This paper explores the challenges of implementing client-centred occupational therapy practice. While many occupational therapists believe in the principles of client-centred practice and espouse them, it seems much more difficult to implement these into everyday practice. Findings from three qualitative studies with three different populations (i.e., family-centred care for children and their families, community-based home care, facility-based care for older adults) are used to illustrate the challenges which are divided into three broad categories: challenges at the level of the system, at the level of the therapist and at the level of the client. Suggestions for change at each level are addressed. Organizations, therapists and clients must work together to facilitate these changes and ensure that each occupational therapy client receives respectful, supportive, coordinated, flexible and individualized service.

Mental Health Clinicians are accustomed to being confronted with not only difficult situations, but difficult conversations. Although discussing issues of diversity can be challenging, these dialogues are vital to the therapeutic process. In order to work under a multicultural framework, a clinician must minimally have basic knowledge on the culture of the client(s) being treated. Therapists are to use culturally appropriate intervention strategies and be mindful of the rules of the client’s culture. It is advised to work with the client(s) to define their culture, what it means to them, and what it means to society. It is the duty of a clinician to exemplify this for clients and give clients the power, permission, and invitation that they may believe they need, to do the same. This article discusses issues of diversity in the incarcerated system, clarifies the clinician’s role, and empowers clinicians to utilize multicultural techniques in treatment.

Canadian occupational therapists aspire to be client-centred. As a profession we continue to debate what this means. The question of how to be client-centred becomes more acute when the client has cognitive deficits, and capacity, including the legal right to consent to treatment, is called into question. Further, in current literature and law, capacity is described as situational (i.e., specific to the time, circumstances and question being asked) and thus, difficult to determine. In this discussion, the author will explore two strategies, graded decision-making and advocacy, for practicing in a client-centred manner with cognitively impaired clients.

This article discusses the case of a male vowed religious clergy, who was also in residential treatment for sexual misconduct and interpersonal difficulties. Importantly, this client also had a childhood history of sexual trauma. The case, difficult and complex in its own right, posed unique clinical challenges. The first author and therapist, a Catholic, feminist, woman often works with child trauma clients. Thus, the experiences of transference and countertransference were particularly important therapeutic considerations working with this client. Themes of power, sex, shame, guilt, and blame needed to be explored and processed in depth from the client’s and therapist’s perspectives both during session and supervision. Concurrent issues include personality disorders, physical disability, and psychosexual disorders. This client was referred by their religious institution and took part in a mandated fourteen to twenty-week residential programme. Therapeutic modalities include trauma-informed, attachment-oriented, and psychodynamic individual and grouporiented psychotherapy.

Occupational therapists, known for their ability to coach others through difficult changes, now find themselves in changing environments that include clients' desire for participation in service delivery, and the need for evidence of the outcome of occupational therapy intervention. This paper proposes a process model that is based on core concepts of occupation and client-centred practice, that incorporates theoretical approaches, and can be applied to all clients. The process model coaches the occupational therapist through a client-centred problemsolving process. It represents a description or plan for occupational therapy assessment and intervention that leads to a collaborative approach to client-identified occupational performance issues. Examples of the application of the process to occupational therapy practice are included. This model helps to articulate core concepts and values, and provides guidance for occupational therapy practice within changing environments and expectations.

In home care, bathroom activities—particularly bathing and toileting—present a unique set of challenges. In this focus group study, professional home care providers identified factors that increase the danger and difficulty of assisting their clients with bathing and toileting. These included small restrictive spaces, a poor fit between available equipment and the environment, a reliance on manual handling techniques (but insufficient space to use optimal body mechanics), attempts to maintain normalcy, and caring for unsteady and unpredictable clients. Specific elements of each activity that care providers found difficult included multitasking to support client stability while performing care below the waist (dressing/undressing, providing perineal care) and helping clients to lift their legs in and out of a bathtub. Participants did not feel that available assistive devices provided enough assistance to reduce the danger and difficulty of these activities.

