Der volle Inhalt der QuelleThe objective of this study was to compare neonatal morbidity and mortality between late-preterm infants (gestational age at birth: 34 to 36 weeks and 6 days) with and without fetal growth restriction (FGR). A longitudinal study was conducted between October 2004 and October 2006 involving 50 pregnant women with pre-term delivery associated with FGR (group I) and 36 women with spontaneous preterm delivery not associated with FGR (group II). The following postnatal outcomes were evaluated: weight and gestational age at birth, cesarean section rate, 5-minute Apgar score, umbilical artery pH at birth, and need for and duration of orotracheal intubation and hospitalization in the neonatal intensive care unit (NICU), as well as the presence of respiratory distress syndrome (RDS), sepsis, thrombocytopenia, hypoglycemia, intracranial hemorrhage (ICH) and jaundice, need for phototherapy, length of hospital stay, and occurrence of death. The chi-square test, Fishers exact test and nonparametric Kruskal-Wallis test were used for statistical analysis, adopting a level of significance of 5%. Gestational age was similar in groups I and II, with a mean of 35.5 weeks in both groups. A higher frequency of the following adverse postnatal outcomes was observed in group I: lower birth weight (p<0.001), higher incidence of cesarean section (92% versus 25% in group II; p<0.0001), greater need for NICU treatment (58% versus 33%; p=0.041), longer hospital (p<0.001) and NICU stay (p<0.001), higher frequency of ICH (12% versus 0; p=0.037) and hypoglycemia (24% versus 6%; p=0.047), and longer duration of phototherapy (p=0.005). No differences in Apgar scores, cord pH, orotracheal intubation, RDS, thrombocytopenia, sepsis, or jaundice were observed between groups. There were no cases of hyaline membrane disease, bronchopulmonary dysplasia, pulmonary hemorrhage, or neonatal death. In conclusion, the group of late-preterm infants with FGR presented more neonatal complications than the group without FGR

Der volle Inhalt der QuelleMethods. Subjects were 444 term controls drawn from 5,337 participants of a multi-centre nested case-control study of preterm birth in Montreal. Maternal hair, collected after delivery, was measured for average nicotine and cotinine concentration across the pregnancy, assuming hair growth of 1 cm/month. The BW for GA z-score used Canadian population-based standards. Multiple linear regression was used to assess effects on the z-score, after controlling for potential confounders.
Results. In regression models for maternal active smoking analysis, the addition of hair nicotine to models containing either self-report or hair cotinine or both self-report and cotinine explained significantly more variance in the BW for GA z-score (p=0.009, p=0.017, and p=0.033, respectively). In maternal passive smoking analysis, no significant effect of ETS on BW for GA was found using hair biomarkers.
Conclusion. These results indicate that hair biomarkers are sensitive tools capable of predicting reductions in birthweight for maternal active smoking. The stronger results obtained for nicotine are reflective of the fact that hair nicotine is a better measure of maternal smoking, but it could also suggest that nicotine plays an aetiologic role in affecting foetal growth.

Resumo: A restrição do crescimento intrauterino (RCIU) limita o desenvolvimento fetal adequado aumentando a morbidade e mortalidades perinatais. Os mecanismos fetais adaptativos na RCIU podem desencadear alterações endócrinas e metabólicas que explicariam a ocorrência de doenças na idade adulta. O objetivo do estudo foi avaliar na RCIU experimental pela ligadura da artéria uterina se existem alterações na morfometria e histologia do fígado, intesti no e rins e se existem diferenças na expressão dos receptores de insulina, IR-(3, IRS-1, IRS-2, IGF-IR(3 no grupo de fetos submetidos à RCIU. O presente experimento foi submetido ao Comitê de Ética e Experimentação Animal da Universidade Estadual de Campinas (CEEA-UNICAMP) e aprovado como projeto de pesquisa N° 1644-1. Para realização do estudo utilizamos fetos de ratas Sprague Dawley divididos em 3 grupos. Grupo I (RCIU) - 40 fetos submetidos à ligadura da artéria uterina unilateral com 18,5 dias de gestação, Grupo II (Controle-RCIU) - 40 fetos do corno oposto ao da ligadura da artéria uterina e Grupo III (Controle Externo) - 40 fetos sem procedimento cirúrgico ou alimentar. Os resultados mostraram no modelo experimental de RCIU uma diminuição do peso corporal (PC), hepático (PH) e intestinal (PI) (p<0,01) no grupo RCIU, as relações entre PH/PC, PI/PC, PR/PC foram mantidas, fetos RC IU tem diminuição das camadas submucosas e mucosas intestinais (p<0,05); diminuição da camada cortical renal e do número de glomérulos, com aumento do volume glomerular (p<0,05). Na RCIU encontramos menor expressão hepática do IR-(3, IRS-1 e IRS-2, menor expressão do IRS-2 no intestino e rins e maior expressão do IGF-IR(3 em todos os tecidos. O modelo experimental estudado causou uma RCIU simétrica com alterações morfométricas e do metabolismo da glicose que poderiam justificar no futuro um maior risco de doenças metabólicas
Abstract: Intrauterine growth restriction (IUGR) limits appropriate fetal development increasing morbidity and perinatal mortality. Adaptive mechanisms in fetal IUGR may leave to endocrine and metabolic alterations that could explain the occurrence of diseases in adulthood. The aim of this study was to evaluate whether experimental IUGR by uterine artery ligation causes changes in morphology and histology of the liver, intestines and kidneys. We also evaluated if there were differences in the expression of insulin receptors, IR-(3, IRS-1, IRS-2, IGF-IR(3 of fetuses subjected to IUGR. This experiment was submitted to the Ethics and Animal Experimentation of the Campinas State University (UNICAMP CEEA) and was approved as a research project No. 1644-1. The study used fetuses Sprague-Dawley rats divided into 3 groups. Group I (IUGR) - 40 fetuses who underwent uterine artery ligation sided with 18.5 days of pregnancy Group II (Control-IUGR) - 40 fetuses of the horn opposite to the uterine artery ligation, and Group III (External Control) - 40 fetuses without surgery or food The results showed the experimental model of IUGR, a reduction in body weight (BW), liver (PH) and intestine (PI) (p <0.01) in IUGR, the relationship between PH/PC, PI/PC, PR/PC have been retai ned, IUGR fetuses have reduced layers of the intestinal mucosa and submucosa (p<0,05), decreased renal cortical layer and the glomerular number and increased volume rate (p<0,05). In IUGR found lower hepatic expression of IR-(3, IRS-1 and IRS-2, reduced expression of IRS-2 in the intestine and kidney and increased expression of IGF-IR(3 in all tissues. The experimental model studied caused a symmetrical IUGR with histological changes and glucose metabolism that could justify a greater risk of metabolic diseasesin the future

Der volle Inhalt der QuelleIntrodução: A restrição do crescimento fetal, também denominada crescimento intrauterino restrito (CIUR), é uma das principais complicações da gravidez. Está associada a elevados índices de morbimortalidade perinatal e na infância, requerendo investimentos financeiros elevados para possibilitar assistência adequada a esses recém-nascidos. Justificativa: Necessidade de pesquisa de marcador clínico-ecográfico, através do qual se possa predizer o risco de um feto vir a ter restrição do crescimento ou desfecho gestacional desfavorável, possibilitando intervenção precoce, e melhor assistência perinatal. Objetivos: Verificar relação entre a medida do volume placentário obtida pela ultrassonografia antenatal, e imediatamente após o nascimento pela macroscopia, em gestantes de fetos com suspeita de CIUR e gestantes de baixo risco, e os achados perinatais. Materiais e métodos: estudo transversal, prospectivo e observacional, realizado com 30 gestantes de baixo risco e 19 gestantes de fetos com suspeita de CIUR (peso fetal estimado e/ou circunferência abdominal abaixo do percentil 10 para a idade gestacional), atendidas no Hospital Universitário de Santa Maria (HUSM). O volume placentário anteparto, em cm3, foi mensurado pelo método descrito por Azpurua et al, que utiliza as medidas do comprimento, altura e espessura placentários; através do aplicativo disponibilizado pelo mesmo autor, determinou-se o percentil em que se encontrava o volume, para a determinada idade gestacional. O volume pós-parto foi medido pelo Princípio de Arquimedes. Dados perinatais foram obtidos dos registros de nascimento e prontuários dos recém-nascidos. As medidas das variáveis estudadas foram analisadas sob a forma de média e desvio padrão (dados paramétricos), mediana e quartis (dados não paramétricos). Testes estatísticos: t-Student, Mann-Whitney, correlação de Pearson; foi considerado satisfatório um nível de significância de 5%, e os dados armazenados e analisados no pacote estatístico SPSS versão 21.0. Resultados: houve diferença altamente significante entre o volume placentário ecográfico e macroscópico, em ambos os grupos (p<0,001); foi verificada correlação entre o volume placentário e o APGAR no primeiro minuto no grupo CIUR (p<0,02); existiu associação altamente significante entre internação na UTI-Neonatal, sendo maior no grupo CIUR (p<0,01); 94,7% das pacientes do grupo CIUR tinham placentas com volume abaixo do p10, no aplicativo utilizado. Conclusões: o volume da placenta no pós-parto foi menor que o calculado antes do nascimento, em ambos os grupos, o que é esperado, em razão da perda de sangue pela placenta após dequitação. Desfechos perinatais desfavoráveis estiveram presentes quando o volume placentário é pequeno, mas que poderiam ser justificados pela prematuridade. Assim, os achados do presente estudo são sugestivos de que o volume placentário em fetos com CIUR é reduzido, e associado a alguns desfechos perinatais adversos, mas estudos com amostras maiores são necessários para confirmar essas hipóteses.

