesses and products for culinary businesses. Empowerment in the form of social capital will strengthen the values of relationships with business partners, trust to customers and employees and strengthen the work culture "mapalus" (local wisdom for the community) such as cooperation and helping each other. Empowerment in the form of organizational will strengthening the management and organizational professionalism as well as the acknowledgment of business license and product brands by consumers.

Abstract Background The transition to parenthood is a period of major stressors and increased risk of anxiety for all parents. Though rates of perinatal anxiety are similar among women (4%-25%) and men (3%-25%), perinatal anxiety research on nonbirthing partners remains limited. Objective We aimed to examine whether demographic characteristics or digital perinatal support preferences differed among nonbirthing partners with compared to without self-reported high parenthood-related anxiety. Methods In this large cross-sectional study of nonbirthing partners using a digital perinatal health platform during their partner’s pregnancy, users reported their parenthood-related anxiety through a 5-item Likert scale in response to the prompt “On a scale of 1=None to 5=Extremely, how anxious are you feeling about parenthood?” High parenthood-related anxiety was defined as reporting being very or extremely anxious about parenthood. During the onboarding survey, in response to the question “Which areas are you most interested in receiving support in?” users selected as many support interests as they desired from a list of options. Chi-square and Fisher exact tests were used to compare demographic characteristics and support interests of nonbirthing partners with low versus high parenthood anxiety. Logistic regression models estimated the odds ratios (ORs), with 95% CIs, of high parenthood-related anxiety with each user characteristic or digital support interest. Results Among 2756 nonbirthing partners enrolled in the digital platform during their partner’s pregnancy, 2483 (90.1%) were men, 1668 (71.9%) were first-time parents, 1159 (42.1%) were non-Hispanic White, and 1652 (50.9%) endorsed an annual household income of >US $100,000. Overall, 2505 (91.9%) reported some amount of parenthood-related anxiety, and 437 (15.9%) had high parenthood-related anxiety. High parenthood-related anxiety was more common among non-White nonbirthing partners: compared to those who identified as non-Hispanic White, those who identified as Asian, Black, or Hispanic had 2.39 (95% CI 1.85-3.08), 2.01 (95% CI 1.20-3.23), and 1.68 (95% CI 1.15-2.41) times the odds of high parenthood-related anxiety, respectively. Lower household income was associated with increased odds of reporting high parenthood anxiety, with the greatest effect among those with annual incomes of <US $50,000 compared to >US $100,000 (OR 2.13, 95% CI 1.32-3.34). In general, nonbirthing partners were interested in receiving digital support during their partner’s pregnancy, but those with high parenthood-related anxiety were more likely to desire digital support for all support interests compared to those without high parenthood anxiety. Those with high parenthood-related anxiety had more than 2 times higher odds of requesting digital education about their emotional health compared to those without high parenthood-related anxiety (OR 2.06, 95% CI 1.67-2.55). Conclusions These findings demonstrate the need for perinatal anxiety-related support for all nonbirthing partners and identify nonbirthing partners’ demographic characteristics that increase the odds of endorsing high parenthood-related anxiety. Additionally, these findings suggest that most nonbirthing partners using a digital health platform with high parenthood-related anxiety desire to receive perinatal mental health support.

The fisher et al formula for calculating the sample size was used to yield a sample size of199. Data was collected using questionnaires and analyzed using statistical package of social sciences (SPSS) version 21 as a tool of analysis.Results: The study findings revealed that the companies that increased buffer stock at various levels in the supply chain. Increasing buffer stock at various levels in the supply chain resulted to decreased lead time, improved quality and reduced cost. Results also showed that most of the companies ‘conducted reduce order cycle times. Conducting reduce order cycle times resulted to decreased lead time, improved quality and reduced cost. Further, the results revealed that most of the companies shared supply chain costs with partners. Sharing supply chain costs with partners resulted to decreased lead time, improved quality and reduced cost.Policy recommendation:the study recommended that manufacturing companies should put in place a risk analysis and evaluation management strategy to enhance supply chain performance. In particular, companies should consider conducting whole life costing of suppliers and also internal quality of suppliers.

Breeding systems affect the timing of reproduction, spacing patterns and social organization, individual fitnesses, and population sizes. For many species, information on breeding systems and mating is limited or untested in wild populations, resulting in management actions that are incompletely informed. We used photographic data collected on a reintroduced fisher (Pekania pennanti (Erxleben, 1777)) population in northern California, USA, to test hypotheses about the breeding system, the timing of breeding, and the potential for male infanticide. We documented fishers of both sexes breeding with multiple partners in the same year, demonstrating polygynandry. We use logistic and linear regression to evaluate the timing and frequency of male visitation at 262 reproductive dens used by 50 individual females. Of 46 documented copulations and 577 male visits, 100% and 95%, respectively, occurred while females had kits in their natal dens. Seventy-five percent of documented male visits occurred before 31 March and 95% occurred before 17 April. Observed breeding occurred within 3.1 ± 1.6 days (mean ± SD) of locating females’ natal dens. We found no evidence for male-directed infanticide. Our results add precision to the timing of the reproductive cycle and provide the first descriptions of male–female interactions for wild fishers.

The Nolia Nuagaon (Bada Noliya and Sana Noliya Nuagaon), fisher communities rehabilitated between the newly revived Tampara Lake, Bay of Bengal, and Rushikulya Estuary. The coastal ecosystem and habitation within this sandy beach vegetation in the past are regularly affected by storm surges, coastal erosion, and floods. The fisher group relies on the sea and the estuary for their livelihood. They are politically, and socio-economically deprived. The present study envisages the societal and economic changes due to the altered ecosystem due to climate changes, Regional Sea Level Rise (RSLR), and human interventions. The basic tools involved are Hazard Vulnerability and Capacity Assessment (HVCA), and Ecosystem Services Shared Value Assessment (ESSVA) using the policies piloted by the International Lake Environment Committee (ILEC) for Eco-DRR, Partners for Resilience (PfR). The Nolia Nuasahi (Bada and Sana) villages were under threat on the coastal front. The search helped to train the community to understand the pioneer issues and coercions that are surging up with time and the fishing profession, which is now replaced by migration and marginalization adding to their age-old lifestyle. The growth of tourism activities in the nearby Tampara Lake shall bring challenges to the landscape, supporting Ecosystem services and the fishing community’s regime. A joint Action Plan between the Lake and coastal ecosystem is warranted to be developed to maintain a better livelihood for the villagers after acceptance from the Gram Sabha and then included in the Gram Panchayat development plan (GPDP) to boost SDG-14.

The relevance of this research is due to significant changes in the labor market caused by the transformation of the modern world, when the demand for professionals who can study in a digital environment, as well as carry out cross-cultural communication and interaction with remote business partners, has sharply increased. This article identifies the main difficulties arising in the process of teaching intergroup dialogue and professionally-oriented interaction to non-linguistic students and also proposes new organizational and methodological approaches aimed at overcoming these difficulties. The aim of the research is to test the experimental methods of distance teaching of dialogic speech as a means of adaptation of non-linguistic students to intergroup communication in the digital environment. Such research methods as the experiment, observation, and the method of open and closed-ended questionnaires were used. The statistical significance of the obtained data was tested with the help of the student’s paired t-test and the Fisher test. The article describes the experimental distance teaching of communication in a virtual environment simulating professional intercultural interaction. The organizational and methodological approaches elaborated by the authors give an opportunity to form an educational foreign language environment, which trains non-linguistic students in intergroup communication with remote partners. The pedagogical tools tested in practice contribute to the effectiveness of teaching students with a low level of foreign language proficiency. The materials presented in the article allow us to create new technologies of foreign language distance teaching and will be in demand by specialists in the field of higher education pedagogies.

When historians proffer historical truths they “must not merely tell truths,” they must “demonstrate their truthfulness as well,” observes Hackett Fisher. As against this standard, Frederick Vaughan's intellectual biography of Richard Burdon Haldane does not fare so well. Vaughan argues that Viscount Haldane’s jurisprudential tilt, which favoured the provinces in Canadian federalism cases before the Judicial Committee of the Privy Council, was rooted in Haldane’s philosophizing about Hegel. He does so, however, without much reference to the political and legal currents within which Haldane thought, wrote, and thrived. More remarkably, Vaughan does not derive from his reading of Haldane and Hegel any clear preference for the local over the national. We are left to look elsewhere for an explanation for Haldane’s favouring of the provincial side in division-of-powers cases. Vaughan additionally speculates about why Haldane’s predecessor Lord Watson took a similar judicial path, yet offers only tired and unconvincing rationales. Vaughan, lastly, rips Haldane out of historical context for the purpose of condemning contemporary Supreme Court of Canada decision making under the Charter. Under the guise of purposive interpretation, Vaughan claims that the justices are guilty of constitutionalizing a “historical relativism” that Vaughan wrongly alleges Hegel to have propounded. While passing judgment on the book’s merits, the purpose of this review essay is to evaluate the book by situating it in the historiographic record, a record that Vaughan ignores at his peril.