The case study illustrates how adherence to auditing standards may place auditors in a difficult situation when balancing the auditor's risk of litigation, the clients' rights to confidentiality, and the auditor's duty to the public. Additionally, the case provides exposure to prior litigation cases against auditors with unfavorable outcomes when auditors chose to protect their client's confidentiality, and, conversely, in cases where auditors chose to protect the greater public interest. The case requires students to engage in critical thinking by providing their viewpoints as to the optimal balance of limiting auditor liability, adhering to client confidentiality requirements, and simultaneously serving the public interest. The case study is appropriate for both undergraduate and graduate auditing courses.

Periodic broadcasting is an effective approach for delivering popular videos. In general, this approach does not provide interactive (i.e., VCR) functions, and thus a client can tolerate playback latency from a video server. The concept behind the approach is partitioning a video into multiple segments, which are then broadcast across individual communication channels in terms of IP multicast. The method improves system throughput by allowing numerous clients to share the channels. For many broadcasting schemes, client receiving bandwidth must equal server broadcasting bandwidth. This limitation causes these schemes to be infeasible in mobile networks because increasing receiving bandwidth at all client sites is expensive, as well as difficult. To alleviate this problem, the fibonacci broadcasting (FiB) scheme allows a client with only two-channel bandwidth to receive video segments. In comparison with other similar schemes, FiB yields smallest waiting time. Extending FiB, this work proposes a new scheme (called FiB+) to achieve smaller client buffering space and the same waiting time under two-channel receiving bandwidth. Extensive analysis shows that FiB+ can yield 34.5% smaller client buffer size than that of FiB. Further simulation results also indicate that FiB+ requires lower client buffering space than several previous schemes.

In this paper, by analyzing the Communication architecture of integrated supervision and control system, the devices that communicate following the IEC61850 standard in station layer are given, which all need to integrate IEC61850 client. While a communication test is conducted using IED simulator and communication gateway simulator to verify the feasibility of the proposed integration method.

Margaretha Järvinen: The meeting between clients and the system: on research in social work This article is based on the inaugural lecture by the first professor of social work in Denmark. Although coming later than in many other countries, the professorship symbolizes the fact that the field of social work is about to develop into a scientific field of research. The article focuses on social work as a meeting between clients and social welfare institutions. Although it has become popular to describe citizens who come into contact with social welfare institutions as “comsumers” or “users”, this article argues for retention of the idea of client, since these are people with limited freedom of action. Social workers are described as representatives of the welfare state, not as representatives of, or advocates for clients, as they sometimes describe themselves. The article discusses three aspects of the meeting between the client and the system, and is inspired by the work of Pierre Bourdieu. The first is how the system creates the client. Social work is constructed as a field in which clients and their problems are constructed in accordance with the system and its doxa, rather than the other way around. The second aspect is the janus face of help. The article questions the idea that social workers “help” their clients. The traditional helping relation between social workers and their clients is described as a relationship of symbolic power that tends to underline rather than relieve the client’s ‘otherness’. Finally, the article discusses Bourdieu’s concept of practical reason. The field of social work represents rituals, routines and rationalities that may be difficult to put into a neat scientific formula. It argues though, that this concept should not be used (as some social workers have done) as a bulwark against scientific inquiries into the doxa and symbolic power mechanisms of the field.

The 1975 White Paper Better Services for the Mentally Ill recognises different roles for day hospitals, day centres and the voluntary sector in the provision of psychiatric day care. Two broad client groups, needing short-term support or long-term care, are described. However, Vaughn (1983 and 1985) and Wilkinson (1984) have reviewed lack of co-ordinated planning in the provision of services and the placement of clients. Carter (1981) in a major survey of day care, showed that in many cases it was difficult to differentiate between day hospital and day centre services or client groups.

