To assess the effects of housing characteristics and home environmental factors on lung function of Chinese children, 6740 children (aged 6–16 years) were recruited from seven cities in Northeast China in 2012. Performance of lung function was determined by comparison of forced vital capacity (FVC), forced expiratory volume (FEV1), peak expiratory flow (PEF), and maximal mid-expiratory flow (MMEF). Multivariate regression models were used to evaluate the associations with lung function deficit. The results showed that housing conditions were associated with lung function deficit in children. The adjusted odds ratios were 0.47 (95% CI: 0.26–0.83) for FVC for “ping-fang” housing compared with “dan-yuan-lou-fang” housing and 2.90 (95% CI: 2.43–3.47) for FEV1 with home renovations completed within two years compared with counterparts. The linear regression models consistently showed a significant association of housing conditions and home environmental factors with lung function measurements across subjects. A residence taller than seven stories was negatively associated with FEV1 ( β = −55; 95% CI: −97 to −13). In conclusion, housing conditions and home environmental factors are particularly important to the development of lung function and respiratory health in children. These factors are concerning and action should be taken to improve them.

We appreciate Drs Blackman, Samson-Fang, and Gutgesell's interest in our letter to the editor and the opportunity to respond to some of their comments. Pediatricians and child psychiatrists are frequently prescribing the combination of clonidine and methylphenidate to manage children with attention deficit hyperactivity disorder who are unresponsive to other treatments. We agree that monitoring pulse and blood pressure, as well as looking for a history of cardiac sinus node dysfunction, makes clinical sense. Regarding baseline ECGs, there does not seem to be enough data on the use of this combination of medications in children to state a definitive answer.

In this contribution, we examine the question of the transmission of Bantu languages from the generation of the parents to the generation of children, into the specific context of Gabon. Our results are based upon a sociolinguistic survey (Zuè Elibiyo, 2008) made in two different cities of Gabon. One of these cities is located in the north-west (Libreville), and the other one in the center (Lambaréné). The data analysis shows that French language is clearly the language which the great majority of the parents transmit to their children. Nevertheless, at least two Bantu local languages, Fang (A75) and Gisir (B41) are, to a certain extent, also spoken by adults to the children within the family context. Finally, we show that not all local languages of Gabon are really endangered, even though their transmission is globally in decline.

There are many contradictions in previous studies of the status of depressive symptoms among Chinese migrant children, the number of which in the compulsory education stage is about 13.9 million. This systematic review and meta-analysis were designed to compare depressive symptoms among migrant children and their urban children. Around 2,710 papers were retrieved from PubMed, OVID, the Web of Knowledge, CNKI, Wan Fang, Chongqing VIP, and Chinese Social Sciences Citation Index on January 27, 2020. Thirty-Two studies met study criteria with 14,906 migrant children and 10,792 urban children. Migrant children scored significantly higher for depressive symptoms score (SMD = 0.307, 95% CI [0.222, 0.393]). Region significantly influenced the difference of depressive symptoms between migrant children and urban children. Large effect sizes came from studies using scales that focus on depressive mood while studies using scales that focus on depressive behavioural symptoms had small effect sizes. Future research should focus on treatments to decrease depressive mood of migrant children. Future studies can add information about the length of their current stay in the urban location to improve the quality of study. PROSPERO registry: Systematic review registration no. CRD42018090676.

Background Prenatal exposure to pesticides has been linked to adverse neurodevelopmental outcomes. Gaps exist in the current literature about the timing and magnitude of exposures that result in these adverse outcomes. Objective The Study of Asian Women and their Offspring’s Development and Environmental Exposures (SAWASDEE) cohort was established to investigate the impact of prenatal exposure to pesticides on early indicators of cognitive and motor skills, inhibitory control, emotion regulation, and memory that have been found to be important in the development of subsequent neurobehavioral and neurodevelopmental diseases. The overarching goal is to find earlier predictors of potential adverse neurologic outcomes in order to enable earlier interventions that could result in better outcome prognoses. Methods Recruitment of this prospective, longitudinal birth cohort began in July 2017 and was completed in June 2019 in Chom Thong and Fang, 2 farming districts in Chiang Mai Province in northern Thailand. Follow-up of the study participants is ongoing. During pregnancy, 7 questionnaires were administered. Time-resolved biospecimen samples were collected monthly (for urine) and during each trimester (for blood) during antenatal care visits. Medical records were abstracted. Infants were administered the NICU Network Neurobehavioral Scale (NNNS) test at 1 month of age. A total of 322 mother-child pairs completed the NNNS test. All children will be followed until 3 years of age and undergo a series of neurodevelopmental tests. We will complete several additional exposure related analyses. Results A total of 1298 women were screened, and of those, 394 (30.35%) women were enrolled. The mean gestational age at enrollment was 9.9 weeks (SD 2.6). Differences in literacy were observed between Chom Thong and Fang participants. In Fang, about 54 of 105 (51.4%) participants reported being able to read in Thai compared to about 206 of 217 (94.9%) participants in Chom Thong. The percentages were comparable for reporting to be able to write in Thai. Conclusions This longitudinal birth cohort study will inform risk assessment standards for pregnant women in Thailand and other countries. Building awareness of how insecticide exposure during specific windows of pregnancy affects the neurodevelopmental trajectories of children in developing countries is a specific need recognized by the World Health Organization. Venomous snake bites can constitute medical emergencies, and without immediate care may be life-threatening. This study describes the characteristics and management of patients suffering from snake bite injuries (SNIs) in the Jerusalem area. A retrospective analysis of all patients who were admitted to the Hadassah Medical Center emergency departments (EDs) due to SNIs between 1 January 2004 and 31 March 2018 was conducted. During this period, 104 patients were diagnosed with SNIs, of whom 32 (30.7%) were children. Overall, 74 (71.1%) patients were treated with antivenom, 43 (41.3%) were admitted to intensive care units, and 9 (8.6%) required treatment with vasopressors. No mortality was recorded. On ED admission, none of the adult patients presented with an altered mental state compared to 15.6% of the children (p < 0.00001). Cardiovascular symptoms were observed in 18.8% and 5.5% of the children and adults, respectively. Fang marks appeared in all of the children. These findings underscore the severity of SNIs and the differences in clinical presentation between children and adults in the Jerusalem region.

