Individuals with hearing loss face high personal costs for hearing aids, making over-the-counter hearing aids, or headphones featuring custom audio control, alternative options. Apple Inc.’s AirPods Pro contain a variety of headphone accommodations designed to help people hear better. Recent research has evaluated electroacoustic properties of AirPods Pro demonstrating good ability to fit prescriptive targets for different hearing losses. Here, we examine the usability and perceived benefit from engaging custom audiogram-driven features in AirPods Pro. Listeners with self-assigned hearing profiles ranging from normal hearing to a moderate degree of hearing loss take a hearing screener (Mimi Hearing Test). AzBio sentences are presented in quiet and in noise while enabling and disabling the custom features in Apple’s Accommodation settings. Listeners then complete a questionnaire targeting usability of the features and perceived benefit. Overall, listeners found the features easy to use and to be at least a little helpful, regardless of hearing status. Listeners with suspected or known hearing impairment reported slightly greater benefit than normal hearing listeners when listening in noise with the custom audio features. Headphone Accommodations in AirPods Pro may be an affordable option for an improved listening experience. [Work supported by Western Washington University.]

Aging is a complex process with poorly understood genetic mechanisms. Recent studies have sought to classify genes as pro-longevity or anti-longevity using a variety of machine learning algorithms. However, it is not clear which types of features are best for optimizing classification performance and which algorithms are best suited to this task. Further, performance assessments based on held-out test data are lacking. We systematically compare five popular classification algorithms using gene ontology and gene expression datasets as features to predict the pro-longevity versus anti-longevity status of genes for two model organisms (C. elegans and S. cerevisiae) using the GenAge database as ground truth. We find that elastic net penalized logistic regression performs particularly well at this task. Using elastic net, we make novel predictions of pro- and anti-longevity genes that are not currently in the GenAge database.

Professional bicycle racing is a popular sport that has attracted significant attention in recent years. The evolution and ubiquitous use of sensors allow cyclists to measure many metrics including power, heart rate, speed, cadence, and more in training and racing. In this paper we explore for the first time assignment of a subset of a team’s cyclists to an upcoming race. We introduce RaceFit, a model that recommends, based on recent workouts and past assignments, cyclists for participation in an upcoming race. RaceFit consists of binary classifiers that are trained on pairs of a cyclist and a race, described by their relevant properties (features) such as the cyclist’s demographic properties, as well as features extracted from his workout data from recent weeks; as well additional properties of the race, such as its distance, elevation gain, and more. Two main approaches are introduced in recommending on each stage in a race and aggregate from it to the race, or on the entire race. The model training is based on binary label which represent participation of cyclist in a race (or in a stage) in past events. We evaluated RaceFit rigorously on a large dataset of three pro-cycling teams’ cyclists and race data achieving up to 80% precision@i. The first experiment had shown that using TP or STRAVA data performs the same. Then the best-performing parameters of the framework are using 5 weeks time window, imputation was effective, and the CatBoost classifier performed best. However, the model with any of the parameters performed always better than the baselines, in which the cyclists are assigned based on their popularity in historical data. Additionally, we present the top-ranked predictive features.

Apoptosis (type of programmed cell death) is an active process of cellular self-destruction in multicellular organisms. It is characterized by distinctive histomorphological, biochemical, and molecular features. Multiple cellular pathways trigger apoptosis, two of them are the best known: intrinsic and extrinsic. Multiple cellular signals and interactions can influence the course of apoptotic pathways. Bcl-2 family proteins play a key role in regulatory mechanisms of intrinsic apoptosis. Mitochondrial outer membrane permeabilization (MOMP) is an essential step for intrinsic apoptosis that is controlled by pro-apoptotic and anti-apoptotic members of Bcl-2 protein family. Pro-apoptotic effector proteins Bax and Bak represent the only Bcl-2 proteins inducing formation of MOMP, whose pores facilitate the subsequent releasing of several pro-apoptotic proteins from mitochondrial intermembrane space into cytosol. These proteins initiate a caspase cascade, resulting in rapid elimination of the doomed cells.

Macrophages show high plasticity and result in heterogenic subpopulations or polarized states identified by specific cellular markers. These immune cells are typically characterized as pro-inflammatory, or classically activated M1, and anti-inflammatory, or alternatively activated M2. However, a more precise definition places them along a spectrum of activation where they may exhibit a number of pro- or anti-inflammatory roles. To understand M1-M2 dynamics in the context of a localized response and explore the results of different mathematical modeling approaches based on the same biology, we utilized two different modeling techniques, ordinary differential equation (ODE) modeling and agent-based modeling (ABM), to simulate the spectrum of macrophage activation to general pro- and anti-inflammatory stimuli on an individual and multi-cell level. The ODE model includes two hallmark pro- and anti-inflammatory signaling pathways and the ABM incorporates similar M1-M2 dynamics but in a spatio-temporal platform. Both models link molecular signaling with cellular-level dynamics. We then performed simulations with various initial conditions to replicate different experimental setups. Similar results were observed in both models after tuning to a common calibrating experiment. Comparing the two models’ results sheds light on the important features of each modeling approach. When more data is available these features can be considered when choosing techniques to best fit the needs of the modeler and application.

Introduction: This study identified users’ indoor navigation concerns, prototype feature preferences, and perceptions of the relative importance and difficulty of certain building types in order to guide the development of an accurate and user-friendly indoor navigation application. Methods: Six-hundred fourteen adult volunteers who are legally blind from the 7,000-member Sendero global positioning system (GPS) electronic discussion group completed a 27-item survey instrument using Survey Monkey Pro online. Data were analyzed using chi-square and cross tabulation statistics. Results: Participants felt points of interest were the most important type of indoor information, that ability to know their location at any time the most important application feature, and verbal output with auditory and vibrational cues, the best output mode. Airports and bus or rail transit facilities scored highest as important buildings for navigation systems, and sports arenas and airports scored highest as difficult buildings to navigate. Chi-square analyses demonstrated that gender and GPS use were related to travel confidence. In addition, GPS use was related to user age, degree of reading vision, and employment. Discussion: The findings demonstrate user preferences for an indoor wayfinding application and high-priority building types. Regardless of stated preferences, it is important to allow the user to customize the application settings. The judgments on building importance of participants provide data on which to prioritize future site development. The chi-square analyses were exploratory in nature and designed to discover possible relationships. Implications for practitioners: Vision education and rehabilitation professionals can use the findings to understand the process of accessible indoor navigation and structure lessons accordingly. Developers can improve their products. Both groups can know and understand the indoor wayfinding perceptions and opinions of more than 600 individuals who are legally blind.

