A healthy reproductive system is inconceivable without normal vaginal microbiota, and full-fledged treatment cannot be carried out without detailed understanding of the arrangement and functions of the human microbiome. Today superbugs are a reality, and the role of such concepts as “microbiome” and “biofilms” is already undeniable in medical practice. Every doctor understands that it is necessary to choose antibacterial drugs based on practicability, global experience and evidence-based medicine. All this clearly demonstrates that there is a need to create an authoritative source of knowledge — a handbook for practitioners. Each chapter contains up-to-date information on the impact of female microbiota on the course and outcomes of pregnancy, on the etiology, pathogenesis and diagnostics of vaginal microbiocenosis disorders, and detailed treatment regimens. The work is intended for obstetrician-gynecologists and heads of women’s health clinics, perinatal centers, departments of general hospitals, fellows and heads of departments of obstetrics and gynecology, students of all forms of continuous medical education, graduate students and clinical residents, as well as students of medical schools.

Annotation:
Research suggests that intrasexual aggression during adolescence functions in competition over dating and reproductive opportunities and that aggressive strategies are more adaptive for females at this developmental stage. This sex difference appears to be related to the differential use of aggressive behavior and slightly uneven developmental trajectory between adolescent males and females. Competition over males is a common motive for female aggression during middle and high school, and, similar to adults, adolescent aggressors often use tactics of competitor derogation to lower the mate value of rivals. Taking an evolutionary perspective, findings demonstrate that adolescent females who engage in intrasexual peer aggression tend to have adaptive dating and sexual patterns, whereas those who are frequently victimized suffer maladaptive fitness outcomes. Recent research also shows that directed female intrasexual aggression during early stages of adolescence can be effective in both disrupting dating relationships of rivals and gaining access to desired dating partners.

This chapter describes the physiological effect of CFTR dysfunction on the development of the reproductive system. Young people with CF and their parents are poorly educated regarding sexual function and becoming parents themselves. They often wish to learn this from their CF teams. Male and female potency, reproductive genetics, and the need for genetic and general counselling before embarking on pregnancy are covered. Contraception, emphasizing the need to avoid unwanted pregnancy and sexually transmitted diseases, and assisted reproductive techniques are described. We discuss the evolving medical and obstetric management of pregnancy, including the likely need for optimizing drug treatment or escalating to more intensive treatment for intercurrent infection. Optimal delivery in the context of maternal health, fetal risks, and longer term maternal outcomes are discussed. Pregnancy post transplantation and termination of unwanted pregnancy or where the mother is too poorly to continue conclude the chapter.

Annotation:
During the 1980s and 1990s, feminist evolutionists were instrumental in demonstrating that primate females, including girls and women, can be aggressive and seek status within their groups. Building on their insights, researchers from across disciplines have found that females use a variety of direct and indirect tactics as they pursue their reproductive success. To better understand women’s aggression and status seeking, one also must examine their social networks. Women must not only deal with the dynamics within their groups, they also must deal with pressures from other groups. Success in maintaining connections in one’s social network is vital for access to the various resources women need for their own reproductive success and to keep competitors in check. Overall, women’s social networks, while serving both supportive and competitive functions, profoundly impact on the reproductive future of women and especially the survival and future reproductive strategies of their children.

It is generally accepted that women are at greater risk of lifetime post-traumatic stress disorder (PTSD) than men. Both gender differences in trauma exposures and cognitive response to trauma as well as sex differences in neuroendocrine function are thought to contribute to the differences in prevalence of PTSD across the lifespan. For women, reproductive transitions such as puberty, pregnancy, and menopause are particularly relevant, as ovarian and stress hormones as well as neurosteroids exert profound effects on the central nervous system. Similarly, pregnancy and childbirth can be experienced as traumatic events leading to exacerbation or new-onset PTSD. This chapter reviews the relevant literature regarding PTSD in women to highlight the importance of considering gender and sex as risk and resilience factors. The chapter is organized according to reproductive stage, as PTSD symptoms and treatment implications vary across the female lifespan.

The musteloids are the most speciose of the carnivorans and have a global distribution. They display a wide diversity of morphological and physiological form and function, which have been shaped by their adaptation to a wide variety of ecological niches, ranging from the Arctic to the tropics and deserts to the seas. This chapter explores how several morphological and physiological adaptations are key to their successful diversification, including an elongated body, a highly insulating pelage, powerful teeth and jaws, anal sacs for olfactory communication or chemical defence, and reproductive physiologies that allow females to optimise their reproductive output. While many of these adaptations are shared by other carnivorans, it is their combination in musteloids that has allowed them to diversify so successfully globally.