Background. The transtheoretical model (TTM) views individual behavioral change as a nonlinear, dynamic process, which is consistent with the complex nature of physical activity (PA) in children and adolescents. However, within this theoretical framework, the elements that facilitate the behavioral change in PA in children and adolescents need to be further explored. Objective. A systematic review of research related to TTM-based exploration of the elements of behavioral change in PA in children and adolescents, an analysis of the strengths and weaknesses in practice, and an outlook for future research. Materials and Methods. After computer searches of the CNKI, Wan-Fang, VIP, WOS, PubMed, and EBSCO databases, two researchers independently screened articles, extracted information, and evaluated the quality of the articles. Results. A total of 25 articles (26 studies) of medium- to high-quality were included in the systematic review. The meta-analysis included 30,106 children and adolescents aged 11.24 to 17.7 years. The counter-conditioning and self-liberation of the process of change, self-efficacy and decisional balance are key elements that facilitate the transition of the PA stage in children and adolescents. Extramodel psychological variables such as exercise motivation play a moderate to large role in the PA stage transition. In addition, VPA is an important discriminator of PA stage transition in children and adolescents. Conclusion. It is recommended that interventions be designed according to the key elements of behavioral change in order to better facilitate the PA stage transition of children and adolescents.

Background: Snake bite is one of the important health problems in tropical and sub-tropical countries including India. Most snake bites present without envenomation as most bites are usually due to non-poisonous snakes and even poisonous snakes can control amount of venom injected. Even though mortality is under reported, India accounts for most of deaths due to snake bite. Methods: A hospital based descriptive case study was conducted at Pediatric ward of VIMS Ballari. All children with definite history of snake bite with fang marks or features of local/systemic envenomation were included in study. Demographic parameters, symptomatology and complications were noted down as per pro forma and data was analyzed. Results: Most bites occurred in lower limbs in older children while playing outdoors. Most cases were from rural area. Majority presented with local toxicity followed by hemotoxicity and neuroparalysis. Two children died, one because of respiratory paralysis and other by acute kidney injury. Conclusions: Snake bite is a preventable health problem. By wearing protective shoes and avoiding outdoor sleep many snake bites can be avoided. Early, aggressive but judicious use of antisnake venom is a cornerstone of management.

Purpose: To present a meta-analysis whether the risks of caries and periodontal problems in autistic children are higher than those in healthy children. Study design: A literature search that included PubMed, Embase, Web of Science, Cochrane, China National Knowledge Infrastructure (CNKI), Wan fang, and Chinese Scientific and Technological Journal (VIP) databases was conducted. The primary outcomes of interest included the DMFT index, Plaque index (PI), Gingival index (GI), and Salivary pH. Quality assessment was performed in accordance with the Newcastle-Ottawa Scale (NOS). Dichotomous variables are presented as relative risk (RR), and continuous variables are presented as weighted mean difference (WMD). Results: Eight studies were included in this meta-analysis. Among these 8 studies, six studies compared the DMFT index, three studies compared PI, three studies compared GI, and three studies compared salivary pH. Meta-analysis showed that the mean DMFT index in autistic children was higher than that in healthy children, and the difference was statistically significant {MD = 0.50, 95% CI [0.04–0.96], P<0.00001}. Similarly, PI and GI in autistic children were higher than those in healthy children, and the difference between PI was statistically significant {MD = 0.59, 95%CI [0.36–0.82], P=0.02}, while the difference between GI was not statistically significant {MD = 0.52, 95%CI [0.30–0.75], P=0.08}. But the salivary pH in autistic children was lower than that in healthy children {MD = −0.28, 95%CI [−0.54–−0.02], P = 0.02}, and the difference was statistically significant. Conclusion: The present analysis suggests that children with autism have poorer oral hygiene, higher risk of caries, and a lower salivary pH than healthy children.

The clinical diagnosis of snakebite is critical, particularly in Southeast Asia where venomous snakebites are a public health concern. Additionally, cases involving unwitnessed snakebite with no species identification, especially in non-verbal children posed a challenge in the emergency setting. A 2-year-2-month-old boy presented to our emergency department with signs of neurotoxicity. He was restless and mildly bradypnoeic with the respiratory rate of 24 to 28 breaths per minute. He also had bilateral ptosis with absent gag reflex. There were faint fang marks noted over the medial aspect of his left ankle with local swelling and bruises, despite no history of animal bite and no eyewitness. A high index of suspicion of neurotoxic envenomation was prompted and a total of 6 vials of neuro-polyvalent anti-venom were administered in scheduled batches. Progressive clinical recovery was subsequently observed after the first batch of anti-venom administration. The case illustrated the importance of clinical recognition of neurotoxic envenomation in the absence of snake bite history or species identification. Early administration of anti-venom may potentially reverse the neurotoxic effects of systemic envenomation and saves lives.

Abstract Objective Snake envenomation has been poorly studied in developing countries. ‘Early morning neuroparalytic syndrome’ (EMNS), the classical clinical constellation caused by krait bites, refers to nighttime, indoor bites where nonspecific symptoms progress to neuroparalysis. Literature regarding EMNS in children is scarce. This study was planned to describe the clinical profile, intensive care needs and predictors of outcome in children with EMNS. Methods It is a retrospective study of children below 12 years admitted with a clinical diagnosis of snake envenomation to the pediatric intensive care unit (PICU) of a tertiary care hospital in North India. Patient records were reviewed from the electronic patient database manager. Comparison was made between the EMNS group and the non-EMNS group and between survivors and nonsurvivors within the EMNS group. Results Of the 111 children with snake envenomation, 76 had neuroparalysis (68%) and 51 had EMNS. In the EMNS cohort, 37 (72.5%) belonged to rural areas, 46 (90.2%) had indoor bites and 39 (76.5%) were witnessed. Patients with EMNS were more likely to have absent fang marks, hypoxemia at admission, bulbar palsy and need for PICU admission. Mortality rate was 13.7% in EMNS; predictors included younger age, presence of ptosis, cardiac arrest at admission and nonavailability of PICU bed (univariable analysis) but none of them independently predicted mortality. Conclusion Younger age, presence of ptosis, cardiac arrest at admission and nonavailability of intensive care beds increase the risk of mortality in children with EMNS. Timely recognition and respiratory support may reduce mortality in these children.