Resumo: A programação fetal é um processo fisiológico que assegura, durante o desenvolvimento intrauterino, a adaptação para o mundo exterior. Ou seja, o organismo é "moldável" por estímulos durante sua formação e dependendo do insulto experimentado, existe a possibilidade de mudanças estruturais e funcionais que podem predispor o indivíduo a doenças na vida adulta. O modelo de restrição proteica, assim como outros modelos, leva a um "stress" gestacional e, segundo a hipótese de Barker, programa a prole ao desenvolvimento de doenças na vida adulta, dentre elas a hipertensão arterial. Os rins são órgãos fundamentais na manutenção do equilíbrio hemodinâmico. Mudanças morfológicas e neuroendócrinas nos rins levam a alterações hidroeletrolíticas frequentemente associadas à patogênese da hipertensão arterial. A gênese desta doença ainda não está bem descrita, por envolver alterações multifatoriais, dentre elas, modificações da atividade neural tanto central quanto periférica, que podem ser um indicativo da elevação pressórica em nosso modelo. A atividade simpática renal é um importante modulador da excreção dos eletrólitos e, quando alterada, promove maior ou menor retenção de sais, principalmente o sódio, podendo contribuir para a elevação da volemia e consequentemente da hipertensão arterial. Diversos neuropeptídeos estão envolvidos na atividade simpática renal e os níveis destes são um importante marcador na gênese da hipertensão. Dentre esses peptídeos estão a Substância P (SP), seu receptor NK1R e o Peptídeo Relacionado ao Gene da Calcitonina (CGRP). Nossos resultados mostraram redução na imunorreatividade de SP, CGRP e aumento do receptor 1 para neurocinina nos gânglios da raiz dorsal da prole de ratas submetidas à restrição proteica gestacional. Identificamos também a elevação dos níveis de CGRP na parede pélvica renal. Assim, acreditamos que haja alterações na neuromodulação da atividade aferente renal, o que pode ser um fator contribuinte para a manutenção do estado hipertensivo neste modelo experimental
Abstract: A fetal programming is a physiologic process that ensures an adaptation for external world during the intra uterine development. In this period, the organism is "moldable" by stimulus that happens during its formation, which ensures adequate phenotypes formation for different environments. Kidneys are the most important organs when it has to do with maintaining the organism hemodynamic balance and also morphological and neuroendocrine alterations, which leads to fluid and eletrolytes changes, frequently associated to arterial hypertension pathogenesis. The genesis of this disease is not well described yet. It involves multifactorial changes like the neural activity in both central as peripheral, which, in our model, may be an indicative of increased pressure. The renal sympathetic activity is an important excretion modulator of electrolytes and when amended, promotes greater or lesser retention of salts, mainly sodium, contributing to the increase in blood volume and consequently hypertension. Several neuropeptides are involved in renal sympathetic activity, and these levels are an important marker in the genesis of hypertension. Among these peptides we find substance P (SP) and its receptor NK1R, and Related Peptide Calcitonin Gene (CGRP). Our research showed reduced immunoreactivity of SP, CGRP and increased neurokinin 1 receptor in dorsal root ganglia among the offspring of rats subjected to gestational protein restriction. According to this result, we believe that there are changes in afferent renal activity neuromodulation which may be a contributing factor for maintenance of hypertension in this experimental model

INTRODUÇÃO: O efeito da idade gestacional e do crescimento fetal nos imunocomponentes do leite materno ainda é pouco conhecido. OBJETIVO: Determinar a quantidade de citocinas no colostro em função da idade gestacional e do crescimento fetal. MÉTODO: Estudo transversal, envolvendo mães de recém-nascidos prematuros (PT) e de termo (T), nascidos na Maternidade da Faculdade de Medicina de Botucatu – UNESP, em 2014-2015. Critério de inclusão: gestação única; ausência de diabetes materno, ou uso de medicações/drogas ilícitas, sorologias negativas; recém-nascido com peso adequado (AIG) ou pequeno para idade gestacional (PIG) e sem malformação. Excluídos: não obtenção do colostro, mastite, uso de medicamento pela puérpera. Foram estudados 4 grupos: PT-PIG (n=18), PT-AIG (n=42), T-PIG (n=45), T-AIG (controle, n=42). No colostro coletado entre 24-72 horas pós-nascimento, foram dosadas as citocinas (IL-1β, IL-6, IL-8, IL-10, IL-12 e TNF-α) por citometria de fluxo. Na comparação entre grupos utilizou-se o Qui-quadrado ou teste Exato de Fisher, ANOVA, e Correlação de Pearson. RESULTADOS: As características maternas foram semelhantes nos 4 grupos. A idade gestacional média foi 34 semanas nos prematuros e 39 semanas nos termos. Os níveis de citocinas do colostro não diferiram entre os grupos de termo e pretermo. O grupo T-PIG apresentou maior quantidade de citocinas comparado aos demais. Nos 4 grupos houve correlação entre as citocinas, especialmente nos T-PIG. CONCLUSÃO: A quantidade de citocinas no colostro foi influenciada pelo crescimento fetal e não pela idade gestacional.
INTRODUCTION: The effect of gestational age and foetal growth in the imunno componentes of human milk is not well stablished. OBJECTIVE: To determine cytokines levels in colostrum according to gestational age and foetal growth. METHOD: Cross-sectional study with mothers of term (T) and preterm (PT) infants, with birth weight appropriate-for-gestational age (AGA) and small-for-gestational age (SGA), born at the Maternity of Botucatu School of Medicine- UNESP, during 2014 -2015. Inclusion criteria were: single gestation; absence of diabetes mellitus; no maternal use of medications or illicit drugs; negative maternal serology; and newborn without malformation. Mothers with mastitis, or use of medication, or failure to obtain colostrum, were excluded. Four groups were studied: PT-SGA (n=18), PT-AGA (n=42), T-SGA (n=45), and T-AGA (control, n=42). Colostrum was collected between 24-72 hours after birth, and cytokines (IL-1β, IL-6, IL-8, IL-10, IL-12 and TNF-α) were measured by flow cytometry. Chi-square or Fisher's exact test, ANOVA and Pearson's correlation were used to evaluate differences among the groups. RESULTS: Maternal characteristics did not differ between groups. The mean gestational age was 34 and 39 weeks for preterm and term groups. Cytokines levels in the colostrum were not different between term and preterm groups. However cytokines levels were significantly higher in colostrum from mothers of T-SGA. A positive correlation between cytokines was found in all groups, especially in T-SGA. CONCLUSION: Cytokines levels in colostrum was influenced by foetal growth, while the correlation between cytokines varied according to gestational age and foetal growth.

Na comparação entre grupos utilizou-se o Qui-quadrado ou teste Exato de Fisher, ANOVA, e Correlação de Pearson. RESULTADOS: As características maternas foram semelhantes nos 4 grupos. A idade gestacional média foi 34 semanas nos prematuros e 39 semanas nos termos. Os níveis de citocinas do colostro não diferiram entre os grupos de termo e pretermo. Abstract: INTRODUCTION: The effect of gestational age and foetal growth in the imunno componentes of human milk is not well stablished. OBJECTIVE: To determine cytokines levels in colostrum according to gestational age and foetal growth. Objetivos: testar a validade da relaÃÃo diÃmetro transverso do cerebelo (DTC) /circunferÃncia abdominal (CA) como mÃtodo diagnÃstico ultra-sonogrÃfico da restriÃÃo de crescimento fetal (RCF). Determinar, atravÃs de curva ROC (receiver operator characteristic), o melhor ponto de corte da relaÃÃo DTC/CA. Verificar se a relaÃÃo DTC/CA tem sua acurÃcia modificada na dependÃncia do tipo de RCF (simÃtrica ou assimÃtrica) ou do tempo entre a ultra-sonografia e o parto. Comparar DTC/CA, no ponto de corte obtido, com a relaÃÃo comprimento do fÃmur (CF) /circunferÃncia abdominal (CA). MÃtodo: estudo prospectivo, seccional, envolvendo 250 gestantes com gravidez Ãnica, idade gestacional precisa, feto vivo. Foram realizadas ultra-sonografias obstÃtricas atà a resoluÃÃo da gestaÃÃo, mas somente a Ãltima foi considerada para anÃlise. Os neonatos cujas relaÃÃes DTC/CA estiveram maiores do que o ponto de corte determinado pela curva ROC foram considerados acometidos por RCF. Idem para a relaÃÃo CF/CA. Considerou-se como padrÃo-ouro para o diagnÃstico de RCF os recÃm-nascidos cujos pesos situaram-se abaixo do percentil 10 para a idade gestacional nas curvas de Lubchenco et al. (1963), corrigidas para sexo. Definiu-se RCF simÃtrica neonatos com Ãndice ponderal de Rohrer situado entre 2,2 e 3.0. Aqueles com RCF cujos Ãndices fossem < 2,2 foram classificados como RCF assimÃtrica. Resultados: a prevalÃncia da RCF foi de 12,4%. O ponto de corte da relaÃÃo DTC/CA determinado pela curva ROC foi 16,15. A sensibilidade, especificidade, valores preditivos positivo e negativo, acurÃcia, razÃes de verossimilhanÃa positiva e negativa foram de 77,4%, 82,6%, 38,7%, 96,3%, 82%, 4,5 e 3,7, respectivamente. Na RCF simÃtrica a sensibilidade e especificidade foram de 80,8% e 81,7%, respectivamente. Na assimÃtrica a sensibilidade e especificidade foram 60% e 75%, respectivamente. Resultados menores do que na simÃtrica, porÃm, nÃo estatisticamente significantes (p > 0,05). No intervalo de zero a sete dias entre a Ãltima ultra-sonografia e o parto, a sensibilidade e especificidade foram de 81,5% e 82,1%, respectivamente. No intervalo de oito a 14 dias, a sensibilidade e especificidade foram de 50% e 84,3%, respectivamente, sem diferenÃa estatisticamente significante entre os dois intervalos (p > 0,05). O ponto de corte da relaÃÃo CF/CA foi de 22,65, com sensibilidade, especificidade, valores preditivos positivo e negativo, acurÃcia, razÃes de verossimilhanÃa positiva e negativa de 67,7%, 81,7%, 34,4%, 94,7%, 80%, 3,7 e 2,5, respectivamente. ConclusÃes: a relaÃÃo DTC/CA no ponto de corte 16,15 mostrou-se mÃtodo eficaz no diagnÃstico de RCF, tanto simÃtrica quanto assimÃtrica, nÃo sendo influenciada pelo tempo entre a Ãltima ultra-sonografia e o parto. Sendo mÃtodo independente da idade gestacional, à especialmente Ãtil nos casos em que este dado à ignorado. A relaÃÃo CF/CA mostrou-se menos eficaz do que a DTC/CA no diagnÃstico da RCF.
Objectives: to evaluate the validity of transverse cerebellar diameter (TCD)/abdominal circumference (AC) ratio as an ultrasonographic diagnosis method of fetal growth restriction (FGR). To calculate by receiver operator characteristic (ROC) curve the best cut-off value of TCD/AC ratio. To verify whether TCD/AC has its accuracy modified according to the dependence of type of FGR (symmetric and asymmetric) or according to the time between ultrasonography and deliverance. To compare TCD/AC ratio at its cut-off with the femur length (FL)/ abdominal circumference (AC) ratio. Method: a prospective cross-sectional study, carried out in 250 pregnant women with singleton pregnancies between 20 and 42 weeks of gestation, known accurate gestational age with ultrasound confirmation, living fetuses. Obstetrics sonographic examinations were accomplished until gestation resolution, but only the last one, within 14 days of the deliverance, was used for analysis. Neonates with TCD/AC ratio greater than the cut-off, established by ROC curve were diagnosed as FGR. The same was considered for FL/AC ratio. We classified as gold standard for FGR in new-born infants, who presented birth weight bellow 10th percentile of gestational age according to the growth curves of Lubchenco et al. (1963), corrected according to their sex. Neonates showing FGR and Rohrer ponderal index between 2,2 and 3,0 were labeled as symmetric FGR. Those showing FGR and ponderal index below 2,2 were labeled as asymmetric FGR. Results: prevalence of FGR among the study group was 12,4%. The best cut-off value calculated by ROC curve for TCD/AC ratio was 16,15. The sensitivity, specificity, accuracy, positive predictive values and negative predictive values, likelihood ratio for positive and negative tests were 77,4%, 82,6%, 38,7%, 96,3%, 82%, 4,5 and 3,7, respectively. In the symmetric FGR, sensitivity and specificity were 80,8% and 81,7%, respectively. In the asymmetric FGR, sensitivity and specificity were 60% and 75%, respectively. Results lower than in the symmetric FGR, but not statistically significant (p > 0,05). In the interval zero to seven days between sonographic examination and deliverance, sensitivity and specificity were 81,5% and 82,1%, respectively. In the interval of eight to 14 days, sensitivity and specificity were 50% and 84,3%, respectively, with no statistically significant difference (p > 0,05). The best cut-off value calculated by ROC curve for FL/AC ratio was 22,65, showing sensitivity, specificity, accuracy, positive predictive values and negative predictive values, likelihood ratio for positive and negative tests of 67,7%, 81,7%, 34,4%, 94,7%, 80%, 3,7 and 2,5, respectively. Conclusions: TCD/AC ratio at cut-off 16,15 proved to be an effective method in antenatal diagnosis of FGR, both symmetric as asymmetric, with no influence of interval between ultrasonography examination and deliverance. As a gestational age-independent method, it is useful enough in the occurrence of cases where these data are unknown. FL/AC ratio proved is not so effective as TCD/AC ratio in diagnosis of FGR.

