Der volle Inhalt der QuelleHepatoblastoma (HB) is the most common liver tumor in children, the survival rate of patients has increased thanks to intensive chemotherapy. Unfortunately, this strategy creates lifelong side effects in survivors who become disabled, with one third ending up with hearing loss and subsequent disability.Another characteristic of this cancer is its low mutational burden, the main pathways that are dysregulated are the Wnt and Hippo pathways. β-catenin, the main effector of the Wnt pathway, harbors activating mutations in 80% of HB tumors. When β-catenin is exon 3 deleted, it removes its ability to be degraded by a destruction complex. β-catenin translocates into the nucleus where it acts as a co-transcription factor promoting the expression of pro-survival genes.Our work, on cell lines, patient samples and mouse models of HB, shows that an actin-binding protein called fascin-1 is an effector of β-catenin and exerts a pro-oncogenic effect. Fascin-1 is responsible for actin bundling that leads to the formation of cell structures such as filopodia and invadopodia, which facilitate cell migration and invasion. The role of fascin is particularly important during embryonic development, but fascin expression decreases in most adult epithelial tissues. Fascin is absent in hepatocytes but interestingly is re-expressed during tumorigenesis. We find it co-expressed with β-catenin in tumors and furthermore fascin is a marker of undifferentiated cells. Its knockdown with siRNA strategy leads to a decrease of cell growth in HepG2 cells.We sought to investigate the effect of fascin inhibition on various HB model cells and patient-derived xenograft cells. We target fascin actin bundling activity with a pharmacological inhibitor already in phase 2 clinical trials for solid tumors. Our study explores the effect on proliferation, cell death and senescence, invasion and migration, epithelial to mesenchymal transition and the underlying mechanism linking fascin to HB tumorigenesis, which could be the Hippo pathway.Our results show a clear effect of fascin inhibition on tumorigenesis with all the listed parameters and most interestingly we show a link between fascin inhibition and increased differentiation. This relationship has never been shown before. Furthermore, we show that Yap may be a potential downstream effector of fascin, closing the bridge between β-catenin and Yap in HB.Our results demonstrate that fascin inhibition in HB could be an interesting new application to explore for the ongoing clinical trial with NPG20444. We hope that positive results would lead to the inclusion of HB in this trial and ultimately that fascin inhibition would lead to the proposal of targeted therapeutic options for HB patients

Der volle Inhalt der QuelleAlthough considered a rare disease, pediatric cancers are the 2nd leading cause of death in children under the age of 14 in Western countries. Considerable progress was made in the 1970s, with an overall survival of 70% achieved in around twenty years. Nevertheless, certain subgroups are still associated with a poor prognosis, and the matter of treatment-related sequelae remains. These challenges partly stem from the fact that the majority of treatments for pediatric cancers were initially developed for adult cancers, even though we now know that they have their own particularities, notably in terms of biology. It is therefore important to identify the specificities of pediatric cancers to better target their vulnerabilities. The work of this thesis focused on the bioinformatic characterization of the molecular components involved in tumor aggressiveness, in two types of pediatric tumors: diffuse midline gliomas (DMG) and rhabdomyosarcomas (RMS). In both cases, we identified oncogenic signals involved in the acquisition of an invasive phenotype, and highlighted a new individual factor likely to play a prognostic role in RMS. Lastly, these studies are concluded by epistemological reflections on the role of multi-omics technologies in oncology, which have been central throughout this work

Der volle Inhalt der QuellePediatric high-grade gliomas (pHGG) and posterior fossa type A ependymomas (EPN-PFA) remain one of the biggest challenges in pediatric oncology. They exhibit vast inter- and intra-tumoral heterogeneity, complicating the development of effective therapeutic strategies. Then to improve their clinical outcome, we absolutely need pre-clinical models that recapitulate key features of their corresponding parental tumors. During my PhD, I developed an optimized protocol for the establishment and biobanking of pHGG organoids (pHGG_O) and EPN-PFA organoids (EPN-PFA_O). These models can be grown long-term and comprehensive histological and molecular analyses showed that they recapitulate inter- and intra-tumoral heterogeneity of their parental tumor even after several passages and cryopreservation as 3D cultures. I further showed that they can be employed to test responses to standard of care therapy as well as new therapeutic options, including drugs from clinical trials as they accurately capture the clinical phenotypes of their respective parental tumors. Moreover, these models led to the identification of the DRD2 inhibitor ONC201 as an unexpected potential therapeutic agent for H3K27M non-altered pediatric brain tumors and helped identify combination strategies to increase the therapeutic response to ONC201. Thus, those models are positioned to support powerful and innovative preclinical studies, particularly those related to personalized assessments of treatment response profiles and identification of novel efficient drug combinations

Der volle Inhalt der QuelleThis research stems from a ten-year experience of Adapted Physical Activities with children and teenagers who were hospitalised in an hemato-oncology ward. This research aims at describing and theorising the impact of ludic adapted physical activities on children, teenagers, the medical staff, and the members of the APA team. The unconscious aspects of the relationships between these intervenants have been studied, where individuals responded in their own way to the difficult time they had to go through. The research emphasises the importance, complexity and particularity of the pedagogical and human relationships - all the more because the APA are proposed to patients whose body also showed signs of suffering from a lethal disease. I attempted to define the nature of the necessary transference between the young patient and the person who engaged him in practising the APA. This was achieved in a constant to and fro between on-the-field practice and analysis of psychanalytical theory. The assumption was that it was necessary to analyse the APA sessions and their effects in light of the transference that had emerged between the patient and the APA teacher. The member of the APA team will then use his unconscious knowledge of this one to one relationship to adapt to the patient, and by this broaden and modify his/her theoretical approach to pedagogy and research

In order to increase cure rates and decrease complications and treatment sequelae, international collaborative efforts led to the development of therapeutic trials stratified on major prognostic factors including biological factors. However, treatment adaptation to individual patient characteristics remains marginal.In this thesis, our objective was to better understand how somatic (tumor-related) and constitutional (patient-related) characteristics could modify efficacy and toxicity of anticancer therapies used in pediatric oncology. Several works were performed: 1- Prognostic analysis of tumor markers: assessment of the alpha-foetoprotéine (AFP) decline prognostic value in childhood malignant germ cell tumors; 2- Prognostic analysis of constitutional factors: (i) assessment of the interaction between gender and type of alkylating agents on efficacy and acute toxicity; (ii) assessment of the efficacy and toxicity impact of genetic polymorphisms in patients with osteosarcoma treated with high-dose methotrexate; 3- Risk factors analysis of long-term toxicities: analysis of severe ototoxicity in the French Childhood Cancer Survivors Study (FCCSS)