We examined the feasibility of high-frequency partial liquid ventilation in the preterm lamb with respiratory distress syndrome and evaluated its effect on pulmonary and systemic hemodynamics. High-frequency partial liquid ventilation increased oxygenation compared with high-frequency gas ventilation over 5 h (arterial oxygen tension 253 ± 21.3 vs. 17 ± 1.8 Torr; P < 0.001). Pulmonary vascular resistance decreased 78% ( P < 0.001), pulmonary blood flow increased fivefold ( P < 0.001), and aortic pressure was maintained ( P < 0.01) in the Liquid group, in contrast to progressive hypoxemia, hypercarbia, and shock in the Gas group. Central venous pressure did not change. The Liquid-Hypoxia group was similar to the Gas group. We conclude that high-frequency partial liquid ventilation improves gas exchange and stabilizes pulmonary and systemic hemodynamics compared with high-frequency gas ventilation. The stabilization appears to be due in large part to improvement in gas exchange.

Peroxisome induction can be expressed as an increase in peroxisome area (proliferation) or as an increase in peroxisomal fatty acid oxidation (activity). This study compares proliferation and activity as endpoints for hepatic peroxisome induction by perfluorodecanoic acid (PFDA). Fluorocarboxylic acids such as PFDA represent a class of compounds possessing commercially important surfactant properties. A single 50 mg/Kg ip. dose of PFDA produces a characteristic “wasting syndrome” in male F-344 rats. Symptoms include hypophagia, weight loss, hepatomegaly, and delayed lethality. Hepatic studies reveal changes similar to those seen with the hypolipidemic agent clofibrate. These include mitochondrial disruption, endoplasmic reticulum and peroxisome proliferation, and increased peroxisomal acyl-CoA oxidase activity.Male Fisher-344 rats received a single ip. dose of 2, 20, or 50mg/Kg PFDA dissolved in 1:1 propylene glycol/water and were sacrificed 8 days post-dose. All control rats received an equal volume of vehicle ip. Animals were provided food and water ad libitum, except pair-fed controls which received the same restrictive food intake consumed by their weight-paired dosed partners (50mg/Kg PFDA group) to simulate the hypophagia associated with PFDA.

In this study we tested the basic premise that fishers’ knowledge provides similar perceptions about exploited stocks to data gathered by standardised monitoring in a small-scale commercial fishery. To that end, we compared temporal trends in catch and individual size for fish species between data obtained from interviews with fishers and a 25-year dataset from a landing monitoring program in the Itaipu Reservoir (Brazil). Fishers described with accuracy changes in species composition of the catches, from large migratory species of high commercial value (common before impoundment) to migratory species of low commercial value and sedentary species following damming. Temporal trends in catches reported by fishers and documented in the landing data matched for most species. Histories diverge when absolute values are involved, such as when fishers were asked to recall their largest catch. Fishers were homogeneous in their reports, indicating that instead of individual opinions, they revealed knowledge resulting from their observation and fishery experience. Fishers can be a reliable source of information for detecting changes in catches over time, especially when large-scale habitat changes have occurred within the time scale of a fisher career. Fishers may be key partners to be considered by managers in information gathering for effective management.

The number of women living with HIV (WLHIV) in Indonesia, especially Papua, is increasing yearly, along with the increasing number of men who transmit HIV to their partners. WLHIV are susceptible to opportunistic infections, leading to increased morbidity and mortality. This study, aiming to identify the characteristics and opportunistic infections among WLHIV in Manokwari Regency, West Papua, was descriptive with a cross-sectional study design involving 70 WLHIV who routinely received antiretroviral treatment at the VCT of RSUD Manokwari as respondent. Data on demographic and clinical variables were obtained through interviews using structured questionnaires. Data were analyzed by fisher exact tests to measure the association of opportunistic infections with demographic and clinical variables. The results showed that the demographic characteristics of WLHIV: aged18-35 years (47.1%), high school education (42.9%), private employment (47.1%), married (65.7%), ethnic Papuan (65.7%), and heterosexual risk factor (97.1%). Meanwhile, their clinical characteristics were clinical stage III (62.9%), CD4+ ≥350 cell/mm3 (62,9%), detectable viral load (55,7%), and underweight (38.6%). The most opportunistic infections among them were pulmonary tuberculosis (58.6%). The clinical stage of HIV and CD4+ cells was significantly associated with opportunistic infections among WLHIV. For these reasons, it is necessary to intervene by applying antiretroviral administration strategies and specific antimicrobial therapy.

AbstractBackgroundInfection with high-risk human papillomavirus (HR-HPV) may be higher in perinatally human immunodeficiency virus (HIV)–infected (PHIV) than HIV-uninfected (HU) adolescents because of long-standing immune deficiency.MethodsPHIV and HU females aged 12–24 years in Thailand and Vietnam were matched by age group and lifetime sexual partners. At enrollment, blood, cervical, vaginal, anal, and oral samples were obtained for HPV-related testing. The Wilcoxon and Fisher exact tests were used for univariate and logistic regression for multivariate analyses.ResultsNinety-three PHIV and 99 HU adolescents (median age 19 [18–20] years) were enrolled (June 2013–July 2015). Among PHIV, 94% were currently receiving antiretroviral therapy, median CD4 count was 593 (392–808) cells/mm3, and 62% had a viral load &lt;40 copies/mL. Across anogenital compartments, PHIV had higher rates of any HPV detected (80% vs 60%; P = .003) and any HR-HPV (60% vs 43%, P = .02). Higher proportions of PHIV had abnormal Pap smears (eg, atypical squamous cells of unknown significance [ASC-US], 12% vs 14%; low-grade squamous intraepithelial neoplastic lesions, 19% vs 1%). After adjusting for ever being pregnant and asymptomatic sexually transmitted infections (STI) at enrollment, PHIV were more likely to have HR-HPV than HU (odds ratio, 2.02; 95% confidence interval, 1.09–3.77; P = .03).ConclusionsPerinatal HIV infection was associated with a higher risk of HR-HPV and abnormal cervical cytology. Our results underscore the need for HPV vaccination for PHIV adolescents and for prevention and screening programs for HPV and other STIs.

In the process of research, it was determined that the cooperation between Ukraine and FAO has a productive activity, in particular, in the formation of national legislation. It is developed together with the government, in particular with the Ministry of Agrarian Policy and Food of Ukraine, and other partners. We developed a special toolkit - a standardized interview questionnaire - for an empirical study of perspectives and strategic priorities in the formation of the FAO activity model and its implementation in Ukraine. In the process of researching enterprises, respondents in the form of managers responsible for ecosystem management were asked to determine the effectiveness of FAO's activities in the field of responsible ecosystem entrepreneurship in order to formalize the model of FAO's activities and its implementation in Ukraine. The answers of the respondents were processed using the developed method of mathematical filtering of the research results, which consists of three filtering blocks, which makes it possible to detect at the first stage the level of variability of the studied data using the mode and median of the studied series. At the second stage we determine the symmetry and sharpness of the distribution, using the calculation of asymmetry, standard deviation and kurtosis. At the third stage of data processing, based on the analysis of the Pearson correlation coefficient, the Fisher F-criterion, the Friedman test and the Kruskal-Wallis H-criterion, correlation-dispersion deviations are determined, which make it possible to exclude such respondent data with significant deviations from the recommendation part from the norm.