Der volle Inhalt der QuelleLos fetos con restricción del crecimiento intrauterino (RCIU) presentan remodelamiento cardiovascular el cual persiste en la infancia y se ha asociado a enfermedades cardiovasculares en el adulto. La hipertensión en la infancia se ha demostrado como un factor de riesgo cardiovascular para la enfermedad adulta. Un seguimiento estricto junto con intervenciones en la dieta se ha demostrado mejora la salud cardiovascular en estos niños, sin embargo no todas las restricciones del crecimiento tienen hipertensión en la infancia. El objetivo principal de esta tesis es definir los parámetros con mayor utilidad de la ecocardiografía fetal para predecir hipertensión y remodelamiento arterial en infantes de 6 meses de edad con restricción del crecimiento intrauterino. Para esto, se realizó un estudio de cohorte incluyendo fetos con RCIU y controles, seguidos desde vida prenatal hasta los 6 meses de edad. La evaluación prenatal consistió en una ecocardiografía funcional completa. A los 6 meses de edad estos niños fueron evaluados para hipertensión y remodelamiento arterial. Posteriormente se realizó la construcción de un score cardiovascular para determinar desde vida prenatal aquellos niños con mayor riesgo a presentar hipertensión en vida postnatal y que pudieran requerir vigilancia o intervenciones.

Resumo: Já está bem documentado que a dieta hipoproteica em ratos durante a gestação produz efeitos no crescimento fetal, uma persistente elevação na pressão arterial e disfunções no desenvolvimento renal da prole. Alterações na pressão arterial parecem estar relacionadas à acentuada redução no número de nefros que acaba por causar um quadro de hipertrofia e hiperfluxo nos nefros remanescentes como adaptação para equilibrar a taxa de filtração, no entanto, os mecanismos utilizados para esta adaptação culminam com o surgimento de albuminúria. Este processo acaba por causar esclerose glomerular culminando em um ciclo, comprometendo cada vez mais os nefros remanescentes. O desenvolvimento deste quadro pode levar à síndrome nefrótica e posteriormente à doença renal terminal. Acredita-se que o principal fator atuante na programação fetal neste modelo é devido à exposição exacerbada do feto aos glicocorticoides materno, o que acaba por comprometer a correto desenvolvimento e diferenciação de tecidos e órgãos, e na expressão ou atividade de uma série de receptores e enzimas. Recentemente, tem surgido a hipótese de que a atividade da mTORC poderia estar envolvida no surgimento de doenças na idade adulta neste modelo experimental. Este trabalho teve por objetivo avaliar se a inibição da mTORC através do tratamento com rapamicina em animais programados poderia ser benéfico, inibindo o surgimento de complicações relacionadas com a estrutura glomerular e como seria seu efeitos sistêmico, sobre a função renal e pressão arterial sistêmica. Ratos Wistar receberam ração com baixa proteína (6% LP) e dieta controle (17% NP) durante o período gestacional. A prole de machos foi tratada com rapamicina diluída em DMSO (5%) e administrada via intraperitoneal na dose de 1mg/kg, 3 vezes por semana, a partir da 4ª semana de vida até a 12ª semana. A aferição da pressão arterial sistólica foi realizada nas idades 8, 12 e 16. Foi observado que nos grupos que receberam rapamicina, a pressão artéria sistólica elevou-se consideravelmente em todas as idades. A avaliação da função renal foi realizada através de clearance de creatinina e lítio nas mesmas idades e observamos que durante todo o tratamento, o grupo NP que recebeu rapamicina excretou mais sódio, na porção pós-proximal do túbulo. Além disso, não houve diferença na taxa da filtração glomerular em nenhuma das idades. Quando a proteinúria foi avaliada, observamos que o grupo programado LP sem rapamicina, apresentou evolução com o passar das semanas sendo significativamente maior a partir das 12ª e 16ª semanas, no entanto, os grupos que receberam rapamicina, não apresentaram a mesma evolução, indicando preservação da estrutura glomerular. Os presentes resultados demonstram que apesar de a rapamicina ter elevado a pressão arterial em ambos os grupos, há uma indicação de que os animais programados tem um controle menos eficaz no controle da pressão arterial através da função renal. Mesmo diante da pressão elevada, a rapamicina foi capaz de inibir injúrias à barreira de filtração
Abstract: It is well established that a low protein diet in rats during pregnancy causes effects on fetal growth, a persistent elevation in blood pressure and renal dysfunction in the offspring at a later age. Alterations in blood pressure seem to be related to the marked reduction in the number of nephrons which ultimately causes an overflow and hypertrophy in the remaining nephrons in the attempt to adapt its balance on the filtration rate. However, the mechanisms used for this adaptation results in the appearance of albuminuria. This process might cause glomerular sclerosis culminating in a cycle, increasing the damage to the remaining nephrons. The development of this framework can lead to nephrotic syndrome and subsequently to ESRD. It is believed that the main factor in the fetal programming model is the fetus overexposure to maternal glucocorticoids, which ultimately compromises the proper development and differentiation of tissues and organs. Additionally, the expression and the activity of a number of receptors and enzymes is also affected. Recently, in this experimental model, there has arisen the hypothesis that the activity of mTORC could be involved in the onset of disease in adulthood. This study assess whether the inhibition of mTORC, through the treatment with rapamycin in programmed animals, could be beneficial by inhibiting the onset complications related to glomerular structure and its affect on renal function and blood pressure. Wistar female rats were fed with low protein (6 % LP) or control diet (17 % NP) during pregnancy. The male offspring were treated with rapamycin diluted in DMSO (5 %) and administered intraperitoneally at a dose of 1mg/kg, 3 times per week, from the 4th week of life until the 12th week. The measurement of systolic blood pressure was measured at 8, 12, and 16 weeks old. We noticed that in the groups treated with rapamycin the arterial systolic pressure rose considerably in all ages. The assessment of renal function was performed by creatinine clearance and lithium at the same ages as the blood pressures assessment. The NPR group that received rapamycin had an increase in sodium excretion at the post- proximal tubule during the whole treatment. Additionally, there was no difference in the rate of glomerular filtration rate at any age among the groups. When proteinuria was assessed, we found that the programmed Group LP without rapamycin, showed an increase along the weeks and as expected, the groups that have received rapamycin did not show the same trend, indicating preservation in the glomerular structure. The present results demonstrate that rapamycin caused increase in the blood pressure in both groups however, it was able still to inhibit the injury into the filtration barrier. Additionally, there is an indication that the programmed animals, has a less effective control in the blood pressure and the excretion of sodium, even when the treatment is interrupted

