Abstract The randomised CLL4 trial was launched in 1999 to answer scientific and management issues. Since then, 730 patients with Binet stages A progressive, B and C have been randomised between 1) chlorambucil (10mg/m2 x 7 days), or 2) fludarabine (FDR) alone (40mg/m2 x 5 d oral or 25mg/m2 x 5 days IV), or FDR plus cyclophosphamide (FDR 24mg/m2 x 5 d oral or 25mg/m2 x 3 d IV plus cyclo 150mg/m2 x 5 d oral or 250mg/m2 x 3 d IV). Information on FISH analysis with 5 probes is available on 440 patients. The incidence of the genetic abnormalities is: 13q del (60%), trisomy 12 (15%), 6q del (8%), 11q del (19%), 17p del (11%); some markers are positive in more than one patient. Analysis by stage showed no differences in the distribution of these markers and analysis by age showed that patients with 13q del were younger (p=0.006) and those with 11q del were older (p=0.01). Analysis by response to all therapies (as the code of individual therapies has not been broken) in 343 patients is shown in Table 1. The results show clearly that almost half the patients with 17p del (p53 locus) are primary non-responders (NR) or show progressive disease (PD) following first line therapy, and have poor survival. 2yr survival of patients with none of these abnormalities was 87% (79%–95%), and using a hierarchical model with normals as the fifth group there was a clear trend in survival between groups (2p<0.00005). When the % of 17p del lymphocytes was correlated with response, it was apparent that the higher the % of 17p del cells the greater the proportion of non-responders (Table 2), with 20% 17p del being the critical threshold. When we grouped together cases with 6q, 11q and 17p deletion plus trisomy 12 (representing 42% of cases) there was a significantly lower response rate compared with the cases without those four abnormalities: CR/Nod PR 35% vs 47%, PR 33% vs 39%, and 32% vs 13%, (Chi-square 13.3; p=0.0003). This study demonstrates that the genetic abnormalities which can be shown in c.80% of patients with active CLL have a significant bearing on the response to treatment and early survival, confirming the survival differences reported by Dohner at al (N Engl J Med 343: 1910–16; 2000). It remains to be determined whether better therapies could overcome the low response rates of patients with poor prognostic genetic abnormalities. AbstractIntroductionAripiprazole 2-month ready-to-use 960 mg (Ari 2MRTU 960) is a new long-acting injectable (LAI) antipsychotic formulation for gluteal administration every 2 months, intended for the treatment of schizophrenia and maintenance monotherapy treatment of bipolar I disorder (BP-I). This 32-week trial evaluated the safety, tolerability, and pharmacokinetic profile of multiple-dose administration of Ari 2MRTU 960 in clinically stable adult patients with a diagnosis of schizophrenia or BP-I, versus that of aripiprazole once-monthly 400 mg (AOM 400; an LAI indicated for the treatment of schizophrenia and maintenance monotherapy treatment of BP-I).MethodsThis was an open-label, multiple-dose, randomized, parallel-arm trial conducted at 16 sites in the US. Patients were randomized to receive Ari 2MRTU 960 every 56±2 days (n=132) or AOM 400 every 28±2 days (n=134). The primary objective was to establish the similarity of aripiprazole concentrations on the last day of the dosing interval, as well as exposure during the dosing interval (area under the concentration-time curve [AUC]), between Ari 2MRTU 960 and AOM 400 following multiple doses. It was pre-specified that the lower bound of the 90% confidence interval (CI) of the geometric means ratio (GMR) for these parameters must be >0.8.ResultsIn the Ari 2MRTU 960 group, 102 patients (77.3%) completed the study; in the AOM 400 group, 92 patients (68.7%) completed the study. The GMR of C56 for Ari 2MRTU 960 to C28 for AOM 400 was 1.011 (90% CI: 0.893, 1.145). The GMR (90% CI) of AUC0–56 for Ari 2MRTU 960 to AUC0–28 for AOM 400 was 1.006 (90% CI: 0.851, 1.190). Mean (standard deviation) maximum aripiprazole plasma concentration was 342 (157) ng/ml after the fourth Ari 2MRTU 960 dose and 344 (212) ng/ml after the eighth AOM 400 dose.ConclusionPharmacokinetic parameters were similar between Ari 2MRTU 960 and AOM 400.FundingOtsuka Pharmaceutical Development & Commercialization, Inc. The aim of this research was to study the effectiveness of local bacterial strains from oil-contaminated soil to degrade phenol. The study consisted of two experiments, using six individual strains and using mix of strains. Bacterial strains used in the first experiment were 1.3, 3.3 dan 8.2.1 (Bacillus sp.), strain 3.2 (Propionibacterium), strain 3.4 (Pseudomonas sp.), and strain 8.1.2 (Enterobacter sp.).Bacterial strains used in the second experiment were mix of all six strains (K6) and mix of three strains (K3) consisted of strain 3.4, 8.1.2 and 8.2.1 with the same ratio. The experiments were conducted in 100 mL Bushnell and Haas medium containing 300-400 ppm phenol for three days.Three strains (strain 3.4, 8.1.2, dan 8.2.1) had the highest phenol removal efficiency at day 3, i.e. 99-100%. COD values were decreased to 345-393 mg/L or 56-61.3% by those three strains. Mix culture K6 effectively removed phenol form the medium, but COD value decreased to only 56.7%. The fate of COD decrease was not the same as phenol removal by these strains (either in idividual or mix cultures), because phenol was degraded into intermediate compounds.

440 Background: Sunitinib (Su) is a standard treatment (tx) for metastatic renal cell carcinoma (mRCC). Pre-clinical and clinical studies in several cancer types suggest that the antidiabetic agent metformin (Met) has antitumor activity. Met may negatively regulate mTOR activity. Its effect on the outcome of targeted therapies in mRCC is poorly defined. We analyzed the effect of Met use on the outcome sunitinib tx in diabetic patients (pts) with mRCC. Methods: We performed a retrospective study of an unselected cohort of diabetic pts with mRCC, who were treated with Su in 7 centers across 2 countries. Pts were divided into 2 groups: (1) Met users and (2) Met naive. The effect of Met use on response rate (RR), progression free survival (PFS) and overall survival (OS), was tested with adjustment of other known confounding risk factors using a chisquare test and partial likelihood test from Cox model. Furthermore, univariate and multivariate analyses of association between clinicopathologic factors and Met use, and outcome were performed using the entire pt cohort. Results: Between 2004-2014, 108 diabetic pts with mRCC were treated with sunitinib. There were 52 Met users (group 1) and 56 nonusers (group 2). The groups were balanced regarding the following clinicopathologic factors: age, gender, HENG risk, past nephrectomy, mRCC histology, ≥2 metastatic sites, lung/liver/bone metastasis, prior targeted tx, smoking status, use of angiotensin system inhibitors (ASIs), pre-tx neutrophil to lymphocyte ratio (NLR) >3, Su-induced hypertension (HTN), and Su dose reduction/tx interruption. Clinical benefit (partial response + stable disease) in group 1 vs. group 2 was 96% vs. 84%, while 4% vs. 16% had disease progression within the first 3 months of tx (p=0.054). Median PFS was 15 vs. 11.5 months (p=0.1). Median OS was 32 vs. 21 months (p=0.001). In multivariate analyses of the entire pt cohort (n=108), factors associated with PFS were active smoking (HR=2.7, p<0.0001) and pre-tx NLR >3 (HR 1.8, p=0.012). Factors associated with OS were Met use (HR 0.2, p<0.0001), HENG risk (HR 3.3, p=0.008), active smoking (HR=2.9, p<0.0001), liver metastases (HR 1.8, p=0.004), and pre-tx NLR >3 (HR 3.3, p<0.0001). Conclusions: Met use may improve the OS of diabetic pts with mRCC that are treated with Su.