Background: Although increasing evidence has revealed the efficacy of acupuncture in obesity/overweight, actual improvement in metabolism in children and adolescents is unclear. Therefore, we conducted a meta-analysis to evaluate this correlation. Methods: A comprehensive search was conducted using multiple databases, including Medline, Cochrane, Embase, Web of Science, Chinese Biomedical Literature Database, China National Knowledge Infrastructure, Chinese Scientific Journal Database, and Wan-fang Data, to identify relevant randomized controlled trials published before February 1, 2023. General information and data for the descriptive and quantitative analyses were extracted. Results: Fifteen randomized controlled trials of 1288 obese/overweight children and teenagers were included. All the trials were conducted in China and South Korea. Regarding quality assessment, no other significant risk of bias was found. The acupuncture groups were more likely to have improved metabolic indicators of obesity/overweight than the control groups, in terms of body mass index (standardized mean difference [SMD] = −0.45, 95% confidence interval [CI]: −0.69 to −0.21, I2 = 71.4%), body weight (SMD = −0.48, 95% CI: −0.92 to −0.05, I2 = 84.9%), and serum leptin (SMD = −0.34, 95% CI: −0.58 to −0.10, I2 = 91.8%). The subgroup analysis showed that for body mass index, the results were consistent regardless of the intervention duration, body acupuncture or auricular acupuncture combined with other interventions. Conclusion: Our results suggest that acupuncture is effective in improving metabolic outcomes of obese/overweight children and adolescents. Owing to the limited number of trials included in this study, the results should be interpreted with caution.

Mental disorders both in adults as well as in children and adolescents are highly prevalent (e.g., Merikangas et al., 2010; The WHO World Mental Health Survey Consortium, 2004). Cognitive-behavioural therapy (CBT) is recommended as first-line treatment in an array of emotional and behaviour disorders, due to its vast empirical support (e.g., National Institute for Health and Care Excellence [NICE], 2020; Tollin, 2010; Walter et al., 2020). More recent third-wave approaches, namely Acceptance and Commitment Therapy (ACT), are also showing their effectiveness across disorders and age ranges (Fang & Ding, 2020; Gloster et al., 2020). Nevertheless, like every other scientific discipline, CBT is in constant evolution, awaiting new and innovative developments. Humans are so complex, and suffering in its multiple shapes is so prevalent that there is always room to develop new theories, to refine already existent theories, to improve practices and to increase dissemination. In this special issue of the journal Psychologica, several researchers and clinicians made valuable contributions, aiming at different populations (children, adolescents and adults), different phases of the life course (preschool, secondary school, expecting a first child), different approaches of CBT (second or third wave therapies), and different levels of action (developing assessment instruments, refining conceptualizations, building programs, developing new delivery formats, and disseminating knowledge). Similar to their Western counterparts, children in Hong Kong generally fail to reach the recommended levels of physical activity (PA). As an ultra-dense metropolis, Hong Kong is different from most Western cities. It is therefore important to update and appraise previous PA research in order to inform future PA promotion for Hong Kong children. Using a scoping review, the current study aimed to evaluate PA research among preschool and school-aged children in Hong Kong aged 3–12 years old who are at a critical development stage. Literature was searched from four English databases: Medline via EBSCOhost, SPORTDiscus, ERIC and PsycINFO via ProQuest; and three Chinese databases: CNKI, CQVIP and WAN-FANG. PA research among Hong Kong children published from 1 January 1997 to the searching date, 31 March 2020 was included. A total of 63 studies were identified, with the majority of studies focused on school-aged children as compared to preschoolers, adopted a cross-sectional design, using self-reported PA measures, and with small to medium sample sizes. We classified eligible studies into five main categories: (a) Health benefits of PA (k = 12). Consistent evidence on the health benefits of skeletal and cardiovascular capacity, quality of life, cognitive function, and sleep quality was revealed. However, inconsistent evidence was found on the benefits of weight-related indicators and academic performance. (b) Patterns of PA (k = 12). There is a general pattern of low levels of PA among Hong Kong children, in particular girls and children with special educational needs. (c) Measures of PA and related constructs (k = 11). The Chinese versions of self-reported measures of PA, PA-related social environment, and PA-related psychological constructs showed acceptable reliabilities and validities. (d) Correlates of PA (k = 18). The correlates of PA include physical environment, social environment, physical factors, psychological factors, and multiple correlates, which is in line with the social-ecological model. (e) Interventions for promoting PA (k = 10). PA interventions among Hong Kong children were conducted for healthy children, children with special educational needs, and children with cancer. Overall, there is a growing volume of PA research among children in Hong Kong in the recent decade. Yet, there is a lack of high-quality research for measuring, understanding, and promoting PA among Hong Kong children. It is highly recommended that future PA research among children should pay more attention on the preschoolers, adopting robust research design (e.g., randomized controlled trials), recruiting large and representative sample, and collecting device-assessed data.

This study was aimed at systematically evaluating the clinical effect and safety of Xiao’er Xiaoji Zhike oral liquid in the treatment of Mycoplasma pneumoniae pneumonia (MPP) in children and providing evidence-based references for clinical application. Methods. The databases like Chinese Biomedical Literature Database, China Network Knowledge Infrastructure, Wan Fang Database, Chinese Scientific Journal Database, PubMed, EmBase, and the Cochrane Library were systematically investigated via searching clinical trials about Xiao’er Xiaoji Zhike oral liquid in treating MPP from the establishment of these databases to Jun 8, 2020, the valid data from which were entered meta-analysis. The quality of evidence was assessed by GRADE criteria. Results. Totally, 15 trials and 1500 patients were involved in this review. It showed that clinical efficacy of trial group was more superior than control group at the outcome measures of cough disappearance time, lung rale disappearance time, fever subsidence time, total effective rate, lung X-ray infiltrates disappearing time, reduction of hospital stay, immunological indexes, and some other measures. And the differences between groups were statistically significant. There was no statistical difference in the adverse effects between two groups. Lung X-ray infiltrates disappearing time and cough disappearance time were separately high- and moderate-quality evidences while lung rale disappearance time and fever subsidence time were all low in accordance with GRADE criteria. Conclusions. In accordance with trials with low methodological quality, Xiao’er Xiaoji Zhike oral liquid combined with azithromycin seems to be safe and superior to azithromycin alone for the treatment of MPP in children. However, further trials with rigorous methodology need to be implemented for these potential benefits.