Der volle Inhalt der QuelleObjective: To examine the association between isolated single umbilical artery (ISUA) and fetal growth restricion. Methods: Case control study with retrospective review of 131 singleton pregnancies with isolated single umbilical artery diagnosed prenataly between 1998 and 2010. Control group consisted of 730 singleton pregnancies prospectively evaluated with histological confirmation of 3 vessels cord. Mean birthweight and frequency of low birthweight ( < 2,500g), very low birthweigh ( < 1,500g) and fetal growth restriction below the 5th and 10th centiles were compared between groups. Odds ratios and 95% confidence intervals were calculated for significant differences. Logistic regression analysis was used to examine the association between fetal growth restriction and significant independent variables. Results: The mean birthweight difference between ISUA (n=131, 2,840+-701g) and control (n=730, 2,983+-671g) pregnancies was 143g (95%CI= 17-269; p= 0.04). Birthweight below the 5th centile was more common in ISUA (21.4% versus 13.6%, p= 0.02, LR= 1.57, 95%CI: 1.07- 2.25); particularly in the subgroup of pregnancies with associated maternal disease or pregnancy complication (28.6% versus 14.1%, p= 0.02, LR= 2.22, 95%CI: 1.12-4.25). No significant differences were observed in low birthweight, very low birthweight or birthweight below the 10th centile. Logistic regression analysis demonstrated that birthweight below the 5th centile was significantly associated with ISUA only. Conclusion: Isolated single umbilical artery is associated with 1.6 times increased risk of birthweight below the 5th centile. In pregnancies with associated maternal disease or pregnancy complication, this risk is increased 2.2 times

Resumo: O objetivo deste estudo foi avaliar e comparar o desempenho mental e o controle motor em lactentes nascidos a termo, pequenos (PIG) ou adequados (AIG) para idade gestacional no 12º mês de vida. Estudo prospectivo durante o 1º ano de vida. A análise dos resultados foi realizada em um corte seccional no 12º mês de vida. Os recém-nascidos (RN) foram selecionados no Centro de Atenção Integral à Saúde da Mulher da Universidade Estadual de Campinas (Unicamp), entre Maio de 2000 e Julho de 2003. Para cada neonato PIG foram selecionados os dois próximos neonatos AIG. Foram incluídos nesta pesquisa: RN nascidos a termo, idade gestacional entre 37 e 41 semanas; peso de nascimento abaixo do percentil 10 para o grupo PIG ou entre os percentis 10 e 90 para o grupo AIG; residentes na região metropolitana de Campinas. O projeto foi aprovado pelo Comitê de Ética em Pesquisa da Unicamp. Foram excluídos RN com síndromes genéticas, malformações ou infecções congênitas diagnosticadas no período neonatal; peso de nascimento acima do percentil 90 e os que necessitaram de internação em unidade de terapia intensiva neonatal. Foram utilizadas as Escalas Bayley de Desenvolvimento Infantil II (BSID-II) (1993), aplicadas no 12º mês de vida, com ênfase em provas motoras de controle postural. Os dados registrados nos roteiros de avaliação foram transcritos e armazenados nos moldes de arquivo para o banco de dados do programa ¿Statistical Package for Social Sciences for Personal Computer¿ (SPSS/PC), versão 11 e analisados através do ¿Statistical Analysis System¿ (¿SAS System for Windows¿), versão 8.02. A população de 95 lactentes compareceu a pelo menos uma avaliação programada no 1º ano de vida e a amostra seccional avaliada no 12º mês foi composta por 70 lactentes (23 PIG e 47 AIG). No estudo da população, o grupo PIG apresentou peso ao nascimento significativamente menor que o grupo AIG. Os grupos apresentaram diferenças na distribuição de algumas variáveis maternas. A escolaridade menor que 8 anos esteve 3,71 vezes mais associada ao grupo PIG e a ocupação esteve 0,18 vezes mais associada ao grupo PIG. No estudo amostral, o grupo PIG apresentou peso ao nascimento significativamente menor que o grupo AIG. Ocupação materna foi 0,22 vezes mais associada ao grupo PIG. Os valores de ¿index score¿ na escala motora foram significativamente menores no grupo PIG (p= 0,046). No estudo do controle postural da amostra, observou-se diferença significativa entre os grupos, nas provas MO 61: fica em pé sozinho, (p= 0,019) e MO 71: caminha para o lado, (p= 0,020), com maior freqüência de execução no grupo AIG. Concluiu-se que lactentes nascidos PIG apresentaram pontuações significativamente mais baixas na escala motora, sendo que as provas: fica em pé sozinho e caminha para o lado, mostraram diferença significativa, executada por menor proporção de lactentes do grupo PIG
Abstract: The objective of this study was to assess and to compare the mental performance and motor control in infants born at term, small (SGA) or appropriate (AGA) for gestational age in the 12th month of life. It was a prospective study during the 1st year of life. The analysis of the results was done in a sectional cohort in the 12th month of life. The neonates were selected in the Center of Integral Attention to the Woman's Health at Universidade Estadual de Campinas (Unicamp) from May 2000 to July 2003. To each SGA neonate, the next two AIG neonates were selected. In this research were included: neonates at term, gestational age between 37 and 41 weeks, birth weight below the 10 percentile for the SGA group or between the 10 and 90 percentiles for the AGA group; living in the Campinas metropolitan area. Ethical permission was obtained from the Research Ethics Committee at Unicamp. Genetic syndromes, multiple congenital malformations or verified congenital infections; birth weight above the 90 percentile and those who needed neonatal intensive care were excluded. The Bayley Scales of Infant Development-II (BSID-II) (1993) were used in the 12th month of life, with emphasis in items for the motor postural control. The data registered in the record form were transcribed and stored in the data bank of the Statistical Package for Social Sciences for Personal Computer (SPSS/PC), version 11, and were analyzed through the Statistical Analysis System (SAS System for Windows), version 8.02. A population of 95 infants was assessed at least once in the 1st year of life. The sectional sample in the 12th month was composed of 70 infants (23 SGA and 47 AGA). In the population study, the SGA group showed birth weight significantly lower than the AGA group. There was difference in the distribution of some maternal variables. The scholarship below 8 years was 3.71 times more associated to the SGA group and the maternal occupation was 0.18 times more associated to the SGA group. In the sample study, the SGA group showed birth weight significantly lower than the AGA group. Maternal occupation was 0.22 times more associated to the SGA group. The motor index score was significantly lower in the SGA group (p= 0.046). In the study of the postural control of the sample, there was significant difference between the groups in the item MO 61: stands up alone (p= 0.019) and MO 71: walks sideways (p= 0.020), with lower frequency for the SGA

Resumo: Introdução: Alguns estudos demonstram que a predição do peso fetal usando a volumetria dos membros fetais é mais precisa do que quando se usam medidas bidimensionais (2D). Até hoje, somente o método multiplanar foi utilizado para a volumetria dos membros fetais. Desta forma, a utilidade do método rotacional (VOCAL®) para este fim nunca foi testada. Objetivos: Avaliar as variabilidades intra e interobservadores e a concordância entre as medidas do volume da coxa fetal realizadas com os métodos multiplanar e VOCAL®. Comparar as acurácias das fórmulas com medidas do volume da coxa fetal com as acurácias das fórmulas com medidas 2D. Comparar as acurácias das fórmulas deste estudo com as acurácias das fórmulas já publicadas. Métodos: 210 pacientes foram avaliadas, formando um grupo para gerar as fórmulas (n = 150) e um grupo para validá-las (n = 60). Os pacientes utilizados para gerar as fórmulas também foram utilizados para avaliar as variabilidades intra e interobservadores e a concordância entre as medidas realizadas pelos métodos multiplanar e VOCAL®. Foram utilizadas análises de regressão polinomial para criar uma equação com medidas 2D, uma com o volume da coxa fetal medido pelo método multiplanar (CoxaM) e uma com o volume da coxa fetal medido pelo método VOCAL® (CoxaV). Utilizaram-se testes t de Student pareados para comparar as acurácias das equações deste estudo com as acurácias das fórmulas já publicadas. Foram utilizadas análises proporcionais de Bland e Altman para avaliar as variabilidades intra e interobservadores e a concordância entre as medidas realizadas pelos métodos multiplanar e VOCAL®. Resultados: A diferença média percentual entre as medidas pelos métodos multiplanar e VOCAL® foi de -0,04 com limites de concordância de 95% de -8,17 e 8,09. A diferença média percentual e os limites de concordância de 95% entre as medidas na avaliação das variabilidades intra e interobservadores foram -1,10 (-7,67 to 5,47) e 0,61 (-7,68 to 8,91) para o método VOCAL® e 1,03 (-6,35 to 8,41) e -0,68 (-11,42 to 10,06) para o multiplanar. As melhores fórmulas para cálculo do peso fetal estimado (PFE) foram: PFE = -562.824 + 11.962 x CA x CF + 0,009 x DBP² x CA² (CA: circunferência abdominal; CF: comprimento femoral; DBP: diâmetro biparietal); PFE = 1033.286 + 12.733 x CoxaM; PFE = 1025.383 + 12.775 x CoxaV. Tanto no grupo que gerou as fórmulas como no grupo utilizado para validá-las não houve diferença significativa entre as acurácias das fórmulas com medidas 2D ou tridimensionais (3D). Quando aplicadas nas pacientes deste estudo, as acurácias das fórmulas 2D e 3D já publicadas foram significativamente piores dos que as das novas fórmulas. Conclusões: Os métodos VOCAL® e multiplanar são intercambiáveis para a volumetria da coxa fetal. Possivelmente as maiores fontes de discrepâncias na estimativa do peso fetal são as diferenças fenotípicas entre as pacientes utilizadas para criar as fórmulas. Os dados deste estudo reforçam a necessidade de fórmulas específicas para cada população, independentemente do uso de medidas 2D ou 3D.
Abstract: Introduction: Some authors have demonstrated that the prediction of birth weight using fetal limb volumetry is more precise than with two-dimensional ultrasound (2DUS). To date, only the multiplanar method has been used for fetal limb volumetry, so the usefulness of the rotational technique (VOCALTM - Virtual Organ Computer- aided AnaLysis) for this purpose has never been tested. Objectives: To evaluate the repeatability, reproducibility and agreement of measurements performed with multiplanar and VOCALTM techniques for total fetal thigh volumetry. To compare the accuracies of birth-weight-predicting models with total fetal thigh volumetry with models derived from 2DUS parameters. To compare the performances of our new formulas with those of previously published equations. Methods: 210 patients were prospectively evaluated to compose a formula-generating group (n = 150) and a prospective-validation group (n = 60). The patients of the formula-generating group were also used to evaluate the repeatability, reproducibility and the agreement of the measurements of multiplanar and VOCALTM techiniques for fetal thigh volumetry. Polynomial regression analysis was performed in the formula-generating group to generate one equation with 2DUS measurements, one with fetal thigh volume measured by the multiplanar technique (ThiM) and one with fetal thigh volume obtained by the VOCALTM method (ThiV). Paired samples t-tests were used to compare the accuracies of our equations with those of previously published 2D and three-dimensional (3D) equations. Proportionate Bland and Altman analyses were performed to determine the agreement between the two methods and to evaluate intra- and inter-observer variability. Results: The mean percentage difference between measurements performed with the VOCALTM and multiplanar techniques was -0.04 and the 95% limits of agreement were -8.17 and 8.09. The mean percentage difference and 95% limits of agreement between paired measurements in the assessment of intra- and inter-observer variability were -1.10 (-7.67 to 5.47) and 0.61 (-7.68 to 8.91) for the VOCALTM technique and 1.03 (-6.35 to 8.41) and -0.68 (-11.42 to 10.06) for the multiplanar method. The formulas with the best fit for the prediction of birth weight (EFW) were: EFW = -562.824 + 11.962 x AC x FL + 0.009 x BPD² x AC² (AC: abdominal circumference; FL: femur length; BPD: biparietal diameter); EFW = 1033.286 + 12.733 x ThiM; EFW = 1025.383 + 12.775 x ThiV. For both the formula-generating and the rospective-validation groups, there was no significant difference between the accuracies of the new 2DUS and 3DUS models. When applied to our population, the accuracies of previously published 2DUS and 3DUS formulas were significantly worse than our models. Conclusions: The VOCALTM and multiplanar techniques can be used interchangeably for total fetal thigh volumetry. We believe that the greatest sources of discrepancies in estimation of birth weight are the phenotypic differences among patients used to create each of the formulas mentioned in this study. Our data reinforce the need for customized birth weight prediction formulas, regardless of whether 2DUS or 3DUS measurements are employed.