Today, modern ballroom dance is the priority direction of the development of modern choreographic art, in contrast to folk and pop art. Nowadays, despite the existence of a sufficiently large number of different directions of modern, folk, pop choreography, ballroom dance is becoming not only more and more popular, but also a dynamically developing direction. Therefore, special attention from the point of view of both historical and theoretical-methodical research of ballroom dance requires substantiation of its features and prerequisites for further development, both as a sport and as a choreographic art. The article examines the conditions for the formation of ballroom dancing, its main features, and determines the prospects for the further development of this type of choreographic art. Dance provides an opportunity for physical, aesthetic, spiritual development and education of both children and youth, as well as adults. Learning the best motor skills and skills in the training process is associated with the mobilization and active development of many physiological functions of the human body: blood circulation, breathing, neuromuscular activity, etc. Thanks to the fast rhythm, complex and varied technique, ballroom dancing helps a person to feel self-confidence, prevents the appearance of complexes, trains endurance, the vestibular apparatus, the respiratory system and the heart. In recent years, a new type of leisure activity in the field of ballroom dancing has been actively developing in Ukraine - dancing in the Pro-Am (Professional - amateur) format. Pro-Am is a direction in ballroom dancing where one participant is a professional teacher and the other is an amateur student. In addition to numerous trainings in the dance hall and demonstration performances, such a couple has the opportunity to participate in dance tournaments and festivals. Pro-Am competitions are held both as part of sports ballroom dance tournaments and as independent events. The Pro-Am system is a unique dance training system. An individual approach to each student makes it possible not only to teach him to dance technically correctly, but also to reveal individual capabilities. Everyone who studies has the opportunity to choose the direction that he likes the most and to study in the mode that suits him best, thus, the effectiveness of classes increases.

The classification of surface myoelectric signals (sEMG) remains a great challenge when focused on its implementation in an electromechanical hand prosthesis, due to its nonlinear and stochastic nature, as well as the great difference between models applied offline and online. In this work, the selection of the set of the features that allowed us to obtain the best results for the classification of this type of signals is presented. In order to compare the results obtained, the Nina PRO DB2 and DB3 databases were used, which contain information on 50 different movements of 40 healthy subjects and 11 amputated subjects, respectively. The sEMG of each subject was acquired through 12 channels in a bipolar configuration. To carry out the classification, a convolutional neural network (CNN) was used and a comparison of four sets of features extracted in the time domain was made, three of which have shown good performance in previous works and one more that was used for the first time to train this type of network. Set one is composed of six features in the time domain (TD1), Set two has 10 features also in the time domain (TD2) including the autoregression model (AR), the third set has two features in the time domain derived from spectral moments (TD-PSD1), and finally, a set of five features also has information on the power spectrum of the signal obtained in the time domain (TD-PSD2). The selected features in each set were organized in four different ways for the formation of the training images. The results obtained show that the set of features TD-PSD2 obtained the best performance for all cases. With the set of features and the formation of images proposed, an increase in the accuracies of the models of 8.16% and 8.56% was obtained for the DB2 and DB3 databases, respectively, compared to the current state of the art that has used these databases.

Responses from Central New Jersey residents (n = 290) to nine survey items about pro-environmental behaviors were analyzed for their connection to survey respondents’ local built and natural landscape characteristics. These nine survey items were of interest due to their clustering in three groups in earlier dimension reduction analysis. Nine logistic regression models (one for each of these items) were built using a suite of built and natural features of landscape at the municipal level—including land use, population density, and access to parks. The logistic regression models using the landscape variables failed to provide effective explanations of engagement in pro-environmental behavior. In total, only ten landscape feature variables—out of a possible thirty-eight -were used in any of the most effective models. The logistic regression model of proper appliance disposal performed best and could correctly classify responses of whether respondents had disposed of an appliance correctly 84% of the time. The rest of the models were of little use. This suggests that there may not be a connection between these built and natural landscape features and these behaviors, or that the study did not look at location at a level that was granular enough to detect any patterns.

After determining cell viability with trypan blue, morphological features of apoptotic degeneration were analysed by fluorescence microscopy with Hoechst 33258 stain. Results:Cetuximab alone mildly inhibited Hep-2 proliferation and showed no pro-apoptotic effects. When administered concomitantly with cisplatin, cetuximab synergistically increased inhibition of proliferation and apoptosis. Conclusion:The antiproliferative activity of cetuximab is consistent with its hypothesised role in inhibiting repopulation. However, the increase in the effects of pro-apoptotic agents induced by cetuximab may be even more relevant to its clinical effectiveness than the inhibition of repopulation.

Abstract Context.—Digital photography is commonly used to document microscopic features of tissue samples, but it relies on the capture of arbitrarily selected representative areas. Current technologic advances permit the review of an entire sample, some even replicating the use of a microscope. Objective.—To demonstrate the applicability of digital video to the documentation of histologic samples. Design.—A Canon Elura MC40 digital camcorder was mounted on a microscope, glass slide–mounted tissue sections were filmed, and the unedited movies were transferred to a Apple Mac Pro computer. Movies were edited using the software iMovie HD, including placement of a time counter and a voice recording. Results.—The finished movies can be viewed in computers, incorporated onto DVDs, or placed on a Web site after compression with Flash software. The final movies range, on average, between 2 and 8 minutes, depending on the size of the sample, and between 50 MB and 1.6 GB, depending on the intended means of distribution, with DVDs providing the best image quality. Conclusions.—Digital video is a practical methodology for documentation of entire tissue samples. We propose an affordable method that uses easily available hardware and software and does not require significant computer knowledge. Pathology education can be enhanced by the implementation of digital video technology.

This paper is an attempt at providing an account of Long GoN within the system of minimalismbased on case-agreement and single cycle syntax, closely related to the system developed in Chomsky (J998, 1999, 2000). First, we propose that a successful relation of case-agreement can be established not only between full Probes and complete Goals but also between 'split' Probes and complete Goals. We define a 'split' Probe as a (predefined) pair of heads that complement each other and jointly have features (+case, +). In the concrete case ofGoN and Long GoN, the two heads involved are F/Neg (+case) and v (+). The distance holding between the two half Probes in the cases ofLong GoN and the punitive power of the PIC, force us to revise certain assumptions concerning the status ofphases in the derivation. We propose that for a successful case-agreement relation both the 'split' Probe and the Goal must be placed within the same derivational phase. We therefore submit that neither the vP nor the infinitive embedded under control verbs constitute strong phases. In order to deal with infinitives embedded under control verbs, we reject the idea that they are CPs and propose instead that they are best treated as bare TPs, following Boskovic (1997) or, preferably, Hornstein (2000, 2001). The latter hypothesis, based on movement into thematic positions and treatment of obligatory control PRO as an NP trace, has an additional advantage ofexplaining lack ofdefective intervention effects that a PRO, with its interpretable (+) features visible to the Probe, is otherwise expected to cause..