To reduce the gap among regularly altering client requirements and existing resources, Client-Awareness Allocation of Resources and Scheduling of jobs in cloud by using social group optimization (SGOCARAJS) is proposed. This algorithm is mainly split into two phases namely allocation of resources using SGO and shortest job first scheduling. The main aim is to map the jobs to virtual machines of cloud group to attain higher client satisfaction and lowest makespan time. Experiments are conducted on datasets and results are compared with present scheduling techniques. This model proved that this algorithm outrun the available algorithms based on concerned metrics.

AbstractWhen developing a (web) interface for a deductive database, functionality required by the client is provided by means of HTTP handlers that wrap the logical data access predicates. These handlers are responsible for converting between client and server data representations and typically include options for paginating results. Designing the web accessible API is difficult because it is hard to predict the exact requirements of clients. Pengines changes this picture. The client provides a Prolog program that selects the required data by accessing the logical API of the server. The pengine infrastructure provides general mechanisms for converting Prolog data and handling Prolog non-determinism. The Pengines library is small (2000 lines Prolog, 150 lines JavaScript). It greatly simplifies defining an AJAX based client for a Prolog program and provides non-deterministic RPC between Prolog processes as well as interaction with Prolog engines similar to Paul Tarau's engines. Pengines are available as a standard package for SWI-Prolog 7.1

SUMMARY: This study examines auditor-client relationships following the high-profile merger of a local Chinese audit firm, Da-Hua CPAs, with a Big 4 firm, Ernst & Young, to create EYDH in early 2002. Of the 46 domestically listed clients Da-Hua had at the time of the merger, 30 switched to other audit firms during 2002–04. This large loss of clients could be attributed either anecdotally to a lack of post-merger managerial and cultural congruence, or to a lack of demand for high quality audits. We examine 11 (13) switching clients in 2002 (2004) as early (late) switchers. Although our archival analyses suggest that the switching decisions of early switchers are more likely to be explained by common factors such as changes in client structural characteristics, post-merger client portfolio management, and client-auditor friction over accounting treatments, late switchers do not differ from late non-switchers in terms of these factors. However, we find some time-serial evidence that late switchers follow their audit partners to a local audit firm mainly for greater discretion over financial reporting. Further, semi-structured focused interviews reveal that late switchers found it difficult to adapt to EYDH’s practices which, in their view, were less cooperative and too risk aversive. Overall, the results of our case study are consistent with the notion that clients switch from Big 4 to local firms mainly for more lenient audit treatments.

Abstract Five key therapeutic functions of metaphors are often discussed by psychotherapists. They (i) help clients express emotions and experiences, (ii) help therapists and clients explain difficult concepts, (iii) introduce new frames of reference, (iv) help work through client resistance, and (v) build a collaborative relationship between therapists and clients. Research on how these functions are enacted in psychotherapy talk tends to assume that they are indeed perceived as such by clients, and that metaphorical language is preferred to comparable literal language in performing them. This paper reports a survey study (N = 84) to critically interrogate these assumptions. Participants read two constructed therapy dialogues, controlled and counterbalanced for presentation sequence, where therapist and client discuss an issue using metaphorical and literal language respectively. Each dialogue is followed by a 15-item questionnaire to rate how well the presumed functions were performed (e.g. the therapist and client can work effectively together, the therapist is able to explain difficult concepts). A combined Confirmatory (CFA) and Exploratory Factor Analysis (EFA) suggests that, instead of the five distinct functions proposed in the literature, participants discerned three functions which reflect a more holistic view of what metaphors can do. A second EFA conducted on literal responses yielded only two factors. This contrast in factor structure further suggests that (i) literal language is less functionally nuanced, and (ii) metaphors are not simply perceived as an ‘add-on’ to literal language, but are evaluated across an extended narrative in fundamentally different ways. Within-subjects metaphor vs. literal ratings of the items under the emergent three-factor structure were then compared. Metaphor ratings were significantly higher in all factors (p < 0.01), suggesting that metaphorical language is indeed perceived as more effective than literal language when discussing clients’ issues. Implications, limitations, and future directions are discussed.