Rape, a mammoth epidemic notoriously having its tentacles covering everywhere like a speed of light that travels through cable implanted in a conduit pipe enclosed in an edifice. This scourge has unmasked its ugly face; of no value nakedness and spiteful fang on the nation in no undersized gauge, thereby consuming and reducing her women-folk to nothing but ciphers of humanity even in their own home; their dignity alienated, emotionally and psychologically dehumanized, made to struggle with societal stigmatization and above all, condemned to a lifetime melancholy trauma. It is on this foundation the paper explores how sexual socialization as a strategy could be utilized in stemming the further increase of the scourge and to ensuring a haven for everyone irrespective of gender. A secondary source was adopted for data collection; it also made use of qualitative analysis which invariably led to inferences drawn and coherent conclusion achieved. Findings revealed that the level of enlightenment and strategy towards rape curtailment in Nigerian society has been so low. The paper recommends that parents must be on their guard by ensuring that children receive sound sexual socialization from home fronts. The paper concluded that rape has been a colossal cankerworm rattling the nation.

Abstract Background: Pediatric sarcomas represent approximately 12% of all pediatric cancers with over 100 subtypes. Although some molecular detection techniques can be used to assist the diagnosis and treatment of pediatric sarcoma, it still remains challenging due to overlapping morphological features and limited biopsy specimens. Here we performed next generation sequencing (NGS) to analyze the molecular profiles of pediatric and adult sarcomas in China. Methods: A total of 700 Chinese patients with sarcoma were collected. The tumor tissue and matching blood specimens were tested by the integrative DNA and RNA sequencing panel Onco Panscan plus࣪ at Genetronhealth. Results: Of the 700 sarcoma patients, there were 224 children (32%) and 476 adults (68%). Difference from the almost same ratio of men to women in adults, there were more males (n = 134) than females (n = 90) in children. Rhabdomyosarcoma was the most common soft tissue tumor in children, accounting for 24.1% (n = 54), followed by Ewing's sarcoma (9.8%, n = 22) and Osteosarcoma (8.5%, n = 34), while Angiosarcoma (3.8%, n = 18) and Fibrosarcoma (3.6%, n = 17) were the most common in adults. Based on mutation types, the frequent alterations were missense mutations (n = 326, 51.2%), fusions (n = 129, 20.3%) and copy number variants (n = 66, 10.4%) in children with a mean of 3 mutations per patient, and missense mutations (n = 1438, 66.8%), fusions (n = 221, 10.3%) and truncating mutations (n = 215, 10.0%) in adults with an average 5 mutations. TP53 (12.9%, n = 29), EWSR1 (8.9%, n = 20) and ALK mutations (6.7%, n = 15) were common in children, which was distinct from TP53 (29.4%, n = 140), NF1 (4.8%, n = 23), CTNNB1 (4.2%, n = 20) and RB1 (4.2%, n = 20) in adults. Based on NGS results, pathological subtypes could be confirmed in 40.2% (90/224) and 48.1% (229/476) of children and of adults sarcoma patients, respectively. In addition, we identified a total of 72 drug-targeted gene mutations in the 57 children patients, of which 35 (48.6%) gene mutations could be targeted by FDA-approved drugs. In adults, 256 drug-targeted gene mutations were detected and the proportion of actionable mutations was up to 78.9% (n = 202). Conclusion: The genomic landscape of pediatric sarcomas is different from that of adults. NGS aids in the subtype classification and clinical guidance of pediatric sarcomas, providing evidence for personalized treatments with clinical benefit. OBJECTIVETo assess the prevalence of healthcare-associated infections (HAIs) in mainland China.DESIGNSystematic review and meta-analysis.SETTINGAdults and children from secondary and tertiary acute-care hospitals in mainland China.METHODSWe searched PubMed, the China National Knowledge Infrastructure, and Wan Fang for multicenter point-prevalence surveys of acute-care hospitals in mainland China from January 2006 to August 2016. All reports related to HAI, using a point-prevalence methodology and published either in English or Chinese were eligible.RESULTSIn total, 3,021 publications were identified; 115 were eligible for quality assessment and data abstraction. The weighted HAI prevalence (95% confidence interval [CI]) overall, in general hospitals, children’s hospitals, maternal and child health hospitals, and oncology hospitals were 3.12% (95% CI, 2.94%–3.29%), 3.02% (95% CI, 2.79%–3.26%), 4.43% (95% CI, 3.39%–5.47%), 1.88% (95% CI, 1.47%–2.29%), and 3.96% (95% CI, 3.12%–4.79%), respectively. In general hospitals, prevalence was highest in adult intensive care units (26.07%; 95% CI, 23.03%–29.12%), followed by surgery (3.26%; 95% CI, 2.96%–3.57%), and internal medicine (3.06%; 95% CI, 2.67%–3.46%). Overall, lower respiratory tract infection was the most frequent HAI (24,185, 47.28%), followed by urinary tract infection (5,773, 11.29%) and upper respiratory tract infection (5,194, 10.15%). Gram-negative bacilli were the most frequently isolated pathogens, and Pseudomonas aeruginosa (3,395, 14.91%), and Escherichia coli (2,918, 12.82%) were the most common single microorganisms.CONCLUSIONSThis study is the largest systematic review on the prevalence of HAI in mainland China. These results provide a benchmark for future PPSs and a reference for infection prevention and control strategies in mainland China.Infect Control Hosp Epidemiol 2018;39:701–709

Background Lamotrigine as a broad-spectrum antiepileptic drug, is widely applied and its clinical efficacy is highly recognized. However, significant differences are observed in blood drug concentration of lamotrigine among individuals, which may have an impact on its efficacy. UGT1A4 is the main metabolic enzyme. However, it was inconsistent for the influence of UGT1A4 genetic polymorphism on concentration and efficacy of lamotrigine therapy. This study aimed to evaluate the influences of UGT1A4*3 genetic polymorphisms on lamotrigine concentration and therapeutic effect through meta-analysis. Methods The literature search was conducted in Medline, Embase, PubMed, Web of Science, Wan Fang Database, China National Knowledge Infrastructure, China Science and Technology Journal Database until January 2024. The primary outcome included the mean serum concentration, concentration-to-dose-ratio by body weight (CDR), or efficacy related to different UGT1A4*3 genotype for lamotrigine therapy. Data were collected to access the Mean Difference or odds ratio with 95% confidence interval. Meta-analysis was performed by RevMan 5.2. Results A total of eleven studies were enrolled. The meta-analysis for mean serum concentration of lamotrigine showed no significant difference between patients carrying TT genotypes and TG and GG genotypes group (MD: 0.12, 95% [-0.35, 0.58], P = 0.62). There was significant difference in CDR (MD: 0.49, 95% [0.03, 0.94], P = 0.04) and therapeutic efficacy (OR: 7.18, 95% [4.01, 12.83], P<0.00001) of lamotrigine, however no significant difference was found in subgroup analysis of CDR of children (MD: 0.03, 95% [-0.35, 0.42], P = 0.87) between patients carrying TT genotypes and TG and GG genotypes group. Conclusions Polymorphism of UGT1A4*3 influenced the CDR and therapeutic efficacy of lamotrigine for antiepileptic therapy. Genotype analysis provided reference for personalized medication in the future. However, more high-quality evidences are necessary for precise and definitive conclusion.