The launch of the new iPad Pro by Apple in March 2020 generated high interest and expectations for different reasons; nevertheless, one of the new features that developers and users were interested in testing was the LiDAR sensor integrated into this device (and, later on, in the iPhone 12 and 13 Pro series). The implications of using this technology are mainly related to augmented and mixed reality applications, but its deployment for surveying tasks also seems promising. In particular, the potentialities of this miniaturized and low-cost sensor embedded in a mobile device have been assessed for documentation from the cultural heritage perspective—a domain where this solution may be particularly innovative. Over the last two years, an increasing number of mobile apps using the Apple LiDAR sensor for 3D data acquisition have been released. However, their performance and the 3D positional accuracy and precision of the acquired 3D point clouds have not yet been fully validated. Among the solutions available, as of September 2021, three iOS apps (SiteScape, EveryPoint, and 3D Scanner App) were tested. They were compared in different surveying scenarios, considering the overall accuracy of the sensor, the best acquisition strategies, the operational limitations, and the 3D positional accuracy of the final products achieved.

Abstract. In recent years, 3D model for indoor spaces have become highly demanded in the development of technology. Many approaches to 3D visualisation and modelling especially for indoor environment was developed such as laser scanner, photogrammetry, computer vision, image and many more. However, most of the technique relies on the experience of the operator to get the best result. Besides that, the equipment is quite expensive and time-consuming in terms of processing. This paper focuses on the data acquisition and visualisation of a 3D model for an indoor space by using a depth sensor. In this study, EyesMap3D Pro by Ecapture is used to collect 3D data of the indoor spaces. The EyesMap3D Pro depth sensor is able to generate 3D point clouds in high speed and high mobility due to the portability and light weight of the device. However, more attention must be paid on data acquisition, data processing, visualizing, and evaluation of the depth sensor data. Hence, this paper will discuss the data processing from extracting features from 3D point clouds to 3D indoor models. Afterwards, the evaluation on the 3D models is made to ensure the suitability in indoor model and indoor mapping application. In this study, the 3D model was exported to 3D GIS-ready format for displaying and storing more information of the indoor spaces.

This study presents the design and programming of a modern new class of radio-controlled electric vertical takeoff and landing (E-VTOL) aircraft using lightweight polylactic acid (LW-PLA). Utilizing the LW-PLA cork foam material is crucial to lowering the aircraft's total weight and improving its mechanical characteristics at the same time. The type of LW-PLA utilized in this study was specific, and its fabrication process was made possible by a Creality Cr 10s pro-3D printer. In this study, a lightweight polylactic acid was used to demonstrate the design of an electric vertical takeoff and landing (E-VTOL) aircraft (LW-PLA). It is essential to use the LW-PLA cork foam material to reduce the overall weight of the airplane and enhance its mechanical properties simultaneously. The particular kind of LW-PLA used in this model was manufactured using a Creality Cr 10s pro-3D printer. Solid Work version 2021 was utilized in the development of the mathematical model. Since radio-controlled aircraft capable of vertical takeoff and landing can carry out both tasks, the goal of this study is to find the best way to utilize the features that both airplanes and drones have in common. Consequently, the suggested model represents a sophisticated enhancement of the aerodynamic characteristics of the many types of radio control aircraft that are capable of performing vertical takeoffs and landings.

The aim of the study is to study the cytokine profile in HIV-infected women with acute vaginitis against the background of complex conservative treatment. Materials and methods of research 55 women aged from 25 to 55 years with acute vaginitis were under our supervision. All women were surveyed for the period from 2018 to 2020, of which 35 women with HIV infection, which constitutes the main group, and 20 women without HIV infection, which constitutes the control group. In all female patients, cytokines (to determine inflammatory processes) (IFNγ, IL-17) were determined in the peripheral blood serum. IFNγ, IL-17 were determined by enzyme-linked immunosorbent assay using test systems "Vector-Best" of the Russian Federation. Results and discussion: the ratio of IFNγ / IL-17 (pro-inflammatory / anti-inflammatory cytokines or Th1 / Th2) in HIV-uninfected women was 2.2. In the presence of a pronounced inflammatory process, that is, in HIV-uninfected women, this indicator was 0.96. After carrying out immunocorrective treatment using Immun-5 (The drug is a balanced mixture of natural biologically active substances, the action of which is aimed at activating the body's immune system. Wetland vegetation is an important component of wetland ecosystems and plays a crucial role in the ecological functions of wetland environments. Accurate distribution mapping and dynamic change monitoring of vegetation are essential for wetland conservation and restoration. The development of unoccupied aerial vehicles (UAVs) provides an efficient and economic platform for wetland vegetation classification. In this study, we evaluated the feasibility of RGB imagery obtained from the DJI Mavic Pro for wetland vegetation classification at the species level, with a specific application to Honghu, which is listed as a wetland of international importance. A total of ten object-based image analysis (OBIA) scenarios were designed to assess the contribution of five machine learning algorithms to the classification accuracy, including Bayes, K-nearest neighbor (KNN), support vector machine (SVM), decision tree (DT), and random forest (RF), multi-feature combinations and feature selection implemented by the recursive feature elimination algorithm (RFE). The overall accuracy and kappa coefficient were compared to determine the optimal classification method. The main results are as follows: (1) RF showed the best performance among the five machine learning algorithms, with an overall accuracy of 89.76% and kappa coefficient of 0.88 when using 53 features (including spectral features (RGB bands), height information, vegetation indices, texture features, and geometric features) for wetland vegetation classification. (2) The RF model constructed by only spectral features showed poor classification results, with an overall accuracy of 73.66% and kappa coefficient of 0.70. By adding height information, VIs, texture features, and geometric features to construct the RF model layer by layer, the overall accuracy was improved by 8.78%, 3.41%, 2.93%, and 0.98%, respectively, demonstrating the importance of multi-feature combinations. (3) The contribution of different types of features to the RF model was not equal, and the height information was the most important for wetland vegetation classification, followed by the vegetation indices. (4) The RFE algorithm effectively reduced the number of original features from 53 to 36, generating an optimal feature subset for wetland vegetation classification. The RF based on the feature selection result of RFE (RF-RFE) had the best performance in ten scenarios, and provided an overall accuracy of 90.73%, which was 0.97% higher than the RF without feature selection. The results illustrate that the combination of UAV-based RGB imagery and the OBIA approach provides a straightforward, yet powerful, approach for high-precision wetland vegetation classification at the species level, in spite of limited spectral information. Compared with satellite data or UAVs equipped with other types of sensors, UAVs with RGB cameras are more cost efficient and convenient for wetland vegetation monitoring and mapping.