SUMMARY Auditors and clients are often required to resolve difficult, complex accounting issues in which they have different views. However, we know little about the effect of contextual factors on auditors' negotiation behaviors. This experimental study involving 63 experienced audit managers and partners examines the impact of the strength of the audit committee (strong or weak) and past relationship with the client (contending or compromising) on auditors' judgments in the pre-negotiation planning phase in resolving a difficult, subjective inventory writedown issue. These two important contextual factors are posited to affect auditors' perceived bargaining power and expectations of difficulties with respect to the impending negotiation. Specifically, we hypothesize an interaction where the most contending position (negotiation strategy) is adopted when the audit committee is strong (enhanced auditor bargaining power) and the past relationship is contending (a difficult negotiation). The findings support these expectations. In additional analyses, we obtain parallel results in the negotiation phase. In all, the findings confirm the importance of strength of the audit committee and past client relationship on auditors' negotiation planning judgments, and the concurrent consideration of these two pervasive contextual factors in the audit environment. Determining if treatment results in a significant change in a client’s communication ability can be a difficult task for clinicians. We addressed this issue by proposing a definition of clinically significant change that contains three dimensions. A clinically significant change is a change in client performance that (a) can be shown to result from treatment rather than from maturation or other uncontrolled factors, (b) can be shown to be real rather than random, and (c) can be shown to be important rather than trivial. Additionally, some methods are described for evaluating the clinical significance of improvements in client performance during treatment.

This study sought the views of 22 Victorian rehabilitation counsellors (RC's) about what they do to try and engage reluctant clients, the behaviour such clients exhibit, whether RC's discuss the reluctance and the mandatory nature of participating in rehabilitation with their clients, and how RC's choose to respond to reluctance. RC's reported that reluctance was most often a result of clients' emotional adjustment difficulties, and that the most difficult clients to engage were those who were passively disengaged rather than those who were initially angry or hostile. Engagement relied on RC's uncovering the clients' own goals for participating in rehabilitation. Implications of the findings for practice, and the need for a model of client reluctance in rehabilitation are discussed.

However, current functional analytic technology is often limited in its effectiveness in the clinical setting because the focus is on labelling and classifying behavioural outcomes rather than conducting a detailed analysis of the manner in which these outcomes assist the client to operate in the environment. The effectiveness of existing functional analytic techniques might be enhanced by moving the focus of the investigation from identifying the consequent variables that impact on behaviour to systematically exploring client–environment interactions as well as the manner in which the client experiences the consequences of his/her actions. One potential method (Valued Outcomes Analysis) that provides a framework for the systematic investigation of the effects of ‘inner skin’ factors on the incidence of behavioural difficulties is described here.

Objective: to identify the main technical difficulties operational faced by the nurse in the sector of politrauma during the assistance for the client politraumatized. Methods: this is about a descriptive study, from qualitative approach, performed with 10 nurses from politraumatized sector at large public hospital in Rio de Janeiro city, Brazil, from July to August 2007. Information was collected through structured interviews script whose analysis was based on content analysis, thematic analysis, method proposed by Bardin. Results: through the testimonies if realizes to real situation of public service emergency from Rio de Janeiro city. When evaluating the assistance provided to the client politraumatized, we note several technical problems operational what make difficult the nurse work with these clients. Conclusion: the emergency care of trauma victims to the customer still needs many improvements. The technology, when put into practice the correct way, become a facilitator, minimizing the difficulties, contributing to the development of technical and scientific professionals and increasing the survival of the client polytraumatized in serious condition. Purpose Microfinance has become an important way to alleviate poverty. Though four decades have passed since its introduction, its impact is still not entirely clear. What makes it difficult to ascertain its efficacy is the existence of diverse types of microfinance organizations and client profiles. Microfinance institutions must primarily pay more attention to the client, and to the mechanism through which financial services are delivered. The purpose of this paper is to identify the profiles of microfinance customers and the features of their operations. Design/methodology/approach In this paper, multilevel latent class models were estimated to reveal clusters of operations and classes of clients. Findings The results show that there are six clusters of operations and four classes of clients in the market, each with distinct profiles and needs. Different strategies are recommended for each cluster and class. Originality/value Numerous studies have focused on the importance of getting to know the clients of microfinance programs, but none as yet have used market segmentation as a way to do so. The goal is to generate better strategies to help clients improve their business results. Applying market segmentation to the microfinance market may point to different products for different groups of clients, taking the real needs of each of them into account.