Hyaluronidases are enzymes that degrade hyaluronan, an important component of the extracellular matrix. The mammalian hyaluronidases are considered to be involved in many (patho)physiological processes like fertilization, tumor growth, and metastasis. Bacterial hyaluronidases, also termed hyaluronate lyases, contribute to the spreading of microorganisms in tissues. Such roles for hyaluronidases suggest that inhibitors could be useful pharmacological tools. Potent and selective inhibitors are not known to date, althoughl-ascorbic acid has been reported to be a weak inhibitor ofStreptococcus pneumoniaehyaluronate lyase (SpnHL). The x-ray structure of SpnHL complexed withl-ascorbic acid has been elucidated suggesting that additional hydrophobic interactions might increase inhibitory activity. Here we show thatl-ascorbic acid 6-hexadecanoate (Vcpal) is a potent inhibitor of both streptococcal and bovine testicular hyaluronidase (BTH). Vcpal showed strong inhibition ofStreptococcus agalactiaehyaluronate lyase with an IC50of 4 μmand weaker inhibition of SpnHL and BTH with IC50values of 100 and 56 μm, respectively. To date, Vcpal has proved to be one of the most potent inhibitors of hyaluronidase. We also determined the x-ray structure of the SpnHL-Vcpal complex and confirmed the hypothesis that additional hydrophobic interactions with Phe-343, His-399, and Thr-400 in the active site led to increased inhibition. A homology structural model of BTH was also generated to suggest binding modes of Vcpal to this hyaluronidase. The long alkyl chain seemed to interact with an extended, hydrophobic channel formed by mostly conserved amino acids Ala-84, Leu-91, Tyr-93, Tyr-220, and Leu-344 in BTH.

Purpose We aimed to update a previously published, multi-institutional nomogram of outcomes for salvage radiotherapy (SRT) following radical prostatectomy (RP) for prostate cancer, including patients treated in the contemporary era. Methods Individual data from node-negative patients with a detectable post-RP prostate-specific antigen (PSA) treated with SRT with or without concurrent androgen-deprivation therapy (ADT) were obtained from 10 academic institutions. Freedom from biochemical failure (FFBF) and distant metastases (DM) rates were estimated, and predictive nomograms were generated. Results Overall, 2,460 patients with a median follow-up of 5 years were included; 599 patients (24%) had a Gleason score (GS) ≤ 6, 1,387 (56%) had a GS of 7, 244 (10%) had a GS of 8, and 230 (9%) had a GS of 9 to 10. There were 1,370 patients (56%) with extraprostatic extension (EPE), 452 (18%) with seminal vesicle invasion (SVI), 1,434 (58%) with positive surgical margins, and 390 (16%) who received ADT (median, 6 months). The median pre-SRT PSA was 0.5 ng/mL (interquartile range, 0.3 to 1.1). The 5-yr FFBF rate was 56% overall, 71% for those with a pre-SRT PSA level of 0.01 to 0.2 ng/mL (n = 441), 63% for those with a PSA of 0.21 to 0.50 ng/mL (n = 822), 54% for those with a PSA of 0.51 to 1.0 ng/mL (n = 533), 43% for those with a PSA of 1.01 to 2.0 ng/mL (n = 341), and 37% for those with a PSA > 2.0 ng/mL (n = 323); P < .001. On multivariable analysis, pre-SRT PSA, GS, EPE, SVI, surgical margins, ADT use, and SRT dose were associated with FFBF. Pre-SRT PSA, GS, SVI, surgical margins, and ADT use were associated with DM, whereas EPE and SRT dose were not. The nomogram concordance indices were 0.68 (FFBF) and 0.74 (DM). Conclusion Early SRT at low PSA levels after RP is associated with improved FFBF and DM rates. Contemporary nomograms can estimate individual patient outcomes after SRT in the modern era.

Tungkal Composite Jambi Liberika Coffee is one of the top qualities of Indonesian coffees that has received a geographic indication certificate (IGs). With its limited production and high prices, currently Tungkal Jambi Liberika coffee is one of the coffees that is prone to being counterfeited. The counterfeiting of Tungkal Jambi Liberika coffee is increasingly difficult to identify, especially in the form of ground roasted coffee. This study evaluated the potential application of UV spectroscopy technology to classify Tungkal Jambi Liberika coffee (with geographic indications) and normal Probolinggo Liberika coffee (non- geographic indications). A total of 120 samples for each Liberika coffee were prepared weighing 1 gram for each sample. Spectra measurements were carried out in the form of a coffee solution. Spectral data were taken using a UV-visible spectrometer with a wavelength interval of 200-400 nm (Genesys™ 10S UV-Vis, Thermo Scientific, USA). By using the average transformed spectra in the 250-350 nm interval, the differences between the two types of Liberika coffee can be clearly seen, especially at some wavelength peaks, namely 270 nm, 300 nm, 315 nm and 346 nm. The classification accuracy obtained for the SIMCA classification is 100% for both Tungkal Jambi Liberika and Probolinggo Liberika coffee.

Mean dose of OHCLQ: 221 mcg/L (64), 3.24 (1) mg/kg/day.Mean serum OHCLQ level: 166 mcg/L (6.5): 18 (28%) patients with level <100 mcg/L (mean: 63 [0.7] mcg/L) and 47 (72%) with level >100 mcg/L L (mean 195 mg/XX [94]). On ECG, mean HR: 69 bpm (10) and mean QTc interval: 429.7 (84) ms, prolonged (>440 ms) in 23 (35%) of the patients.12 (18%) patients reported dizziness at some time and 29 (45%) were being treated with a drug related to the risk of QTc prolongation: Possible (P: at authorized doses no risk of TdP): 1 (2%) patient, Conditional (C: risk under excessive dose, interactions): 13 (20%), P+C: 4 (8%), C+C: 11 (17%).When comparing the mean QTc result of the group with OHCLQ and the control groups without OHCLQ (n: 41), the QTc is higher (p=0.062) than the group of healthy subjects (n: 23) and without differences with the control group EASR (n: 18), not treated with OHCLQ (p=0.70). Among patients with OHCLQ level >100 mcg/L (n: 47) vs <100 mcg/L (n: 18), patient weight (p=0.012), level (p<0.0001), and dose in mg /kg (p=0.03), is significantly higher at levels >100 mcg/L. No relationship was observed between the OHCLQ level and the QTc result. In 6 (9%) patients the OHCLQ dose is >3 mg/kg (mean: 5.35 [1] mg/kg, HR: 73 [14] bpm, OHCLQ dose: 367 [71] mcg/L), no detected significant differences in QTc (1 patient presented QTC >440). In patients with QTc >440 ms, older patient age, QTc result, and heart rate were significant. However, the dose in mg/kg of OHCLQ was lower in the group with QTc <440 ms (p=007).ConclusionIn patients with SARD treated with maintenance doses of OHCLQ of about 3 mg/kg/day, 1) The serum level of OHCLQ is not associated with QTc prolongation on the ECG. 2) QTc is significantly related to age and heart rate. 3) The EASR per se does not affect the QTc result.Table 1.Characteristics of patients with QTc greater than or less than 440 ms and with an OHCLQ level greater than or less than 100 mcg/L.QTc >440 ms N: 23 (35%)QTc ≤440 ms N: 42 (65%)pAge, mean (SD)64 (15)56 (14.5)0.007Female, n (%)22 (96)40 (95)0.85QTc, mean (SD)454 (23.9)416 (23.7)0.0001Symptoms, n (%)4 (17)4 (10)0.77Heart rate, mean (SD)74 (10.3)67 (10.3)0.016Weight, mean (SD)69 (15.1)67 (15.6)0.61Risk factors QTc prolongation Concomitant disease, n (%)4 (17)4 (10)0.77 Drugs, n (%)14 (61)15 (36)0.18OHCLQ;Dosis, mean (SD)209 (64.5)229 (15.8)0.15Mg/kg OHCLQ, media (SD)2.91 (1.06)3.43 (1.08)0.07Years in treatment, mean (SD)9.3 (6.6)10.5 (6.5)0.48OHCLQ serum level, media (SD)174.27 (94.6)162,4 (80)0.51OHCLQ >100 mcg/LOHCLQ ≤100 mcg/LN: 47 (72%)N: 18 (28%)pOHCLQ serum level, mean (SD)166 (94)63 (96.5)0.0001QTc, mean (SD)430 (24)431 (23)0.87Dosis mg, mean (SD)221 (65)208 (66.5)0.48Mg/kg OHCLQ, mean (SD)3.32 (1.1)2.65 (1.08)0.03Years in OHCLQ, mean (SD)10.8 (6.8)11.5 (6.9)0.71Weight, mean (SD)69 (15.3)80 (15.2)0.012AcknowledgementsEl estudio fue apoyado con una beca de investigación de la Asociación para la Investigación en Reumatología de la Marina Baixa (AIRE-MB).Disclosure of InterestsNone declared