Defects in wound closure can be related to the failure of keratinocytes to re-epithelize. Potential mechanisms driving this impairment comprise unbalanced cytokine signaling, including Transforming Growth Factor-β (TFG-β). Although the etiologies of chronic wound development are known, the relevant molecular events are poorly understood. This lack of insight is a consequence of ethical issues, which limit the available evidence to humans. In this work, we have used an in vitro model validated for the study of epidermal physiology and function, the HaCaT cells to provide a description of the impact of sustained exposure to TGF-β. Long term TGF-β1 treatment led to evident changes, HaCaT cells became spindle-shaped and increased in size. This phenotype change involved conformational re-arrangements for actin filaments and E-Cadherin cell-adhesion structures. Surprisingly, the signs of consolidated epithelial-to-mesenchymal transition were absent. At the molecular level, modified gene expression and altered protein contents were found. Non-canonical TGF-β pathway elements did not show relevant changes. However, R-Smads experienced alterations best characterized by decreased Smad3 levels. Functionally, HaCaT cells exposed to TGF-β1 for long periods showed cell-cycle arrest. Yet, the strength of this restraint weakens the longer the treatment, as revealed when challenged by pro-mitogenic factors. The proposed setting might offer a useful framework for future research on the mechanisms driving wound chronification.

The professional development of employees is an integral part of the organization's personnel management, since the effectiveness and efficiency of its activities significantly depend on the level of professional training of personnel in the conditions of the modern economy. Employees increas-ingly have to combine a wide variety of functions in their work, perform many different professional tasks, process large amounts of information, etc. Therefore, the proper choice of the form of profes-sional development, which would meet the demands and capabilities of the company, as well as the employee himself, is an important task of the management. However, despite this, the question of which tools to use for the effective selection of acceptable forms is still unclear. The article considers different approaches to the essence of the interpretation of the concept of "professional development". Various forms of professional development of employees are char-acterized, the importance of choosing a specific form for the development of the company and the problems that arise in this case are indicated. The stages of choosing forms of professional develop-ment for employees of the foreign economic division have been developed and the use of the TOPSIS methodology for choosing the optimal form of professional development of an employee on the ex-ample of a specific enterprise is clearly presented. The research was conducted on the basis of State Enterprise "X". According to the TOPSIS methodology, the chosen option, namely self-education and mentoring, cannot be considered the best forms of professional growth for employees of the for-eign economic division, but only acceptable for the organization. According to the TOPSIS method-ology, the option that receives 1 point is considered the best. According to the results of our research, it can be noted that the choice of forms of profes-sional development of an employee, especially those working in the field of foreign economic activi-ty, is an important issue for companies that seek not only to maintain their positions in the foreign market, but also to expand their activities abroad. Therefore, the stages of selection of forms pro-posed by us and a visual presentation of the use of the TOPSIS methodology will enable domestic en-terprises to more effectively solve this task, taking into account financial capabilities, the needs of employees, their abilities about professions, the needs of companies, etc.

AbstractImplementing clinical outcome assessments electronically in clinical studies requires the sponsor and electronic clinical outcome assessment (eCOA) provider to work closely together to implement study-specific requirements and ensure consensus-defined best practices are followed. One of the most important steps is for sponsors to conduct user acceptance testing (UAT) using an eCOA system developed by the eCOA provider. UAT provides the clinical study team including sponsor or designee an opportunity to evaluate actual software performance and ensure that the sponsor’s intended requirements were communicated clearly and accurately translated into the system design, and that the system conforms to a sponsor-approved requirements document based on the study protocol. The components of an eCOA system, such as the study-specific application, customization features, study portal, and custom data transfers should be tested during UAT. While the provider will perform their own system validation, the sponsor or designee should also perform their due diligence by conducting UAT. A clear UAT plan including the necessary documentation may be requested by regulatory authorities depending on the country. This paper provides the electronic patient-reported outcome (ePRO) Consortium’s and patient-reported outcome (PRO) Consortium’s best practice recommendations for clinical study sponsors or their designee for conducting UAT with support from eCOA providers to ensure data quality and enhance operational efficiency of the eCOA system. Following these best practice recommendations and completing UAT in its entirety will support a high quality eCOA system and ensure more reliable and complete data are collected, which are essential to the success of the study.

Background. One of the most prominent features of secondary edematous breast cancer (SEBC), which is the most malignant form of this type of cancer, is severity of chronic inflammation that is important for pathogenesis and progression of the disease. As of now, there is evidence of association of carcinogenesis and inflammation. The transcription factor (NF-kB) and pro-inflammatory cytokines play a pivotal role in both inflammation and carcinogenesis. The regulation of NF-kB signal pathways is impaired in a lot of malignant diseases, including breast cancer (BC). Thus, the study of the content of pro-inflammatory cytokines and NF-kB is of high priority, as it can provide valuable information about the course of the tumor process. However, there are few research papers that deal with association of cytokine profile and NF-kB in breast tumors. Purpose – is to study the content of NF-kB-р105 and pro-inflammatory cytokines (IL-6, IL-8, TNFα) in the blood serum of patients with secondary edematous breast cancer. Materials and Methods. 87 patients (42 with SEBC, 45 with BC) were examined prior to treatment. The age of 42 patients with T4bN0-3M0 SEBC ranged from 34 to 71 years (median 53.1). The ductal cancer was found in 30 patients (71.43%), the lobular cancer – in 12 patients (28.57%). The tumor of more than 5 cm was detected in 20 individuals (47.6%), the tumor of less than 5 cm – in 22 individuals (52.4%). The comparison group consisted of 45 patients with BC, with their age ranging from 30 to 67 years (median 52.3). They had T3-4N1-3M0 non-edematous locally advanced BC. The tumor of more than 5 cm was detected in 12 patients (26.7%), the tumor of less than 5 cm – in 33 patients (73.3%). The ductal cancer was diagnosed in 33 patients (73.3%), the lobular cancer – in 12 patients (26.7%). The control group consisted of 10 patients with fibroadenomas. The content of cytokines (IL-1B, IL-2, IL-6, IL-8, TNFα) in the blood serum of patients was measured using the ELISA assay and CJSC «Vektor-Best» standard assay kits. The content of NF-kB1 subunit (р105 → р50) was measured using the ELISA assay and the Human NFkB – p105 (Nuclear factor NF-kappa-B p 105 subunit) ELISA Kit. The measurement was performed using the Immunochem-2100 American semi-automatic immunoassay analyzer. Results. It was found that in SEBC, the levels of pro-inflammatory cytokines IL-6, IL-8, TNFα were increased by 1.4 times, compared to the parameters in BC, and the level of IL-8 was the highest. The total level of NF-kB increased by 14,7 times in patients with SEBC and by 2,4 times in patients with BC, compared to individuals with fibroadenomas. The level of NF-kB in SEBC in groups with IL-6, TNFα was higher than in groups with BC by 3.1 and 1,7 times, respectively. It was found that the highest level of NF-kB was in the group with cytokine IL-8. In SEBC, it was higher by 5.7 times than in BC. In patients with SEBC, correlations between NF-kB and cytokines were established: NF-kB and IL-8 (r = 0.80; p < 0.05); NF-kB and IL-6 (r = 0.60; p < 0.05); NF-kB and TNFα (r = 0.60; p < 0.05). Thus, one feature of SEBC is the increase in the content of NF-kB, IL-6, and TNFα, and also a significant increase in the level of NF-kB and IL-8, compared to the parameters in patients with BC as well as in patients with fibroadenomas. Conclusions. It was found that there was activation of the transcription factor NF-kB-p105 and a significant increase in the levels of pro-inflammatory cytokines (IL-6, IL-8, TNFα) in the blood serum of patients with SEBC, compared to patients with fibroadenomas, which indicates high carcinogenic potential of the tumor and the presence of the inflammatory component. It was demonstrated that the highest level of NF-kB-p105 was detected in the group of patients with SEBC with the highest level of cytokines IL-8. It was also found that in patients with SEBC, the level of the transcription factor and pro-inflammatory cytokines IL-6, IL-8, TNFα in the blood serum was significantly higher than in patients with BC, thereby confirming severe aggressiveness of this form of the disease.

