West Nile virus has a potential to emerge in new areas and cause large epidemics as was witnessed in the United States following its introduction in 1999. The virus is now a global public health threat, having been detected on every continent except Antarc tica. Once restricted to Africa, its expansion beyond its natural habitat is related to some viral, vectoral, anthropologic and environmental factors. The successful establishment and spread of the virus depend in part on viral adaptations, availability of competent hosts and mosquito vectors and suitable environmental conditions. A combination of measures can be applied to minimise the likelihood of a WN virus epidemic. Available vaccines are only for veterinary use, human vaccines are still in development . Vector control, animal vaccination, targeted surveillance and strong cooperation between relevant authorities are important in preventing a WN virus epidemic in Malaysia.

Complete emergency measures are one of the important guarantees for effective emergency response. An evaluation method based on User-Generated Content (UGC) data is proposed to assess the completeness of emergency measures. To implement this method, UGC data comprising emergency needs from the users and emergency plans comprising emergency measures for specific emergency were firstly collected using a crawler program. A feature template was then established to identify relationships between different features, and a Conditional Random Field (CRF) model was used to extract emergency measures from the emergency plan and emergency needs from UGC data. The Siamese network was applied thereafter to compute the similarity between the emergency measures and emergency needs. The missing emergency measures were obtained based on the similarities, and a quantitative method to calculate the completeness was defined. Finally, using province A as a case study, the emergency measure completeness was evaluated and the emergency measures that need to be strengthened and improved were identified.

The solution of the problem of developing approaches to the management of the emergency prevention process in the conditions of limited energy supply of emergency response measures is considered. Within the framework of solving the problem, the process of emergency management in the conditions of limited energy supply is analyzed and the scheme of managerial influence on the process of spreading the emergency situation in the conditions of limited energy supply is developed. A structural and logical model of emergency management in conditions of limited energy supply has been developed. Developed a structural and logical model of emergency management in conditions of limited energy supply, which consists of twelve units. These are monitoring of the situation, identification of risks, notification and evacuation of people, search and identification of the danger of emergence and spread of emergency in conditions of limited energy supply, decision-making on the use of special equipment for electricity generation, preparatory measures, information control measures. measures in case of spread of danger, localization of sources of spread of danger, decision-making on recovery without quarantine regime in the region of hostilities, impact on the situation, analysis of the effectiveness of preventive measures and emergency prevention measures. The location of the units on six hierarchical levels, connected by logical connections, provides information on the state of the region in conditions of limited energy supply through the elements of the monitoring system and impact on the region through the system of performers, providing a continuous management process to minimize the consequences of emergency. The obtained results allow, using innovative information and technical methods of emergency prevention of such nature, to further form a modern method of emergency response in conditions of limited energy supply of emergency response measures, supplementing the above methods with an effective crisis management system. Purpose – Emergency relief supply chain (ERSC) design is an important strategic decision that significantly affects the overall performance of emergency management activities. The performance of an ERSC can be measured by several performance measures some of which may conflict with each other. The purpose of this paper is to propose an ERSC design framework by simultaneously taking total logistics cost (TLC), risk level, and amount of demands covered in an ERSC into consideration. Design/methodology/approach – The study considers TLC of an ERSC as the sum of logistics cost from distribution warehouses (DWHs) to Break of Bulbs (BOBs) and from BOBs to affected neighborhoods. The risk level of an ERSC is measured by estimating the expected number of disrupted relief items (EDI) distributed from DWHs through BOBs to neighborhoods. The covered demand (CDM) is defined as total populations that are supported in case of an emergency, the populations within the maximal coverage distance (MCD) from relief facilities. Based on these performance measures, the authors formulate a Goal Programming (GP) model to distribute emergency relief items to affected locations. Ideal values of these performance measures are decided, and the GP model seeks to minimize the weighted sum of the percentage deviations of those performance measures from the ideal values. The relationships among performance measures have been thoroughly analyzed through detailed trade-off studies under two realistic case studies by changing weights of each performance measure. Findings – Three performance measures are interdependent over specific values of weights. TLC and EDI have a trade-off relationship when the weight on each measure increases. TLC and CDM also have a trade-off relationship when the weight on EDI increases. However, this relationship becomes less apparent when the MCD increases. EDI and CDM also have the same trade-off relationship when the weight on TLC changes. Therefore, decision makers should thoroughly analyze these trade-off relationships when they design ERSCs. Overall, the study identified that an ERSC with higher MCD outperforms one with lower MCD in terms of TLC, EDI, and CDM. Originality/value – The study presents a design framework to generate more balanced ERSCs by simultaneously taking three conflicting performance measures into consideration, and demonstrated the feasibility of the framework through realistic case studies. The trade-off analysis provides useful insights and theoretical knowledge to researchers and practitioners in the discipline of emergency logistics management. The results from this study are expected to contribute to the development of more balanced ERSCs.