Purpose Intracranial efficacy of first-line crizotinib versus chemotherapy was compared prospectively in the phase III PROFILE 1014 study in ALK-positive non–small-cell lung cancer. Patients and Methods Patients were randomly assigned to receive crizotinib (250 mg twice daily; n = 172) or chemotherapy (pemetrexed 500 mg/m2 plus cisplatin 75 mg/m2 or carboplatin at area under the curve 5 to 6, every 3 weeks for ≤ six cycles; n = 171). Patients with stable treated brain metastases (tBM) were eligible. Intracranial efficacy was assessed at baseline and every 6 or 12 weeks in patients with or without known brain metastases (BM), respectively; intracranial time to tumor progression (IC-TTP; per protocol) and intracranial disease control rate (IC-DCR; post hoc) were measured. The intent-to-treat population was also assessed. Results Of 343 patients in the intent-to-treat population, 23% had tBM at baseline. A nonsignificant IC-TTP improvement was observed with crizotinib in the intent-to-treat population (hazard ratio [HR], 0.60; P = .069), patients with tBM (HR, 0.45; P = .063), and patients without BM (HR, 0.69; P = .323). Among patients with tBM, IC-DCR was significantly higher with crizotinib versus chemotherapy at 12 weeks (85% v 45%, respectively; P < .001) and 24 weeks (56% v 25%, respectively; P = .006). Progression-free survival was significantly longer with crizotinib versus chemotherapy in both subgroups (tBM present: HR, 0.40; P < .001; median, 9.0 v 4.0 months, respectively; BM absent: HR, 0.51; P < .001; median, 11.1 v 7.2 months, respectively) and in the intent-to-treat population (HR, 0.45; P < .001; median, 10.9 v 7.0 months, respectively). Conclusion Compared with chemotherapy, crizotinib demonstrated a significantly higher IC-DCR in patients with tBM. Improvements in IC-TTP were not statistically significant in patients with or without tBM, although sensitivity to detect treatment differences in or between the two subgroups was low.

The effects of process parameters of biomass grain size (ranging from 0.425 to 3.35mm), rotational speed (ranging from 20 to 60rpm) and the pyrolysis temperature (ranging from 400 to 550 oC) on the yield of bio-oil were investigated in this study by using a fast pyrolysis reactor with a single tapered screw extruder. This study gives the optimal pyrolysis temperatures and rotational speeds for different grain sizes to achieve peak bio-oil yield. The results indicate that higher feed rates are required for larger grain sizes to achieve peak bio-oil yields. For instance, a 20rpm rotational speed is for 0.425~1.18mm grain sizes (50~57% bio-oil yield), while a 40rpm rotational speed is for 1.7~3.35mm grain sizes (56~60% bio-oil yield). This implies that the productivity of bio-oil can be significantly increased in the current system. For the case of 2.5~3.35mm grain size, which corresponds to an optimal rotational speed of 40rpm, the production rate was estimated to be around 4kg/h.

A series of dialdehyde cellulose samples with varied aldehyde content were prepared by oxidation of bacterial cellulose with sodium periodate. The X-ray diffractometry profile showed that the crystallinity degree of bacterial cellulose was 66%, but that of the dialdehyde cellulose (containing 12% aldehyde ) was 56%. Thermal decomposition of these dialdehyde cellulose samples and bacterial cellulose were studied by thermogravimetry. The decomposition of bacterial cellulose in nitrogen started at 230 and rapid weight loss occurred between 300-400. The introduction of dialdehyde group to bacterial cellulose resulted in significant shifts of decomposition to lower temperature range. The decomposition of dialdehyde cellulose (12% aldehyde ) started at 180 and rapid weight loss between 200-350. The temperature of rapidest weight loss shifted from 340 of bacterial cellulose to 290 of the dialdehyde cellulose(12% aldehyde).

331 Background: Testicular microlithiasis (TM) is a entity characterized by multiple areas of increased echogenicity on scrotal ultrasonography (U.S.). Studies have reported association between TM and testicular germ cell tumors (TGCT). However, a study assessing the association of TM and extragonadal germ cell tumors (EGCT) has not been performed. Methods: Scrotal U.S. were reviewed in 441 men randomly selected from our computerized database during the period 1998 and 2010 at our tertiary-care cancer hospital. The association of U.S. findings with medical records and a history of germ cell tumors (GCT) were evaluated. The type of tumor was confirmed at histology after surgery or biopsy. Exclusion criteria included age <15 years and history of orchidectomy. Results: The median age was 32 years (range 15 – 88). TM was present in 95 patients (22%) and GCT were found in 130 patients (29%). 105 of these had TGCT (seminoma, n=56 and no seminoma, n=49) and 25 had EGCT (mediastinal, n=16, retroperitoneal, n=6 and CNS, n=3). Concurrent diagnosis of a TGCT was more prevalent in TM patients than in patients without TM (36/95 [38%] versus 69/346[20%] OR 2.45 95% CI 1.49–4.0; p<0.0001) while a diagnosis of an EGCT was present in 13 patients with TM and 12 patient without TM (13/95 [14%] versus 12/346 [3%] OR 4.41 95% CI 1.94–10.03; p<0.0001). No statistical differences were found for histology type. Conclusions: The prevalence in Latin American of TM is higher than previous reports and strongly associated with TCGT and EGCT. The clinical impact of the relationship between EGCT and TM is yet unknown and further studies are needed to clarify its role in the management. We suggest that young high-risk referral patients with TM should undergo scrotal U.S. annually; when a testicular tumor is not evident by U.S., investigations to rule out an EGCT, as thoracic or abdominal CT scans, should be considered individually in each case.

Abstract. Possible arterio-venous gradients of T4, T3, rT3 and 3,3'-diiodothyronine (3,3'-T2) across the liver and the kidneys were measured in 9 patients with varying degrees of liver failure undergoing diagnostic catheterization. Plasma iodothyronine levels were measured in peripheral, hepatic and renal veins before and at 10-min intervals until 60 min after iv injection of 400 μg of TRH. In 2 patients estimated hepatic plasma flow and effective renal plasma flow were determined as well. In these 2 patients, no significant differences between iodothyronine levels in arterial and peripheral venous plasma were found. T4 and T3 levels were not significantly different between peripheral, renal and hepatic veins. Hepatic vein rT3 and 3,3'-T2 concentrations were 10.7 ± 8.3% (mean ± sd, P < 0.005) and 36 ± 18% (P < 0.001) lower than those in the peripheral vein (N = 9). Renal vein rT3 was just (6.2 ± 7.5%, P < 0.05) lower than rT3 in peripheral vein, whereas 3,3'-T2 was not different between the two veins. Estimates of hepatic and renal plasma flow were in agreement with values from the literature. On the basis of these data approximate hepatic clearance rates of 110 and 380 1/day for rT3 and 3,3'-T2 and a renal clearance rate of about 35 1/day for rT3 were calculated. Sixty min after TRH, plasma T3 was increased to 147 ± 56% (P < 0.05) and 3,3'-T2 in peripheral plasma was increased to 142 ± 36% (P < 0.025), whereas plasma T4 and rT3 did not change. These data suggest that the liver is the major site of plasma rT3 and 3,3'-T2 clearance in patients with mild liver disease and probably also in healthy humans. The kidneys may account for roughly 25% of plasma rT3 clearance. The increase in plasma 3,3'-T2 after TRH may be secondary to the rise in precursor T3, but thyroidal secretion of 3,3'-T2 is not excluded.

Multicomponent eutectic alloys developed in recent years based on the Al–Ca system have high practical application prospects due to their low density, high corrosion resistance, good processability when casting, and high formability in the as-annealed state. Alloy hardening is achieved by doping with Mn, Fe, Zr, Sc and other elements. Obtaining an ultrafine-grained state in aluminum alloys by the methods of severe plastic deformations, e.g. equal channel angular pressing (ECAP), significantly increases the complex of their mechanical properties. In this regard, the purpose this paper was aimed to study the effect of warm ECAP on the structure, mechanical properties and thermal stability of the eutectic aluminum alloy, wt.%: Al–3.5Ca–0.9Mn–0.5Fe–0.1Zr–0.1Sc. The ECAP process was carried out on as-cast alloy specimens with a diameter of 20 mm (temperature 400 °C, route BC, channel intersection angle 110°, number of passes N = 6). It is shown that as a result of ECAP, a developed substructure with high-density dislocations and released nanosized Al6(Mn, Fe), and Al3Sc particles is formed in the alloy, as well as primary coarse Al6(Mn, Fe) particles and eutectic Al4Ca particles are reduced in size. Such a change in the structure during ECAP leads to the significant hardening of the alloy: its strength properties increased by 1.5–2.0 times, and relative elongation decreased by 1.3 times in the longitudinal section sample and slightly changed in the «transverse» section sample as compared to the initial condition.