This institution adopt the old Standard of feeding, growth and development, by the breast feeding incentive, exclusive until 4-6 mouths of age and follows the norms approved by the Nutrition Department of the Brazilian s Pediatric Society that refers to the mineral supplement and supplement of D vitamin. The sample was constituted by 28 children aged between 6 and 60 months, born at the right moment, with more than 37 weeks, being classified according with the weigh of birth as Normal, weighting over 3000g, and small to their gestational age (SGA), weighting less than 2500g. About the oral health, the following factors were valued: visit to a dentist surgeon, oral hygiene orientation, teeth cleanliness, usage of dental floss, visible bacterial plaque at the vestibular surface of the superior incisors, usage of feeding bottle, suck habits (dummy or finger), prevalence of caries and development defects of the enamel (DDE). The nutrition condition was valued (NCHS) at the dental exam. There was no significant difference at the breast feeding time that was about 9 mouths for both groups(Mann-Whitney test, Z+0,02 / critics Z =1,96). The normal children and the ones SGA did not present significant differences when took into account the possible alteration of the valued factors, except of the usage of dental floss, that was significant at children that was small for the gestational age (Exact Fisher test p=0,0126). By the Spearman s correlations coefficient was observed that to both groups, the three more important factors to the oral health were: teeth cleanliness, no visible plaque and oral hygiene orientation. The conclusion was that in spite of the nutritional condition of birth, the children that were small for the gestational age, did not show differences in the oral health, what can be explained by the educational actions done to prevention and promotion of global health that are learned at the Motherly-Infant and child care outpatient department of UNISA, with orientation of Pediatric dentistry of Dental College discipline. Resumo: O objetivo deste estudo foi comparar o comportamento de lactentes nascidos a termo, pequenos para a idade gestacional (PIG) e lactentes nascidos com peso adequado para a idade gestacional (AIG), no primeiro ano de vida. Foram selecionados 125 neonatos na maternidade do Centro de Atenção Integral à Saúde da Mulher (CAISM/UNICAMP), obedecendo aos critérios de inclusão: pais ou responsáveis legais assinaram o Termo de Consentimento Livre e Esclarecido; neonatos que não necessitaram de cuidados especiais; com idade gestacional entre 37 e 41 semanas; com avaliação no 1º, 2º,3º,6º,9º,12º meses. A casuística foi composta por 95 lactentes que compareceram para pelo menos uma avaliação programada no 1º ano de vida, foi dividida em dois grupos de acordo com a adequação peso/idade gestacional; grupo PIG, constituído por 33 lactentes com peso ao nascer abaixo do percentil 10 e grupo AIG por 62 lactentes com peso entre o percentil 10 e 90 da curva de crescimento fetal de Battaglia e Lubchenco (1967). Para a avaliação do neurodesenvolvimento foram utilizadas as Escalas Bayley de Desenvolvimento Infantil II (BISID-II). Para a avaliação do comportamento do lactente, elegeu-se as Escalas de Classificação do Comportamento (ECC), das BSID-II. O estudo seccional avaliou no 1º mês: 63 lactentes (18PIG/45AIG). No 2º mês: 68 lactentes (25PIG/43AIG). No 3º mês: 68 lactentes (22PIG/46AIG). No 6º mês: 67 lactentes (25PIG/42AIG). No 9º mês: 61 lactentes (22PIG/39AIG) e no 12º mês: 68 lactentes (21PIG/47AIG). Os grupos foram semelhantes quanto às variáveis neonatais, exceto peso ao nascer. O peso foi significativamente menor no grupo PIG (p<0,001). Os grupos não apresentaram diferenças estatisticamente significativa na performance das Escalas Mental e Motora. Considerando o Index Score (IS) da Escala Mental, os resultados não demonstraram diferença estatisticamente significativa entre os grupos. Os resultados de IS da Escala Motora demonstraram diferença estatisticamente significativa entre os grupos, no 2º mês (p=0.008) e 12º mês (p=0.047). O grupo PIG apresentou valores medianos inferiores nestes meses. Considerando os resultados da performance comportamental na ECC, observou-se valores de significância estatística no 2º mês (pvalor=0.048), com maior freqüência relativa de lactentes PIG classificados como inadequados. Entre os fatores considerados na ECC nos primeiros meses de vida, apresentou valor de significância estatística o Fator Atenção/Vigília no 2º mês (p=0.005). Considerando a comparação dos resultados do percentil da ECC, os grupos demonstraram diferença estatisticamente significativa no 2º mês (p=0.001) e 3º mês (p=0.003). Os valores medianos foram inferiores no grupo PIG. No Fator Atenção/Vigília, os grupos de lactentes apresentaram diferença estatisticamente significativa no 2º mês (p=0.001) e 3º mês (p=0.003), sendo que os valores medianos foram inferiores no grupo PIG. No Fator Qualidade Motora os grupos apresentaram diferença estatisticamente significativa no 2º mês (p=0.045), sendo que os valores medianos foram inferiores no grupo PIG
Abstract: The objective of this study was to compare the behavior of full-term small-for-gestational age (SGA) with full-term appropriate-for gestational age (AGA) infants in the first year of life. A hundred twenty five full-term neonates were selected at Neonatology Service in the Center of Integral Attention to Woman¿s Health (CAISM) of the State University of Campinas (UNICAMP), São Paulo, Brazil observing the inclusion criteria as follow: parents or legal guardians who signed the Informed Consent; neonates who did not need special care; gestational age between 37 and 41 weeks. They were assessed in the 1st , 2nd ,3rd ,6th ,9th and 12th months of life. Ninety five infants who came at least to one assessment during the first year of life were the sample. This sample was divided into two groups, according to weight and gestational age. In the SGA group there were 33 infants with birth weight less than percentile 10th and, in the AGA group, there were 62 infants whose birth weight varies between the 10th and 90th percentile on fetal growing Battaglia and Lubchenco method (1967). The Bayley Scales of Infant Development-II (BSID-II) (BAYLEY, 1993) were used with emphasis on the Behavior Rating Scale (BRS). The cross-sectional study evaluated in the 1st month, 63 infants (18 SGA and 45 AGA); in the 2nd month, 68 infants (25 SGA and 43 AGA); in the 3rd month, 68 infants (22 SGA and 46 AGA); in the 6th month, 67 infants (25 SGA and 42 AGA); in the 9th month, 61 infants (22 SGA and 39 AGA); in the 12th month, 68 infants (21 SGA and 47 AGA). The groups showed similar distribution in biologic variables on birth, except birth weight. The SGA group showed lower birth weight than AGA, with significant difference between them (p<0.001). No differences were observed in Mental and Motor Scales performance. No differences were observed in the IS of the Mental Scale. The Motor Scale median comparison showed lower IS in the SGA with significant difference in the 2th month (p=0.008) and in the 12th month (p=0.047). Considering in the BRS, it was observed that the inadequate performance was associated in the 2nd month (p=0.048) to a bigger number of SGA infants, classified as inadequate. As the results of performance of BRS factors are concerned, the Attention/Arousal Factor displayed significantly lower average values (p=0.005) in SGA group. The percentile results in BRS showed significant difference in the 2nd (p=0.001) e 3rd (p=0.003) months, with lower medium values in the SGA group. Resumo: A desnutrição intra-uterina tem sido associada a morbidade neurológica em longo prazo. Tendo em vista que os lactentes nascidos pequenos para a idade gestacional (PIG) representam um modelo de estudo para essa situação e que a maioria dos trabalhos focaliza a idade escolar, o presente estudo teve como objetivo comparar o desempenho motor de lactentes nascidos a termo PIG com lactentes nascidos a termo adequados para a idade gestacional (AIG) no 1°, 2°, 3° e 6° meses. Tratou-se de um estudo prospectivo e seccional. Os neonatos foram selecionados na maternidade do Centro de Atenção Integral a Saúde da Mulher da Universidade Estadual de Campinas, no período de maio de 2000 a julho de 2003, obedecendo aos seguintes critérios de inclusão: recém-nascidos (RN) residentes na região de Campinas, que permaneceram no alojamento conjunto, resultantes de gestação de feto único, com idade gestacional entre 37 e 41 semanas, com peso ao nascimento classificado entre o percentil 10 e 90 da curva de crescimento fetal para o grupo AIG e, abaixo do percentil 10 para o grupo PIG. Foram excluídos: RN com síndromes genéticas, malformações e infecções congênitas. Para avaliação foi utilizada a Escala Motora das Bayley Scales of Infant Development-II. A partir da pontuação do Index Score (IS), com media 100 e desvio padrão de 15, os lactentes foram classificados com performance acelerada (IS=115), performance dentro dos limites normais (IS=85-114), performance levemente atrasada (IS=70-84) ou performance significantemente atrasada (IS=69). Para analise dos dados foi considerado o valor do IS obtido no 1°, 2°, 3° e 6° meses. Quando houve diferença significativa de IS entre os grupos PIG e AIG, as provas daquela idade e as características familiares que poderiam contribuir para as diferenças foram investigadas. A amostra compreendeu 63 lactentes (18 PIG; 45 AIG) no 1° mês, 68 lactentes (25 PIG; 43 AIG) no 2° mês, 68 lactentes (22 PIG; 46 AIG) no 3° mês e 66 lactentes (24 PIG; 42 AIG) no 6° mês. O grupo PIG apresentou media de IS significativamente menor que o grupo AIG no 2° e 6° meses. Nesses períodos, houve menor proporção de lactentes do grupo PIG que realizaram com sucesso as seguintes provas: "faz movimentos alternantes para arrastar em prono", "troca de decúbito lateral para dorsal", "equilibra a cabeça", "senta sozinho momentaneamente por 2 segundos" e "senta sozinho por 30 segundos". Considerando as características familiares, os grupos diferiram quanto a ocupação materna, escolaridade materna e renda per capita, de modo que no grupo PIG houve maior freqüência de mães que não trabalhavam fora do lar, que apresentavam menos de 8 anos de estudo e com baixa renda familiar. Os resultados obtidos sugerem que os lactentes nascidos a termo PIG estão sob maior risco para apresentar alterações no desenvolvimento motor
Abstract: Intrauterine malnutrition has been associated with long-term neurological morbidity. Considering that infants born small for gestational age represent a study model for this condition and that most studies focus on school age children, the present study aimed to compare the motor performance of infants born small for gestational age (SGA) with those appropriate for gestational age (AGA) at 1, 2, 3, and 6 months. This was a cross-sectional and prospective study. The neonates were selected at the Neonatology Service of the Center for Integral Attention to Women's Health-University of Campinas, between May 2000 and July 2003, according to the following criteria: healthy newborns resident in the region of Campinas, resulting of single fetus pregnancies, with gestational age between 37 and 41 weeks, with birthweight between the 10th and 90th percentiles of fetal growth curves for the AGA group and under the 10th percentile for the SGA group. Newborns with genetic syndromes, congenital malformations and infections were excluded. The Motor Scale of Bayley Scales of Infant Development-II was used for evaluation. Using the index score (IS), with a mean of 100 and standard deviation of 15, the infants were classified as presenting accelerated performance (IS=115), within normal performance limits (IS=85- 114), mildly delayed performance (IS=70-84) or significantly delayed performance (IS=69). The IS during the 1st, 2nd, 3rd and 6th months of life were considered in the analysis of the results obtained. When a significant difference in IS occurred between the SGA and AGA groups, the items at that age and the family characteristics that could contribute to these differences were investigated. The sample comprised 63 infants (18 SGA; 45 AGA) aged 1 month, 68 infants (25 SGA; 43 AGA) aged 2 months, 68 infants (22 SGA; 46 AGA) aged 3 months and 66 infants (24 SGA; 42 AGA) aged 6 months. The SGA group presented a mean motor IS lower than the AGA group at 2 and 6 months. For these periods, the SGA group presented a lower proportion of infants who successfully performed the following skills: "makes crawling movements", "turns from side to back", "balances head", "sits alone momentarily" and "sits alone for 30 seconds". Considering the family characteristics, the groups differed with respect to maternal occupation, maternal education and family income; therefore, the SGA group showed a large number of mothers who did not work outside the home, had less than 8 years of study and low family incomes. The results obtained suggest that the infants who are SGA present a greater risk for adverse motor outcomes