Annotation:
The focus in this chapter is on why honor killing ever came into existence as a social practice. The units for analysis are sociocultural systems and ecological pressures on the demographic groups among whom honor killing evolved. Here a population-level model of cultural evolution is employed to advance an argument for the best explanation for the development of honor killing. Only cultural systems performing adaptive functions continued among early desert nomads and pastoralist of the arid mountain uplands. Historical and anthropological research supports claims that severe ecological challenges led to two major functional systems: consanguine hierarchical patriarchy and the segmentary lineage system. Honor killing likewise evolved, first as a costly signaling system to avert loss of female reproductive assets and to avoid group splintering. It later evolved further as an exaptation and as a means of avoiding blood-related conflicts within segmentary lineage systems.

Annotation:
The lysosomal storage disorders (LSDs) are a group of inborn organelle disorders, clinically heterogeneous, and biochemically characterized by accumulation of nondegraded macromolecules primarily in the lysosomal and other cellular compartments. Given the common and essential cellular function of the lysosomal system in different organs and systems, patients afflicted with these disorders present a broad range of clinical problems, including neurological problems, visceromegaly, and skeletal deformities. Onset of symptoms may range from fetal period to adulthood. The neurological problems include developmental delay, seizures, acroparesthesia, motor weakness, muscle wasting, behavioral/psychiatric disturbances, cerebrovascular ischemic events, and extrapyramidal signs. Patients may present with symptoms later that include psychiatric manifestations, are slowly progressive, and may precede other neurologic or systemic features. Most of LSDs are autosomal recessive; however, a few are X-linked with symptpmatic female carriers (e.g., Fabry disease). In most of them, the diagnosis is established by biochemical and/or molecular assays. In terms of management, disease-modifying therapies include enzyme replacement, hematopoietic stem cell transplantation, and substrate reduction therapy. Patients and their families require genetic counseling regarding reproductive risks, disease prognosis, and therapeutic options. Investigations of disease molecular mechanisms provide insights into potential targets for the development of therapeutic strategies. Supportive care has been the key and essential for most LSDs, resulting in substantial improvement in quality of life of patients and families.

Research on young individuals and childbearing adults being treated for neoplasia has revealed a rising number of requests for treatments aimed to maintain the possibility to conceive. To answer such requests, it is important for medical professionals to consider the necessity to cure the woman, to preserve her fertility, to give information both on the effect of neoplasia and treatments on pregnancy. Patients have to be informed on the possible treatment alternatives that are less aggressive towards the reproductive function, but at the same time, give desirable results in terms of survival. Neoplasia and Fertility describes the state-of-the-art on fertility preservation in women affected by neoplasia. The 11 book chapters inform the reader with the goal of equipping them with the required information needed to present the condition and to discuss the possibility of conceiving, and how to manage patients after oncologic treatments at different stages of pregnancy. Key Features - Informs the reader about the relationship between gynecological cancer and fertility in women through 11 chapters - Describes a broad range of cancers and relevant treatment options for maintaining fertility - Explains the role of a Fertility Sparing Team in clinics - Familiarizes the reader with the ethics behind oncology treatments with reference to female fertility - Describes fertility issues related to hereditary cancers in women - Includes references for further reading The book serves as an informative reference on the subject to medical doctors in the gynecology, obstetrics and midwife specialties, and nurses training the gynecological oncology. It will also be of interest to healthcare administrators involved in fertility and oncology clinics, as well as general practitioners in family medicine.

This chapter reviews recent data on the evolution of sexual strategies in humans and shows how, in the natural environment, the adaptive functions of sexual infidelity were substantially different in males and females. The meaning of technical terms used by evolutionary biologists to describe different behavioral strategies related to maximization of reproductive success are explained, including the Coolidge effect, good-gene sexual strategy, and serial monogamy. Biological analysis of motivations for sexual infidelity integrate evolutionary hypotheses with recent data from molecular genetic studies of personality showing that carriers of some genetic polymorphisms related to novelty seeking are more prone to sexual promiscuity. Finally, the chapter reports a clinical case showing how cultural prejudice can distort individual expectations about what is normal sexual desire.

Annotation:
Within their cosmic ecology, the Athenians took it for granted that their polis was a “communion” (koinonia) of households, so in their experience there could be no equivalents of our modern distinctions between state and society or political and social realms. Households (oikoi) functioned as the cells of the social body, such that the vitality of the parts was inseparable from the vitality of the whole. Thus, the human “government” of the polis began not with assembly meetings but with the management of its constituent oikoi, which were the primary means of life and livelihood for all Athenians. The Athenians also took it for granted that the gods had deliberately designed males and females to play different, but complementary roles in the reproduction of social being. Women were expected to serve as “partners” to their husbands in the business of household management, performing a wide range of functions that were essential to the lives of their oikoi and therefore to the life of their polis. While they may not look like “citizens” to us, they were considered full members of the polis (politides) at the time. Terms like “patriarchy” and “misogyny,” so common in the modern literature, are accordingly unhelpful when describing gender relations in classical Athens.