Aluminum alloys alloyed with rare earth and transition metal are promising materials for electric energy transportation due to their high properties of strength, thermal stability, and electrical conductivity. The features of strengthening, their mechanical properties and electrical conductivity of Al–0.2Y–0.2Sc–0.3Er alloy after cold rolling have been established. The alloy as a cast structure is presented by aluminum solid solution (Al) and dispersed eutectics with τ2 (Al75-76Er11-17Y7-14) phase upon complete dissolution of scandium in (Al), and a content of yttrium and erbium at the level of 0.2–0.3 % each. Cold rolling the ingot accelerates strengthening upon annealing at 270 and 300 °C, reducing the time of achieving peak hardness. The maximum strengthening due to precipitation of L12 dispersoid of Al3(Sc,Y,Er) phase with the average particle size up to 10 nm is achieved after 7 h of annealing at 300 °C after cold rolling. This shows the prevailing heterogeneous mechanism of nucleation due to defects accumulated during cold rolling which stimulates strengthening. The eutectic particles are located mainly along the boundaries, elongated in the rolling direction. Irrespective of the mode of sheet fabrication, the alloy demonstrates high thermal stability up to 400 °C. During annealing of the sheets to 450 °C, their non-recrystallized structure is retained. Ingot annealing at t = 300 °C in 7 h and cold rolling with subsequent annealing under the same conditions provide a high level of mechanical properties and electrical conductivity: σ0.2 = 194 MPa, σu = 210 MPa, δ = 12.1 % and IACS – 60,1 %. The alloy has demonstrated high yield stress up to 100 h of annealing at t = 300 °C.

Developmental cerebral hemodynamic adaptations to chronic high-altitude exposure, such as in the Sherpa population, are largely unknown. To examine hemodynamic adaptations in the developing human brain, we assessed common carotid (CCA), internal carotid (ICA), and vertebral artery (VA) flow and middle cerebral artery (MCA) velocity in 25 (9.6 ± 1.0 yr old, 129 ± 9 cm, 27 ± 8 kg, 14 girls) Sherpa children (3,800 m, Nepal) and 25 (9.9 ± 0.7 yr old, 143 ± 7 cm, 34 ± 6 kg, 14 girls) age-matched sea level children (344 m, Canada) during supine rest. Resting gas exchange, blood pressure, oxygen saturation and heart rate were assessed. Despite comparable age, height and weight were lower (both P < 0.01) in Sherpa compared with sea level children. Mean arterial pressure, heart rate, and ventilation were similar, whereas oxygen saturation (95 ± 2 vs. 99 ± 1%, P < 0.01) and end-tidal Pco2 (24 ± 3 vs. 36 ± 3 Torr, P < 0.01) were lower in Sherpa children. Global cerebral blood flow was ∼30% lower in Sherpa compared with sea level children. This was reflected in a lower ICA flow (283 ± 108 vs. 333 ± 56 ml/min, P = 0.05), VA flow (78 ± 26 vs. 118 ± 35 ml/min, P < 0.05), and MCA velocity (72 ± 14 vs. 88 ± 14 cm/s, P < 0.01). CCA flow was similar between Sherpa and sea level children (425 ± 92 vs. 441 ± 81 ml/min, P = 0.52). Scaling flow and oxygen uptake for differences in vessel diameter and body size, respectively, led to the same findings. A lower cerebral blood flow in Sherpa children may reflect specific cerebral hemodynamic adaptations to chronic hypoxia. NEW & NOTEWORTHY Cerebral blood flow is lower in Sherpa children compared with children residing at sea level; this may reflect a cerebral hemodynamic pattern, potentially due to adaptation to a hypoxic environment.

The administration of sustained-release FSH-MAP5 in a 2-injection protocol has been shown to be as effective as a multiple-FSH injection protocol in inducing superovulation in cattle (Bo and Mapletoft 2014 Theriogenology 81, 38) and in sheep (Fry et al. 2016 Reprod. Fertil. Dev. 28, 250); however, the effect on embryo quality in the latter experiment was unclear. The following experiment further investigated the effect of FSH-MAP5 on ovulation rate and embryo quality in a sheep multiple-ovulation embryo transfer (MOET) program conducted in the breeding season. Two hundred sixteen Dohne merino ewes received a 12-day CIDR-S device containing 0.33 g of progesterone (Zoetis, Florham Park, NJ, USA) plus 200 mg of FSH IM (Folltropin-V; Vetoquinol, Belleville, Canada) and 400 IU of eCG IM (Pregnecol: Vetoquinol) in 4 treatment groups. Group 1 (n = 51) received 7 injections (a.m., p.m.) of FSH in saline (2.5, 2.0, 1.5, 1.5, 1.0, 1.0, and 0.5 mL) starting 2.5 days before CIDR withdrawal and 400 IU of eCG in saline at the time of the first FSH injection. Group 2 (n = 53) received 6 injections (a.m., p.m.) of FSH in saline (3.0, 2.0, 1.5, 1.5, 1.0, and 1.0 mL) starting 2.5 days before CIDR withdrawal and 400 IU of eCG in saline at CIDR withdrawal. Group 3 (n = 56) received 3.3 mL of FSH in hyaluronan (50 mg of MAP5, Vetoquinol) and 400 IU of eCG in saline 2.5 days before CIDR withdrawal and 1.7 mL of FSH-MAP5 at 0.5 days before CIDR withdrawal. Group 4 (n = 56) received 3.3 mL of FSH-MAP5 at 2.5 days before CIDR withdrawal, 1.7 mL of FSH-MAP5 at 0.5 days before CIDR withdrawal, and 400 IU of eCG in saline at CIDR withdrawal. Ewes were inseminated with semen collected and pooled from 5 rams at 36 to 40 h after CIDR withdrawal. Donor ewes were slaughtered 6 days after AI, corpora lutea were counted on the ovary, and ova/embryos were collected. Data for corpora lutea, total ova/embryo, and transferable embryo was analysed by the Kruskal–Wallis test and differences between groups were determined by the Dunn’s test. Results are shown in Table 1. There were no differences in the mean number of ovulations or total ova/embryo collected between any group (P > 0.05); however, those receiving eCG at the time of CIDR withdrawal (Groups 2 and 4) had significantly more transferable embryos than those receiving eCG at the time of the first FSH injection (Groups 1 and 3). In conclusion, the administration of the sustained-release FSH-MAP5 in a 2-injection protocol in sheep was as effective as a multiple-FSH injection protocol in inducing multiple ovulations and yielded similar number of transferable embryos, whereas the administration of eCG at the time of CIDR withdrawal resulted in more transferable embryos, possibly by improving fertilisation rates.

Porous zinc oxide (ZnO) thin films were synthesized via the calcination of molecular layer-deposited (MLD) “zincone” layers. The effect of the MLD process temperature (110 °C, 125 °C) and of the calcination temperature (340 °C, 400 °C, 500 °C) on the chemical, morphological, and crystallographic properties of the resulting ZnO was thoroughly investigated. Spectroscopic ellipsometry reveals that the thickness of the calcinated layers depends on the MLD temperature, resulting in 38–43% and 52–56% of remaining thickness for the 110 °C and 125 °C samples, respectively. Ellipsometric porosimetry shows that the open porosity of the ZnO thin films depends on the calcination temperature as well as on the MLD process temperature. The maximum open porosity of ZnO derived from zincone deposited at 110 °C ranges from 14.5% to 24%, rising with increasing calcination temperature. Compared with the 110 °C samples, the ZnO obtained from 125 °C zincone yields a higher porosity for low calcination temperatures, namely 18% for calcination at 340 °C; and up to 24% for calcination at 500 °C. Additionally, the porous ZnO thin films were subjected to piezoelectric measurements. The piezoelectric coefficient, d33, was determined to be 2.8 pC/N, demonstrating the potential of the porous ZnO as an, e.g., piezoelectric sensor or energy harvester.