ABSTRACT Auditors frequently use benchmarking analysis to evaluate the appropriateness of a client's estimates. Client management may strategically select benchmark data, making an auditor's evaluation task more difficult. Psychology research suggests that the composition of the benchmark set can lead to contrast effects, because evaluations of an option in a choice set can change based on the inclusion of other options in the set. In Experiment 1, we examine and find that auditors' judgments of the reasonableness of a client-preferred discount rate for a Level 3 investment are inappropriately influenced by the set of peer companies provided by the client as justification. In Experiment 2, audit managers performing the same task similarly exhibited contrast effects. However, as managers' investment experience increased, the influence of contrast effects from the benchmark set decreased. Given the widespread use of benchmark data, contrast effects from benchmark set composition have implications for accounting and auditing contexts.

This article discusses the treatment of a young person with obsessive-compulsive disorder (OCD) characterized primarily by symptoms of physical and mental contamination, as well as religious obsessions. The treatment was complicated by several factors that will be reviewed and addressed. First, this client had a comorbid diagnosis of attention-deficit/hyperactivity disorder. Second, the young client's mother was also diagnosed with OCD and had a significantly anxious presentation. This impacted treatment in two primary ways. One important way treatment was complicated was that the client frequently attempted to manage her mother's emotions by underreporting her own distress at times. The other implication of the mother's diagnosis was that it became difficult to involve her in the treatment of her daughter and reduce her accommodation of her daughter's symptoms. The manner in which these complicating factors were addressed in the successful treatment of this client by adapting evidence-based practice will be presented as a guide for clinicians facing similar challenges.

This research is a case study of the effect of Dramatherapy with a client who was suffering from prolonged grief following stillbirth. The assumption was that an analysis of the Dramatherapy sessions could shed light on processes that may assist the client in relief of her unresolved grief and integration of her stillbirth, which were necessary in order to get on with her life. The Dramatherapy was based on Jennings’ (1992, 1993, 1998) EPR model. The analysis focused on the dramatherapeutic processes, specifically, dramatic reworking, retelling and projection, that accompanied the client's experience of relief and integration of her loss. The client, ‘Iris’ (a fictional name), is a career woman, who sought dramatherapeutic treatment in the hope that it might help her overcome her overwhelming feelings of distress and sorrow, which made it difficult for her to function, both physically and emotionally. The research examined the first six sessions of a longer-term therapy, during which the client's prolonged grief emerged as a major issue for her. The records kept by a participant observer (the researcher), the products of the client's art work, and audio recordings of the sessions served as the research data. The presentation of the data focused on changes in the client's appearance, body language, vocal presence, emotionality, and issues raised in the sessions.

This study was used to examine how practicing veterinarians protect the self when dealing with people at work. Veterinarians interact with the people attached to their patients in myriad ways in order to accomplish their organizational goals. Data collected during semistructured interviews with 17 veterinarians revealed that these veterinarians considered client interactions just as important as treating nonhuman animals. Impression management concepts were used to describe many of the interactional strategies veterinarians employ to protect the self when dealing with difficult clients. Building trust with clients and giving front stage performances were common strategies adopted. Despite veterinarians wanting their jobs to be animal-centric, these data show that they are not.