Background/Aims: Published studies indicated that the MTHFR gene polymorphisms C677T and A1298C are associated with congenital heart disease (CHD) risk in children, but obtained inconsistent results. Our study aims to reach a more accurate association between these two polymorphisms and CHD risk. Methods: Eligible studies were obtained by screening the PubMed, Embase, China National Knowledge Infrastructure, Wan Fang and VIP databases based on designed searching strategy. The odds ratio (OR) and 95% confidence interval (CI) were calculated. Moreover, a trial sequential analysis was introduced to confirm the positive results and an RNA secondary structure analysis was also applied to discover the potential molecular mechanism. Results: Based on thirty-two published articles, involving 6988 congenital heart disease subjects and 7579 healthy controls, the pooled results from the C677T polymorphism in the fetal population showed increased risks in allelic model (OR=1.32, 95%CI=1.14-1.53), recessive model (OR=1.69, 95%CI=1.25-2.30), dominant model (OR=1.35, 95%CI=1.11-1.64), heterozygote model (OR=1.20, 95%CI=1.01-1.41) and homozygote model (OR=1.75, 95%CI=1.31-2.33). An increased risk was only detected in the A1298C polymorphism in the overall fetal popalation in a recessive model (OR=1.42, 95%CI=1.10-1.84). In the subgroup stratified by region, sample size, genotyping method and source of controls, the increased risks were widely observed in both the C677T and A1298C polymorphisms with CHD risk. Furthermore, trial sequential analysis confirmed our positive results, and the RNA secondary structure analysis detected the changes in the RNA secondary structure caused by the mutant 677T allele and 1298C allele. Conclusion: In summary, we found that the MTHFR C677T polymorphism is associated with a significant increased risk in congenital heart disease in the fetal population. Moreover, an increased risk in the CC genotype of MTHFR A1298C polymorphism was observed, but the protective role of the 1298C allele needs further study.

Background:Migration of Romanians to work abroad began after 1990 with the aim to provide a better income and life for their family. Current studies show that the migration of one parent or both, even when it is temporary, produces negative long-term effects on the health and psychosocial evolution of the children affected. Children and adolescents exposed to chronic stress due to migration, misunderstandings between the parents, alcoholism, violence, divorce of the parents etc., present an increased risk of illness. More and more data from the literature suggest that prolonged stress and depression induces inadequate cortisol along with norepinephrine secretion, increased synthesis of proinflammatory cytokines, which are the basis for autoimmune pathologies, such as chronic arthritis. Objectives:Given the extended phenomenon of migration from Romania and the increase in the cases with autoimmune pathology in children and adolescents, we aimed to evaluate the association between the disorders related to the permanent stress induced by the parental migration abroad and the risk of developing arthritis during childhood. Methods:The study included 201 children and adolescents aged 13.4 ± 3.7 years, who were in evidence of an outpatient health unit, from 2016-2019. These cases were included in a chronic disease registry with the diagnosis of Juvenile Idiopathic Arthritis (JIA), established by a pediatric rheumatologist. For the initial evaluation, we used a questionnaire that included the socio-demographic data. In comparison, we studied 40 healthy children (control group). The family drawing test was used for patients between 5 and 16 years of age to identify possible conflicts with certain family members, to assess the emotional and psychological maturity of the child or adolescent, and to find out if are there any problems at home. Results:At the end of the study, only 181 (90%) of the eligible patients completed the questionnaire and the family drawing test. Demographic data showed that patients from rural areas predominated (71.8%), compared to 28.2% from urban areas. In terms of sex, 52.5% were male, compared to 47.5% female. Family history (mother, father, sister, brother, grandfather, aunt, uncle) of autoimmune disease was encountered in 28.1% of patients, as follows: spondylarthritis in 9.4% cases, rheumatoid arthritis in 8.8% cases, JIA in 3.9% cases and other autoimmune diseases (Systemic Lupus Erythematosus, Scleroderma, Diabetes, Asthma) in 6% cases. Patients from low-income families were in 82.3% of cases. 72.5% of the cases had a prolonged state of stress by migrant parents for working abroad [38.7% only one parent (30% mother) and both parents in 17.7% of cases], divorce in the family in 11.6% of cases, unmarried mother in 2.8% of cases, and a close relative recently deceased in 1.7% of patients.Subcategories of JIA included: polyarticular JIA negative Rheumatoid Factor (RF) in 39.77% of cases, enthesitis-related arthritis in 27.07% of cases, polyarticular JIA positive RF in 14.36% of cases, oligoarticular JIA in 14.9% of cases, systemic JIA in 3.31% of cases and psoriatic JIA in 0.59% of patients. Conclusion:Both the data from the questionnaires, but especially the family drawing tests, suggest that the prolonged state of stress with anxiety, sadness, pain and depression, in combination with starvation, lack of parental love and the genetic predisposition, have contributed to the emergence of chronic arthritis, pathology that is growing more and more in recent years in Romania.References:[1] Dube SR, Fairweather D, Pearson WS, Felitti VJ, Anda RF, Croft JB. In fan studies scholarship, the term ‘young fans’ tends to refer to university-age fans, thereby often overlooking school-age children and the meaning of fandom among future generations of fans. While it can be difficult to study children due to issues around access and consent, it is important that we do ‐ they can tell us a lot about not only where fan cultures are right now but also where they might be heading in the future. In this article, we offer the results of a large-scale survey presented to approximately 1700 children aged 7‐17. The answers reveal why it is important for media studies scholarship to develop new methods for understanding children’s media consumption behaviours. First, despite the ‘mainstreaming’ of fandom in popular culture, our research evinces that the traditional depictions of fans as ‘unruly’ or ‘obsessive’ persist. And second, these young fans are not viewing themselves as fans of specific objects, but rather as ‘fans’ first and foremost, with the specific object of fandom unstated. Fan studies’ traditional view of fandom as being devoted to one thing needs expansion.