This article analyzes early experiments of Musica Elettronica Viva. The music of this avantgarde group, which emerged on the wave of widespread interest in collective creative process and free improvisation, is usually considered jointly the socio-political upheavals of the late 1960s. Although the historical canvas — student unrest, mass riots, the rise of counterculture — reflects the spirit of the era, external facts obscure the artistic innovation of MEV. Meanwhile, the aim of the group, created to be a composer’s laboratory, was to find a new sound through spontaneous interaction with stage partners. While in the first experiments (Spacecraft) only MEV participants were engaged, in further ones (Soup, Soundpool) everyone could join the improvisation. Such an approach was supposed to not only destroy professional authoritarianism, but also to transform the music art itself. For such a courageous idea, a plan was required. And this plan was suggested by one of MEV founders, the American composer and pianist Frederick Rzewski. The analysis of his articles, lectures, essays, and documents allows us to identify the key points of the MEV strategy, which can be interpreted as the principles of music participation. Despite the fact that the group was fully focused on improvisation, its achievements also follow music composition. This fact proves the personal heritage of each of the MEV participants. Nowadays, due to the rising interest in the compositions created in co-authorship with the listener, the early works of MEV deserve closer attention. They do not just employ one of the possible approaches to engage the audience in the creative process, but, in fact, they have designed and tested an original model of participation, the study of which will join the list of music participation resources.

Voluntary counseling and testing (VCT) service plays an essential part in the prevention of human immunodeficiency virus (HIV) infection. The purpose of this study was to investigate the characteristics of participants and analyze the major factors of HIV infection in VCT in Nantong, China. This study was conducted between January 2010 and December 2015, based on the responses to questionnaires and blood test results retrieved from the Chinese National HIV/AIDS Comprehensive Control Information System (CNHCCIS). Multivariate logistic regression analyses were used to identify factors related to HIV infection. Differences between first-time testers and repeat testers were assessed using the chi-squared or Fisher test. Over six years, a total of 11,560 VCT participants were included, and 420 cases were confirmed to be HIV-positive. Overall, the annual number of participants was relatively stable with a mean of 1927, while there was a rapid increase in the HIV detection rate (from 1.03% in 2010 to 7.52% in 2015). In multivariate analysis, referral counseling and having a HIV-positive spouse/fixed sex partners were found to be significantly associated with HIV infection among all participants, while being unmarried or divorced, having commercial heterosexual behaviors, and male-male sexual behaviors are additional HIV-related factors for males. Compared to first-time testers, repeat testers were more willing to engage in high-risk sexual behaviors and had higher HIV detection rates ( P < 0.001 ). In conclusion, the HIV epidemic in Nantong is still not controlled. Therefore, in the future, it is critical to expand VCT services to increase the detection rate of HIV, which can prevent the transmission of HIV effectively.

Conclusión: Establecer un emparejamiento sexual en el contexto del VIH y tener una orientación no heterosexual presentó diferencias estadísticas entre las variables sociodemográficas y comportamentales. Methods: A cross-sectional study carried out in a medical assistance service specialized in the treatment of people with Human Immunodeficiency Virus, with 173 participants. Pearson's Chi-square or Fisher's exact test was used to analyze the results. Results: We identified an association between sexual orientation and variables such as gender, age, marital status, age, education, income, type of exposure, type of birth, consistent condom use, presence of infection, sexual practice, partner in routine consultations, disclosure of HIV to the partner and considers disclosure of their condition to the partner. Conclusion: Establishing a sexual partnership in the context of HIV and having a non-heterosexual orientation presented statistical differences between sociodemographic and behavioral variables. Objetivo: Analisar a influência da orientação sexual sobre as variáveis sociodemograficas, clínicas e comportamentais entre parcerias sexuais de pessoas que vivem com vírus da imunodeficiência humana/Síndrome da imunodeficiência adquirida. Métodos: Estudo transversal realizado em um serviço de assistência médica especializada no tratamento de pessoas com o Vírus da Imunodeficiência Humana, com 173 participantes. Na análise dos resultados utilizou-se o teste Qui-quadrado de Pearson ou exato de Fisher. Resultados: Identificou-se associação entre a orientação sexual e as variáveis: sexo, idade, estado civil, faixa etária, escolaridade, renda, forma de exposição, tipo de parceria, uso consistente do preservativo, presença de infecção, prática sexual, acompanhamento do parceiro nas consultas de rotina, divulgação do HIV para o parceiro e considerar importante a divulgação da sua condição sorológica para o parceiro. Conclusão: Estabelecer uma parceria sexual no contexto do HIV e ter uma orientação não-heterossexual apresentou diferenças estatísticas entre as variáveis sociodemográficas e comportamentais.

Background: Community pharmacists play an important role in the wellness of patients, families and friends affected by prescription and illicit opioid drugs. They are key partners of the Community Based Naloxone (CBN) Program in Alberta and similar programs across other Canadian jurisdictions. This publicly funded program is an evidence-based response to the opioid overdose crisis, facilitating access to and distribution of naloxone kits through pharmacies. The study aimed to describe Alberta community pharmacists’ practices, training, comfort levels and views in dispensing naloxone kits through the CBN program and detail potential perceived barriers to program participation. Methods: The study was conducted as a cross-sectional online survey of Alberta pharmacists. Data collected from the survey were descriptive and evaluated using Microsoft Excel. Fisher exact tests were used to study the associations in responses among several demographic characteristics and related to dispensing practices, pharmacists’ beliefs and perceived barriers. Results: A total of 255 responses were included in the final analysis, with 89.8% of pharmacists replying “yes” to CBN program participation. Pharmacists on average were “comfortable” dispensing naloxone to patients for varying indications, with 85% reporting always providing education when dispensing naloxone to an individual for the first time. About 41% of pharmacists reported no barriers to the program, with the most common perceived barriers being lack of time (29%), demand (20%) and funding (19%). Conclusion: Most community pharmacists who responded to the survey participate in the CBN program in Alberta. They held positive beliefs about their role in screening patients for the risk of opioid overdose and are confident in their abilities to recommend and educate on naloxone kits. Proactive screening appeared lower, however, and dispensing kits were potentially variable. The assembly of functional holoenzymes composed of regulatory D-type cyclins and cyclin-dependent kinases (cdks) is rate limiting for progression through the G1 phase of the mammalian somatic cell cycle. Complexes between D-type cyclins and their major catalytic subunit, cdk4, are catalytically inactive until cyclin-bound cdk4 undergoes phosphorylation on a single threonyl residue (Thr-172). This step is catalyzed by a cdk-activating kinase (CAK) functionally analogous to the enzyme which phosphorylates cdc2 and cdk2 at Thr-161/160. Here, we demonstrate that the catalytic subunit of mouse cdc2/cdk2 CAK (a 39-kDa protein designated p39MO15) can assemble with a regulatory protein present in either insect or mammalian cells to generate a CAK activity capable of phosphorylating and enzymatically activating both cdk2 and cdk4 in complexes with their respective cyclin partners. A newly identified 37-kDa cyclin-like protein (cyclin H [R. P. Fisher and D. O. Morgan, Cell 78:713-724, 1994]) can assemble with p39MO15 to activate both cyclin A-cdk2 and cyclin D-cdk4 in vitro, implying that CAK is structurally reminiscent of cyclin-cdk complexes themselves. Antisera produced to the p39MO15 subunit can completely deplete mammalian cell lysates of CAK activity for both cyclin A-cdk2 and cyclin D-cdk4, with recovery of activity in the resulting immune complexes. By using an immune complex CAK assay, CAK activity for cyclin A-cdk2 and cyclin D-cdk4 was detected both in quiescent cells and invariantly throughout the cell cycle. Therefore, although it is essential for the enzymatic activation of cyclin-cdk complexes, CAK appears to be neither rate limiting for the emergence of cells from quiescence nor subject to upstream regulatory control by stimulatory mitogens.

This step is catalyzed by a cdk-activating kinase (CAK) functionally analogous to the enzyme which phosphorylates cdc2 and cdk2 at Thr-161/160. Here, we demonstrate that the catalytic subunit of mouse cdc2/cdk2 CAK (a 39-kDa protein designated p39MO15) can assemble with a regulatory protein present in either insect or mammalian cells to generate a CAK activity capable of phosphorylating and enzymatically activating both cdk2 and cdk4 in complexes with their respective cyclin partners. A newly identified 37-kDa cyclin-like protein (cyclin H [R. P. Fisher and D. O. Morgan, Cell 78:713-724, 1994]) can assemble with p39MO15 to activate both cyclin A-cdk2 and cyclin D-cdk4 in vitro, implying that CAK is structurally reminiscent of cyclin-cdk complexes themselves. Antisera produced to the p39MO15 subunit can completely deplete mammalian cell lysates of CAK activity for both cyclin A-cdk2 and cyclin D-cdk4, with recovery of activity in the resulting immune complexes. By using an immune complex CAK assay, CAK activity for cyclin A-cdk2 and cyclin D-cdk4 was detected both in quiescent cells and invariantly throughout the cell cycle. Therefore, although it is essential for the enzymatic activation of cyclin-cdk complexes, CAK appears to be neither rate limiting for the emergence of cells from quiescence nor subject to upstream regulatory control by stimulatory mitogens.