56 Background: The biological trajectory of Gleason score 6 (GS6) prostate cancer (PCa) in men on active surveillance (AS) is unknown. We herein evaluate the potential for high grade PCa to arise clonally from GS 6 disease and determine the capacity of tissue-based biomarkers to predict grade progression. Methods: Men on AS with GS6 PCa who underwent magnetic resonance imaging/ultrasound (MRI/US) fusion biopsy on two occasions from 2012 through 2017 were enrolled. Tumor foci were tracked and re-sampled using the MRI/US fusion biopsy platform. ERG immunohistochemistry (IHC) and DNA/RNA next generation sequencing (NGS) were performed on formalin-fixed paraffin-embedded (FFPE) initial and repeat biopsy specimens to assess tumor clonality and evaluate candidate molecular markers of PCa grade progression. Results: Sixty-seven men of median age 64 years (IQR 59-69) and PSA 4.9 ng/ml (IQR 3.3-6.4) underwent repeat sampling of a single tracked tumor focus using MRI/US fusion biopsy. The median interval to repeat biopsy was 11 months (IQR 6-13). On repeat biopsy, 31 (46%) men progressed to high-grade (GS≥7) disease (n = 24, GS 3+4 = 7; n = 7, GS ≥4+3 = 7). Among the 67 subjects, ERG IHC status was concordant between initial and repeat biopsy in 64 (96%). Of 134 total specimens obtained (67 initial + 67 repeat biopsies), ERG status determined by NGS was concordant with ERG status by IHC in 132 (99%). Comparing the initial biopsy specimens in men who did versus did not undergo grade progression on follow up biopsy, derived cell cycle progression (CCP) scores (median 57.3 vs. 44.0, p = 0.11) and genomic prostate scores (GPS; median 73.8 vs. 64.4, p = 0.15) were not significantly different. Similarly, expression of FOLH1, PCAT4, SCHLAP1, and SPINK1 on initial biopsy did not significantly differ among men who did and did not undergo grade progression. Conclusions: Use of MRI/US fusion biopsy facilitated resampling of the same clonal focus of cancer over time, with high concordance of ERG status determined by both IHC and NGS. Derived genomic classifiers and candidate individual gene expression markers measured on initial biopsy tissue were not significantly different between patients who progressed and those who did not.

Maddox Dairy, located in Riverdale, CA, USA, is a Holstein herd that milks 3500 cows with a 305-day mature-equivalent milk production of 12 800 kg, and they have been producing high genetic animals by embryo transfer (ET) since the early 1980s. Invivo-derived embryos from Holstein donors were transferred fresh (grade 1 or 2) or frozen (grade 1), at morula (4), early blastocyst (5), or blastocyst (6) stage, to virgin heifers (VH, natural oestrus, 13-15 months old) or lactating cows (LC, Presynch-Ovsynch, 86 days in milk, first or second lactation) 6 to 9 days after oestrus. Pregnancy diagnosis was done by transrectal ultrasonography at 32-46 days in VH and by the IDEXX PAG test at 30 days in LC. June, July, August, September, and October were called critical months (first service AI conception rate drops below 44%) and compared with the other months. The data from 32 503 ETs between January 2008 and December 2018 are summarised on Table 1. Pregnancy rates (PR) are lower for LC recipients than for VH. Embryo transfers performed 7 or 8 days after oestrus had higher PR in both types of recipients and embryos, but Day 6 and 9 oestrus are also used with fair results. The season does not seem to affect PR. There is not enough difference in the combination of stage and days from oestrus for invivo-derived embryos. These numbers do not belong to a planned experiment. Several management changes during the years were made, which make it very difficult to apply statistical methods to analyse the data correctly. They are used as a tool to make decisions in an attempt to improve future results. To assess intermittently scanned continuous glucose monitoring (isCGM) performance for different rates of change in plasma glucose (RCPG) during glycemic challenges in type 1 diabetes (T1D). Nineteen people with T1D (7 females; age 35 ± 11 years; HbA1c 7.3 ± 0.6% (56 ± 7 mmol/mol)) performing two glycemic challenges (OGTT) were included. During OGTTs, plasma glucose was compared against sensor glucose for timepoints 0 min (pre-OGTT), +15 min, +30 min, +60 min, +120 min, +180 min, and +240 min by means of median absolute (relative) difference (MARD and MAD) and Clarke Error Grid (CEG), then was stratified for RCPG and glycemic ranges. Overall, MARD was 8.3% (4.0–14.8) during hypoglycemia level 1 18.8% (15.8–22.0), euglycemia 9.5% (4.3–15.1), hyperglycemia level 1 9.4% (4.0–17.2), and hyperglycemia level 2 7.1% (3.3–11.9). The MARD was associated with the RCPG (p < 0.0001), detailing significant differences in comparison of low, moderate, high, and very high RCPG (p = 0.014). Overall, CEG resulted in 88% (212 values) of comparison points in zone A, 12% (29 values) in zone B, and 0.4% (1 value) in zone D. The isCGM system was accurate during OGTTs. Its performance was dependent on the RCPG and showed an overestimation of the actual reference glucose during hypoglycemia.

Background: College-level American football injury data are routinely collected; however, data relating to American football injuries at universities in the United Kingdom have never been reported. Purpose: To describe the epidemiology of UK university American football injuries. Study Design: Descriptive epidemiology study. Methods: An online survey tool was used to collect the injury data of 410 players from 56 UK university teams who participated in the 2014-2015 British Universities and College Sports American football season. Survey data were collected from January to February 2016 and were analyzed to determine the incidence and patterns of injury. Results: Overall, 710 injuries and 204 concussions were self-reported among the 410 participants, of which 334 (81.5%) were injured and 131 (32.0%) experienced concussion symptoms. The rate of injury per 100 athlete-seasons was greater in defensive players (195.3) than offensive players (155.1). The most common injuries were knee and ankle ligament injuries. Most injuries were classified as severe (time loss of >4 weeks). Conclusion: UK university American football injuries differ markedly from those reported for US colleges. UK university players appear to have less playing experience, greater concussion risks, more severe injuries, and a greater proportion of injuries in defensive players versus offensive players.

Background:While salivary gland ultrasound (SGUS) has widely used for evaluating Sjögren’s syndrome, information on SGUS findings of systemic sclerosis (SSc) is limited. Objectives:We aimed to evaluate the salivary gland involvement in patients with SSc using SGUS.Methods:We consecutively included patients with SSc fulfilling American College of Rheumatology/European League against Rheumatism (ACR/EULAR) 2013 classification criteria, primary Sjögren’s syndrome (pSS) fulfilling ACR/EULAR 2016 classification criteria, and idiopathic sicca syndrome. All patients underwent SGUS examination using the Outcome Measures in Rheumatology (OMERACT) definition of a SGUS scoring system. The hyperechoic bands using 0-3 scoring system (none/<25% of the parenchyma/25-50% />50%) and intraglandular power Doppler signal (PDS) were assessed. Hocevar scoring system (0-48) comprising parenchymal echogenicity, homogeneity, hypoechoic areas, hyperechogenic reflections, and clearness of posterior borders were also evaluated. Results:A total of 147 patients were included in the study: SSc (n=59), pSS (n=56), and idiopathic sicca syndrome (n=32). The proportion of the highest OMERACT grades among the four glands≥ 2 were significantly higher in SSc (54.2%) and pSS (62.5%) than idiopathic sicca syndrome (3.1%). Total OMERACT SGUS scores, total fibrosis scores, Hocevar score were significantly higher in SSc and pSS compared to idiopathic sicca syndrome (Table 1). The proportion of the highest fibrosis grades among the four glands ≥2 were significantly higher in SSc (79.7%) than pSS (62.5%) and idiopathic sicca syndrome (46.9%). The orthogonal central compositional planning of the experiment was used to study the effect of three factors: sublimation temperature (350 to 400 °С), duration (40 to 80 min), and weight of the catalyst fluorinated sintered mass (5 to 10 g) on the (NH4)2SiF6 sublimation completeness. Results obtained in the experiment were used to build a second-order model, which correlate with experimental data. The dynamics of (NH4)2SiF6 sublimation removal was determined for sublimation durations of τ = 10, 20, 40 and 80 min at processing temperatures of 350, 375 and 400 °C. The (NH4)2SiF6 removal degree values calculated based on the second-order model for τ = 44, 48, 52, 56, 60, 64, 68, 72, and 76 min fit well the experimental curves. Spectra of fluorinated catalyst samples before and after sublimation were studied using X-ray phase analysis and IR spectroscopy. The data of IR spectroscopy and X-ray phase analysis are in good agreement and show that (NH4)2SiF6, (NH4)3AlF6 and unreacted NH4F are present in the catalyst with NH4F sintered mass, and only aluminum compounds are detected – NH4AlF4 and AlF3 after sublimation. These data indicate the completeness of the sublimation removal of silicon from the catalyst and NH4F sintered mass with NH4AlF4 and AlF3 aluminum compounds only observed after sublimation. REE concentration is 15 % due to silicon removal.