Der volle Inhalt der QuelleThe aim of this study was to evaluate the ability of prenatal ultrasound scans to predict fetal growth discordance in twin pregnancies and perinatal morbidity/mortality associated with these cases. This was a retrospective study (1998-2004) involving twin pregnancies that were scanned and had their delivery at our Institution (HCFMUSP). Cases with fetal malformations (n=43) or twin to twin transfusion syndrome (n=24) were excluded. The study of ultrasound scans consisted of 221 twin pregnancies. The final morbidity/mortality study group consisted of 151 twin pregnancies. Birth weight was evaluated based on twin growth charts published by Alexander et al (1998) and weight discordance as a difference >= 20%. Small for gestacional age (SGA) was defined as birth weight below the 10th centile. The study of ultrasonographic prediction of interwin discordance was made using four different intervals between ultrasound examination and delivery (0 to 7 days, n = 96; 8 to 14 days, n = 66; 15 to 21 days, n = 58; 22 to 28 days, n = 59 pregnancies), with a total of 279 ultrasound examinations. In group 0 to 7 days, the sensitivity was 93,6%, specificity was 79,4%, positive predicted values was 89,2%, negative predicted values was 87,1% and accuracy was 88,6%. In the groups 8 to 14 days, 15 to 21 days and 22 to 28 days the sensitivity and accuracy were 95,8% and 84,9%, 95,6% and 84,5%, 90,9% and 84,8%, respectively. Birthweight discordance was observed in 40 sets of twins (26.5%) and 12 cases were monochorionic MC (30%). Twenty five cases (22.5%) in the non discordant group were MC. In the non discordant group, monochorionic pregnancies showed lower gestational age at delivery (34.3 versus 36.2 wks, p=0.004), lower mean birth weight (2067g versus 2334g, p=0.0016) and longer length of stay in hospital (10.6 versus 7.3 days, p=0.0023) compared to dichorionic twins. In the group with twin birthweight discordance, there were no significant differences between MC and DC pregnancies and 75% of the cases had at least one newborn with SGA. These cases were showed lower gestational age at delivery (35.2 versus 36.8wks, p=0.009) and longer length of stay in hospital (17.5 versus 8.2 days, p=0.026). In the discordant group, the smaller twin had a higher frequency of first minute Apgar score < 7 (27.5% versus 7.5%, p=0.01). Perinatal mortality rate was similar in both groups (discordant 4.5% and concordant 3.7%, p=1.0). There were no significant differences in morbidity and mortality between concordant and discordant twins when birth weight was between the 10 th and 90 th centile. In conclusion, there was a good correlation between fetal growth discordance predicted by prenatal scan and actual birth weight discordance. Neonatal morbidity was related to SGA. Excluding fetal malformation and TTTS cases, birth weight discordance in twin pregnancies is not a significantly associated with neonatal mortality.

Der volle Inhalt der QuelleINTRODUCTION: Fetal growth restriction (FGR) is a major complication of pregnancy and it is associated with high rates of perinatal morbidity and mortality. The frequency of neonatal adverse outcome is directly related to the severity of FGR and the cases of poor prognosis are related to weight below the 3rd percentile. Abnormal placental development and decreased uteroplacental perfusion is one of the main causes of FGR. This study aimed to evaluate placental volume and vascular indices by three-dimensional ultrasonography (3DUS) in pregnancies with severe FGR, and their correlation with normal values and maternal-fetal doppler. METHODS: We studied 27 pregnant women whose fetuses had estimated weight below the 3rd percentile for gestational age. Placental volume (PV) and placental indices- vascularity index (VI), flow index (FI) and vascularization and flow index (VFI)- were measured by the 3DUS using the VOCAL technique. These were compared to volume and vascularization normogram described by De Paula e cols. (2008, 2009). Since placental volumes vary throughout pregnancy, the observed values were compared to the expected values at the gestational age and to fetal weight. The observed-to-expected placental volume ratio for gestacional age (PVR-GA) and the observed-to-expected placental volume ratio for fetal weight (PVR-FW) were calculated. Placental parameters were correlated with uterine arteries (UtA) mean pulsatility index (PI) of umbilical artery(UA) PI and they were also evaluated according to the presence of bilateral protodiastolic notch in UtA. RESULTS: Compared to normal curve, the placentas of severe FGR pregnancies showed PV, VI, FI and VFI significantly lower (p < 0,0001 for all parameters). There was a statistically significant inverse correlation of UtA mean PI to PVR-GA (r= - 0,461, p= 0,018), VI(r= -0,401, p= 0,042) and VFI(r= -0,421, p= 0,048. In the group of women who had bilateral protodiastolic notch in UtA, PVR-GA (p= 0,014), PVR-FW (p= 0,02) and VI (p= 0,044) were significantly lower. None of placental parameters correlated significantly with UA PI. CONCLUSIONS: Volume and placental vascularization indices are decreased in fetuses with severe FGR. UtA mean PI has a negative correlation with PVR-GA, VI and VFI, and PVR-GA, PVR-FW and VI are decreased in cases with UtA bilateral notch. There was no correlation of placental parameters to UA PI

