Lead (Pb) is a highly potent neurotoxin, especially for children. A vast epidemiological literature documents the effects of Pb exposure in young children. Pb can cross the placental barrier to induce early fetal exposure. This prenatal exposure is associated with impaired cognitive function in children, resulting in learning problems, decreased intelligence quotient (IQ) and behavioral problems. For obvious ethical reasons, it is not possible to measure Pb concentrations in the fetus during pregnancy. Consequently, a single measurement obtained from umbilical cord blood at delivery is available to assess prenatal exposure. Thankfully, Pregnancy physiologically based pharmacokinetic (p-PBPK) models can be used to simulate prenatal exposures throughout pregnancy. The aim of this thesis is to provide a tool capable of simulating internal fetal exposures, from incomplete data, such as those obtained during biomonitoring campaigns.A literature search was carried out on the impact of pregnancy on the ADME processes of Pb in both the mother and fetus. This research highlighted an increase in maternal bone turnover (i.e. cycle of formation and resorption) during pregnancy. Indeed, urinary markers of bone formation and resorption linearly increase from the 2nd trimester until the end of pregnancy. Furthermore, data on the kinetic of Pb during pregnancy reveal a distinctive U-shaped trend, an initial phase of decrease during the first 4 months followed by a second phase of increase in maternal blood lead levels. The parameter values modeling bone turnover and placental transfer of Pb were calibrated using literature data. The predictive performance of our p-PBPK model was assessed using paired concentrations of Pb in umbilical cord and maternal blood at delivery, demonstrating satisfactory performance between the model predictions and the observed data. A global sensitivity analysis of the model highlighted à strong influence on fetal exposures from the Pb binding to erythrocytes, Pb accumulation in bones and the partition coefficient in the brain.Pb concentrations measured in the umbilical cord at delivery, from the French Longitudinal Study since Infancy (Elfe), were used in the p-PBPK model to simulate Pb concentrations in the fetal brain. Simulations carried out on the Elfe cohort reveal variations in predicted Pb concentrations in fetal brain over the course of pregnancy. By the end of pregnancy, almost half the fetuses in the Elfe cohort had predicted brain concentrations likely to induce in vitro and ex vivo effects on neurodevelopmental.In conclusion, this thesis highlights the potential of p-PBPK models for assessing prenatal exposure. We have demonstrated the ability of our model to simulate fetal exposure indicators, taking into account the physiological mechanisms essential to Pb toxicokinetics during pregnancy. In the future, this model can be coupled with an effect model such as an Adverse Outcome Pathway (AOP) to help predict the effects of Pb on the neurodevelopment of children

Electromagnetic fields in the environment - whether extremely low frequency or radiofrequency - are responsible for continuous population exposure. However, the potential impact of exposure to electromagnetic fields on neonatal health during pregnancy is still not well known, in particular due to the difficulties for epidemiological studies to assess in sufficient detail and precision the exposure to electromagnetic fields, which is ubiquitous. The first objective of this thesis was to study the relationship between maternal exposure to extremely low frequency electromagnetic fields and two indicators of neonatal health: prematurity and small for gestational age. The second objective was to build a tool for quantitative estimation of occupational exposure to radiofrequencies. The work carried out in response to the first objective is based on data from two French birth cohorts. Cumulative exposure to extremely low frequency electromagnetic fields during pregnancy was assessed using a job-exposure matrix. The results were in favor of an absence of association with the risk of small for gestational age. However, for the risk of spontaneous prematurity, a modest association was reported among the most exposed mothers up to 28 weeks of gestation. The work carried out in response to the second objective is based on an international collaboration. On the basis of the job calendars collected in an international case-control study and exposure measurements extracted from the literature, a first quantitative job-exposure matrix to estimate occupational exposure to high frequency electromagnetic fields in epidemiological studies was created