The objective of this study is to evaluate the association between quality and features related to internet users of the most viewed YouTube videos about COVID-19 and chloroquine, during the first wave of the pandemic. We conducted a cross-sectional observational study focusing on the most viewed YouTube videos on COVID-19 and chloroquine treatment, in French and English. The primary outcome was the association between video quality as assessed by DISCERN and modified JAMA scores, and video features related to internet users as assessed by number of viewership and likes. By June 2020, 168 videos accumulating more than 57 million views and nearly 2 million reactions from Internet users, were included. Most of the videos did not support or oppose the treatment and came from news channels (N = 100; 60%). Videos taking sides were mostly pro-chloroquine (N = 69; 89%). The number of non-factual videos analyzed was very low (N = 3; 2%). The quality of the videos was average (mean DISCERN score = 2.4 (DS 1.0) and mean modified JAMA score = 2.6 (DS 0.6)) and declined over time. The best quality videos were those published by health care professionals or those from educational channels. Most experts interviewed were men (N = 136; 81%). More than 1 in 5 videos featured a political figure (N = 35; 21%), and these were mostly pro-chloroquine with lower DISCERN or modified JAMA scores (p<0.001). We found an association between the number of likes and the quality of the videos evaluated by the DISCERN score (rho = 0.29; p<0.001) and the modified JAMA score (rho = 0.30; p<0.001). The association observed between the quality of the videos and the number of likes suggests a better health literacy and critical thinking of lay internet users. Although YouTube has become a major player in the dissemination of medical information, more involvement of health professionals and governmental organizations is needed.

Abstract This research focuses on the synthesis and adhesive properties of mucoadhesive mats, prepared with poly(vinylic alcohol) as a base polymer for the oromucosal release of propranolol (PRO) by the electrospinning technique. The nanofibers mats were evaluated by scanning electron microscopy (SEM), Fourier-transform infrared spectroscopy, thermogravimetric analysis, and differential scanning calorimetry; in vitro drug entrapment efficiency, degradation time, and adhesion studies were performed. SEM images of the electrospun mats show the correct formation of fibers with a variable diameter and porosity. Thermal studies indicate excellent thermal stability of the scaffolds, The fibrous mats loaded with 10% of the drug exhibit the best thermal stability with decomposition after 450°C. In vitro studies indicate a drug content of 88% loaded in the mats. In the cytotoxicity test, loaded mat presents cell proliferations of 97% and 88% for drug concentrations of 10% an 15%, respectively. To conclude, the formed electrospun adhesive mats exhibited excellent thermal stability, adhesive properties, and drug entrapment efficiency, promising features for a successful drug topical release system on mucosal tissue in the oral cavity.

The study reveals several flaws in the MDC which reinforce the role of the state as a transmission belt for transnational capital, rather than as a facilitator for development. For instance, the neo-liberal market fundamentalism and big-bang approach inherent in the MDC spells ‘jobless growth’. Similarly, the notion that ‘good governance is less government’, implies that the state is to a large extent reduced to an ‘investment promotion agency’. Having said this, the MDC does contain several novel and positive features, and its problematic aspects can be overcome through some strategic changes in the MDC approach. These may be summarised as a heavier focus on ‘development’, a more pro-active state, and more comprehensive and inclusive governance structures.

Aim. To study the peculiar features of roentgenological picture of pulmonary tuberculosis in patients with polymorphous HLA-DRB1 allels in the genotype in associations with pro-and-antiinflammatory cytokines concentration. Materials and methods. The complex clinicoimmunogenetic examination of 64 patients with pulmonary tuberculosis was carried out. The patients’ age ranged 18 to 52 years. The gene HLA-DRB1 polymorphism was determined using the method of polymerase chain reaction with DNA, previously extracted from blood samples. Cytokine concentration was determined in the peripheral blood before specific treatment, using the assay kits of “Vector-BEST Company” (Russia). Results. The prevailing clinical form of pulmonary tuberculosis among patients was infiltrative one – 40 (62.5). Bacterioexcretion was stated in 89.5 % of cases. Roentgenological picture of pulmonary tuberculosis in patients with HLA-DRB1 *04, *13, *14, *16 alleles was characterized as disseminated injury of lung tissue with destructive changes, the concentration of IL-1b, TNF-α exceeded the standard values by more than 3 times (r = 0.88; р = 0.040), IL-2, IL-10, g-INF was lower than standard values by more than 4–5 times (r = 0.75; р = 0.003). Conclusions. When studying HLA-DRB1 gene polymorphism and pro-and-antiinflammatory cytokines concentrations in patients with pulmonary tuberculosis, it was established that patients with HLA-DRB1 *01, *07, *08, *10, *15 alleles more often, compared to the others, had disseminated injury of lung tissue, in most cases with its destruction as well as dysbalanced cytokine balance in the direction of deep immunosuppression.