Der volle Inhalt der QuelleINTRODUCTION: Gastroschisis is a congenital abdominal wall defect of the fetus and one of its main complications is related to fetal growth restriction. OBJECTIVES: Primary: To evaluate the growth pattern of fetuses with gastroschisis according to each biometric parameter; Secondary: to evaluate growth deficit in three gestational periods and to predict low birth weight from measures of biometric parameters below the 10th percentile. METHODS: This is a retrospective cohort study. We selected 70 cases for evaluation of the growth pattern. The measurements of each biometric parameter: head circumference, abdominal circumference, femur length, head circumference/abdominal circumference ratio and estimated fetal weight were plotted in a growth chart for comparison with the curve of normality. The percentage difference between the mean values of the fetuses with gastroschisis in relation to normal fetuses was then determined. For the evaluation of growth deficit 59 cases with at least one exam in each gestational period (I: 20 to 25 weeks and 6 days; II: 26 to 31 weeks and 6 days; III: 32 weeks until delivery) were included. The deficit of each biometric parameter was obtained from the comparison between these gestational periods. For the prediction of low birth weight, the measures below the 10th percentile of each biometric parameter in periods I and II were tested. RESULTS: In the evaluation of the growth pattern a significant difference between the fetuses with gastroschisis and normal fetuses from 20 weeks of gestation (p < 0.005) is observed. In the evaluation of growth deficit only estimated fetal weight showed a significant difference (p= 0.030). The percentage of fetuses with estimated fetal weight values below 10 percentile in period 2 was 40% higher than that in period I, and 93% higher in period III than in I. In the prediction of low birth weight, only head circunference (odds ratio= 6.07; sensitivity= 70.8 %; specificity = 71.4 %) and abdominal circunference (odds ratio= 0.558; sensitivity = 41.7 %; specificity = 80 %) in period II were predictive. CONCLUSION: Fetuses with gastroschisis show biometric parameters measures significantly smaller than the measures of normal fetuses with 20 weeks of gestation and/or more. In the evaluation of growth deficit, there is a higher incidence of fetal growth restriction in periods II and III. It is possible to predict newborns with low birth weight from measures of head circunfernce and abdominal circunference below the 10th percentile in period II

Resumo: A restrição de crescimento intra-uterina (RCIU) tem sido associada à maior mortalidade perinatal e maior morbidade no neonatal e lactente. Todavia, persiste considerável controvérsia em relação ao desenvolvimento neuromotor, no que se refere ao desempenho inferior dos nascidos a termo pequenos para a idade gestacional (PIG). O presente estudo teve por objetivo avaliar e comparar o desenvolvimento das habilidades motoras finas de lactentes a termo PIG com a termo adequados para a idade gestacional (AIG) no 6º, 9º e 12º meses. Tratou-se de um estudo seccional e longitudinal. Foram selecionados 125 recémnascidos (RN) no Centro de Atenção Integral à Saúde da Mulher (CAISM) da Universidade Estadual de Campinas (UNICAMP). A casuística foi composta por 95 lactentes que compareceram em pelo menos uma avaliação entre o 6º, 9º e 12º meses. Foram estudadas duas coortes de lactentes de acordo com a adequação peso/idade gestacional: grupo PIG, constituído por 33 lactentes com peso ao nascimento abaixo do percentil 10 e grupo AIG por 62 lactentes com peso entre o percentil 10 e 90 da curva de crescimento fetal de Battaglia e Lubchenco (1967). Foram incluídos: RN cujos pais ou responsável legal assinaram o Termo de Consentimento Livre e Esclarecido, com idade gestacional entre 37 e 41 semanas, residentes na região metropolitana de Campinas, que permaneceram no alojamento conjunto, resultantes de gestação de feto único. Foram excluídos: RN com síndromes genéticas, malformações e infecções congênitas. Como instrumento de avaliação foram utilizadas as Bayley Scales of Infant Development-II (1993). Na análise dos resultados para o perfil sócio-demográfico da família, foi evidenciado que no grupo PIG houve maior número de mães sem ocupação fora do lar e que tinham o hábito de fumar. No estudo seccional, o index score (IS) motor demonstrou diferença entre os grupos PIG e AIG no 6º e 12º meses de vida. Houve diferença significa entre os grupos na escala cognitiva para duas provas no 6º mês e uma prova no 9º mês. No estudo longitudinal, para o IS cognitivo houve diferença significativa apenas entre os meses avaliados, sendo que o 6º mês foi diferente do 9º e do 12º mês. Para o IS motor, observou-se diferença significativa entre os grupos PIG e AIG e entre os meses, sendo o 6º mês diferente dos demais. Para as provas avaliadas, não houve diferença entre os grupos, porém, entre os meses encontrou-se diferença significativa em 12 provas cognitivas e uma prova motora. Concluí-se que os lactentes nascidos a termo PIG estão sob maior risco para o desenvolvimento neuromotor atípico, em especial para as habilidades motoras finas, no 6º, 9º e 12º meses de vida
Abstract: Intrauterine growth restriction (IUGR) has been associated with higher perinatal mortality and major morbidity in newborns and infants. However, there is still considerable controversy regarding the neuromotor development with reference to the inferior performance of term infants born small for gestational age (SGA). The present study aimed to evaluate and compare the fine motor skill development in term infants born SGA and term infants born appropriate for gestational age (AGA) on the 6th, 9th and 12th months of life. This was a cross-sectional and prospective study. One hundred and twenty-five fullterm newborns were selected at the Neonatology Service in the Center of Integral Attention to Women's Health (CAISM) of the University of Campinas (UNICAMP); however, only 95 infants comprised the sample, who attended at least one assessment among 6, 9 and 12 months. Two cohorts of infants were studied according to the adequacy of gestational age/weight: the SGA group comprised 33 infants with birthweight under the 10th percentile and the AGA group comprised 62 infants with birthweight between the 10th and 90th percentiles of fetal growth curves (Battaglia & Lubchenco, 1967). Inclusion criteria were: newborns whose parents or legal guardian signed a consent form, with gestational ages between 37 and 41 weeks, resident in the Metropolitan Region of Campinas, who stayed in nursery, resulting of single fetus pregnancies. Exclusion criteria were: infants with genetic syndromes, malformations and congenital infections. Bayley Scales of Infant Development- II (1993) were used as evaluation tools. In the analysis of results for the family's sociodemographic profile, it was evidenced a great number of mothers in the SGA group with no occupation outside the home and who had the smoking habit. In the cross-sectional study, the motor index score (IS) showed differences between SGA and AGA groups at 6 and 12 months. There was a significant difference between groups in cognitive scale for two tests during the sixth month and one test during the 9th month. In the longitudinal study, there was a significant difference among the months for the cognitive IS, i.e. the 6th month was different from the 9th and the 12th months. Significant differences were found between SGA and AGA groups and among the months for the motor IS, i.e. the 6th month was different from others. There were no differences between groups for the tests; however, a significant difference was found among the months in 12 cognitive tests and one motor test. We concluded that term infants born SGA are at higher risk for atypical neuromotor development, particularly for fine motor skills at 6th, 9th and 12th months of life

Der volle Inhalt der QuelleIntrodução: A medida da espessura placentária pode se constituir em um marcador clínico importante para a predição de recém-nascidos afetados pela restrição de fluxo sanguíneo uteroplacentário. Com estabelecimento de técnica simples e barata, após o nascimento, e de posse de resultados ultrassonográficos obtidos previamente durante a gestação, acrescenta-se mais um método de baixo custo e eficácia preditiva acurada na propedêutica perinatal, adicionando maior segurança no manejo destas gestantes de alto risco portadoras de doença hipertensiva, diabetes melito e crescimento intrauterino restrito (CIUR). Objetivos: Estudar a espessura placentária em gestantes de baixo risco e portadoras de Síndromes Hipertensivas da Gestações, CIUR e Diabetes melito; buscar possíveis associações entre a espessura placentária diagnosticada ao exame ultrassonográfico pré-natal e imediatamente após o nascimento; buscar possível associação entre a espessura placentária e desfechos perinatais. Metodologia: Estudo transversal, prospectivo e observacional de um grupo de gestantes portadoras de doença hipertensiva, diabetes melito e crescimento intrauterino restrito que foi comparado a um grupo controle. Tal estudo foi desenvolvido entre os meses de outubro de 2013 e fevereiro de 2015. O primeiro grupo foi constituído de gestantes portadoras de diabetes mellitus gestacional, o segundo grupo, foi constituído de gestantes portadoras de síndromes hipertensivas da gestação, o terceiro grupo foi constituído por gestantes portadoras de CIUR e, o quarto grupo, por gestantes de baixo risco. Durante a internação hospitalar da parturiente foram realizadas seis medidas de espessura placentária pelo exame de ultrassonografia. As placentas foram examinadas macroscopicamente logo após o nascimento sendo avaliada a espessura placentária, realizando-se cinco cortes no sentido transversal da placenta. Com um total de seis fatias, a medida da espessura foi realizada com um especímetro digital no ponto central de cada fatia. Resultados: amostra total deste estudo foi constituída por 82 pacientes, sendo 29 pacientes hígidas, com gestação de baixo risco, 20 portadoras de síndromes hipertensivas da gestação, 17 portadoras de diabetes melito e 16 portadoras de crescimento intrauterino restrito, sendo que dessas, 8 apresentavam, também, PE associada. Quando se buscou a correlação entre as medidas das espessuras placentárias de cada fatia, avaliadas pela ecografia e macroscopia, houve correlação fraca porém significante entre as primeiras (r = 0,26; p = 0,02) e sextas fatias (r = 0,28; p<0,01) e correlação moderada e significante entre as terceiras (r = 0,33; p = 0,02), quartas (r = 0,41; p<0,0001) e quintas (r = 0,38; p<0,0001) fatias. Quando buscou-se a correlação entre a espessura média na macroscopia com a espessura média na ecografia, por grupos de estudo, observou-se correlação moderada e significante no grupo de crescimento restrito (r = 0,60; p<0,05). Conclusões: as medidas da espessura placentária avaliadas pela ultrassonografia anteparto e ao exame macroscópico da placenta após o nascimento, possuem uma correlação positiva e significante, independente de as gestantes serem ou não portadoras de patologias prévias ou durante a gravidez; não houve correlação entre a espessura média da placenta na macroscopia pós-parto com a espessura média à ecografia nos grupos de BXR, SHG e DM, porém houve correlação moderada no grupo de CIUR; a divisão da placenta em fatias tanto no exame de ultrassonografia como no exame macroscópico após o parto mostrou uma correlação positiva e significante entre as primeiras, terceiras, quartas, quintas e sextas fatias, entre os métodos de medida; não houve correlação entre as medidas da espessura placentária antenatal e pós-natal com as diversas variáveis perinatais. Pelos achados do presente estudo, recomenda-se que de forma sistemática a medida da espessura placentária pela ultrassonografia seja realizada no centro do disco placentário, ou seja, na terceira ou quarta fatia.