Informing research participants is a fundamental ethical principle. . While information delivered during the inclusion in research has been described in numerous recommendations and guides, initiatives for providing general information during the course of research are more rarely considered whereas it represents a duty to respect the participants. When it exists, post-inclusion information is mainly seen in terms of return of the overall results at the end of the research. In some contexts, such as cohorts and long-term studies, the need to disclose general information during the course of research, in order to keep participants informed of the progress of the research and its developments, begins to emerge. The goal of this study was to explore the viewpoints of patient representatives, research professionals and patient groups affected by the question of information after inclusion, in the context of two cohorts of HIV-infected patients (AQUITAINE and COPILOTE) from the French Agency for Research on AIDS and Viral Hepatitis (ANRS). Face to face semi-direct interviews were conducted with 1) members of the TRT-5 group, a coalition of HIV patient associations working close with the ANRS to improve access to research information; 2) professionals of two cohorts (AQUITAINE and COPILOTE) identified with the help of the principal investigators and 3) a representative of the ANRS designated by the head of the agency. The interviews were fully transcribed and citations that may highlight clues to improving the process of information are reported. Patient groups were interviewed by questionnaire. Eleven patient representatives; 16 professionals (principal investigators, clinicians, researchers and the representative of the ANRS) and 134 patients agreed to participate. A qualitative analysis of interviews was conducted and compared to patient’s responses to the questionnaire. The results showed that written information is a relevant tool which: 1) helps to palliate the perceived lack of information; 2) participates in the education of participants; 3) helps to distinguish between the care and the research and 4) motivates professionals to communicate about research. Despite this relevance, written information should not be a substitute for verbal information, but should complete the latter. Oral information helps, indeed, to better address the heterogeneous nature of participants' expectations. This work supports the idea that the development of tools aiming the delivering of general information on ongoing research, even if it does not necessarily correspond to the preferred approach of participants, is essential to show them respect, to stimulate their interest and commitment in research and finally to initiate spoken information from professionals. Although the specific context of the work (the participants are patients followed by professionals in the field of medical care) makes it difficult to generalise the results, it is believed that it is essential to share initiatives and empirical studies such as that carried out here to better understand the content of information that can, or should be issued during the research and its dissemination. This must indeed be balanced with the difficulties of developing such information for professionals

Context. Maternal BMI at conception and weight gain during pregnancy were related in numerous studies with offspring’s birth weight and postnatal growth, as well as with mothers’ and children’s obesity and metabolic risk later on. Nevertheless, few studies addressed the weight trajectory of the mother before pregnancy and its association with children’s growth and adiposity.Objective: To study the associations between different parameters of maternal weight history before and during pregnancy and their relations with the risk of gestational diabetes and with children’s BMI and adiposity.Populations. We used data from two French mother-child cohorts: ELFE and EDEN. The ELFE study (Etude longitudinale française depuis l’enfance) included 18329 newborns in a random sample of 344 maternity wards in 2011 in mainland France. The EDEN cohort (study of pre- and early postnatal determinants of child development and health) recruited 2002 pregnant women in the maternity wards of Nancy and Poitiers between 2003 and 2006. We used data from obstetric files and from questionnaires filled-in by parents in both cohorts. In addition, the EDEN cohort had data from clinical examinations (of mothers during pregnancy and after delivery and of children at birth and at 5-6 years of age), including the estimation of children’s body composition by bio-impedancemetry at 5-6 years.Results. In the ELFE study, either maternal dieting to lose weight or an important weight gain or loss in the year before pregnancy were associated with a higher weight gain during pregnancy, independently of maternal pre-pregnancy BMI and socioeconomic status. The positive association between weight loss before pregnancy and weight gain during pregnancy was stronger in women who were obese or who had reported weight-reducing diets before pregnancy. Women who had gained an important amount of weight in the year before pregnancy had a higher risk of gestational diabetes, independently of the BMI reached at the beginning of pregnancy.Our analyses on the EDEN cohort showed that maternal pre-pregnancy BMI was positively related with children’s BMI, fat mass percent and central adiposity at 5-6 years of age. These associations were observed independently of women’s weight gain during pregnancy and socioeconomic status. A more thorough analysis of the shape of these relations showed associations essentially at the extremities of the maternal BMI range. Gestational weight gain was positively associated with children’s BMI Z-score at 5-6 years, after adjustment for maternal pre-pregnancy BMI, especially in women who were thin before pregnancy. Additional adjustment for children’s birth weight decreased the force of associations between children’s BMI Z-score and both maternal pre-pregnancy BMI and gestational weight gain, but both associations remained statistically significant.Conclusion. Important weight loss before pregnancy, especially if intentional, seems to enhance a compensatory increase in gestational weight gain. Important weight gain before pregnancy, beyond its role in maternal BMI reached before pregnancy, might be considered as an independent risk factor of gestational diabetes. At 5-6 years, the persistent effect of the intra-uterine environment on children’s BMI and adiposity is only detected in women who were thin or severely obese before pregnancy. Additional studies on children are necessary in order to conclude on weight loss recommendations before pregnancy in overweight and obese women

La anormalidad que representa el fenómeno interesa a la comunidad educativa de la institución y, por ende, a sus directivos académicos que, en general, formulan hipótesis para explicar el fenómeno mediante el análisis de los factores socio-económicos y demográficos que incidirían en el rendimiento académico de los alumnos y sus decisiones de abandonar los estudios universitarios, quedando constante el resto de las variables.