Abstract Objectives: To determine the support of physicians for the recently published ASCO evidence-based clinical practice guidelines for venous thromboembolism (VTE) prophylaxis and treatment in patients with cancer (J Clin Oncol. 2015;33:654-6). Methods: ASCO key recommendations were presented as part of three CME seminars to physicians not specialized in hem/onc or hemostasis. In addition two specialists in the field discussed a list of arguments for and against each of the recommendations considering (1) quality of scientific evidence underlying each recommendation, (2) feasibility in daily practice including sensitivity to patients preferences, and (3) medicolegal implications and reimbursement aspects. After each presentation attendees were asked to fill out a questionnaire on how much they supported the ASCO recommendations and each of the pro&con arguments. Results: A total of 89 physicians attended the three meetings. 56 questionnaires were returned. The ASCO recommendations with the highest degree of support were: ‒ Patients undergoing major cancer surgery should receive prophylaxis for 7-10 days and even 4 weeks after major abdominal or pelvic surgery with high-risk features (84% pro) ‒ LMWH should be given for the initial treatment as well as for long-term secondary prophylaxis (80% pro). The recommendations with the lowest degree of support were: ‒ Patients with cancer should be educated about signs and symptoms of VTE. They should be periodically assessed for VTE (59% pro). ‒ Anticoagulation should not be used to extend survival of patients with cancer in the absence of other indications (56% pro). Conclusion: After presenting ASCO recommendations as part of CME training the degree of physicians' support was still at best moderate and sometimes even close to neutrality. In daily medical practice scientific evidence has no priority to clinical practicability, patients preferences, medicolegal aspects and cost-restrictions. Guideline authors should complement their recommendations with advice from their own practice on how to implement guidelines in daily care and how they manage financial and medicolegal restrictions. In addition there is an unmet need for more education on VTE prophylaxis and treatment among physicians not specialized in hem/onc or hemostasis who are caring for cancer patients. Background: There is paucity of detailed studies of adult T cell acute lymphoblastic leukemia (T-ALL) in developing countries reflecting the condition of these patients including clinical and biological features. Objective: This study was carried out to analyze the immunophenotypic characteristics of 40 Moroccan patients with T-ALL and its association with biological and clinical features. Patients and Methods: Between 2006 and 2009, 130 adult patients diagnosed with acute lymphoblastic leukemia (ALL) were immunophenotyped by 3-color flow cytometry using a panel of monoclonal antibodies. Cases presenting features of a T-lineage phenotype were subjected to detailed analysis including immunophenotypic, clinical and biological parameters. Results: Proportion of T-ALL among ALL Moroccan patients was 31.0%. Median age of patients was 28 years. Twenty-nine patients were females and 11 were males. 45.0% of patients (18/40) had features of immature T-ALL stages (pro-T and pre-T ALL), 30.0% (12/40) of CD1a+ cortical T-ALL stage and 25.0% (10/40) had a characteristic phenotype of medullary T-ALL. The frequencies of progenitor cell markers CD10, CD34 and TdT expression were 14.0; 57.5% and 50.0% respectively. The aberrant expression of B lineage associated antigen CD79a were positive in 20.5% of the cases and the aberrant expression of myeloid antigens CD13 and/ or CD33 was found in 22 (55.0%) cases. No significant association was encountered between TdT, CD34 or myeloid antigens positivity and high risk features at presentation as age, sex, and white blood cells. However, myeloid antigens (CD13 and/or CD33) was significantly associated with T-cell maturation stages (p = 0.009). Conclusion: To the best of our knowledge, this is the first report from North Africa of immunophenotypic study on adult T-ALL. Our findings indicate that the proportion of T-ALL among ALL in Morocco is similar to that reported in others Mediterranean countries like France and Italy and that myeloid-associated antigens expression is frequently associated with immature immunophenotype.

Cell proliferation and escape from apoptosis are important pathological features of hepatocellular carcinoma (HCC), one of the tumors with the highest mortality rate worldwide. The aim of the study was to evaluate the expression of the pro-apoptotic p66shc and the anti-apoptotic SerpinB3 in HCCs in relation to clinical outcome, cell fate and tumor growth. p66shc and SerpinB3 were evaluated in 67 HCC specimens and the results were correlated with overall survival. Proliferation and cell death markers were analyzed in hepatoma cells overexpressing SerpinB3, under different stress conditions. p66shc−/− mice and xenograft models were also used to assess the effects of p66shc and SerpinB3 on tumor growth. In patients with HCC, the best survival was observed in the subgroup with p66shc levels below median values and SerpinB3 levels above median values. Mice p66shc−/− showed high levels of SerpinB3, while in HepG2 cells overexpressing SerpinB3, p66shc expression was trivial. HepG2 overexpressing SerpinB3 cells were more prone to die after oxidizing treatments, such as diamide or high concentration H2O2. These cells injected in nude mice developed tumors five times smaller than those from control HepG2 cells. Tumors originating from HepG2 overexpressing SerpinB3 cells showed decreased activated Caspase-8, with concomitant increase of RIP3K and decreased levels of cleaved RIP3K, typical features of necroptosis. In conclusion, in patients affected by HCC, the pattern characterized by p66shc downregulation and elevated SerpinB3 levels was associated with markedly better survival. This pattern favored necroptosis in experimental high-stress conditions.

Although autism spectrum disorder (ASD) is a multifaceted neurodevelopmental syndrome, accumulating evidence indicates that oxidative stress and inflammation are common features of ASD. Flavonoids, one of the largest and best-investigated classes of plant-derived compounds, are known to exert antioxidant, anti-inflammatory, and neuroprotective effects. This review used a systematic search process to assess the available evidence on the effect of flavonoids on ASD. A comprehensive literature search was carried out in PubMed, Scopus, and Web of Science databases following the PRISMA guidelines. A total of 17 preclinical studies and 4 clinical investigations met our inclusion criteria and were included in the final review. Most findings from animal studies suggest that treatment with flavonoids improves oxidative stress parameters, reduces inflammatory mediators, and promotes pro-neurogenic effects. These studies also showed that flavonoids ameliorate the core symptoms of ASD, such as social deficits, repetitive behavior, learning and memory impairments, and motor coordination. However, there are no randomized placebo-controlled trials that support the clinical efficacy of flavonoids in ASD. We only found open-label studies and case reports/series, using only two flavonoids such as luteolin and quercetin. These preliminary clinical studies indicate that flavonoid administration may improve specific behavioral symptoms of ASD. Overall, this review is the first one to systematically report evidence for the putative beneficial effects of flavonoids on features of ASD. These promising preliminary results may provide the rationale for future randomized controlled trials aimed at confirming these outcomes.

Relevance. At the present time, despite the large number of studies, the root causes of pre-eclampsia remain unknown. In recent years, attention is paid to the study of the role of vascular growth factors in the pathogenesis of various obstetric pathologies, including pre-eclampsia. The aim of our study was to examine the features of the angiogenic imbalance in determining the severity of pre-eclampsia, and further tactics of conducting pregnant women. Matherials and Methods. 135 pregnant women were examined: I group - 50 pregnant women with preeclampsia moderate; II group - 36 pregnant women with preeclampsia-severe; Group III - 49 pregnant women with physiological pregnancy. All patients special methods of investigation have been carried out - the definition of levels of pro-angiogenic (PlGF) and anti-angiogenic (sFlt-1, sEng) factors in the serum in 28-34 weeks gestation. In our study we found significant differences in the content of vascular growth factors in patients in the three comparison groups. Results. With an increase in the severity of pre-eclampsia decrease pro-angiogenic placental vascular factors to a greater extent than other vascular factors points to the aggravation of the pathological process and can be used as an additional criterion of pre-eclampsia severity. The level of vascular growth factors and their relationship with a certain probability, reflects the extent of endothelial dysfunction that predicts the best time of prolongation of pregnancy in patients with pre-eclampsia. Conclusion. Use of vascular growth factor in the diagnosis and determining the severity pre-eclampsia, along with traditional clinical and laboratory research will optimize the obstetric tactics in these patients.