PurposeThe purpose of this paper is to investigate the rationale for the failure of management and auditors to identify material internal control weaknesses (ICWs) in their initial Sarbanes‐Oxley Act of 2002 (SOX) 404 reviews, resulting in subsequent restatement of their opinions.Design/methodology/approachThe paper focuses on the factors associated with the failure of management and auditor to identify material internal controls weaknesses in their initial SOX 404 reports. Logistic regression is run on a sample of 56 firms that reported material internal controls weaknesses in their amended internal control reports and a control group of 344 firms that reported material internal controls weaknesses (i.e. ineffective internal controls) in their initial internal control reports for 2004.FindingsThe results show that firm size, the use of a Big 4 auditor, the ratio of non‐audit to total fees, and the need for accounting restatements are positively associated with the probability to file an amended internal control report. The number of ICWs and the number of audit committee meetings are negatively associated with the probability to file an amended internal control report.Practical implicationsThe paper's findings suggest that regulators and corporate boards should consider providing more guidelines on audit committee practices in addition to the audit committee structure. For example, more guidance by the board is needed to ensure that the audit committee is active in overseeing the company's auditor–client relationship and its internal audit function.Originality/valueEmpirical findings on factors associated with the failure of management and auditor to identify material ICWs in their SOX 404 review can contribute to an understanding of factors affecting the efficiency of the SOX 404 review by attributing such failure to either inherent factors such as operational complexity and industry membership or to managerial choices in auditor‐client relationship and corporate governance issues. An understanding of these factors can help companies and the Public Company Accounting Oversight Board and the Securities and Exchange Commission in their efforts to improve the effectiveness and the efficiency of the current SOX 404 process.

Background: In 2017, hydroxyurea was approved for the treatment of all sickle cell patients with genotype HbSS in Uganda following overwhelming support from the NOHARM trial conducted in a malaria endemic region at Mulago National Referral Hospital's Sickle Cell Clinic. Between January 2019 and June 2022, immediately following nationwide approval and distribution of hydroxyurea, hydroxyurea was prescribed a total of 4017 (51.2%) times out of the 7851 HbSS patient visits to the clinic according to data available from the clinic. Aims:The purpose of this study is to investigate the knowledge, perceptions, and beliefs of patients with sickle cell disease and their caregivers attending this clinic in Kampala regarding their disease, complications, genetic impact, general anticipated course of treatment or natural history of disease, and hydroxyurea. Methods: A total of 400 (56%) out of 713 patients were surveyed over a 3-week period using standardized, pretested questionnaires administered orally by trained research personnel in both English (253, 63%) and Luganda (147, 36.8%), which are the most common local languages and dialects spoken by patients in Kampala. Three-hundred and twenty-one (80.3%) participants were caregivers and 79 (19.8%) were children with sickle cell disease between 12- and 17-years-old. Results: Most participants reported receiving counseling about sickle cell disease (333; 83.3%) largely by hematologists (286; 85.9%), however, several knowledge gaps were identified. Two-hundred and ninety-seven (74.3%) participants reported not knowing the type of sickle cell or genotype of the patient. Moreover, presentation of various options did not significantly improve recognition of the correct genotype, with only 121 (30.3%) participants correctly identifying HbSS. Three-hundred and twenty-eight (82%) participants correctly identified the heritable transmission of the disease from both parents; however, caregivers exhibited limited understanding of genetic risk with only 31 (9.7%) correctly identifying the one in four chances of having a subsequent child with sickle cell disease when both parents are carriers. Hydroxyurea counseling was reported by 326 (81.5%) of participants, and 307 (76.7%) participants reported that the patient received hydroxyurea, with most patients taking the medication for at least 1 year. Of the 93 (23.5%) participants who identified the patient had never received hydroxyurea, 32 (34.4%) were uncertain if the patient would be eligible and 56 (57%) believed the patient was eligible to take the medication. Participants identified hydroxyurea benefits as decreases in pain (332; 83%), hospitalization (162; 40.8%), need for blood transfusion (148; 37%), need for pain medication (87; 21.8%), infections (42; 61%), weight loss (22; 31.9%), length of hospital stay (18; 4.5%), and incidence of stroke (10; 2.5%). Slightly more than half of patients had no concerns about hydroxyurea (218; 54.5%). Most patients with concerns (182; 45.5%) identified hydroxyurea's high cost as their main concern (141; 77.5%). Other rumors and fears about hydroxyurea included: negative affects on internal organs such as the liver or kidneys (54; 29.7%), other negative side effects (27; 14.8%), inaccessibility in pharmacies and remote areas (18; 10%), harm due to stopping or missing doses (16; 8.8%), ineffectiveness (16; 8.8%), additional indications as a cancer medication (9; 4.9%), and younger children's difficulty swallowing pills (6; 3.3%). Conclusion:Given the significantly improved morbidity and mortality of sickle cell patients who are adherent to hydroxyurea, it is necessary to address the rumors, misconceptions, and gaps in understanding of the genetic impact of the disease as well as the risks and benefits of hydroxyurea therapy. At the population level, learning is impacted by education levels, literacy rates, health literacy rates, and other cultural and societal factors, thus teaching should be culturally competent and inclusive with verbal and visual methods. The high cost of the medication and lack of consistent supply seem to be the most prohibitive factors in patient acceptance and adherence to take hydroxyurea. With only 1% of Uganda's population covered by health insurance, it is imperative for system-wide changes to address the affordability and accessibility of hydroxyurea and/or to extend the coverage of health insurance to those in need.

The concentrations of thyroxine (T4), 3,3',5-triiodothyronine (T3) and 3,3',5'-triiodothyronine (reverse T3; rT3) in thyroid gland tissue and serum of the fetal pig (n = 68) from day 39 to 113 of gestation were measured. Tracer quantities of iodothyronines, displaying the onset of thyroid hormone activity, were found in the thyroid tissue on day 39, i.e. before the appearance of a measurable quantity of iodothyronines in the serum. The T4 and T3 thyroidal content showed the first rise between days 56 and 76. Then, T3 was increasing sharply from day 92 till birth, while T4 content was decreasing from about day 76 to a low value between day 92 and 105, and then showing an increase shortly before birth. The rT3 content was the highest on day 39 and then it was steadily decreasing to reach a nadir on about day 76. Measurable amounts of thyroid hormones (TH) in the serum were observed not earlier than on day 46 of gestation. Near birth, the tissues of the pig fetus are in a milieu characterised by the highest blood TH concentrations. The serum to thyroid concentration ratio for rT3 and T4 was generally below 1.0 until the last trimester of gestation, when it was over 5.0 for rT3 and over 4.0 for T4. By contrast, the T3 serum to thyroid ratio was below 0.5 throughout the gestation. The results show that the fetal pig thyroid displays a low rT3 and T4 content, but the marked T3 elevation observed near term supports the view that a high production and secretion of T3 near term may be a critical factor for normal postnatal adaptation to extrauterine cooling in the pig.

AbstractAutologous hematopoietic stem cell transplantation (autoHSCT) requires collection of sufficient number of hematopoietic stem cells. The goal of this study was to evaluate efficacy of plerixafor used in patients with lymphoid malignancies failing conventional stem cell mobilization. This was a prospective, non-interventional study. All consecutive patients (n = 109) treated with plerixafor in 11 centers were reported. The drug was used either in case of previous mobilization failure (n = 67) or interventionally, in case of insufficient CD34+ cell output during current mobilization (n = 42). Successful mobilization was defined as resulting in collection of ≥ 2 × 106 CD34+ cells/kg for single autoHSCT or ≥ 4 × 106 CD34+ cells/kg for double procedure. The overall rate of successful mobilization was 55% (55% for single and 56% for double autoHSCT). The median total number of collected CD34+ cells/kg was 2.4 (range, 0-11.5) for patients intended for a single transplantation while 4.0 (0.6-16.9) for double procedure. The number of circulating CD34+ cells increased after the use of plerixafor regardless of baseline values. The median fold increase was 3.3 (0.3-155). Data from this observational study confirm high efficacy of plerixafor used in routine clinical practice as salvage for patients with lymphoid malignancies failing conventional stem cell mobilization.