This study tested two hypotheses involving the use of sport spectating as an opportunity to spend time with one's family. First, it was hypothesized that scores on Family Motivation would be higher for fans who were married or had children than for unmarried fans without children. Second, it was predicted that among sport fans who were married or had children, those preferring a Nonaggressive sport would report higher scores on Family Motivation than those preferring an Aggressive sport. Analysis confirmed the first hypothesis while refuting the second.

As fan studies increasingly explores understudied fan communities this article calls for the inclusion of children as a crucial overlooked group of fans. Drawing upon research that considers fandom throughout the life course, it argues that fandom is an important part of childhood and one that may look different than it does for adult fans. I argue, the extent to which children’s media consumption practices resemble fan practices has obscured the fact that children can be fans. In this article I consider the impediments to considering the fandom of children, the ways in which children may be acting as ‐ if not labelling themselves as ‐ fans, what doing so can offer both fandom studies and children’s media studies and I lay out some suggestions for beginning the process of examining the practices and sites of children’s fandom.

Purpose Sport management research that examines children as a distinct group of sport consumers is sparse, and therefore the authors know relatively little about how and why children become fans of sport teams. The purpose of this paper is to explore the game-day experiences of children in order to better understand how these experiences allow children to socialize into the team community and become fans of the team. Design/methodology/approach The authors examine this through exploratory observational analysis and 26 semi-structured interviews with children at professional sporting events. Findings Among the results, it was found that children primarily focus on exploring ways to build membership in the fan community as opposed to initially building connections to the team itself. In addition, those children that watched the games with their peers demonstrated greater in-game emotional responses than those children that viewed the game with family. Research limitations/implications This study provides support for the importance of community membership in the initial stages of sport team fandom as well as the varying effects of different groups within fan communities on child fans. However, further research is needed to increase the generalizability of the results. Practical implications It is recommended that sport teams increasingly target groups that will bring children to games with their peers in order to enhance their game experience and increase their socialization into fandom. Originality/value This paper is one of the first in sport management to directly look to better understand children and the ways in which they become fans of sports teams.

BackgroundChildhood-onset systemic lupus erythematosus (cSLE) is associated with higher disease severity than adult-onset lupus. Abnormal activation of B cells is a crucial link in their pathogenesis. Belimumab is a specific inhibitor of the soluble B lymphocyte stimulator and inhibits its binding to receptors and thus its activity[1,2]. However, the current clinical research evidence of this drug in children is insufficient, the relevant clinical data are mostly from prospective studies in adults, and there are no exact guidelines for clinical application in children.ObjectivesThis study aimed to investigate the efficacy and safety of belimumab for treating children with cSLE.MethodsWe systematically searched PubMed, EMBASE, Wan Fang Data,Web of Science, the Cochrane Library, and Medline for randomized controlled trials, original case reports, and case series that described belimumab’s efficacy in treating cSLE. A random-effects meta-analysis was performed to calculate its efficacy. Inconsistency was evaluated using the I2 and Egger tests to evaluate potential publication bias (STATA v.12.0).ResultsFive studies with 325 patients were included in the meta-analysis (Table 1). The age range of the included participants was less than 18 years; most were women. All patients received belimumab at a dosage of 10 mg/kg every 2 weeks for the first three doses, then every 4 weeks thereafter. SLEDAI-2K was the main score, which was significantly reduced compared with the baseline[SMD= -1.106, 95%CI (-1.311,-0.901), P<0.001]. The number of oral corticosteroids was significantly decreased after the belimumab therapy[SMD = -1.21,95%CI (-1.72, -0.70), P<0.001]. Both Anti-double stranded DNA (Anti-dsDNA) and anti-nuclear antibodies (ANA) positive patients also decreased obviously[RR= 0.56, 95%CI (0.37,0.85), P=0.007; RR= 0.90, 95%CI (0.83,0.98), P=0.012]. Infection and thrush were the main adverse effects with rates of 26% and 4%,respectively, but all were classified as mild-moderate treatment-emergent adverse events. Serious adverse reactions are rarely reported.ConclusionBelimumab treatment in cSLE minimizes the use of hormones, and the incidence of adverse events such as infections was low, suggesting belimumab can effectively reduce disease activity safely and reliably. Long-term efficacy and safety still need a multicenter, large sample, long-term, in-depth research.References[1]Kamphuis S, Silverman ED. For four days, community service activities were given to early childhood children at Rainbow Kiddy Kindergarten, Surabaya. Eight children participated, and all of them were kindergarten B students. The aim of holding this activity was to provide understanding and instill habits for productive and diligent living. The learning method is done through puppet stages that present educational stories, block assembly games, drawing on scratch paper, puzzle games, and coloring fans and piggy banks. The results of this community service activity show that the socialization activity has made children understand the importance of being diligent. Puzzle games and assembling blocks train children's motoric, sensory, and mental cognitive abilities to make efforts. Drawing activities on scratch paper and coloring fans encourage children to be more expressive and creative. Coloring piggy banks encourages creativity and the intention to start saving.

Non-steroidal anti-inflammatory drugs (NSAIDs) are the main triggers of drug hypersensitivity reactions in children. According to the EAACI latest classification NSAIDs hypersensitivity reactions are differentiated into cross-reactive reactions, with non-immunological mechanisms (based on COX-1 inhibition), and selective reactions, with immunological mechanisms. Paediatric clinical manifestations of NSAID hypersensitivity are typically cutaneous, but sometimes, similarly to anaphylaxis, can involve other systems, especially the respiratory one. Differentiating between NSAID intolerance and NSAID allergy through drug provocation tests is crucial for the patient because the two clinical entities require different management.

Scholars have begun exploring how parenthood impacts individuals’ sport fandom. Limited work to date, however, has considered such a question in light of new parenthood. Thus, the purpose of this study was to examine how new parent sport fans negotiate their multiple identities. To this end, I completed semi-structured long interviews with 27 sport fans with young children (i.e., individuals presently raising children of age 0–6 years). Drawing on the social–psychological foundations of identity theory, I examined these new parents’ salience hierarchy negotiation. I identified and analyzed two consumer groups: Maintainers (who have sustained the centrality of their fan identity despite a change in life circumstances) and Modifiers (who have “de-escalated” their fandom). These new parents’ voices are used to guide the findings. This study advances the theoretical understanding of how parenthood impacts fandom and illuminates how the sport industry can optimally serve new parent sport fans.