In this article, the problem of political leaning classificationof the text resource is solved. First, a detailed analysis of ten stud-ies on the work’s topicwas performed in the form of comparative characteristicsof the used methodologies.Literary sources were compared according to the problem-solvingmethods,the learning that was carried out, the evaluation metrics, and according to the vectorizations. Thus, it was determined that machine learning algorithms and neural networks, as well as vectorizationmethods TF-IDF and Word2Vec, were most often used to solve the problem.Next, various classification models of whether textual information is pro-Ukrainian or pro-Russian were built based on a dataset containing messages from social media users about the events of the large-scale Russian invasion of Ukraine from February 24, 2022.The problem was solved with the help of Support Vector Machines, Decision Tree, Random Forest, Naïve Bayes classifier,eXtreme Gradient BoostingandLogistic Regressionmachine learning algo-rithms, Convolutional Neural Networks, Long short-term memory and BERT neural networks, techniques for working with unbal-anced dataRandom Oversampling, Random Undersampling , SMOTE and SMOTETomek, as well as stacking ensembles of models.Amongthe machine learning algorithms, LR performed best, showing a macro F1-scorevalue of 0.7966 when features were trans-formed by TF-IDF vectorization and 0.7933 when BoW.Among neural networks, the best macro F1-scorevalue of 0.76was ob-tained using CNN and LSTM.Applying data balancing techniques failed to improve the results of machine learning algorithms.Next, ensembles of models from machine learning algorithms were determined. Two of the constructed ensembles achieved the same macro F1-scorevalue of 0.7966 as with LR. Ensembles that wasable to do so consisted of the TF-IDF vectorization, the B-NBC meta-model, and the SVC, NuSVC LR, and SVC, LR base models, respectively. Thus, three classifiers, the LR machine learning algorithmand two ensembles of models, which were defined as a combination of existing methods of solving the problem, demon-strated the largest macro F1-score value of 0.7966. The obtained models can be used for a detailed review of various news publica-tions according to the political leaning characteristic, information about which can help people identify being isolated by a filter bubble.

Background. Сhronic atrophic gastritis certainly remains an urgent problem of gastroenterology but data on sexual differences in the content of cytokines in this pathology are quite contradictory. The purpose of the study: to assess the gender characteristics of the cytokine profile in patients with chronic atrophic gastritis. Materials and methods. The study included 120 patients with gastric atrophy, according to histological examination of biopsies. The control group consisted of 20 healthy individuals, men and women equally. In all patients, we have evaluated the levels of interleukins (IL-8, IL-10, IL-18), tumor necrosis factor alpha (TNF-α), vasculoendothelial growth factor (VEGF) by enzyme-linked immunosorbent assay using appropriate reagent from Vector-BEST kits and the Stat Fax 303 Plus analyzer. Results. In men with chronic atrophic gastritis, there is a more pronounced imbalance towards pro-inflammatory cytokines, in particular the level of IL-18 is 1.7 times higher (p < 0.05) than in women. In 46.2 % of cases, the content of IL-8 was also elevated in men by 1.3 times (p > 0.05) compared to women. The level of anti-inflammatory cytokine IL-10 does not have a significant gender difference in patients with precancerous conditions of the stomach. The median of IL-18/IL-10 ratio in men is 2 times higher than in women: 65.36 (21.67; 154.25) vs. 32.15 (12.76; 191.85) (p < 0. 05). In males, IL-8/IL-10 ratio is also 1.5 times higher, which is 2.25 (1.29; 7.68) vs. 1.49 (0.75; 9.78) but this difference was not statically significant. Serum content of VEGF in men exceeded the same indicator in women by 1.4 times (p < 0.05). Direct correlation between VEGF content and the levels of TNF-α (r = 0.47, p < 0.05), IL-8 (r = 0.42, p < 0.05), IL-18 (r = 0.58, p < 0.05) confirm the evidence of increased VEGF expression under the influence of many proangiogenic growth factors and proinflammatory cytokines. Conclusions. With an increase in the level of IL-18 and VEGF by more than 30 %, men require dynamic monitoring for early detection of precancerous structural changes in the gastric mucosa.

Sufficient physical activity and restful sleep play a major role in the prevention and cure of many chronic conditions. Being able to proactively screen and monitor such chronic conditions would be a big step forward for overall health. The rapid increase in the popularity of wearable devices pro-vides a significant new source, making it possible to track the user’s lifestyle real-time. In this paper, we propose a novel unsupervised representation learning technique called activ-ity2vecthat learns and “summarizes” the discrete-valued ac-tivity time-series. It learns the representations with three com-ponents: (i) the co-occurrence and magnitude of the activ-ity levels in a time-segment, (ii) neighboring context of the time-segment, and (iii) promoting subject-invariance with ad-versarial training. We evaluate our method on four disorder prediction tasks using linear classifiers. Empirical evaluation demonstrates that our proposed method scales and performs better than many strong baselines. The adversarial regime helps improve the generalizability of our representations by promoting subject invariant features. We also show that using the representations at the level of a day works the best since human activity is structured in terms of daily routines.

Purpose This paper aims to investigate the methods for the prediction of tags on a textual corpus that describes diverse data sets based on short messages; as an example, the authors demonstrate the usage of methods based on hotel staff inputs in a ticketing system as well as the publicly available StackOverflow corpus. The aim is to improve the tagging process and find the most suitable method for suggesting tags for a new text entry. Design/methodology/approach The paper consists of two parts: exploration of existing sample data, which includes statistical analysis and visualisation of the data to provide an overview, and evaluation of tag prediction approaches. The authors have included different approaches from different research fields to cover a broad spectrum of possible solutions. As a result, the authors have tested a machine learning model for multi-label classification (using gradient boosting), a statistical approach (using frequency heuristics) and three similarity-based classification approaches (nearest centroid, k-nearest neighbours (k-NN) and naive Bayes). The experiment that compares the approaches uses recall to measure the quality of results. Finally, the authors provide a recommendation of the modelling approach that produces the best accuracy in terms of tag prediction on the sample data. Findings The authors have calculated the performance of each method against the test data set by measuring recall. The authors show recall for each method with different features (except for frequency heuristics, which does not provide the option to add additional features) for the dmbook pro and StackOverflow data sets. k-NN clearly provides the best recall. As k-NN turned out to provide the best results, the authors have performed further experiments with values of k from 1–10. This helped us to observe the impact of the number of neighbours used on the performance and to identify the best value for k. Originality/value The value and originality of the paper are given by extensive experiments with several methods from different domains. The authors have used probabilistic methods, such as naive Bayes, statistical methods, such as frequency heuristics, and similarity approaches, such as k-NN. Furthermore, the authors have produced results on an industrial-scale data set that has been provided by a company and used directly in their project, as well as a community-based data set with a large amount of data and dimensionality. The study results can be used to select a model based on diverse corpora for a specific use case, taking into account advantages and disadvantages when applying the model to your data.