This commentary discusses the therapy of a hybridized client (Blimling, 2019) with a difficult relational trauma history in which client-chosen music was combined with a short-term treatment utilizing Accelerated Experiential Dynamic Psychotherapy (AEDP). This combination bypassed rigid defenses and allowed access to a level of affect not ordinarily available to the client, allowing significant symptomatic and relational shifts to occur. Primary goals of helping the client deal with a major loss, reduce his level of depression, and improve his interpersonal functioning were met. Implications for the use of integrative methods with short-term models and the importance of therapist flexibility are discussed.

SUMMARY In this paper, we present the results of an experiment that investigates the effects of two factors—auditors' negotiation experience and client negotiation style—on auditors' perceived ultimate negotiation outcome in terms of an asset impairment writedown. We show that negotiation experience leads to a higher perceived ultimate negotiated writedown for a client with a contentious negotiation style, suggesting that negotiation experience reaps benefits when it is needed most (i.e., when the client is difficult to deal with). Negotiation experience has no effect for a client with a collaborative negotiation style. We also find that the effect of client negotiation style on auditors' perceived negotiation outcome is contingent on the auditors' negotiation experience. Inexperienced auditors perceive a lower ultimate negotiated writedown when dealing with a contentious rather than a collaborative negotiation style client. This makes it difficult for a lawyer to deal with many clients and different cases. The purpose of this study is to design the Expert System for Handling Land Dispute Cases using the Backward Chaining Method. The study was conducted to analyze the existing needs, collect data, design, implement the results of the design into the application that is used, after that will be tested and maintained the system. The results of the study show that the existence of this expert system, it provides very meaningful benefits for the client, namely facilitating the client to carry out consultations regarding the handling of land dispute cases.

The possibilities of using such spiritual practices as Islamic meditation and Islamic reasoning in psychological work with Muslims are shown. Also the article provides an analysis of the main problems arising during the family therapy of Muslims and describes the professional qualities of a psychologist that are necessary for effective work with clients who profess Islam. In conclusion, it is shown that the solution of psychological problems of believers is difficult or impossible to achieve without a trust that is formed between a client and a psychologist and based on their mutual understanding of religious matters.

Existential themes and meaning in life are often embedded in all types of psychotherapy, but can be difficult to handle in a systematic way. This article presents a method of assessing and exploring sources of personal meaning with a client during a 1-hour session. The method is rooted in existential theory as well in contemporary empirical psychology on sources of meaning. The Sources of Meaning Card Method comprises three stages. (a) From a total of 26 cards with printed statements concerning possible sources of meaning, the clients are asked to select 3 to 5 cards of most important personal content. (b) With each of these cards, a semistructured conversation is initiated with regard to the statement’s meaning, personal significance, actual importance, threats, and possibilities for personal change. (c) The therapist summarizes the client’s priorities, decisions, and essential parts of the conversation at the end of the session. The method is presented in four language versions.

Abstract Introduction The Support. Empower. Learn. Parenting Health Initiative (SELPHI) provides expectant and parenting youth ages 16–24 in Philadelphia with supports to improve educational, social, and economic outcomes to shape their health and the health of their children. Phone, text, video-based, and social media communication technology is built in to SELPHI’s program design to facilitate case management and connect clients to a broad referral network. Given the novelty of using information and communication technology (ICT) in case management, the reported lessons learned seek to give providers a specific and nuanced picture of ICT in case management. Methods In its initial 6-month implementation period, SELPHI’s five case managers, called Navigators, served 59 clients. Data from feedback surveys and case records were collected from clients and Navigators. Data included client demographic characteristics, needs assessment, and contact records to inform continuous quality improvement (CQI). Results ICT’s benefits included having multiple ways to connect to difficult-to-reach clients, the ability to be more responsive to clients, and the flexibility to address scheduling and transportation barriers. ICT’s challenges are related to Navigators’ boundary setting, limitations on rapport building, and data security considerations. CQI data are presented to illustrate the lessons learned. Text messages were the most prevalent ICT; phone calls were most successful in engaging clients. Clients’ ICT preferences differed by purpose of communication. Discussion Findings suggest that programs should understand the nuances of client contact preferences. To maximize the benefits of ICT, programs must develop or adapt protocols based on preference and purpose of communication.