It is 5 pm on New Year’s Eve and you are on service as an emergency physician in training for the London Helicopter Emergency Medical Service (HEMS). The London Ambulance Service Control informs you of a stabbing incident in the town of Watford, England. Your helicopter arrives on the scene 10 minutes after the call and 13 minutes after the incident, along with land ambulance and police. After the scene is declared safe, you are directed to the patient, JJ, a 24-year-old male who has suffered a stabbing outside of a local bar. His friend, who made the call to EMS, tells you JJ lost consciousness about 3 minutes after the insult. The patient has no signs of life, with no palpable carotid pulse or measurable blood pressure. He is apneic and his pupils are fixed and dilated. You notice a 2-cm wound in the midclavicular line, just superior to the left nipple, consistent with a penetrating stab wound. Cardiac monitors are attached and show an initial rhythm of asystole. Closed cardiac massage is undertaken immediately with rescue breaths given using bag-valve-mask. Simultaneously, the patient is cannulated in both antecubital fossae and is administered 1 mg adrenaline with 1000 mL 0.9% saline. Emergent decompressive needle thoracotomy is performed on the left chest by placing a 14-gauge, 3.5-inch angiographic catheter into the chest cavity at the midclavicular line in the second interspace. There is no release of air but periodic bubbles and blood appear to come from the left lung. After 2 minutes of unsuccessful CPR, there is still no palpable pulse and you notice the patient’s neck veins are distended, with normal air entry and no dullness to percussion in the left hemithorax. Your helicopter is about 10 minutes away from the nearest major trauma centre.

Objective: To determine the efficacy of a fixed dose of ultrasound energy to treat acute lateral ligament sprains of the ankle joint. Study design: Double-blind randomised controlled trial. Setting: Accident and Emergency department of University Teaching Hospital. Subjects: Patients presenting at Accident and Emergency with ankle injuries. Intervention: Ultrasound or placebo, and Tubigrip. Outcome measures: Pain measured with visual analogue scales, swelling using a tape measure, range of movement using a fluid-filled goniometer, and weight bearing using two scales simultaneously. Results: Patients in both groups improved symptomatically. There were no statistically significant differences between groups in any outcome measure. Within groups, statistically significant differences were detected in pain perceived, and range of movement (dorsiflexion). Conclusion: At the dose and duration used, ultrasound therapy is no better than placebo in the management of lateral ligament injuries.

Emergency management is crucial to finding effective ways to minimize or even eliminate the damage of emergent events, but there still exists no quantified method to study the events by computation. Statistical algorithms, such as susceptible-infected-recovered (SIR) models on epidemic transmission, ignore many details, thus always influencing the spread of emergent events. In this paper, we first propose an agent-based modeling and experiment framework to model the real world with the emergent events. The model of the real world is called artificial society, which is composed of agent model, agent activity model, and environment model, and it employs finite state automata (FSA) as its modeling paradigm. An artificial campus, on which a series of experiments are done to analyze the key factors of the acute hemorrhagic conjunctivitis (AHC) transmission, is then constructed to illustrate how our method works on the emergency management. Intervention measures and optional configurations (such as the isolation period) of them for the emergency management are also given through the evaluations in these experiments.

