This paper describes successful clozapine treatment in a young woman with a five year history of treatment-resistant schizophrenia. After various treatment-resistant strategies had been given, clozapine was commenced. Symptom resolution within 4 months and sustained high level of functioning within 7 months was achieved. The role of clozapine in general psychiatric practice is evident. Clozapine should be considered earlier in the illness course than was formerly the case.

Background: Psychiatric patients are subjected to considerable stigmatization, in particular, because they are considered aggressive, uncontrollable, and dangerous. This stigmatization might influence the approval of coercive measures in psychiatry by the public and healthcare professionals and might have an influence on the clinical practice of coercive measures. We examined whether the general approval of coercive measures for psychiatric patients with dangerous behaviors differs from case-specific approval. Method: We conducted a representative survey of the general population (n = 2207) in the canton of Basel-Stadt, Switzerland. In total, 1107 participants assessed a case vignette depicting a fictitious character with a mental illness and indicated whether they would accept coercive measures (involuntary hospitalization, involuntary medication, and seclusion) for the person in the vignette. It was explicitly stated that within the last month, the fictitious character displayed no dangerous behavior (Vignette ND) or dangerous behavior (Vignette D). Another 1100 participants were asked whether they would approve coercive measures (involuntary hospitalization, involuntary medication, and seclusion) for psychiatric patients with dangerous behavior in general (General D), i.e., without having received or referring to a specific case vignette. Findings: The logistic regression model containing all variables explained 45% of the variance in approval of any type of coercive measures. Assessment of case vignettes without dangerous behavior (Vignette ND) was associated with significantly reduced approval of coercive measures compared to assessment of a case vignette with dangerousness (Vignette D), while approval for coercive measures in a person with mental health disorder with dangerous behavior in general (General D) was significantly higher than for the case vignette with dangerousness. Conclusions: The general approval of coercive measures for people with mental disorders seems to differ depending on if the respondents are asked to give a general assessment or to examine a specific and detailed clinical case vignette, indicating an increased role of stigmatization when asking about generalized assessments. This may contribute to diverging findings on the acceptance of coercive measures in the literature and should be considered when designing future studies.

Previous observational studies have indicated that social influences, such as arising from herding-like behaviour, can contribute to medical errors. In this study, we experimentally examined whether general practitioners (GPs) would follow incorrect prescription recommendations from fellow GP or specialists. To investigate this, we conducted an online survey with 475 GPs practicing in England that included two case vignettes. Case vignette 1 focused on sleeping tablets, and case vignette 2 was centred around antibiotics. The vignettes were presented in random order, and within each vignette, study participants were assigned to one of three experimental conditions: control condition (lacking peer recommendation), fellow GP condition (including a recommendation from a fellow GP not aligned with best practice clinical guidelines), or specialist condition (including a recommendation from a specialist not aligned with best practice clinical guidelines). The primary outcome measure was the proportion of GPs who prescribed medication that deviated from best practice clinical guidelines. We found that, in both case vignettes, the percentage of respondents prescribing such medication was highest in those assigned to the specialist condition, followed by those assigned to the control condition. It was lowest in those assigned to the fellow GP condition (case vignette 1: 73.8% vs. 55.6% vs. 36.6% and case vignette 2: 24.0% vs. 12.4% vs. 10.1%). In the case of vignette 1, the difference between the fellow GP condition and the control condition is statistically significant, suggesting that GPs are less likely to prescribe sleeping tablets when recommended by a fellow GP. This implies that GPs are more inclined to prescribe non-guideline-recommended medication when advised by specialists. This study is the first to experimentally demonstrate that physician herding behaviour can result in prescription errors. Future research could extend this inquiry to diverse contexts, including diagnosis.