A de-escalation of team fandom model was created based on identity work theory. To both test the model and understand how once highly identified fans of sports teams could eventually become non-fans, 23 former fans of National Hockey League (NHL) teams were interviewed. The reasons given for their de-escalation in fandom can be categorized into seven themes: sport in general, the sport of hockey, the league, the team, individual players, media, and life. For those fans who remain fans of the sport, watching national teams play in international competition has been a common practice in the years since the bond with their former favorite NHL team was severed. While only a minority of participants believes it realistically possible they could ever become NHL team fans again in the future, some suggested their children or grandchildren might pull them back into fandom.

Three-fourths of these fans were males and nearly all were white. HSS fans more often claimed to know all the lyrics of their favorite songs than the non-HSS rock fans. HSS fans more often said young children should be permitted to listen to rock music with destructive themes and fewer of them believed that adolscents might commit murder or suicide after having listened to such songs. A large proportion of the students share the concern of adult citizens and professional groups about destructive lyrics in rock music and their effects.

This study aimed to develop a digital module for Digital Literacy for children and young people who were KPop fans in Makassar, to prevent cyberbullying. Data showed that that K-Pop fans consisting of children and teenagers in Indonesia were one of the largest Internet users in the world. The high use of the internet was also directly proportional to the cyberbullying that occured to them, not only as victims but also as perpetrators. This was the reason, why this study was important to investigate even though many researches on digital literacy had been carried out, but not many had been found specifically targeting KPop fans, while they were active internet users. This study was development research referring to the 4-D model (Four) that proposed by S. Thiagarajan and Semmel (1974), which consisted of 4 stages of development, namely the stage of defining (Define), Design , Development (Develop), and dissemination (Disseminate), then reduced to a 3-D model which only consisted of 3 stages of development, namely the stage of defining (Define), Design, Development (Develop). The results produced a Digital Literacy Module, which emphasized content on digital skills, consisting of Devices used, Search Engines, social media applications and online conversations and shopping, all of which were adapted to the characteristics of KPop Fans.

While the sport fan literature suggests that it is common for parents to socialize their children to cheer for specific sports and teams, recent literature proposes that children can socialize their parents into changing the parents’ sport fandom in a process sociologists and consumer behavior researchers refer to as reverse socialization. To ascertain whether children can socialize and influence their parents’ sport fandom, 20 sport fan parents were interviewed. Evidence of reverse socialization was found in 15 of the participants, manifesting itself in ways that can be categorized as either developing new or additional fandom, or changing one’s behaviors or attitudes towards their existing fandom. However, further exploration of the data suggests that future research reexamine the term “reverse socialization,” as we do not see this as a directionality of influence, but as children as socializing agents.

Their prognosis is uniformly pejorative and the guidelines for management are still not codified. we have realized a retrospective study in order to search clinical and pathological features as well as any prognostic factors. PATIENTS AND METHODS: This is a retrospective study of 8 children with GBM confirmed by histology, collected at the neurosurgery department of the Fann National University Hospital Center, from January 1, 2016 to December 31, 2018. RESULTS: The annual frequency was 2.6 cases. There was a male predominance with a sex ratio of 1.6. The time limit consultation time was three and a half months. The main reason for consultation was intracranial hypertension syndrome (HTIC), found in 62.5% of cases. The region most affected was the parietal lobe (n=4). The mean time to surgical management was 2.3 months. Excisional surgery resulted in macroscopically complete resection in half of the cases. The histological study found eight cases of WHO grade IV glioblastoma. Only one patient had received adjuvant radiochemotherapy. The six months’ mortality was 85.7% with a median survival of 6.5 months. CONCLUSION: Glioblastomas remain a rarity in children. Multimodal treatment combining excisional surgery, chemo and radiotherapy remains the guarantee for a better prognosis.

We report findings of a follow-up on rock music preferences and of views of themes advocating homicide, suicide, and satanic practices (HSSR). A total of 120 adolescent offenders (ages 13–18 years), in two youth detention centers, were administered a questionnaire of Likert-type, categorical, and open-ended questions. Ninety-one students were fans of rock music. Of those, approximately 54 percent were HSSR fans. HSSR fans were more likely to be white and school dropouts, to spend more time listening to music, to think it is harmless for young children to listen to HSSR music, and to assume that HSSR lyrics do not lead to destructive acts. Contrary to previous findings, gender and parents' marital status did not predict whether a juvenile offender was an HSSR fan: both males and females, from intact and broken homes, were fans. Findings suggest a strong relationship between a preference for rock music with destructive themes and antisocial or destructive behavior.