Objective: To evaluate, through care indicators, the quality of services rendered to patients considered urgency and emergency cases at an advanced emergency care unit.Methods: We analyzed data from managerial reports of 64,891 medical visits performed in the Emergency Care Unit of the Ibirapuera Unit at Care during the period from June 1st, 2012 through May 31st, 2013. The proposed indicators for the assessment of care were rate of death in the emergency care unit; average length of stay of patients in the unit; rate of unplanned return visits; admission rate for patients screened as level 1 according to the Emergency Severity Index; rate of non-finalized medical consultations; rate of complaints; and door-to-electrocardiogram time.Results: The rate of death in the emergency care unit was zero. Five of the 22 patients classified as Emergency Severity Index 1 (22.7%) arrived presenting cardiac arrest. All were treated with cardiopulmonary resuscitation and reestablishment of vital functions. The average length of stay of patients in the unit was 3 hours, 33 minutes, and 7 seconds. The rate of unscheduled return visits at the emergency care unit of the Ibirapuera unit was 13.64%. Rate of complaints was 2.8/1,000 patients seen during the periodConclusion.: The model of urgency and emergency care in advanced units provides an efficient and efficaious service to patients. Both critically ill patients and those considered less complex can receive proper treatment for their needs.

AbstractConservation measures applied to sea turtle nesting sites have a beneficial effect on population trends and dynamics. Such measures aim to protect nesting females, increase nesting success (proportion of female emergences resulting in nests) and/or to improve hatching and hatchling emergence success. However, taking into account financial and time constrains it is important to identify those measures that have the most positive impact on the sea turtle population. The aim of this paper is to assess and compare the relative importance of the different factors that may influence the efficiency of conservation actions and to investigate which factors, those associated with decreased nesting success, or others leading to higher embryonic and hatchling mortality have a higher impact on overall hatchling recruitment. We developed a model that simulates the nesting activity of sea turtles. For model parameterization, we used data collected from nesting sites of the loggerhead sea turtles (Caretta caretta) in the Eastern Mediterranean. We conducted a series of simulations by simultaneously changing model input parameters. The results of the model illustrate that an increase in hatchling recruitment success (i.e., hatching and hatchling emergence success) would have a more positive effect on overall hatchling production than a similar in nesting success. Our analysis further suggests that changes in hatchling recruitment success even at a single site, could have an important impact on overall hatchling production of the rookery.

Critical deterioration events (CDEs) and emergency transfers (ETs) are two proximal measures to cardiopulmonary arrest, and both aim to evaluate how systems recognize and respond to clinical deterioration in children. This retrospective observational study sought to (1) characterize CDEs and ETs by timing, overlap, and intervention category, and (2) evaluate the performance of the watcher identification system and the pediatric early warning score (PEWS) to identify patients who experience these events. A total of 359 CDEs and 88 ETs occurred during the study period. Respiratory events were most common and accounted for 80.5% of CDEs and 47.7% of ETs. A narrow majority of patients were identified as watchers (55.4% of CDEs and 51.1% of ETs). In total, 85.5% of CDEs and 87.5% of ETs were identified as watchers, elevated PEWS, or both. Opportunities exist for improved escalation plans for high-risk patients to prevent the need for emergent intervention.

BackgroundThe most obvious deficiency in the current evaluation of disaster response is the lack of objective, quantifiable measures of performance. This frequently leads to assessments that are highly subjective depending on the evaluator, does not provide those who are planning with targets to achieve, and does not allow for measures that they have improved their preparedness. The goal of this research project is to offer recommendations for government agencies at the federal, regional, and local levels, public health departments, and health care institutions to aid in the development of pediatric emergency management performance measures.InterventionsThe goal was achieved through the application of traditional quality principles to the assessment of emergency management efforts and to the use of innovative analytic methodologies to develop comprehensive approaches to performance measurement in emergency management.Discussion and ObservationsWhen one discusses performance measures, it is important to remember that these are metrics we use to improve the quality of care. With regard to emergency management, performance measures are used to increase capacity and efficiency. A classic approach to health care performance measures is to discuss them with regard to the domains of structure, process, and outcome. Recently, in addition to these domains, volume has also become an important predictor of clinical outcomes. Although we believe that these domains can be applied to emergency management functions and the development of performance measures for disasters, there are some fundamental differences when compared with their use in development and categorization of traditional health care metrics which have been built in to our modification of these domains to emergency preparedness. This approach, quantitative methodology and consensus development process, when applied, will significantly advance pediatric preparedness. Ultimately, these pediatric specific measures must exist and be used to assess current levels of performance and guide resource allocation and targeted improvement efforts.

