Advancing vascular sheaths, catheters, balloons, stent grafts, or drainage catheters can prove difficult in tight or tortuous anatomy, leading to prolonged procedure and fluoroscopy time. Overcoming the static forces of friction requires greater magnitude of force compared to the kinetic forces of friction. Static forces of friction can result in catheter or device kinking, particularly in tight or tortuous anatomy. By applying slow, steady force (as opposed to multiple isolated applications of force) in a manner described in this chapter, one can advance a device in a slow and steady manner, thus reducing the amount of pain, tissue damage, and potentially fluoroscopy time.

Advancing a drainage catheter can be difficult in certain situations, particularly when the catheter “accordions” upon itself. Most tubes are built with a small shelf in the tip of the catheter, and metal and plastic stiffeners are provided. Applying the majority of the advancing force at the hub of the metal stiffener rather than along the midportion of the catheter exerts force at the tip of the catheter via the internal metal stiffener, which is lodged against the small shelf inside the catheter. Using this method, the operator is able to translate a majority of the forward force to the catheter tip, thereby “pulling” the rest of the catheter through tissues and thus reducing the accordion effect.

Peripheral oedema is a palpable swelling caused by increased interstitial fluid in soft tissues, and can be due to local or systemic disease. Fluid distribution between capillaries and the interstitium is governed by Starling forces. The lymphatic system returns excess fluid and protein from the extracellular, interstitial space to the bloodstream. Thus, interstitial oedema may arise from factors that increase capillary pressure or permeability, factors that reduce plasma colloid osmotic pressure, factors that impede lymphatic drainage, or a combination of these causes. This topic addresses the diagnosis of peripheral oedema.

Lower urinary tract trauma is common. Indeed, iatrogenic lower urinary tract trauma is the commonest type of urological injury. External trauma is much less common but potentially life-threatening, if only because of the force needed to cause it. This chapter describes the incidence, aetiology, pathology, clinical features, and management of both types of injury and draws attention to the controversies in management of the most serious of these injuries which remain controversial after nearly 100 years of debate. Despite the controversies, the three fundamental principles of treatment remain unchallenged: to provide urinary drainage to prevent or relieve urinary retention; to prevent or treat urinary extravasation; and to provide the best possible conditions for recovery of the injury.

The pleural cavity is normally a virtual space that is essential to guarantee the mechanical coupling between the lung and the chest wall. The volume of pleural liquid is determined by the equilibrium of fluid turnover. The determinants of this balance are the Starling forces, the lymphatic drainage, and the active trans-membrane transport. When fluid or air accumulate inside the pleural cavity, pleural pressure rises to atmospheric level causing the lung to collapse while the chest wall to expand. The displacement is not equally distributed between lung and chest wall, because it depends upon their own compliance. Pneumothorax and pleural effusion are common diseases in critically-ill patients. Pneumothorax is divided in two groups based upon the aetiological mechanism—spontaneous and traumatic. Pleural effusion is classified as transudates or exudates, mainly based on protein content; this classification comprises different pathological mechanisms beneath the two kind of pleural effusion.

This book is an authoritative guide to the rules of international law governing the navigational and non-navigational uses of international rivers, lakes, and groundwater. The continued growth of the world’s population places increasing demands on Earth’s finite supplies of fresh water. Because two or more States share many of the world’s most important drainage basins, competition for increasingly scarce fresh water resources will only increase. Agreements between the States sharing international watercourses are negotiated, and disputes over shared water are resolved, against the backdrop of the rules of international law governing the use of this precious resource. The basic legal rules governing the use of shared freshwater for purposes other than navigation are reflected in the 1997 UN Convention on the Law of the Non-Navigational Uses of International Watercourses. This book devotes a chapter to the 1997 Convention but also examines the factual and legal context in which the Convention should be understood, considers the more important rules of the Convention in some depth, and discusses specific issues that could not be addressed in a framework instrument of that kind. It reviews the major cases and controversies concerning international watercourses as a background against which to consider the basic substantive and procedural rights and obligations of States in the field. This new edition covers the implications of the 1997 Convention coming into force in August 2014, and the compatibility of the 1997 and 1992 Conventions.