Objective: The aim of this study is to determine whether physicians have an age bias regarding sexual dysfunction in older vs. younger patients in terms of diagnosis, attributed etiology, proposed treatment and perceived prognosis. Design and Method: An on-line survey consisting of one of two, randomly administered, case vignettes, which differed only by age (28 or 78). In both cases, the patient was described as suffering from occasional erectile dysfunction with a clear psychosocial indication. A total of 236 physicians responded to the survey. Overall, 110 physicians received an old patient vignette and 126 physicians received a young patient vignette. Results: Even though both cases presented with a clear psychosocial etiology, the young patient vignette was more likely to be diagnosed with performance anxiety, whereas the old patient vignette with erectile dysfunction. The old patient vignette dysfunction was more likely to be attributed to hormonal changes, health problems and decreased sexual desire. Physicians were more likely to recommend hormonal and PDE5 inhibitors (PDE5i; such as Sildenafil; Vardenafil; Tadalafil) treatment as well as urology referral to the old patient vignette. The young patient vignette was more often referred to a sexologist and received a more positive prognosis than the older patient. Conclusions: This study demonstrates an age bias among physicians regarding sexuality in later life. Of particular note is the increased prescription of PDE5i to the older patient, despite the clear psychosocial indication presented in the case vignette.

The illusory truth effect (ITE) is the tendency to believe false information as being accurate after it has been presented repeatedly over time. ITE has been shown to hold true in many different contexts; however, there have been no studies that examine the influence of ITE in jurors’ deliberation. Given the importance of weighing legally relevant facts in the decision-making process, and the potential influence of ITE, this study examined whether the repetition of key evidence in testimony matters in this context. This study also examined whether critical information would be influenced by the location of ITE. In that context, jurors may process critical information differently when introducing ITE early (i.e., primacy effect) or later (i.e., recency effect) in the vignette of a murder case. To examine this effect, 100 participants were recruited and asked to read a vignette where pertinent evidence related to a murder was strategically repeated throughout the case narrative. Participants were assigned to one of four groups: control; ITE throughout vignette; ITE at the beginning of vignette; and ITE at the end of vignette. After reading the vignette, participants were asked to complete a short questionnaire and provide a final decision about various aspects of the case. Results revealed that repetition of pertinent evidence matters. The placement of evidence also has the potential to influence jurors’ perceptions of certain case relevant details. These findings suggest that within a sensitive legal context, such as jurors weighing evidence of an accused’s culpability, ITE could alter one’s perception of the facts.

Using vignette experiments, this thesis examines individuals’ decision-making in various social dilemmas. A case study of littering behavior amongst university students in Lao People's Democratic Republic is used to investigate whether individual preferences are stable across littering dilemmas and other social dilemmas. This study further investigates if a visual prompt can encourage prosocial behavior in littering situations. The results show that behavior in social dilemmas is dependent on individual preferences. Additionally, the study finds little evidence that visual prompts could be used in order to efficiently decrease littering in Laos. However, a negative relationship is found between littering behavior and knowledge regarding the consequences of littering. These findings indicate that policy makers could use knowledge increasing campaigns in order to increase prosocial decision making regarding littering. Yet, further studies are needed in order to validate the results. Lastly, we also find differences in what influence littering behavior, depending on the item being littered.