The point of theoretical analysis get constructed the real social interaction Indonesian imigrant workers in Malaysia. Beside it, this research uses naration theory of Fisher De Aubrey which believe that grouping is the important aspect for humanbeing interaction. Paradigm of this research is constructivism, in qualitative model. Research interviews 14 persons Indonesian imigrant workers who had worked in Malaysia. Finding of research, Indonesian imigrant workers have special meaning for pluralism. For Indonesia imigrant workers Malaysia is a dream country, they do not hate that’s country so much. In Indonesian imigrant workers mind there is no pretension to make conflicts in their workplace. Over there work situations very busy and professionalism are completing work if talk about pluralism. Saga in this research find in success stories are so real because local lifestyle and culture of Indonesia and Malaysia are so similar. Construction pluralism for Indonesian workers so positive but they still love of their homeland, Indonesia.

This study, empirical assessment of the effects of social responsibility (SR) strategy on the performance of SMSE’s and possible improvement on entrepreneurs in the north east region of Nigeria was undertaken and found that social responsibility has made an impact on the operation, economic growth and survival of SMSE’s sub-sector in Yobe State of Nigeria. The study was built in SMSE’s sector of the region, the “Thermo Fisher’s 4i Values” (Integrity, Intensity, Innovation and Involvement) it made up the sector in questions a great interaction with customers, suppliers and partners, communities and each other. These four values are foundational to our SR approach (Thermo Fisher scientific, 2020). Incognizance of the importance of SMSE, this study has expanded the body of knowledge in respect to social responsibility of small and micro enterprises. Specifically, the study worked out means in which SMSE sector be encouraged profitably. Only few of SMSE’s do participate and recognized the activities of SR. The study also recognized that financial institutions like banks, individual financer and government are attracted to few SMSE that participated in SR (Basariya, Al Kake, 2019). The study realized that “Social Responsibility (SR) has become a fundamental way of defining the role of business in society” (Itziar & Josep, 2011). Other prominent writers in this context believe that, the needs of current and future generations cannot be met unless there is respect for natural systems and international standards protecting core social and environmental values. It is increasingly recognized the critical role of business sector. As a part of society, it is in business’ interest to contribute to addressing common problems. Strategically speaking, the study has captured that business can only flourish when the communities and ecosystems in which they operate are healthy (Marylyn, Caroline & Sheena, 2011., Judy & Victoria, 2011., Cameron, 2011., Tobias & Frank, 2011., Min-Dong, 2011, &Babafemi, 2015).

BackgroundGlucocorticoids (GCs) are a key treatment for inflammatory rheumatic diseases, but they cause a wide range of adverse side-effects which are of concern both to patients and clinicians.ObjectivesThe objective of this study was to explore the impact of GC therapy on health-related quality of life (HRQoL) during treatment for rheumatic diseases, as a basis for development of a Patient-Reported Outcome Measure (PROM) to be used in clinical trials and practice.MethodsPatients from the UK, USA and Australia who were treated with GCs in the last two years for a rheumatic condition were invited to take part in semi-structured qualitative interviews. Purposive sampling was used to include participants with a range of demographic and disease features. A steering committee of patient research partners, clinicians and methodologists devised an initial conceptual framework, which informed interview prompts and cues. Interviews were carried out by experienced qualitative researchers who encouraged participants to tell their stories and talk about the effects, both adverse and beneficial, of their experiences and perceptions of treatment with GCs, to identify salient physical and psychological symptoms and aspects of HRQoL. The interview data were organised using NVivo, and inductive analysis identified initial themes and domains. Candidate questionnaire items were developed and refined using cognitive interviewing, linguistic assessment, and input from patient research partners.ResultsSixty semi-structured qualitative interviews were conducted (UK n=34, USA n=10, Australia n=16). Mean participant age was 58 years; 39 (66.1%) were female. Purposive sampling of participants provided a broad range of demographic features, GC dosages and inflammatory rheumatic conditions, with 27% having connective tissue disease, 25% inflammatory arthritis, 30% systemic vasculitis and 16% other rheumatic conditions.Initial domains were developed to identify key themes relating to treatment using GCs and their impact on HRQoL; see Figure 1.Figure 1.Steroid PRO Initial ThemesA long-list of 134 initial candidate questionnaire items was developed from the individual themes. These items were reviewed by a qualitative working group of patient research partners, researchers and clinicians to reduce duplication and ambiguity of items. The resulting 62 items were tested and refined by piloting with patient research partners, iterative rounds of cognitive interviews with patients with a range of rheumatic conditions from the UK, USA and Australia, and a linguistic translatability assessment, to define a draft questionnaire of 40 items.ConclusionThis international qualitative study underpins the development of candidate items for a treatment-specific PROM for patients with rheumatic diseases. Background/aims Studies have identified risks for unintended pregnancies, globally and in Nigeria, which include ineffective contraception, strong opposition to family planning by partners, number of living children and birth interval. These factors have contributed to the increasing rate of unintended pregnancy and the high rate of induced abortion, with associated consequences such as obstetric haemorrhage, infection and increased maternal morbidity and mortality. However, there is a paucity of information regarding the influence of culture and religion on pregnancy intentions. This study aimed to examine the influence of culture, religion, sociodemographic characteristics, and reproductive characteristics on nursing mothers' perception of unintended pregnancy in southwest Nigeria. Methods This study used a sequential explanatory mixed-method approach, with both quantitative and qualitative elements. A conceptual hierarchical model was used to analyse the influence of three levels of characteristics (sociodemographic, religious and cultural, reproductive) on unintended pregnancy in southwest Nigeria. A total of 400 nursing mothers attending either a postnatal, immunisation, infant welfare or under-five clinic were selected via multistage sampling from primary healthcare centres. Quantitative data were collected from these participants using a semi-structured questionnaire, administered by a researcher. These data were analysed using both bivariate and multivariate analysis. First, they were analysed with either a chi-squared or Fisher exact test, then subjected to a regression model analysis. Qualitative data were collected and subjected to content analysis via focus group discussions with a total of 32 purposively selected participants. Results Approximately 36.5% participants reported their index pregnancy as being unintended. With regression analysis, age (25–34 years: relative risk ratio=0.42, P=0.02; 35–44 years: relative risk ratio=0.21, P=0.003), parity (relative risk ratio=10.38, P<0.00), ethnicity (relative risk ratio=0.13, P=0.002) and religion (relative risk ratio=0.26, P=0.048) were found to be significant risk factors for unintended pregnancy. Conclusions Age, parity, ethnicity and religion were the main determinants of unintended pregnancies. Intervention programmes should therefore be aware of these variables and address myths and misconceptions about pregnancy intentions.