AbstractBackgroundAripiprazole 2-month ready-to-use 960 mg (Ari 2MRTU 960) is a new long-acting injectable (LAI) antipsychotic formulation for gluteal administration every 2 months. This 32-week trial evaluated the safety, pharmacokinetics, and efficacy of multiple-dose administration of Ari 2MRTU 960 in clinically stable adults with schizophrenia or bipolar I disorder (BP-I), versus that of aripiprazole once-monthly 400 mg (AOM 400; an LAI indicated for the maintenance treatment of schizophrenia in adult patients stabilized with oral aripiprazole [indication varies by country]). Safety and efficacy outcomes in the subpopulation of patients with schizophrenia are reported here.MethodsPatients with schizophrenia were randomized to receive Ari 2MRTU 960 every 56±2 days or AOM 400 every 28±2 days. Safety and tolerability assessments included adverse event (AE) reporting, Visual Analogue Scale [VAS] scores (scale range: 0–100) for patient-reported injection site pain, and extrapyramidal symptom (EPS) monitoring. Efficacy was evaluated at Week 32 using mean (standard deviation [SD]) Clinical Global Impression – Improvement (CGI-I) score, and mean (SD) change from baseline in Clinical Global Impression – Severity (CGI-S) score, Subjective Well-being under Neuroleptic Treatment – Short Form [SWN-S] Total score, and Positive and Negative Syndrome Scale (PANSS) Total score.ResultsStudy completion rate was 79.3% (73/92 patients) in the Ari 2MRTU 960 group and 67.7% (63/93 patients) in the AOM 400 group. Demographics and disease characteristics were well balanced between groups at baseline (mean [SD] PANSS Total score: Ari 2MRTU 960, 62.0 [13.5]; AOM 400, 61.8 [13.5]; mean (SD) CGI-S score: Ari 2MRTU 960, 3.3 [0.9]; AOM 400, 3.1 [0.9]). Treatment-emergent AE (TEAE) incidence was 66.3% with Ari 2MRTU 960 and 63.4% with AOM 400. The most frequent TEAEs were increased weight (Ari 2MRTU 960, 21.7%; AOM 400, 18.3%) and injection site pain (Ari 2MRTU 960, 15.2%; AOM 400, 9.7%). Mean (SD) VAS score for pain after last injection was 1.5 (4.58) with Ari 2MRTU 960 and 1.3 (2.79) with AOM 400. Minimal change was seen in EPS in either group. At Week 32, mean (SD) CGI-I score was similar between groups (Ari 2MRTU 960, 3.5 [1.0]; AOM 400, 3.6 [0.9]). Minimal change from baseline was seen at Week 32 in CGI-S score and SWN-S Total score. There was no clinically meaningful difference between the groups for PANSS Total score (difference of least squares mean change from baseline [95% confidence interval]: -0.9 [-3.5, 1.8]; p=0.5154).ConclusionsIn patients with schizophrenia, administration of Ari 2MRTU 960, as compared with AOM 400, was generally well tolerated, and clinical stability was maintained during the study.FundingOtsuka Pharmaceutical Development & Commercialization, Inc. Abstract Limited research has compared no beta-agonist, ractopamine and lubabegron in feedlot cattle. We evaluated carcass quality and yield variables using a camera grading instrument and also examined objective tenderness outcomes. Treatments were negative control (no beta-ligand; CON), Optaflexx (ractopamine hydrochloride, 300 mg·steer-1·d-1 for 35 d; OPT), and Experior (lubabegron fumarate, 36 mg·steer-1·d-1 for 56 d + 4 d removal; 0EXP) fed for 198 d. Additional EXP treatments investigated total days on feed (DOF) of 184 d (-14EXP), 212 d (+14EXP) and 226 d (+28EXP). Carcasses (n = 2,517) were instrument graded 24 h post-mortem. Longissimus tenderness and lipid content were evaluated in a subset (n = 361) of carcasses via slice shear force and ether extract. Statistical analyses were conducted using mixed models; pen was the experimental unit, treatment was the fixed effect, block was a random effect, and means were separated with the Tukey-Kramer adjustment. Longissimus muscle area (LMA) was 4.0 to 6.2 cm2 larger (P &lt; 0.01) for steers fed EXP compared with CON, which did not differ from OPT. Weight corrected LMA (LMA:HCW) was 1.3 cm2/100 kg larger (P &lt; 0.01) for -14EXP compared with CON. Marbling score was 31 to 39 points lower (P &lt; 0.01) for -14EXP compared with CON and OPT, whereas 0EXP, +14EXP and +28EXP were similar to CON and OPT. Total number of fat pieces within the longissimus did not differ (P = 0.11) across treatments, however number of fat pieces in the lateral one-half of the longissimus decreased (P ≤ 0.03) for -14EXP and +28EXP, whereas +14EXP tended (P = 0.09) to decrease compared with CON. Total meat area was 3.7 percentage points greater (P &lt; 0.01) for -14EXP compared with CON; +14EXP and +28EXP tended (P = 0.07) to be greater than CON. Total fat area was 1.6 to 1.8 percentage points less (P ≤ 0.02) for 0EXP, +14EXP, and +28EXP when compared with CON. Compared with CON, dorsal-ventral longissimus distance increased (P &lt; 0.01) by 3.3 to 5.4% for -14EXP, 0EXP, +14EXP and +28EXP. Medial-lateral longissimus distance increased 1.9 to 2.5 % (P &lt; 0.01) for +14EXP and +28EXP compared with CON, whereas -14EXP tended (P = 0.08) to increase. Percentage intramuscular fat (IMF) was similar (P ≥ 0.44) for +14EXP, +28EXP, and OPT compared with CON, whereas 0EXP tended (P = 0.09) to be less. Marbling score and IMF were moderately correlated (r = 0.66). Slice shear force values were 20.1% greater (P &lt; 0.02) for +14EXP compared with CON, whereas 0EXP tended (P = 0.09) to be greater and +28EXP, -14EXP and OPT were similar (P ≥ 0.28) to CON. These findings illustrate the effect of EXP on carcass traits and tenderness in feedlot cattle.

Iron Cove catchment (~1500 ha) on the southern shores of Port Jackson (Sydney) has been highly urbanised (>90%) for a considerable time (1800s). A map of the area was divided into squares 200 by 200 m and a soil sample was taken randomly in each defined area from the dominant land use type in each square (n = 374). The objectives of the study were to determine the distribution and sources of heavy metals (Cu, Pb, Zn) in surface soils over the catchment and to assess soil quality in terms of guidelines and bioavailability of these metals. The concentrations of Cu, Pb, and Zn were determined for total soil and on a normalised basis. These metals are elevated across the whole catchment, with mean total concentrations for Cu, Pb, and Zn of 62, 410, and 343 mg/kg and 50 percentiles of 44, 203, and 224 mg/kg, respectively, whereas mean enrichment over background is 2, 5.5, and 4.6 times, respectively. Such a wide, regional metal enrichment is probably related to atmospheric deposition from industry inside and beyond the catchment. The highest concentrations of most metals were in soils from the north-east part of the catchment in an area where dwellings and buildings are oldest and where major roads and railway lines converge. Normalise data suggest sources of these metals are roads, railway lines, and older lead-painted houses. Independent studies in the catchment support roads as being an important source of metals in the area. Total soil chemistry indicates that 34, 33, and 56% of the samples were above the Australian and New Zealand Environment and Conservation Council (ANZECC) and National Health and Medical Research Council (NH&MRC) guidelines for Cu, Pb, and Zn, respectively, and selective extraction procedures (EDTA and HCl) indicate that 46–86% of these metals may be bioavailable.

54 Background: Due to high symptom burden in advanced cancer patients, ongoing symptom management for outpatient palliative care patients is vital. More patients are receiving outpatient care; Yet, most palliative care patients receive less than 2 follow ups. Nurse telephone care can improve quality of life in these patients. Our aim was to determine frequency and care provided by Supportive Care Center Telephone Program (SCCTP) in advanced cancer patients. Methods: 400 consecutive patients who utilized palliative care service, 200 from outpatient Supportive Care Center (SCC) and 200 from inpatient Palliative Care (IPC), were followed for 6 months starting 3/2012 to examine call frequency and reason and outcomes including pain and other symptoms [Edmonton Symptom Assessment Scale (ESAS) and Memorial Delirium Assessment Scale (MDAS)] associated with utilization of SCCTP. We also examined the effect of SCCTP interventions on pain, ESAS and counseling needs. Results: 375 patients were evaluable. Median age 59 years, 53% female, 70% white. Most frequent cancer type were gastrointestinal (20%, p < 0.0001) for IPC and thoracic (23%, p <0.0001) for SCC. SCC patients had higher prevalence of CAGE positivity (28% SCC vs 11% IPC, p <0.0001), ESAS SDS(p=0.0134), depression(p=0.0009), anxiety(p=0.0097) and sleep(p=0.0015); MDAS scores were significantly higher in IPC (p<0.0001).115/400 patients (29%) utilized SCCTP. 96/115 outpatients (83%) used the SCCTP vs 19/115 IPC (17%). Common reasons for calls were pain (24%), pain medication refills (24%) and counseling (12%). Of 115 phone calls, 340 recommendations were made; 43% (145/340) were regarding care at home; 56% of these recommendations were regarding opioids. Patients who utilized SCCTP had worse pain(p=0.0059), fatigue(p=0.0448), depression(p=0.0410), FWB(p=0.0149) and better MDAS scores(p=0.0138) compared to non-utilizers. Conclusions: There was more frequent SCCTP use by outpatients than inpatients. Most common reason for utilization was pain control. Frequently, recommendations were made to continue symptom management at home. Patients who utilized SCCTP had worse pain, fatigue, depression, well-being scores and better delirium scores.