Introduction: A post hoc study was conducted to compare visual and anatomic outcomes of vascularized serous pigment epithelial detachment (Group 1) with fibrovascular pigment epithelial detachment (Group 2) due to age-related macular degeneration treated with either 0.5 or 2.0 mg ranibizumab injections. Methods: A prospective, randomized trial was performed with the following regimens for 12 months: (1) 0.5 mg monthly, (2) 0.5 mg monthly for 4 months followed by pro re nata injections, (3) 2.0 mg monthly, and (4) 2.0 mg monthly for 4 months followed by pro re nata injections. Primary measure was best-corrected standardized vision. Secondary measures included central subfield, thickness surface area A2, greatest linear diameter, heights of pigment epithelial detachment and choroidal neovascularization (CNV), subretinal fluid, cystoid macular edema, and adverse events. Results: For 36 eyes (8 in Group 1 and 28 in Group 2), follow-up time was 12 months. There were no differences in baseline features between groups except for pigment epithelial detachment A2 (Group 2 > Group 1). Two-way analysis of variance showed comparable improvements in anatomic and vision outcomes. Three-way analysis of variance also showed similar responses for both lesion subtypes with high-dose treatment. There was a trend toward greater pigment epithelial detachment resolution in Group 1 eyes. There were no differences in retinochoroidal angiomatous proliferation (Type-3 CNV) and cataracts between groups, although greater percentages of eyes in Group 1 developed retinal pigment epithelial tears (25% vs 10.7%). Conclusion: There were no differences in vision and anatomic outcomes between lesion subtypes, and similarly, more rapid responses to high-dose than conventional-dose ranibizumab occurred for eyes with both lesion subtypes. More retinal pigment epithelial tears may develop in eyes with vascularized serous pigment epithelial detachment.

Recurrent stakeholder meetings with experts in the field of breast cancer care, in-depth one-on-one qualitative interviews with a clinical sample of patients with breast cancer, and focus groups with Dana-Farber/Harvard Cancer Center (DF/HCC) Breast Cancer Advisory Group members, were used to elicit feedback for the design features and functions of a patient mobile application and clinician dashboard.SettingThis study was conducted at two academic hospitals in the USA.ParticipantsParticipants included experts in the field of breast cancer care, value-based healthcare, and health information technology, a clinical sample of patients with breast cancer, and members of the DF/HCC Breast Cancer Advisory Group.Main outcome measuresimPROVE incorporates the International Consortium for Health Outcomes Measurement (ICHOM) breast cancer standard outcome set as well as the complete BREAST-Q Breast Cancer Module.ResultsFeedback was elicited from eight stakeholder meetings (n=28 members), interviews with a clinical sample of patients (n=28), and two focus groups with members of the DF/HCC Breast Cancer Advisory Group (n=17 members in each focus group). Participant feedback led to the development of a patient mobile application consisting of five components (myCare, myStory, myResources, myCommunity, and myNotes) and a clinician dashboard that includes an overview table and individual patient profiles with data displays.ConclusionsimPROVE has the potential to transform the way we deliver care to patients. Developed from best practices in user-centered design, agile development, and qualitative methods; imPROVE addresses the needs of multiple stakeholders, including patients, clinicians, healthcare administrators, and researchers.

Abstract Activation of DNA-sensing pathways can induce an anti-tumour immune response, but the mechanisms by which tumour-derived cell-free DNA (cfDNA) activates pro-inflammatory signaling is not well-understood. In particular, the topological features of cfDNA that contribute to DNA sensor activation in phagocytes are unknown. As such, we investigated how tumour-derived cfDNA topology influences DNA sensor activation in macrophages and dendritic cells. We treated a murine colon adenocarcinoma cell line (MC38) with chemotherapy (topotecan or staurosporine) for varying durations to induce cfDNA release. We then harvested cfDNA, separated the protected (membrane-bound) and accessible (non-membranous) fractions, and quantified nuclear and mitochondrial cfDNA abundance. The cfDNA fractions were then added to murine RAW 264.7 macrophages or bone marrow-derived dendritic cells (BMDCs), and a panel of inflammatory markers was measured by RT-qPCR. We found that accessible cfDNA from MC38 cells treated with chemotherapy for 48h increased Il6expression in RAW 264.7, but other inflammatory markers were not elevated. Interestingly, both nuclear and mitochondrial cfDNA abundance directly correlated with Il6expression. Surprisingly, cfDNA did not induce pro-inflammatory signalling in BMDCs. These results demonstrate that tumour-derived cfDNA immunogenicity depends on its mechanistic origins, abundance, and the immune cell type to which it is exposed. We are currently examining the impact of other cfDNA biophysical properties on macrophage activation, as well as elucidating the DNA sensing pathways responsible for inducing Il6expression. Our findings will help uncover the role of cfDNA in mediating anti-tumour immunity. E.Z. P. oleracea L. contains high level of nutrients and biologically active compounds. Recently, lactic fermentation has been proposed as a biotechnological option to enrich the profile of biogenic compounds of Portulaca oleracea L. puree. This study investigated the capability of fermentation by selected lactic acid bacteria to enhance the restoring features of Portulaca oleracea juice towards intestinal inflammation and epithelial injury. Lactic acid fermentation markedly increased the total antioxidant capacity of P. oleracea juice, preserved the inherent levels of vitamins C, A, and E, and increased the bioavailability of the level of vitamin B2 and that of phenolics. The effects of fermented P. oleracea juice on a Caco-2 cell line were investigated using an in vitro model closest to the in vivo conditions. Fermented P. oleracea juice strongly decreased the levels of pro-inflammatory mediators and reactive oxygen species. It also counteracted the disruption of the Caco-2 cell monolayers treated with the inflammatory stimulus. We used a diversified spectrum of lactic acid bacteria species, and some effects appeared to be strains- or species-specific. Fermentation with Lactobacillus kunkeei B7 ensured the best combination for the content of bioactive compounds and the ability to counteract the intestinal inflammation and epithelial injury.