HIV/AIDS cases are like an iceberg phenomenon, where the number found is far lower than the actual number of sufferers. In Semarang Regency, the number of people living with HIV/AIDS in 2019 has increased compared to before. According to risk factors, Sero Discordant (one partner is HIV positive, while the other is not) is 92.19%, sex worker customers (PS) are 10.57%, and men who have sex with men (MSM) are 8.75%. This study aims to analyze the relationship between knowledge and attitudes toward HIV/AIDS transmission prevention behavior in PLHIV couples. The research design is the quantitative analytic cross-section sectional approach. The population was this study was at the Dahlia Clinic which had partners, totaling 33 people. The sample was taken as a total population, but there were 5 people who were not willing to be respondents, so that the sample was 28 people living with HIV/AIDS. The variables studied were knowledge, attitudes, and HIV/AIDS prevention behavior. Data collection was carried out using a questionnaire in the form of a google form. Data were analyzed descriptively and analytically. For the purposes of testing the hypothesis used fisher's exact test. From the research, the results for the variable knowledge of PLWHA were 23 respondents (82.10%) with a high level of knowledge and 5 respondents (17.90%) had a moderate level of knowledge. For the attitudes of PLWHA, 25 respondents (89.29%) had a positive attitude, while 3 respondents had a negative attitude (10.71%). There is no relationship between the level of knowledge and behavior to prevent HIV/AIDS transmission with a p-value of 0.29, nor is there a relationship between attitudes and behavior to prevent transmission of HIV/AIDS with a p-value of 1.00. It was concluded that the knowledge and attitude variables were not related to the behavior of preventing the transmission of HIV/AIDS in PLWHA partners. prevent transmission of HIV/AIDS with a p-value of 1.00. Desain penelitiannya yaitu analitik kuantititatif dengan pendekatan cross sectional. Populasi dalam penelitian ini adalah ODHA di Klinik Dahlia yang mempunyai pasangan, sebanyak 33 orang. Sampel diambil secara total populasi, namun ada 5 orang yang tidak bersedia menjadi responden, sehingga sampel didapatkan sebanyak 28 orang ODHA. Variabel yang diteliti yaitu pengetahuan, sikap, dan perilaku pencegahan HIV/AIDS. Pengumpulan data dilakukan dengan menggunakan kuesioner dalam bentuk google form. Data dianalisis secara deskriptif dan analitik. Untuk keperluan uji hipotesis digunakan uji fisher exact. Dari penelitian didapatkan hasil untuk variabel pengetahuan ODHA sebanyak 23 responden (82,10 %) dengan tingkat pengetahuan tinggi dan 5 responden (17,90%) memiliki tingkat pengetahuan sedang. Untuk sikap ODHA, didapatkan responden dengan sikap yang positif sebanyak 25 responden (89,29 %), sedangkan 3 responden mempunyai sikap negatif (10,71%). Tidak ada hubungan antara tingkat pengetahuan dengan perilaku pencegahan penularan HIV/AIDS dengan p value 0,29, juga tidak ada hubungan antara sikap dengan perilaku pencegahan penularan HIV/AIDS dengan p value 1,00. The Morningside Initiative is a public-private activity that has evolved from an August, 2007, meeting at the Morningside Inn, in Frederick, MD, sponsored by the Telemedicine and Advanced Technology Research Center (TATRC) of the US Army Medical Research Materiel Command. Participants were subject matter experts in clinical decision support (CDS) and included representatives from the Department of Defense, Veterans Health Administration, Kaiser Permanente, Partners Healthcare System, Henry Ford Health System, Arizona State University, and the American Medical Informatics Association (AMIA). The Morningside Initiative was convened in response to the AMIA Roadmap for National Action on Clinical Decision Support and on the basis of other considerations and experiences of the participants. Its formation was the unanimous recommendation of participants at the 2007 meeting which called for creating a shared repository of executable knowledge for diverse health care organizations and practices, as well as health care system vendors. The rationale is based on the recognition that sharing of clinical knowledge needed for CDS across organizations is currently virtually non-existent, and that, given the considerable investment needed for creating, maintaining and updating authoritative knowledge, which only larger organizations have been able to undertake, this is an impediment to widespread adoption and use of CDS. The Morningside Initiative intends to develop and refine (1) an organizational framework, (2) a technical approach, and (3) CDS content acquisition and management processes for sharing CDS knowledge content, tools, and experience that will scale with growing numbers of participants and can be expanded in scope of content and capabilities. Intermountain Healthcare joined the initial set of participants shortly after its formation. The efforts of the Morningside Initiative are intended to serve as the basis for a series of next steps in a national agenda for CDS. It is based on the belief that sharing of knowledge can be highly effective as is the case in other competitive domains such as genomics. Participants in the Morningside Initiative believe that a coordinated effort between the private and public sectors is needed to accomplish this goal and that a small number of highly visible and respected health care organizations in the public and private sector can lead by example. Ultimately, a future collaborative knowledge sharing organization must have a sustainable long-term business model for financial support.

Gas diffusion layers are an important part of all types of polymer electrolyte membrane fuel cells (LT-PEMFC, HT-PEMFC or AEMFC). Albeit the conversion reaction is not taking place in the GDL the contributions of the GDL to the function of the cell are essential. So, the GDL needs to homogenise the distribution of reacting gases over the electrode area compensating inhomogeneities created by the channel fin structure of the flow-field of the bipolar plate and at the same time cause a focussing of product streams towards the channels. The GDL needs to conduct electric current between the catalyst layer and the bipolar plate at high current density and transport released thermal energy. The GDL also needs to distribute mechanical pressure of the stack compression. In order to accomplish all those tasks, the GDL needs to feature the right properties with high homogeneity through-out its area and with high reproducibility batch to batch. Unfortunately, a rather high number of properties can be assigned to a GDL. So, the review paper by Yuan et al. on properties for different LT-PEMFC components1 lists about 15 different properties for GDL which influence their function. As many of these properties are difficult to measure measuring all of them is not a viable approach to the quality assurance of GDL as part of their industrial production. Within the project QM-GDL Fraunhofer ICT together with the other partners of the project is analysing the relevance of the different properties, comparing the expected impact of deviation from the designed values and the effort, and determining the actual values. The goal is to determine the most relevant parameters which will allow to assure proper functioning of the GDL if their values are controlled keeping the effort for measurements at a reasonable value. In the presentation the approach for the selection of relevant parameters will be explained at a number of examples. Also, a tentative list of parameters will be presented and discussed. Background Oncogenic fusion genes play an instrumental role in the pathogenesis of haematological malignancies and the identification of these genes is crucial for diagnosis, risk stratification and treatment decisions. Conventional diagnostic methods such as karyotyping and fluorescence in situ hybridization (FISH) are the current standard of care but are limited in the sensitivity and availability of different FISH probes for detection of fusion genes. In our institution, we have routinely integrated a targeted next-generation sequencing (NGS) panel in our diagnostic workflow for patients with suspected myeloid malignancies. Here we aim to review the cases with RNA fusion genes detected via NGS and compare these results with that of conventional diagnostic methods. Methods We analysed bone marrow samples from patients with suspected myeloid malignancies at the time of diagnosis in Singapore General Hospital (SGH). Extracted genomic DNA and RNA were amplified, and library preparation was performed with the Oncomine TM Myeloid Research assay (Thermo Fisher Scientific) to interrogate 40 DNA genes and 29 fusion driver genes. NGS libraries were sequenced on the Ion GeneStudio S5 System (Thermo Fisher Scientific) and analyzed using in-house bioinformatics pipelines and the Ion Reporter software. Limit of detection was determined as 3% allele frequency for DNA variants and 10 -2 for RNA fusions. RNA fusions detected by the NGS panel were consolidated and compared with cytogenetics (karyotyping and FISH) results. Results A total of 436 samples from adult patients with suspected myeloid malignancies were received for NGS testing from October 2021 to March 2023 in SGH, the largest tertiary hospital in Singapore. 306 had paired cytogenetics and NGS testing performed. 27 of the 306 cases (9%) had RNA fusions detected by NGS as summarized in Table 1 and Figure 1; their disease subgroups comprised of 16 acute myeloid leukaemia (AML), 5 chronic myeloid leukaemia (CML), 3 BCR-ABL1-negative myeloproliferative neoplasm (MPN), 2 acute promyelocytic leukaemia (APL) and 1 myelodysplastic syndrome (MDS). Eight AML cases had RUNX1-RUNX1T1.R3R3 fusions, with 6/8 (75%) harbouring additional DNA single nucleotide variants or indel mutations. Two AML cases had CBFB-MYH11 fusions, with 1 associated with KIT D419 deletion, and the other with U2AF1, NF1, and ASXL1 mutations. We also identified 2 adult AML cases (aged 49 and 65 years respectively) with DEK-NUP214.D9N18 fusions, both of which had associated FLT3-ITD mutations. There were 5 cases with KMT2A fusions, with a variety of partners including MLLT3.K9M6 (n=2), ELL.K9E2 (n=1), MLLT1.K8M4 (n=1), and MLLT6.K8M9 (n=1). Two patients with APL had PML-RARA detected for different variants: P3R3 and P6R3. Amongst the 8 MPN cases, 5 had BCR-ABL1 fusions consistent with diagnosis of CML. There was 1 case of MPN-Eo with FLP1L1-PDGFRA fusion, and the remaining 2 with ETV6-ABL1 and KMT2A-ELL.K9E2 rare fusions. The detected RNA fusions had clinical impact in establishing diagnosis (93%), prognosis (96%), and guidance of treatment decisions (89%). Out of the 27 cases with RNA fusions detected by NGS, 23 (85%) had concordant karyotype and FISH results. Four RNA fusions, namely KMT2A-ELL, CBFB-MYH11, FIP1L1-PDGFRA and ETV6-ABL1, were not detected by karyotyping or conventional FISH probes performed in our institution. It likely reflects the sensitivity of the NGS panel in detecting RNA fusions in cases of cryptic translocations. Conclusion We have demonstrated that NGS has a high sensitivity for identification of RNA fusion genes, is complementary to conventional cytogenetics testing, and has vast clinical impact in terms of diagnosis, prognostication and clinical management. We advocate for the integration of NGS with DNA and RNA sequencing into routine investigation of suspected myeloid malignancies for a more precise and comprehensive diagnostic approach.