Résumé : L'hémorragie du post-partum représente une des causes principales de morbi-mortalité maternelle en France. Malgré les progrès médicaux, la gestion optimale de l'hémorragie du post-partum reste un défi, notamment en raison de la complexité de coordonner différents traitements de façon pluri professionnelle dans un contexte d'urgence. Ce travail de thèse s'est concentré sur l'optimisation de la prise en charge de l'hémorragie du postpartum à travers une analyse approfondie des facteurs de risque et des pratiques professionnelles liés en particulier à l'utilisation des utérotoniques. Ce travail de recherche est structuré en trois volets. Tout d'abord, nous avons effectué une analyse des facteurs de risque associés à l'hémorragie du post-partum, en stratifiant nos analyses sur la parité afin de mieux en comprendre les déterminants et d'identifier les populations à haut risque grâce aux données d'un essai clinique randomisé. Ensuite, le deuxième volet a évalué les pratiques professionnelles des sage-femmes en mettant l'accent notamment sur l'utilisation parfois inappropriée de l'oxytocine en traitement de première intention en identifiant les écarts aux recommandations grâce à une enquête par vignette clinique diffusée aux sages-femmes françaises.Enfin, ce travail a permis d'identifier les facteurs d'échec du traitement par sulprostone, utérotonique de seconde ligne, ayant pour but de réduire les procédures invasives génératrices d'une morbidité importante à l'aide une analyse statistique bayésienne, à partir des données d'une cohorte bicentrique.En conclusion, ce travail fournit des pistes de réflexions pour une détection et une prise en charge optimisée de l'hémorragie du post-partum, en mettant l'accent sur l'identification précoce des facteurs de risques modifiables tel que l'utilisation appropriée des utérotoniques
Abstract : Post-partum hemorrhage is one of the most important causes of maternal morbidity and mortality in France. Despite medical advances, the optimal management of post-partum hemorrhage remains a challenge, not least because of the complexity of coordinating different treatments in a multi-professional emergency setting. This thesis focused on optimizing the management of postpartum hemorrhage through an in-depth analysis of risk factors and professional practices, particularly in relation to the use of uterotonics. This research is structured in three parts. Firstly, we carried out an analysis of the risk factors associated with postpartum hemorrhage, stratifying our analyses by parity in order to gain a better understanding of the determinants and to identify high-risk populations using data from a randomized clinical trial. Secondly, this study assessed the professional practices of midwives, focusing in particular on the sometimes-inappropriate use of oxytocin as a first-line treatment, by identifying deviations from recommendations using a clinical vignette survey distributed to French midwives.Finally, this work identified the factors leading to failure of treatment with sulproston, a second-line uterotonic, to reduce the invasive procedures that generate significant morbidity, using Bayesian statistics analysis based on data from a bicentric cohort.In conclusion, this work provides food for thought for optimized detection and management of post-partum hemorrhage, with the emphasis on early identification of modifiable risk factors such as appropriate use of uterotonics

This chapter examines seven case studies of the macabresque during the twentieth century. The macabresque is portrayed within each by adopting the “vignette” as a narrative form. A vignette is a patch, a semiotic sign, representative of a “mentality” that endows genocide and mass atrocity with a “noble” cause or “moral” if not, indeed, a “sacred” purpose. Similarities across cases of the macabresque emerge; but contrasts also appear that demonstrate major political, cultural, ideological, and attitudinal differences. These contrasts are not incidental side effects of violence. They reveal the relationships of social fantasy, specifically “thing-enjoyment” and mimetic desire and rivalry, in shaping not only ideological constructions of otherness but also the psychodynamics of a political ontology in which ideology is ontology. The case studies include the Armenian Genocide, Stalin’s purges, “Hitler’s Diabolical Laboratory,” blood trauma and the Rwandan Genocide, “Confessional Archives and Angkar’s Torture,” the Argentinian neo-inquisition, and “Bosnian Shame-Camps.”

Human capital investments are at the core of the SIA. In the knowledge-based service economy, the SIA is a promising model for how to ensure economic growth and social cohesion simultaneously through investments in human capital. Yet investing in human capital raises challenges that require attention towards how people accumulate skills and retain these over time. Due to particular features of human capital, this chapter argues, a comprehensive approach is needed that both incentivizes skill acquisition over the life course and protects acquired skills via policies that facilitate labour-market transitions, thus enabling reintegration into the labour market. The discussion elaborates on ways in which social investment policies are complementary to each other, with particular attention to how policies that invest in skills complement policies that protect these skill investments. Evidence for such institutional complementarities is assessed through descriptive statistics and a vignette analysis of the Swedish case.