This study dealt with the arbitrator of emergency in commercial arbitration and this development is the most innovative in the rules of commercial arbitration of the International Chamber of Commerce in Paris in 1912, where a new trend was adopted with regard to interim and urgent measures before the final form of the arbitral tribunal. The International Chamber of Commerce has already adopted the rules of this system before the Arbitration Institute of the Stockholm Chamber of Commerce. The emergence of new systems in commercial arbitration needs to be examined in order to understand, interpret and check its suitability to the needs of the parties to the dispute in the framework of commercial arbitration. Among the most important new systems are the rules of emergency arbitrator, which have been taken by many commercial arbitration centers because of the advantages of one or both parties when it needs urgent and incidental measures that cannot wait until the formation of the arbitral tribunal. Therefore, the appointment of an emergency arbitrator may be requested for such measures. Many of the centers have organized such rules as Stockholm Rules of Arbitration in Sweden and ICC in Paris and others. The study concluded with a set of results, the most important of which was that the emergency arbitrator is one of the modern rules that serve commercial arbitration, which began by relying on Western legal systems not Arab laws.

Con la presente investigación se busca principalmente establecer un perfil de ansiedad, estrés y estados de ánimo del Personal Especialista en Extinción de Incendios Forestales (PEEIF) o bomberos forestales tanto a nivel general como en situaciones de emergencia. La muestra se compuso de 24 sujetos a los que se administró una batería de cuestionarios en tres ocasiones (sin emergencia, primer a emergencia y última emergencia), compuesta por un cuestionario sociodemográfico adhoc, el Perfil de Estados de Ánimo (POMS); y el Inventario de ansiedad estado-rasgo (STAI- E/R), realizando un análisis descriptivo y comparativo a través de la prueba t-student y comparaciones múltiples con Anova de medidas repetidas. Se observó que los miembros PEEIF, a nivel general, tenían valores bajos de ansiedad estado, ansiedad rasgo, tensión, depresión, cólera, fatiga y confusión, mientras que el vigor fue elevado. En situaciones de emergencia, los valores se vieron modificados en su totalidad, debido a la fuente de estrés que supone. Se demuestra que el trabajo de extinción de incendios forestales se caracteriza por tener efectos negativos a nivel emocional sobre sus profesionales, mediados por la experiencia, cargo y condiciones familiares. Abstract: The present research mainly seeks to establish a profile of anxiety, stress and moods of the Forest Fire Extinction Specialist Personnel (PEEIF) or wildland firefighters both at a general level and in emergency situations. The sample was made up of 24 subjects who were administered a battery of questionnaires on three occasions (no emergency, first emergency and last emergency), composed of an ad-hoc sociodemographic questionnaire, the Profile of Mood States (POMS); and the State-Trait Anxiety Inventory (STAI-E / R), carrying out a descriptive and comparative analysis through the bat-student test and multiple comparisons with repeated measures Anova. It was observed that the PEEIF members, at a general level, had low values ​​of state anxiety, trait anxiety, tension, depression, anger, fatigue and confusion, while vigor was high. In emergency situations, the values ​​were modified in their entirety, due to the source of stress involved. It is shown that forest fire extinguishing work is characterized by having negative emotional effects on its professionals, mediated by experience, position and family conditions.