Der volle Inhalt der QuelleAnalysis of chromosomic copy number variants (CNVs) have demonstrated the important role of these genomic imbalances in population diversity and human disease. The model of CNV disease association involves deletions and/or duplications that are individually rare but encompass chromosomal segments of relevant size. Prenatal onset short stature patients constitute a complex group characterized by clinical heterogeneity. The causes of prenatal growth impairment frequently involve genetic changes that disturb the mechanisms and the pathways of fetal growth and development. Thus, we hipothesized that rare CNVs might contribute to the genetic etiology of prenatal onset short stature. In order to evaluate this assumption, our study analyzed the presence of submicroscopic deletions and/or duplications in a selected group of patients born small for gestational age with persistent short stature but without a recognized cause. A total of 51 patients with prenatal and postnatal growth retardation associated with dysmorphic features, developmental delay and/or intellectual disability, but without criteria for known syndromes, were selected. All patients had normal G-banded karyotyping. Array-based comparative genomic hybridization (aCGH) in a whole-genome 60K platform was performed using DNA obtained from all patients. Detected CNVs were compared with CNV data from healthy controls individuals, excluding common copy number polymorphisms. In 17 of the 51 patients screened (33%), 18 rare CNVs were identified. The pathogenicity of CNVs was assessed by considering the following criteria: inheritance and familial segregation; overlap with genomic coordinates for a known genomic imbalance syndrome; overlap with CNVs previously identified in other patients with prenatal onset short stature; and gene content. Four distinct CNVs, found in three patients, were classified as pathogenic: 1) del 22q11.21; 2) dup 10q26.2-26.3 and del 10q26.3; and 3) del 4q28.2-q31.21. Five CNVs, found in five patients, were classified as probably pathogenic: 4) del 3q27.1-q27.3; 5) del 20p13; 6) dup 14q11.2-q12; 7) dup 16q24.1-q24; and 8) dup Xq31.1-q13.2. Taken both groups together, we found pathogenic or probably pathogenic CNVs in 16% of patients. According to familial segregation, four variants were considered as variants of uncertain clinical significance, while five variants were considered as benign. In an attempt to establish a causal genotype-phenotype correlation and to identify genes involved in growth impairment of prenatal onset, the gene content of the variants was analyzed using bioinformatics tools for gene prioritization. Based on our results, it is possible to make the following conclusions: 1) the frequency of pathogenic or probably pathogenic CNVs was at least 16%, indicating that rare CNVs are probably among the genetic causes of prenatal onset short stature; 2) aCGH clarified the diagnosis and the genetic etiology involved in the phenotype of 8 selected patients; and 3) we found CNVs in distinct chromosomal regions with several candidate genes that may be involved in the mechanisms of growth regulation and/or in the regulatory pathways of intrauterine development

Der volle Inhalt der QuelleIn order to compare the behavior of visual and auditive orientation in the imediate neonatal period between fullterm, small for gestational age (SGA) to those of adequate weight to gestational age (AGA), we studied 174 healthy neonates, divided in two groups. Subjects and Methods: we conducted a controlled, cross-sectional study, where the exposure factor was the intrauterine growth retardation (IUGR) and the endpoints were the babie’s visual and auditive orientation behaviors, as well as the cost to it (supplementary itens). The complete NBAS was performed but at this moment we enfasize the orientation behavior and supplementary itens to study. Group 1 was made by 56 SGA babies and group 2 by 118 AGA neonates. As instrument, we used the Brazelton's Neonatal Behavior Assessment Scale (NBAS) and the exam ocurred between 48 and 72 hours after birth. With the goal of minimize the effects of drugs used for obstetric analgesia in the neonatal behavior, only babies whose mothers received no more than epidural blockage with marcaíne were included in the study. We excluded children with malformations, those who required intensive care, those whose mothers has no prenatal care, the simetric SGA babies, as well as those whose mother received medication other than marcaíne or used drugs, except tobaco. We looked at the baby's orientation behavior as well as the cost to it. Other studied variables were: maternal age, parity, type of delivery, Apgar score at 1 and 5 minutes, gestational age, and baby's colour, gender, weight, lenght, cephalic perimeter and prevalence of breastfeeding. Statistical analysis was carried out after treating data by the mean and standard-deviation and comparisons between groups were made using the t test. To evaluate the magnitude of the differences, we calculated the Cohen's Effect Sizes. Results: we weren't able to find any significant difference between groups regarding maternal age, parity, Apgar scores, gestational age as well as babie's colour, gender or breasfeeding. However, we found significant differences in birth weight, as expected due to the study design. And also, in lenght and cephalic perimeter. Orientation behavior, that is, response to animated and inanimated visual and auditory stimulous was significantly different between groups, as well as the behavior in the supplementary itens. Discussion: our results support the idea that SGA babies perform poorly than their AGA pairs in the NBAS orientation group as a whole. And also, that they need more support from the examiner. This has allready been reported by others. However, in this study, differently than others, AGA babies behaved better than SGA newborns in every item of the orientation exam. The significant differences found in the supplementary itens support the idea that SGA babies represent a risk group for interaction with their caretakers.

Der volle Inhalt der QuelleSmall for gestational age (SGA) children are at a greater risk of having a short stature in adulthood. The type 1 and 2 insulin-like growth factors (IGF-1 and IGF-2) are the main endocrine factors determining fetal growth, and most of the known actions of these hormones are mediated via a receptor tyrosine kinase, IGF-1R. Inactivating mutations and deletions of the IGF1R gene in heterozygosis have been reported with increasing frequency in the last 9 years in patients with a history of a failure to thrive pre- and postnatally. It is postulated that at least 2-3% of the children born SGA could be defective for the IGF1R. The clinical presentation of these patients is highly variable with regard to the severity of growth retardation and pre- and post-natal hormonal parameters. Objectives: The goal of this study was to identify the in vitro insensitivity to IGF-1 in patients with defects in IGF1R. Methods: We developed fibroblast cultures from two controls (C1 and C2) and 4 patients born SGA (SGA1, SGA2, SGA3 and SGA4) with a suspected insensitivity to IGF-1 by the absence of catch-up growth during their postnatal life. Of these patients, one (SGA1) carried a heterozygous missense mutation in the IGF1R gene (p.Arg511Trp), and the others exhibited low expression levels of IGF1R in their peripheral leukocytes, as evaluated using real-time PCR. One of these patients (SGA2) was also heterozygous for the allelic variant p.Gly6Arg of IGF1R, an alteration also found in the controls and family members not displaying growth restriction. The actions of IGF-1 were determined using proliferation assays, an analysis of the expression of IGF- 1R and phosphorylation studies of proteins of the IGF-1 signaling pathway in fibroblasts (via PI3K).Results: The SGA1, SGA2, SGA3 and SGA4 fibroblasts proliferated 55%, 66%, 64% and 28%, respectively, less under the stimulation of IGF-1 compared to the control lines. Using real-time PCR, the IGF1R mRNA expression showed reduced levels of IGF1R in the SGA2, SGA3 and SGA4 cells compared to the controls, and the total IGF-1R protein was also decreased in these cells. Moreover, the SGA1 cells showed increased expression levels of IGF1R mRNA and protein compared to the controls. In relation to the activation of PI3K, all of the cells showed decreased phosphorylation of AKT after the stimulation with IGF-1 compared to the control cells. Conclusion: We demonstrated the presence of a partial insensitivity to IGF-1 in the studied samples. The low expression of IGF1R observed in the peripheral leukocytes in the SGA2, SGA3 and SGA4 fibroblasts was confirmed at both the mRNA and protein levels, and the Andréa de Castro Leal Doutorado insensitivity of the cells to IGF-1 is secondary to the decreased expression of the receptor. In contrast, the cells with the mutation p.Arg511Trp (SGA1) displayed normal IGF-1R expression on the cell surface. The patients with alterations in the IGF1R gene do not exhibit a characteristic phenotype that differentiates them from other children born SGA and with no changes in this gene, thus showing the importance of molecular studies for this group of patients

Der volle Inhalt der QuelleIntrauterine growth restriction (IUGR) refers to a condition in which a foetus is not able to achieve its genetic potential for growth. IUGR is a serious clinical problem, and has recently been linked with diseases of adulthood, such as hypertension, insulin-independent diabetes mellitus, dyslipidemia, and ischemic heart disease. Erythropoietin is the major regulator of proliferation and differentiation of erythroid progenitor cells, thanks to its anti-apoptotic activities. The aim of this study was to investigate the effect of IUGR on the kidneys, and the impact of erythropoietin on the kidneys with IUGR. The experimental study was conducted in Pasteur Institute of Novi Sad on 60 mice of NMRI race. IUGR has been imposed with the application of dexamethasone to pregnant females. After birth, the pups were divided into seven groups. DA was administered to the pups on the 1st and 7th day of life (dose 1, 4 and 10 μg/kg). Two groups represented the offspring of the mothers who during pregnancy received DA. After 4 weeks, kidney samples were taken and morphological and stereological analysis of the glomeruli was performed. The application of dexamethasone (100 μg/kg) to pregnant mice leads to their offspring with IUGR. Application of DA to newborn mice with IUGR leads to faster weight gain in the first 7 days of life ("catch-up" growth). Mice born with IUGR have a reduced glomerular surface. Application of DA after birth and on the 7th day of life (4 and 10 μg/kg) in mice with IUGR leads to hypertrophy of the kidney glomeruli. IUGR has no effect on the number of kidney glomeruli. Application of DA has no effect on the number of kidney glomeruli. Mice born with IUGR have a reduced cortical thickness. Application of DA (4 and 10 μg/kg) in mice born with IUGR leads to increased thickness of the kidney cortex. Application of DA to mice with IUGR significantly increases the surface area of the kidney glomeruli and cortical thickness.