More than any other clinical intervention, civil commitment is subject to strict legal requirements and oversight. Although the rules and processes vary by jurisdiction, the legal proceedings in all jurisdictions are designed to balance the autonomy interests and constitutional rights of the patient against the state’s legitimate exercise of authority. This chapter reviews the legal principles underlying civil commitment and describes how those principles are applied in different ways, using examples from various jurisdictions. Using a case vignette, it explores a common scenario in which an emergency room physician who must make a decision regarding a patient’s need for hospitalization obtains a psychiatric consultation, and it follows the civil commitment process as it would play out in one jurisdiction. By its nature, civil commitment is both a clinical intervention and a legal process, and the chapter addresses some of the clinical challenges encountered in the course of involuntary hospitalization.

By 1900, the medical phenomenon of neurasthenia had grown into an image of general disorder manifesting the malaise of modernity. Neurasthenia was always more than the psychological suffering of individuals – it was also the price society had to pay for progress. Thomas Mann, too, was influenced by his contemporaries‘ obsession with time. Especially in his early works, such as Buddenbrooks and Tonio Kröger, these subjects come together as if focussed in a burning glass: between degeneration and the psychology of the self, neurasthenia has become a symptom of a crisis of the bourgeois subject; from today's perspective, for example, the symptomatology of Thomas Buddenbrooks can be read as the first case vignette in the history of burnout. In this volume, however, the aim is not to apply the interpretation of neurasthenia as a contemporary illness around 1900 to the present, which application has by now turned into a simple template. Rather, it aims to interrogate the discourses surrounding neurasthenia and artistic potential as time-bound debates and to ask about the conditions of writing in the mirror of neurasthenia. This revisiting of a famous topos – illness and art in Thomas Mann – thus aims to take a new look at an old topic.

AbstractAddiction and its comorbid disorders lead not only to massive losses in lifestyle but also to a loss of values. On the other hand, therapy and thus healing always means a resumption of the value discussion. In the present case vignette, the change in values of an alcohol-dependent woman and her turn to the beautiful in the sense of a resource-oriented therapy are described. Enjoying beautiful things led the patient to increase the self-efficacy expectation; at the same time, it was the initial step to emerge from the victim role and to start a self-responsible life again. The Will to Beauty is an integral part of the Orpheus Programme, a resource-oriented therapy for treating substance use disorders and behavioural addictions.

CHOPPA-M is a multi-resolution, multi-agent simulation framework developed to complement the existing CHOPPA (Combined Helicopter OPerations and Performance Analysis) framework. Like CHOPPA, it is designed to support operations research methods through simulation. It does so by presenting a consistent user interface and architecture for the modelling of autonomous systems across a variety of physical environments. These autonomous systems, or agents, may then participate in a pre-defined operational scenario. This describes CHOPPA-M's scene/vignette approach. In this paper, the background, architecture and user interface of CHOPPA-M are described. The development of some of the software's agent models are detailed and then employed in a case study: a maritime surveillance helicopter participating in a search and engage operation. From this case study, some conclusions are drawn. Finally, the future direction of CHOPPA-M is discussed.

This work is directed towards the improvement of programming skills for students on the 2nd semester of Electrical Engineering (EE) BEng signed up for the “Digital Electronics and Programming” (DEP) course. The goal is to support students who tend to drop out in the first half of the semester or give up and not show up at oral exams. In our research hypothesis we state: Decreasing and eliminating negative emotional experiences will increase student’s self-efficacy thereby lowering dropout. We experiment with a programming process guide in text and video to gain students' capability in solving programming problems and increase metacognitive awareness. Additionally, we measure students' emotional experience of programming by using a special self-assessment vignette inquiry. For comparison purposes we introduced the same measurement methodology in 2021 on the courses where there is no focus on the process guideline. The results show a positive effect on lowering the negative emotional impact when students use a programming process guide. The article describes the theoretical background based on literature studies for both the process and the students’ self-assessment, and discusses results achieved in relation to the findings in the literature. The preliminary results are described in [8] and in paper here we present recent outcomes for the autumn 2021 and spring 2022 semester over 9 weeks.