Paniagua, et al. suggested that the definition of a “difficult” case in psychotherapy implies a relational definition involving the characteristics of the patient, the case, and the therapist. This study concerned this hypothesis with 44 graduate students in social work who received the Difficult Case Questionnaire representing examples of variables across each domain, e.g., motivation of the client, nature of the disorder, generic factors, and orientation of the therapist. The results were compared with the 1993 findings reported by Paniagua, et al., for a group of professionals in mental health practice. As in the earlier study, the present sample also agreed that, although all such domains are important in the formulation of that definition, their importance is not equal. Whereas professionals rated patient's, case's, and therapist's characteristics as the most important order of domains in that definition, students in this study agreed on the order of therapist's, patient's, and case's characteristics.

Family support agencies that undertake work with families where protective concerns are evident, are frequently placed in the difficult situation of deciding whether to involve Protective Services, and if so, how to do this and remain a helping agency in the eyes of the parents.Alys Key Family Care has found it immensely valuable to meet with a group of service users to consider the issues of (1) passage of information from the agency to Protective Services, and (2) circumstances which warrant involvement of Protective Services.Unlike many professionals, the client group expressed clear and consistent views about what situations warranted notification to a Protective Service, and as a result, the agency has now developed a pamphlet for all new client families that outlines how the agency handles these issues.The paper describes the difficult situations faced by professionals in family support services when Protective Services become involved with their client families and a clear and open policy does not exist between the agency and its clientele. It outlines the process adopted by Alys Key Family Care to actively involve service users in the development of agency policy, and describes the specific guidelines for Protective Service involvement generated by the client advisory group.

Abstract. Background: Individuals experiencing suicidal ideation and/or behaviors are often difficult to engage in treatment. Up to 50% of individuals presenting to an ER for treatment of a suicidal crisis refuse outpatient treatment referrals. This study examined outpatient mental health clinicians' perceptions of the barriers to and facilitators of mental health treatment engagement of individuals at risk for suicide. Method: Thirty-six mental health clinicians working with clients with past and/or current suicidality (ideation, behaviors) in outpatient mental health centers participated in four focus groups. Data were collected using semistructured interview. A process of content analysis was utilized in which data were analyzed thematically. Results: Five main domains of barriers and facilitators emerged including (a) client-level, (b) clinician-level, (c) service-level, (d) agency-level, and (e) intervention-level variables. In all, 38 barriers and 31 facilitators were identified. The most frequently identified facilitator was the clinician's own sense of engagement with the client, identified by 31% of the clinicians. The most frequently identified barrier was housing instability, identified by 34% of the clinicians. Conclusion: Knowledge of clinicians' perspective on treatment engagement of clients at risk for suicide may inform the development and implementation of interventions and guide training/supervision programs for those working with this vulnerable population.

Abstract Providing reliable and surreptitious communications is difficult in the presence of adaptive and resourceful state level censors. In this paper we introduce Tithonus, a framework that builds on the Bitcoin blockchain and network to provide censorship-resistant communication mechanisms. In contrast to previous approaches, we do not rely solely on the slow and expensive blockchain consensus mechanism but instead fully exploit Bitcoin’s peer-to-peer gossip protocol. We develop adaptive, fast and cost effective data communication solutions that camouflage client requests into inconspicuous Bitcoin transactions. We propose solutions to securely request and transfer content, with unobservability and censorship resistance, and free, pay-per-access and subscription based payment options. When compared to state-of-the-art Bitcoin writing solutions, Tithonus reduces the cost of transferring data to censored clients by 2 orders of magnitude and increases the goodput by 3 to 5 orders of magnitude. We show that Tithonus client initiated transactions are hard to detect, while server initiated transactions cannot be censored without creating split world problems to the Bit-coin blockchain.