Purpose – The purpose of this paper is to show how elements from queueing theory can be used to obtain objective measures of effective capacity in the triage function at Skaraborg Hospital in Sweden without direct observation of the function itself. Design/methodology/approach – Approximately 30,000 patients arrived to the emergency department at Skaraborg Hospital in Sweden during 2011. The exact time of arrival and the exact time of triage were recorded for each patient on an individual level. Basic queueing theory uses arrival rates and system capacity measures to derive average queueing times. The authors use the theoretical relation between these three measures to derive system capacity measures based on observed arrival rates and observed average queueing times. Findings – The effective capacity in the triage process is not a linear function of the number of nurses. However, the management of capacity seems well adapted to the actual demand, even though service levels vary substantially during the day and night. Originality/value – This paper uses elements from queueing theory in an innovative way to measure the effective capacity of a service process without direct observation, thereby also avoiding the potential risk of the Hawthorne effect.

The weakness of supervision and management measures in railway emergency rescue led to difficulties of whole of monitoring and disposal on the completion time and effect. In order to solve this problem, the GERTS stochastic network schedule is introduced to the process supervision of railway emergency rescue based on the analysis of process of railway emergency rescue. With critical procedures serving as the nodes and execution process as the edges of the network, the random network model is thus formed. In case of emergency, the simulation of GERTS random network can be used to predict necessary rescue time and disposal efficiency, with previous rescue time and effect for reference. The analysis of the case shows that the simulation obtained by this method is consistent with the practical situation of the rescue. Measure standards from this method can be acquired to evaluate the work schedule and efficiency, efficiently supervising the delay of different divisions during the rescue. The proposed measure improves the efficiency and handling capacity of railway emergency rescue, having certain theoretical guidance significance and application value on railway emergency rescue.

The paper discusses the preventive measures carried out in the penitentiary institutions of foreign countries, preventing the penetration and spread of coronavirus infection. Persons serving sentences in places of detention are at increased risk of infection in the event of an outbreak of the disease. Their situation requires separate consideration in planning and responding to crises. Measures to ensure social distancing are implemented through a special legal regime, the introduction of which limits the subjective rights of convicts. The introduced legal restrictions in some states provoked the emergence of criminal emergencies, which required the optimization of criminal and penal legal relations. Due to the emergency in the healthcare sector, it seems possible to use such institutions of criminal law as release from serving a sentence, deferment from serving a sentence, replacing the unserved part of a sentence with a milder type of punishment as an exceptional measure, and developing alternative ways to maintain socially useful ties. These methods include: increasing the duration of calls in correctional facilities, conducting visits through video conferencing, organizing a prompt exchange of information on the health status of relatives and convicts using a hotline, and using secure mobile devices.

In his lecture onThe Repressed State of Emergency, Ernst-Wolfgang Böckenförde argues for the constitutional possibility of a partial superimposition of certain laws in times of emergency. Repressing the state of emergency would ultimately lead to greater and more permanent damage to the rule of law and liberal rights. This warning bears some similarities to Giorgio Agamben's analysis of the emergence of a permanent state of emergency, which loses the feature of exceptionality by becoming “the dominant paradigm of government in contemporary politics.” Agamben thus proposes a shift from structure to practice, which focuses on the features of emergency politics instead of states of exceptions. The European Union's recent financial crisis is a promising object of research in this regard. After a brief discussion of Böckenförde's illuminating lecture on the repressed state of emergency, this Article will analyze a comment written by Böckenförde at the beginning of the Euro crisis. While this comment has sometimes been interpreted as an unconditional defense of unlawful emergency measures, a closer look at this statement reveals a much more nuanced argumentation. Böckenförde concurs with other legal scholars—such as Paul Kirchhof—in emphasizing the danger of an erosion of the rule of law through illegal measures. Böckenförde's intervention, however, leaves room for interpretation that this emergency action could—in principle and under specific circumstances—nevertheless be legitimate, although he was critical towards the handling of the crisis. On a more general level, this Article will discuss if the suspension of law can ever save the legal order it disregards. Next, the legality of EU crisis management—which has not been judged unanimously—will be scrutinized. At least, the circumvention and adaptation of EU primary law involved in this management has crucially changed the law of “normal affairs.” Finally, this Article will argue that the managerial style of emergency politics challenges the EU's project of “integration through law,” hollows out democratic procedures, and enforces, with its one-sided austerity policy, a form of economic integration that bears strong traces of an “authoritarian constitutionalism.”