IntroductionThe COVID-19 pandemic has taken a great toll on world population in the last 2 years across different groups. Women in pregnancy and in the perinatal period are especially vulnerable to sources of stress, with repercussions on their own mental health and on the wellness of the newborn.ObjectivesWe aimed at assessing the impact of COVID-19 related stress on women’s psychosocial status and evaluate the potential development of anxiety or depressive symptoms in our cohort.MethodsFrom March 2020 to November 2020, our hospital was appointed as an HUB for treating pregnant women positive for Sars-CoV-2 infection. Sociodemographic and clinical data were collected, and patients were delivered different self-report psychometric tests such as the GHQ-12, CD-RS, MSPSS, UCLA-8, ISI, IES-6 and DASS-21. We identified two main types of stressors in our population. Being diagnosed with a pregnancy complication, having an ectopic pregnancy or a spontaneous abortion were identified as pregnancy-related stressors. On the other hand, infections or deaths among partners or relatives were defined as covid-related stressors. Scale scores were compared considering main sociodemographic data and possible stressors, both pregnancy-related and covid-related, that may affect women’s mental health. Fisher exact test, Chi square Test and Student’s T test were used when appropriate.ResultsAmong 98 women with complete data at baseline, 21,3% suffered from at least mild anxiety, 21% suffered from at least mild depression, and 41% experienced at least mild stress according to the DASS-21 scale. Moreover, 26,4% reported PTSD symptoms according to the IES-6 scale. Women who experienced a stressor were more likely to report psychiatric symptoms, even more so when exposed to a pregnancy-related stressor. Being Caucasian, a personal psychiatric history or a familial psychiatric history correlated with scoring positive to a higher number of scales. Regarding DASS-21, high total scores in this scale correlated with being Caucasian and having had previous deliveries. More specifically, being Caucasian, older age, previous deliveries and a positive psychiatric history correlated with a higher score in the depression subscale. A positive psychiatric history and a familial psychiatric history correlated with higher scores in the anxiety subscale, while being hospitalized at a late gestational age correlated with lower scores in this section. Scoring above the cut-off of the DASS stress subscale correlated with a positive psychiatric history, a positive familial psychiatric history, being Caucasian, and with an older age of the women in the sample.ConclusionsOur results underline the burden of the COVID-19 pandemic on women in pregnancy and in the perinatal period. Tailored interventions should be staged to provide adequate care to this particularly sensitive population, for the wellbeing of both the mother and the child.Disclosure of InterestNone Declared

Background The 2022 multicountry mpox outbreak positioned the condition as a public health emergency of international concern. By May 2023, Brazil ranked second globally in the cumulative number of mpox cases and deaths. The higher incidence of mpox among gay and other men who have sex with men in the current mpox outbreak deepens the stigma and discrimination against sexual and gender minorities (SGM). This might worsen the structural barriers impacting access to health services, which ultimately leads to undertesting and underreporting of cases. There are no data available on mpox knowledge and stigma in Latin America. Objective We aimed to evaluate mpox knowledge, stigma, and willingness to vaccinate for mpox among SGM, and to describe sociodemographic and behavioral characteristics according to self-reported mpox diagnosis. Methods A cross-sectional, internet-based survey was conducted in a convenience sample of adults (aged >18 years) living in Brazil recruited through advertisements on dating apps, social media, referral institutions for infectious diseases websites, and mass media (October-November 2022). We compared participants’ characteristics according to self-reported mpox diagnosis using chi-square test or Fisher exact test for qualitative variables and Kruskal-Wallis test for quantitative variables. Results We enrolled 6236 participants: 5685 (91.2%) were cisgender men; 6032 (96.7%) were gay, bisexual, or pansexual; 3877 (62.2%) were White; 4902 (78.7%) had tertiary education; and 4070 (65.2%) reported low or middle income. Most participants (n=5258, 84.4%) agreed or strongly agreed that “LGBTQIA+ individuals are being discriminated and stigmatized due to mpox.” Mpox awareness was 96.9% (n=6044), and 5008 (95.1%) were willing to get vaccinated for mpox. Overall, 324 (5.2%) reported an mpox diagnosis. Among these, 318 (98.1%) reported lesions, 178 (56%) local pain, and 316 (99.4%) sought health care. Among participants not reporting a diagnosis, 288 (4.9%) had a suspicious lesion, but only 158 (54.9%) of these had sought health care. Compared to participants with no diagnosis, those reporting an mpox diagnosis were younger (P<.001), reported more sex partners (P<.001), and changes in sexual behavior after mpox onset (P=.002). Moreover, participants diagnosed with mpox reported more frequently being tested for HIV in the prior 3 months (P<.001), living with HIV (P<.001), currently using HIV pre-exposure prophylaxis (P<.001), and previous sexually transmitted infection diagnosis (P<.001). Conclusions Our results point to high mpox knowledge and willingness to vaccinate among SGM in Brazil. Participants self-reporting mpox diagnosis more frequently reported to be living with HIV, STI diagnosis, and current pre-exposure prophylaxis use, highlighting the importance of an mpox assessment that includes comprehensive sexual health screenings. Efforts to decrease stigma related to mpox among SGM are necessary to avoid mpox underdiagnosis.