LBA6 Background: RTOG 0436 is a randomized Ph III trial designed to evaluate the benefit of cetuximab added to the concurrent chemoradiation for patients undergoing non-operative management of esophageal carcinoma. Methods: Pts with biopsy-proven squamous cell or adenocarcinoma of the esophagus (T1N1M0; T2-4 AnyN M0; Any T/N M1a) were randomized to weekly concurrent cisplatin (50 mg/m2), paclitaxel (25 mg/m2), and daily radiation 50.4 Gy/1.8 Gy fractions ± weekly cetuximab (400 mg/m2 day 1 then weekly 250 mg/m2). Patients were stratified by histology, tumor size (< 5 cm vs > 5cm), and the status of celiac lymph nodal involvement. Overall survival (OS) was the primary endpoint, with a planned accrual of 420 pts to detect an increase in 2-year OS from 41% to 53%; 80% power and 1-sided 0.025 alpha. An interim analysis of cCR was planned for the first 150 of each histology. Results: The study accrued 344 pts from 2008-2013 and 328 were eligible. Based on interim analyses, the study stopped accruing adeno pts in 5/2012 and SCC pts in 1/2013. Pts were well matched for pretreatment characteristics: 80% with T3/4 disease, 66% N1, and 19% with celiac nodal involvement. Incidence of grade 3/4/5 treatment (tx) related AEs was 45%, 22%, 4% in Arm 1 (cetuximab) and 49%, 17%, 1% in Arm 2 (no cetuximab). A cCR rate of 56% was observed in Arm 1 vs 59% in Arm 2 (p=0.72). No differences were seen in cCR between tx arms for either histology. The 12 and 24 mo OS rates for cCR pts were 79% and 58% vs 53% and 30% for those with residual disease (p<0.0001). Median follow-up for all pts is 15.4 mos. The 12 and 24 mo OS (95% CI) for Arm 1 is 64% (56%, 71%) and 44% (36%, 52%) vs 65% (57%, 72%) and 42% (34%, 50%) for Arm 2 (p=0.70). Adeno pts (n=203) had a 12 and 24 mo OS of 65% and 43% for Arm 1 vs 64% and 41% for Arm 2 (p=0.37). The 12 and 24 mo OS for the 125 SCC pts was 62% and 46% for Arm 1 vs 67% and 43% for Arm 2 (p=0.97). Conclusions: The addition of cetuximab to concurrent chemoradiation did not improve OS. There were no differences in cCR rates by tx arm. These Ph III results point to little benefit for current EGFR targeted agents in the tx of esophageal cancer. Supported by RTOG CA21661 and CCOP CA3742 NCI grants and Bristol Myers Squibb. Clinical trial information: NCT00655876.

Abstract Matched unrelated donor hematopoietic stem cell transplantation (MUD HSCT) is a potentially curative treatment for patients with acute myeloid leukemia (AML). The graft-versus-leukemia (GVL) effect may be potent enough to overcome the otherwise poor prognosis associated with AML though its efficacy for high risk cytogenetic subgroups is uncertain. To test this hypothesis, we analyzed outcomes by cytogenetic risk group in 324 patients in first complete remission (CR1), and 440 in CR2 undergoing NMDP-facilitated MUD HSCT between 1988–2002. Using the SWOG/ECOG classification of cytogenetic risk groups (Slovak et al. Blood, 2000) cytogenetics were classified as favorable in 14% of patients, intermediate in 71% and unfavorable in 16%. 56% of the patients were male and 42% were > 35 years at HSCT. 76% of patients and donors were matched at HLA-A, -B and -DRB1, 17% were mismatched at one or more loci and 7% were potentially matched (serologically matched at HLA-A and -B and potentially allele matched at -DR). Disease Status N Kaplan-Meier Estimate for Survival at 5 years Kaplan-Meier Estimate for Disease-Free Survival at 5 years Cumulative Incidence for 100 Day Transplant-Related Mortality Cumulative Incidence for Relapse at 5 years * p-value indeterminate; ** p=0.01 CR1 324 32 ± 6% 32 ± 5% 32 ± 5% 18 ± 4% Intermediate 227 33 ± 7% 32 ± 7% 31 ± 6% 16 ± 5%* Unfavorable 85 31 ± 11% 31 ± 10% 29 ± 10% 23 ± 9%* CR2 440 36 ± 5% 35 ± 5% 25 ± 4% 16 ± 3% Favorable 93 46 ± 10% 44 ± 10% 25 ± 9% 10 ± 6%** Intermediate 313 33 ± 6% 32 ± 5% 27 ± 5% 16 ± 4%** Unfavorable 34 37 ± 17% 38 ± 16% 15 ± 12% 32 ± 15%** These data suggest that with the exception of the 5-year relapse rate, results of cytogenetics have little apparent influence on the outcome for patients undergoing MUD HSCT for AML in CR1. In CR2, results in patients with favorable cytogenetics appear to be better than those with intermediate or unfavorable cytogenetics, but are not statistically significantly different. Effective GVL and protection against relapse is observed, even in high risk cytogenetic subgroups. In this retrospective study, other prognostic factors may influence the outcome, but overall survival for patients with unfavorable cytogenetics appears at least as good as previously reported for matched sibling HSCT.

Abstract. This study validates trace gas and thermodynamic retrievals from nadir infrared spectroscopic measurements recorded by the UK Met Office Airborne Research Interferometer Evaluation System (ARIES) – a thermal infrared, Fourier transform spectrometer (TIR-FTS) on the UK Facility for Airborne Atmospheric Measurements (FAAM) BAe-146 aircraft. Trace-gas-concentration and thermodynamic profiles have been retrieved and validated for this study throughout the troposphere and planetary boundary layer (PBL) over a range of environmental variability using data from aircraft campaigns over and around London, the US Gulf Coast, and the Arctic Circle during the Clear air for London (ClearfLo), Joint Airborne IASI (Infrared Atmospheric Sounding Interferometer) Validation Experiment (JAIVEx), and Measurements, process studies, and Modelling (MAMM) aircraft campaigns, respectively. Vertically resolved retrievals of temperature and water vapour (H2O), and partial-column retrievals of methane (CH4), carbon monoxide (CO), and ozone (O3) (over both land and sea) were compared to corresponding measurements from high-precision in situ analysers and dropsondes operated on the FAAM aircraft. Average degrees of freedom for signal (DOFS) over a 0–9 km column range were found to be 4.97, 3.11, 0.91, 1.10, and 1.62 for temperature, H2O, CH4, CO, and O3, respectively, when retrieved on 10 vertical levels. Partial-column mean biases (and bias standard error) between the surface and ~ 9 km, when averaged across all flight campaigns, were found to be −0.7(±0.3) K, −479(±56) ppm, −11(±2) ppb, −3.3(±1.0) ppb, and +3.5(±1.0) ppb, respectively, whilst the typical a posteriori (total) uncertainties for individually retrieved profiles were 0.4, 9.5, 5.0, 21.2, and 15.0 %, respectively. Averaging kernels (AKs) derived for progressively lower altitudes show improving sensitivity to lower atmospheric layers when flying at lower altitudes. Temperature and H2O display significant vertically resolved sensitivity throughout the column, whilst trace gases are usefully retrieved only as partial-column quantities, with maximal sensitivity for trace gases other than H2O within a layer 1 and 2 km below the aircraft. This study demonstrates the valuable atmospheric composition information content that can be obtained by ARIES nadir TIR remote sensing for atmospheric process studies.