Abstract Fanconi anemia (FA) is a genetic disorder due to mutations in one of the sixteen FANC genes involved in DNA repair. Many FA patients develop bone marrow failure (BMF) during childhood, and FA strongly predisposes to myelodysplasia syndrome and/or acute myeloid leukaemia. The pathogenesis of the BMF remains uncompletely understood. Low hematopoietic progenitor cell (HPCs) counts observed early in life and preceeding the onset of blood cytopenia in patients led we, and other, to hypothesize that the hematopoietic development might be abnormal in the FA embryo. Indeed, unlike adult hematopoietic stem cells (HSCs) which are quiescent in the BM niche, during embryonic life HSCs are in active proliferation in sites of expansion such as fetal liver and placenta, where they get amplified and acquire properties of adult HSC .We hypothesized that in FA, the FA defect in response to the replicative stress could impair the expension of the HSC pool.In order to investigate this hypothesis, we carried out studies in Fancg-/- knock out mice and in human FA fetuses obtained with informed consent from medical abortion. In Fancg-/- mice, FACS analysis revealed a 1,5- to 3-fold deficiency in hematopoietic stem and progenitor cells (HSPC) very early during embryonic development (i.e 11.5 days of gestation - E11.5) in fetal liver (FL) and placenta (Pl) (p <0.001). In both organs, this defect persists during the whole period of amplification (until E14.5 for FL and E12.5 for Pl). In vitro clonogenic assays also demonstrated a 2- fold defect in granulocyte, erythrocyte and macrophage (GEM) progenitors both in Fancg-/- FL or Pl compared to WT (p <0.001), and 4 to 5- fold defect in more immature mixed GEM progenitors in FL (p <0.001). LTC-IC frequency of the HSC-enriched Lineage- Sca1+ AA4.1+ population (LSA) of E14.5 Fancg-/- FL comforted this later result, since it was 5-fold lower than for WT. In vivo long-term hematopoietic reconstitution (LTR) assays confirmed a deficit of the HSC enriched LSA population of E14.5 Fancg-/- FL. Indeed, although the percentage of mice reconstituted was as good as that obtained with the same number of WT LSA, the CD45 Ly5.2 chimerism was reduced (49±20% vs 84±4% for 1000 LSA injected, and 56±12% vs 87±2% for 5000 LSA). Interestingly, bone marrow analysis of mice reconstituted with Fancg-/- LSA 22 weeks after injection showed a level of CD45 Ly5.2 chimerism 3-fold lower than that found in blood, spleen and thymus, as well as a very low chimerism for myeloid GEM lineages, contrasting with a high chimerism for B and T lymphoid lineages. Moreover, we were able to demonstrate that this deficit is already present at E12.5, both in Fancg-/- FL and Pl. Indeed, no mice reconstituted with 3.105 total Fancg-/- fetal liver cells, while 100% injected with the same number of WT FL cells got reconstituted with a chimerism of 59,5±5%. For Pl, when 500 000 cells were injected, reconstitution was observed in only 1 out of 3 mice for Fancg-/- (29% chimerism), and in 3 out of 3 mice for WT (88±4% chimerism). In human FA FL of 14 weeks of gestation, we also observed a 4-fold defect of HSPC with a total lack of in vitro amplification compared to control, in agreement with the mice data. Taken together, these data demonstrate that a profound deficit of HSCs and progenitors cells is present since the earlier stages of embryonic development in FA. In addition, using organotypic cultures of E11 aortas, we could show that this defect of amplification is already present in HSCs emerging from Fancg-/- aorta, which showed a 2-fold lower rate of amplification compared to WT. More importantly, our results show for the first time exhaustion in myeloid lineage of FA, in agreement with what is observed in children with FA disease. Altogether, our work suggests a role of the FA pathway during the development of the hematopoietic system leading to a deficit of amplification of HSC. Comparison of FA HSC transcriptome with that of control HSC in FL and Pl is in progress. It should allow to identify the key pathways involved in the embryonic HSC amplification that are deregulated in FA, and hopefully getting more insights in the pathogenesis of the BMF and leukemogenesis in FA patients. Purpose – The purpose of this paper is to explore children’s relationships with football teams and players and the influences on these. Design/methodology/approach – A child-centric (Banister and Booth, 2005) inductive qualitative approach was utilised to capture children’s voices. The children were asked to take photographs around the theme of “football in my life” and these served as interview prompts when talking to friendship pairs. Findings – Football played a central role in children’s lives in terms of interest, activity and consumption. The children articulated a portfolio of team (club) and player connections of varying strength. This contrasts with the existing adult fandom literature which focuses on individuals supporting a single team. Another strong theme emerging from the data was the children’s market-centred relationships with football clubs. Children’s connections were shaped by a complex web of influences including family and family history, friends, media and geography. Research limitations/implications – Existing fan literature has an adult focus which does not appear to fully explain the child fan. This research provides impetus for developing new theory that better captures child fandom. The findings reinforce the idea that football plays an important part in children’s lives and in doing so they establish their own meanings. The findings presented in this paper provide important insights into the lives of children that could be reflected on in the design of policy across a number of areas including education. Originality/value – This paper presents the first child-centred football fan study.

Purpose The purpose of this study is to evaluate children’s perceptions and attitudes towards sponsorship transition, specifically the change from Nike to PUMA as kit sponsors for Manchester City Football Club (MCFC) in July 2019. Design/methodology/approach A sample of 368 children, between 7 and 16 years of age were recruited for the study. Using electronic diaries, 1,577 diary entries were captured between February 2019 and March 2020. Findings Data reveals that children conceptualise sponsorship as a social exchange, with sponsoring brands seen as human entities and interaction with them reflecting the dynamism of social and familial relationships. Consequently, children in this study demanded prosocial and interpersonal behaviours from sponsors and sponsee during the transition period. Research limitations/implications The research has an immediate and direct application for brand managers and the sponsee when considering terminating long-term sponsorship. Both the departing and incoming sponsors can maximise their relationships with these younger fans through an orchestrated departure, arrival and dedicated handover. Practical implications The findings enable marketing brand managers to effectively evaluate sponsor transition to maximise opportunities to maintain, and indeed start, brand relationships with younger fans. Originality/value This is the first study that has examined sponsorship children’s responses to sponsorship transition.

Fanconi anemia (FA) is a rare autosomal recessive disorder characterized by congenital malformations, haematological problems and predisposition to malignancies. It was first described by Guido Fanconi, a Swiss Paediatrician in 1927. The prevalence of FA is 1 to 5 cases per million. The genes that have been found to be mutated in FA patients are called FANC. 16 different FANC genes have been reported, among which 60-65% account for the mutations seen in FANCA genes which is the most frequently seen in FA patients. The disease is most commonly seen in children between 5-15 years. Diagnosis is based on the congenital physical abnormalities and confirmed by genetic testing. Here we report a rare case of Fanconi Anemia in a 4 year old female child with the characteristic clinical findings and the diagnosis was confirmed by genetic studies.

Participation in boxing, mixed martial arts, and wrestling is extremely popular among children and adolescents. The objective of this study was to determine themes depicted in a select number of combat-related films. Twenty films were independently viewed and analyzed by 4 reviewers. The average number of positive and negative themes was 34.4 and 18.1 mean events/hour for all included films, respectively. The most common positive themes were “positive interactions with family/friends/fans,” “importance of mentorship,” and “positive interactions with a coach/trainer” (4.23, 3.63, and 3.06 mean events/hour, respectively). The most common negative themes were “choosing to do the wrong thing,” “negative interactions with family/friends/fans,” and “poor sportsmanship” (2.70, 2.00, and 1.58 mean events/hour, respectively). In conclusion, the co-viewing of combat-related films among pediatric athletes and their coaches/trainers and/or parents in order to focus on “teachable moments” may encourage the acquisition and development of positive themes and the avoidance and de-emphasis of negative themes.