Abstract Cyclin-dependent Kinase 2 (CDK2) is a Ser/Thr kinase activated via interaction with its cyclin partners CCNE and CCNA, driving G1/S progression of the cell cycle. CDK2 inhibition has the potential to address multiple resistance mechanisms to CDK4/6 inhibitors in breast cancer. In addition, CCNE1 is frequently amplified and over-expressed in cancers including ovarian, uterine, breast, gastric and others, with pre-clinical data linking CDK2 inhibition sensitivity to high CCNE1. The CDK2 inhibitor field has historically suffered from high attrition in Phase I due to off-target toxicity. AZD8421 is a uniquely selective CDK2 inhibitor, that offers potential to show improved therapeutic index and combinability. As well as achieving CDK family selectivity in cells versus key off-targets (CDK1, CDK4/6, CDK9), AZD8421 had no significant kinase inhibition outside the CDK family. Only 7 distinct targets showed greater than 50% inhibition in a panel of 403 kinases when screened with 1µM of AZD8421. In nanoBRET assays measuring cellular target engagement, AZD8421 had an IC50 against CDK2 of 9nM with selectivity over CDK1, CDK4 and CDK6. In vitro cellular assays demonstrated that AZD8421 inhibits endogenous phospho-substrate at 58nM with &gt;327-fold selectivity over CDK9 phospho-substrate pSer2 of RNAPII (&gt;19.2 µM). In a CCNE1 amplified cell line, AZD8421 potently inhibited cell proliferation (69nM, OVCAR3), correlated with inhibition of pRB, arrest in G1/S phase of the cell cycle and induction of senescence, compared to a CCNE1 non-amplified cell line (SKOV3) with IC50 of 2.05 µM. In vitro combination assays in CDK4/6 inhibitor resistant breast cancer cell lines showed combination benefit with AZD8421 plus approved CDK4/6 inhibitors. In vivo, AZD8421 potently suppressed phosphorylation of Rb, and demonstrated robust monotherapy and CDK4/6i combination activity in breast and ovarian in vivo models. In vivo PD marker suppression and efficacy was demonstrated in CDK4/6 inhibitor resistant breast PDXs in combination with palbociclib (CDK4/6i). AZD8421 showed robust monotherapy activity in a CCNE1 amplified ovarian model OVCAR3 with regressions seen with monotherapy and in combination with palbociclib. Monotherapy activity and combination benefit in vivo with standard of care agents was also observed in ovarian PDX models with elevated levels of CCNE1. AZD8421 is a potent and highly selective CDK2 inhibitor, with double digit nM potency in cells, as well as suitable physical and pharmacokinetic properties for progression into the clinic. Abstract Introduction/Background Endocrine therapy remains the fundamental treatment for advanced HR+ breast cancer (BC). For those patients who become refractory to endocrine therapy, resistance may be associated with mutations, amplifications, and fusions in the ESR1-encoded estrogen receptor. Here we examine the frequency of ESR1 alterations and the associated genomic landscape. Methods HR+/HER2- BC samples were sequenced with the OncomapTM ExTra assay, which uses whole-exome DNA sequencing with germline subtraction to detect somatic single nucleotide substitutions, indels, and copy number alterations (CNAs), and uses RNA sequencing to detect gene fusions. Tumor mutational burden (TMB) and microsatellite instability (MSI) are also reported. For analysis, BC samples were grouped by site: local (primary breast or regional lymph node) versus metastatic. Prior treatment history was unknown. Testing for a possible association between ESR1 and other biomarkers (genes, MSI, and TMB) was done using Fisher’s Exact Test (p≤0.05). Results A total of 988 HR+HER2- breast cancer patient samples were included in the analysis. Of these, 821 (83.1%) were local samples and the remaining 167 were metastatic samples, with liver (63, 37.7%), bone (20, 12.0%), skin (16, 9.6%) and chest wall (15, 9.0%) being the most common locations. ESR1 alterations were present in 84 tumor samples, 37 local and 47 metastatic, representing 4.5% and 28.1% of samples, respectively. ESR1 alterations included missense mutations (54 samples), fusions (29 samples), and amplifications (8 samples). The most common missense mutations were Y537S (20 samples, 37.0%), D538Q (20 samples, 37.0%), and E380Q (6 samples, 11.1%), which were located in the ligand binding domain and included both clonal and subclonal events. There were 30 ESR1 fusions identified, 17 (2.1%) in local and 13 in metastatic (7.7%) samples. Most fusions (24 samples, 82.8%) involved the same partner, CCDC170, while the other 6 fusions had unique partners. Examination of the 143 other biomarkers altered among ESR1-altered samples revealed 15 genes and MSI-high status that appeared to be over-represented (Table 1). However, none of the associations were statistically significant after correcting for multiple comparisons. Further, there was no evidence that the prevalence of ERBB2, TP53, AKT1 and PIK3CA alterations differed by ESR1 status (Table 1). Conclusions ESR1 alterations were significantly more common in HR+/HER2- metastatic breast cancer samples compared to local samples. Comprehensive genomic profiling with RNA sequencing identified both common and rare ESR1 fusions, which were most frequent in the metastatic samples. No significant difference in the molecular profile of ESR1 altered vs ESR1 wildtype samples was found in this cohort. CDK2 is a Ser/Thr kinase activated via interaction with its cyclin partners CCNE and CCNA, driving G1/S progression of the cell cycle. CDK2 inhibition has the potential to address multiple resistance mechanisms to CDK4/6 inhibitors in breast cancer. In addition, CCNE1 is frequently amplified and over-expressed in cancers including ovarian, with pre-clinical data linking CDK2 inhibition sensitivity to high CCNE1. First-generation inhibitors targeting CDK2 suffered from poor tolerability in the clinic likely due to off-target activities across both the CDK family and across the protein kinome. As well as achieving CDK family selectivity in cells versus key off-targets (CDK1, CDK4/6, CDK9), it’s noteworthy that AZD8421 had no significant kinase inhibition outside the CDK family. X-ray co-crystal structures of AZD8421 with CDK2 provided the basis for the observed selectivity among the CDK-family. For example, hydrogen bonding interaction of AZD8421 with Lys89 (a CDK2-specific residue) near the solvent region of the ATP-binding pocket was the key requisite for achieving &gt;300-fold selectivity over CDK9. In nanoBRET assays measuring cellular target engagement, AZD8421 had an IC50 against CDK2 of 9nM with selectivity over CDK1, CDK4 and CDK6. Further kinetic characterization of AZD8421 binding revealed a long residence time at CDK2 (2 hours) but not CDK1 (7 minutes). These kinetic studies showed that AZD8421 has a low Kd,app for CDK2 and thus even higher selectivity vs CDK1 than was measured by single timepoint assays. In a CCNE1 amplified cell line, AZD8421 potently inhibited cell proliferation (69nM, OVCAR3), correlated with inhibition of pRB, arrest in G1/S phase of the cell cycle and induction of senescence. In vitro combination assays in CDK4/6 inhibitor resistant breast cancer cell lines showed combination benefit with AZD8421 plus approved CDK4/6 inhibitors. Background:Glucocorticoids (GCs) are a key treatment for the autoimmune rheumatic diseases; however, they produce numerous physical and psychological side effects.1 The Outcome Measures in Rheumatology (OMERACT) Glucocorticoid Working Group have identified that there are no Patient Reported Outcome Measures (PROMs) for assessing the impact of systemic GC therapy across multiple rheumatic diseases from the patient’s perspective.2,3Objectives:The aim is to explore the impact of GCs on the symptoms and health-related quality of life of adults with rheumatic inflammatory diseases, to inform items for inclusion in a PROM. Key considerations will include patient perceptions of GC therapy at diagnosis and over the course of treatment, for use in future randomised controlled trials or in clinical practice.Methods:An international steering committee comprising researchers, rheumatology clinicians, methodologists and patient partners in the UK, Australia and USA developed an initial conceptual framework informed by a review of the literature. Semi-structured interviews were conducted in each country with patients who had an autoimmune rheumatic disease and had received GC therapy. The interviews explored salient aspects of health-related quality of life associated with being treated with GCs.Results:Interviews have been completed in three continents with patients who had a range of demographic features, rheumatological conditions and duration and dosage of GC therapy. Figure 1 shows the initial conceptual framework for developing the GC PROM (Steroid PRO).Figure 1.Conclusion:This conceptual framework will act as an evolving guide in the development of a PROM for assessing patients’ perspectives of systemic glucocorticoid therapy. Introduction: Unexplained erythrocytosis is a common reason for consultation in hematology. Identification of JAK2V617F mutation has facilitated the diagnosis of Polycythemia Vera (PV), but a proportion of patients without clear secondary causes for erythrocytosis remain undiagnosed or presumptively diagnosed with an either Exon 12 mutation or JAK2 negative PV. Since 2005, our institution has been performing JAK2V617F testing by PCR. In 2018 we switched to an NGS panel which includes JAK2/exon 12 and 40 other genes implicated in myeloid malignancies. We reviewed all previously diagnosed PV who had NGS myeloid panel performed to determine whether patients with a clinical diagnosis of JAK2 negative PV had other myeloid mutations that might explain their erythrocytosis and alter their management. Methods: We identified all cases with clinically suspected PV or confirmed JAK2 mutated PV who went on to have had NGS testing performed between January 2018 and February 2019 at London Health Sciences Centre, a tertiary care center servicing a population of approximately 2.5 million in Ontario, Canada. The Oncomine Myeloid NGS panel (Thermo-Fisher, MA, USA) examines DNA sequence variants in 40 genes (17 full genes and 23 hotspot genes) along with an RNA-based panel of 29 fusion driver genes and their over 600 fusion partners. Diagnosis was based on the WHO 2016 Classification of Tumours of Haematopoietic and Lymphoid Tissues. The clinical impact was assessed from retrospective review of electronic medical record to determine whether there was a diagnostic or management impact. Results: A total of 143 patients followed for PV or unexplained erythrocytosis had NGS testing during the study period. Of those, 137/143 (95.8%) patients had previous JAK2V617F PCR tested and 48/137 (35%) were identified with JAK2V617F mutation. Of the 48 patients with previous JAK2V617F PCR detected, NGS confirmed JAK2 mutation in 40/48 (83.3%) with additional non-JAK2 mutations in 17/40 (42.5%) patients. Of note 8/48 (16.7%) patients previously detected JAK2V617F by PCR had undetectable JAK2 mutation when repeat testing was performed by NGS. Of those 89/137 (65%) patients with previous JAK2V617F PCR negative result, NGS revealed JAK2 exon 12 mutation in 3/89 (3.4%) patients and JAK2V617F/JAK2L611V mutations in 1/89 (1.1%) patient resulting in diagnosis as PV, whereas non-JAK2 mutations in 6/89 (6.7%) patients. No MPL or CALR positive cases were identified in this cohort. Remaining 79/89 (88.8%) had no mutations identified (Figure1) and in this group, 13/79 (16.5%) patients were discharged from hematology clinic, 7/79 (8.9%) had therapies such as phlebotomy, aspirin or hydroxyurea stopped or reduced, whereas 2/79 (2.5%) patients had further evaluation or testing for unexplained erythrocytosis. (Table 1) Conclusions: In the unexplained erythrocytosis JAK2V617F PCR negative group, JAK2exon 12 mutation was identified in 3.4% in keeping with known incidence of this mutation. Some previously positive PCR JAK2V617F mutation were not identified by NGS panel (16%) which may reflect changes in clone size either with time or therapy or inherent differences in assay sensitivity (2.5% mutational alleles for NGS versus 0.1% for PCR). Lack of identifiable myeloid mutation and clonal hematopoiesis by NGS testing influenced clinical management. Specifically, mutation negative patients were more likely assigned to non-MPN group and called secondary erythrocytosis which resulted in reducing interventions. Non-JAK2 mutations occurred in more than 1/3 of previously identified JAK2 positive PCR tested PV. The clinical impact of most these mutations is uncertain and requires longer follow up. Background: A common referral to hematology is for patients with suspected myelodysplastic syndrome (MDS) or cytopenias of undetermined origin. We recently implemented a Next Generation Sequencing (NGS) panel which covers a wide spectrum of genes related to the major myeloid disorders, including DNA based gene sequencing and RNA based gene fusion analysis, as a tier one clinical diagnostic test (Levy et al. Eur. J. Haematol 2019). We compared the diagnostic and prognostic information derived from conventional cytogenetics and NGS testing as well as the clinical impact on management in this patient group. Methods: We identified all new cases with suspected MDS or cytopenias of undetermined origin referred between January 2018 to February 2019 that had both NGS and cytogenetic testing at London Health Sciences Centre, a tertiary care centre servicing a population of approximately 2.5 million in Southwestern Ontario, Canada. From the retrospective review of electronic medical record, patient demographics, diagnosis and management were ascertained. Diagnosis was based on the 2016 WHO classification and the ICD-10.Previously reported definitions for ICUS, IDUS, CHIP, and CCUSwere also adopted (Bejar et al. Leukemia 2017). Patients not meeting these criteria were defined as either cytopenia not yet determined (NYD) or secondary to other systemic disease. The impact of NGS and cytogenetics results on diagnosis, prognosis, and management of each disease were assessed by referring to the latest National Comprehensive Cancer Network (NCCN) Clinical Practice Guidelines in Oncology. Either a bone marrow (BM) or peripheral blood (PB) sample from each patient was assessed by the Oncomine Myeloid NGS panel (Thermo-Fisher, MA, USA), which examines DNA sequence variants in 40 genes (17 full genes and 23 hotspot genes) along with an RNA-based panel of 29 fusion driver genes and their over 600 fusion partners. Patients' BM samples were also tested by the conventional cytogenetic G-banding method. Results: Of the 1100 samples assessed by NGS, 178 met the study inclusion criteria. Overall, 120 (67.4%) patients had both cytogenetics and NGS performed on their BM samples. Of those 120 patients, 41 (34.2%) had DNA mutations, 17 (14.2%) had cytogenetic abnormalities, 22 (18.3%) had both molecular and cytogenetic findings, and 40 (33.3%) had neither abnormality identified. NGS Information contributed in diagnosing 13 (10.8%) patients, while cytogenetics in 6 (5%) patients. In addition, of 38 patients who were diagnosed as MDS with good/intermediate risk cytogenetics, 24 (63.2%) harbored poor prognostic risk mutations as detected by NGS (Table), influencing the management of disease. Additionally, 22 (12.4%) of all patients had NGS testing using PB and cytogenetics using BM samples, and of these 13 (52.2%) patients had NGS abnormalities. Finally, 36 (20.2%) of all patients had only NGS testing by PB samples. Of those, 12 (33.3%) were found to harbor at least one gene mutation including ASXL1, TP53, ZRSR2, and STAG1 suggesting poor prognostic significance for diagnoses of MDS, or JAK2, SF3B1 which could support a disease specific diagnosis. Conclusion: NGS had enhanced diagnostic capabilities including classification of newly described entities such as ICUS or CCUS and more importantly yielded additional prognostic information compared to cytogenetics alone for this patient population. Cytogenetic findings were mainly aneuploidy or deletions, either clinically evident constitutional abnormalities such as Trisomy 21 and +X in Klinefelter syndrome, or loss of Y chromosome in a small proportion of cells and thus of questionable clinical significance. Based on this information consideration should be given to using the NGS panel as the primary molecular diagnostic and prognostic tool with karyotyping being reserved for subsets of patients being assessed for suspected MDS or cytopenias of undetermined origin. Abstract Abstract 2485 T-cell acute lymphoblastic leukemia (T-ALL) is caused by multiple lesions affecting genes involved in cell cycle control, proliferation, survival and differentiation. Type A abnormalities are driving events in the leukemogenic process; they are mutually exclusive genomic rearrangements, mainly translocations, which delineate specific T-ALL subgroups such as the TAL/LMO, TLX1, TLX3, and HOXA (MLL, CALM-AF10, and SET-NUP214). Type B abnormalities, on the other hand, are much greater in number and cooperate with different molecular lesions, as they are widespread in diverse genetic subgroups. They include genomic imbalances, chromosome translocations and mutations. To confront the heterogeneity of T-ALL associated genomic lesions we used CI-FISH, SNP and GEP, explored their utility as potential diagnostic tools in pediatric T-ALL and correlated the genetic lesions at diagnosis with the Minimal Residual Disease (MRD) clinical stratification. We examined 51 children with T-ALL enrolled in Italy into the AIEOP-BFM ALL2000 protocol; MRD was performed by RQ-PCR monitoring of IG and TCR clonal rearrangements at days 33 and 78. Based on tumor load levels at both time points, patients were classified as MRD standard (MRD-SR), intermediate (MRD-IR) or high risk (MRD-HR)(Schrappe M, et al. Blood 2011;118:2077-84). Combined Interphase FISH (CI-FISH) was done using a panel of probes for 42 candidate genes. Cases with TCR-rearrangements were investigated for partners; aneuploidies were confirmed with probes for the centromeric alpha satellite regions. Single Nucleotide Polymorphism (SNP) arrays were done with Whole-Genome Cytogenetic 2.7M array (Affymetrix, Santa Clara, CA, USA) following the manufacturer's protocol. Data were analyzed by Chromosome Analysis Suite (ChAS) Software (Affymetrix) with hg19 genome built (http://genome.ucsc.edu/) as reference. Copy number (CN) filters were set at ≥ 20 markers and ≥ 30kb size for losses, ≥50kb for gains, and ≥ 2Mb for CNN-LOH. Gene expression profiles (GEP) (Affymetrix HU-133 Plus 2.0 arrays) were obtained following previously described protocols. Data were normalized with Robust Multi-array Avarage algorithm (RMA). Prediction was performed using the method Prediction Analysis of Microarrays (PAM): a threshold for the selection of predictive probe sets was established using cross validation. This procedure was used to predict and compare classification of 34 cases with CI-FISH and 6 samples for which CI-FISH was not available. CI-FISH grouped 85% of patients according to Type A mutations with distribution in each category reflecting the estimated incidence in childhood T-ALL: 37.5% of cases in the TAL/LMO subgroup, 22.5% in the HOXA, 20% in the TLX3, and 5% in the TLX1. GEP class prediction was concordant with the CI-FISH results. PAM analysis identified 15 probe sets, 11 of which are part of the signature used by Homminga et al. (Cancer Cell 2011;19:484–497) to distinguish the three categories LMOTAL, TLX3 and HOX. SNPs complemented the findings by detecting Type B abnormalities in all cases, thus linking Type A and B lesions. CDKN2A/B abnormalities, as previously observed, were prevalently associated with TLX1 and TLX3(∼90% of TLX1/TLX3 positive cases vs 68% of TAL/LMO and 55.5% of HOXA); PTEN/10q23 deletions occurred in the TAL/LMO subgroup; monoallelic RB1 deletions were found only in the TLX3 subgroup. Interestingly, the TLX3 subgroup was also significantly associated with trisomies and/or tetrasomies of chromosomes 1, 7, 8, and 18 (Fisher exact test; p=0.01). Patients' stratification according to MRD showed most cases with TAL/LMO (15/18, 83.3%) and HOXA (6/9, 66.6%) fell within MRD-HR while TLX3 positive cases were mainly in MRD-SR (4/8) and MRD-IR (3/8). The 2 TLX1 positive cases were respectively assigned to MRD-SR and MRD-IR. CI-FISH and SNP arrays provided us with an accurate and exhaustive genomic diagnosis of pediatric T-ALL. Based on CI-FISH results a successful predictive GEP test was elaborated. In a prospective design, dissection of type A/B abnormalities within MRD categories might represent a refined prognostic stratification at diagnosis for the majority of patients.
